    Tumeur maligne des gaines nerveuses périphériques révélant une neurofibromatose type 1

    La Neurofibromatose type 1 (NF1) ou maladie de Von Recklinghausen est une affection à expression clinique variable. La transformation maligne est rare et redoutable. Le but de ce travail est d’étudier les critères diagnostiques ainsi que la prise en charge thérapeutique de cette transformation maligne. Observation : A.B, âgé de 16 ans, a consulté pour une tuméfaction basicervicale antérieure d’apparition récente, ayant augmenté progressivement de taille sans signes de compression ni de dysthyroïdie. L’examen clinique a trouvé une masse dure, bien limitée, indolore, de 4 cm de grand axe siégeant en regard du manubrium sternal, adhérente au plan profond et débordant sur le creux sus sternal. A cette masse s’associaient des taches cutanées café au lait disséminées sur tout le corps, des nodules de Lish iriens et des neurofibromes occipitaux et parotidiens. Après une exérèse de la masse par voie cervicale, l’examen histologique a conclu à une tumeur maligne de la gaine nerveuse. Le malade a eu dans un 2ème temps, une exérèse plus large emportant le manubrium sternal et les tissus adjacents  avec une reconstruction par des fragments de côtes mais avec des limites chirurgicales envahies. L’évolution a été marquée par une poursuite évolutive précoce imposant une exérèse chirurgicale plus large suivie d’une chimiothérapie et d’une radiothérapie externe (RT). Le malade était cependant toujours en poursuite évolutive et le décès est survenu dans un contexte d'envahissement médiastinal.Mots-clés : Neurofibromatose type 1, Sarcome, tissus mous, gaines nerveuses périphériques, Chirurgie

    No-wait scheduling of a two-machine flow-shop to minimize the makespan under non-availability constraints and different release dates

    International audienceIn this paper, we consider the two-machine no-wait flow-shop scheduling problem, when every machine is subject to one non-availability constraint and jobs have different release dates. The non-availability intervals of the machines overlap and they are known in advance. We aim to find a non-resumable schedule that minimises the makespan. We propose several lower bounds and upper bounds. These bounding procedures are used in a branch-and-bound algorithm. Computational experiments are carried out on a large set of instances and the obtained results show the effectiveness of our method

    Conductance statistics from a large array of sub-10 nm molecular junctions

    Devices made of few molecules constitute the miniaturization limit that both inorganic and organic-based electronics aspire to reach. However, integration of millions of molecular junctions with less than 100 molecules each has been a long technological challenge requiring well controlled nanometric electrodes. Here we report molecular junctions fabricated on a large array of sub-10 nm single crystal Au nanodots electrodes, a new approach that allows us to measure the conductance of up to a million of junctions in a single conducting Atomic Force Microscope (C-AFM) image. We observe two peaks of conductance for alkylthiol molecules. Tunneling decay constant (beta) for alkanethiols, is in the same range as previous studies. Energy position of molecular orbitals, obtained by transient voltage spectroscopy, varies from peak to peak, in correlation with conductance values.Comment: ACS Nano (in press

    Formability analysis of pre-strained AA5754-O sheet metal using Yld96 plasticity theory: Role of amount and direction of uni-axial pre-strain

    Automotive industries are very much interested in formability of different pre-strained aluminum alloy sheets in the context of multistage stamping to fabricate complex components. In the present work, different uni-axial pre-strains of 6.4% and 12.2% were induced in AA5754-O aluminum alloy both along rolling direction (RD) and transverse direction (TD). The true stress-strain response, limiting dome height (LDH) and strain based forming limit diagram (ε-FLD) of as received and all pre-strained materials were evaluated experimentally. The anisotropy constitutive material model was developed using the Yld96 plasticity theory in-conjunction with the Hollomon isotropic hardening law to predict the yield strength evolution of the pre-strained materials. Also, it was found that the limiting strains in ε-FLD shifted significantly depending on the amount and direction of uni-axial pre-strain. Hence, the limiting strains of the as-received materials were transposed into stress space to estimate the stress based forming limit diagram (σ-FLD) using the anisotropy constitutive material model. Further, the dynamic shifts of ε-FLDs of four different pre-strained materials were predicted by successfully decoupling the σ-FLD of as-received materials within root mean square error of 0.008. Finite element models of both uni-axial pre-straining and subsequent LDH tests were developed, and the forming behavior of the pre-strained materials were predicted implementing the Yld96 plasticity model and estimated σ-FLD. It was found that LDH was significantly influenced by the amount of pre-strain, and the maximum thinning location shifted close to pole in the case of 12.2% pre-strained materials. However, the effect of uni-axial pre-strain direction on both LDH and maximum thinning location in AA5754-O material was very negligible

    Antibacterial activity of Thymoquinone, an active principle of Nigella sativa and its potency to prevent bacterial biofilm formation

    <p>Abstract</p> <p>Background</p> <p>Thymoquinone is an active principle of <it>Nigella sativa </it>seed known as "Habbah Al-Sauda" in Arabic countries and "Sinouj" in Tunisia. Bacterial biofilms tend to exhibit significant tolerance to antimicrobials drugs during infections.</p> <p>Methods</p> <p>The antibacterial activity of Thymoquinone (TQ) and its biofilm inhibition potencies were investigated on 11 human pathogenic bacteria. The growth and development of the biofilm were assessed using the crystal violet (CV) and the 2, 3-bis [2-methyloxy-4-nitro-5-sulfophenyl]-2H-tetrazolium-5-carboxanilide (XTT) reduction assay.</p> <p>Results</p> <p>TQ exhibited a significant bactericidal activity against the majority of the tested bacteria (MICs values ranged from 8 to 32 μg/ml) especially Gram positive cocci (<it>Staphylococcus aureus </it>ATCC 25923 and <it>Staphylococcus epidermidis </it>CIP 106510). Crystal violet assay demonstrated that the minimum biofilm inhibition concentration (BIC50) was reached with 22 and 60 μg/ml for <it>Staphylococcus aureus </it>ATCC 25923 and <it>Staphylococcus epidermidis </it>CIP 106510 respectively. In addition our data revealed that cells oxidative activity was influenced by TQ supplementation. In the same way, TQ prevented cell adhesion to glass slides surface.</p> <p>Conclusion</p> <p>The ability of TQ to prevent biofilm formation warrants further investigation to explore its use as bioactive substances with antibiofilm potential.</p

    Efficacy of the motile sperm organelle morphology examination (MSOME) in predicting pregnancy after intrauterine insemination

    Background: Although the motile sperm organelle morphology examination (MSOME) was developed merely as a selection criterion, its application as a method for classifying sperm morphology may represent an improvement in the evaluation of semen quality. The aim of this study was to determine the prognostic value of normal sperm morphology using MSOME with regard to clinical pregnancy (CP) after intrauterine insemination (IUI).Methods: A total of 156 IUI cycles that were performed in 111 couples were prospectively analysed. Each subject received 75 IU of recombinant FSH every second day from the third day of the cycle. Beginning on the 10th day of the cycle, follicular development was monitored by vaginal ultrasound. When one or two follicles measuring at least 17 mm were observed, recombinant hCG was administered, and IUI was performed 12-14 h and 36-40 h after hCG treatment. Prior to the IUI procedure, sperm samples were analysed by MSOME at 8400x magnification using an inverted microscope that was equipped with DIC/Nomarski differential interference contrast optics. A minimum of 200 motile spermatozoa per semen sample were evaluated, and the percentage of normal spermatozoa in each sample was determined.Results: Pregnancy occurred in 34 IUI cycles (CP rate per cycle: 21.8%, per patient: 30.6%). Based on the MSOME criteria, a significantly higher percentage of normal spermatozoa was found in the group of men in which the IUI cycles resulted in pregnancy (2.6+/-3.1%) compared to the group that did not achieve pregnancy (1.2+/-1.7%; P = 0.019). Logistic regression showed that the percentage of normal cells in the MSOME was a determining factor for the likelihood of clinical pregnancy (OR: 1.28; 95% CI: 1.08 to 1.51; P = 0.003). The ROC curve revealed an area under the curve of 0.63 and an optimum cut-off point of 2% of normal sperm morphology. At this cut-off threshold, using the percentage of normal sperm morphology by MSOME to predict pregnancy was 50% sensitive with a 40% positive predictive value and 79% specificity with an 85% negative predictive value. The efficacy of using the percentage of normal sperm morphology by MSOME in predicting pregnancy was 65%.Conclusions: The present findings support the use of high-magnification microscopy both for selecting spermatozoa and as a routine method for analysing semen before performing IUI

    Perceived risk of infection and death from COVID-19 among community members of low- and middle-income countries: A cross-sectional study [version 1; peer review: awaiting peer review]

    Background: Risk perceptions of coronavirus disease 2019 (COVID-19) are considered important as they impact community health behaviors. The aim of this study was to determine the perceived risk of infection and death due to COVID-19 and to assess the factors associated with such risk perceptions among community members in low- and middle-income countries (LMICs) in Africa, Asia, and South America. Methods: An online cross-sectional study was conducted in 10 LMICs in Africa, Asia, and South America from February to May 2021. A questionnaire was utilized to assess the perceived risk of infection and death from COVID-19 and its plausible determinants. A logistic regression model was used to identify the factors associated with such risk perceptions. Results: A total of 1,646 responses were included in the analysis of the perceived risk of becoming infected and dying from COVID-19. Our data suggested that 36.4% of participants had a high perceived risk of COVID-19 infection, while only 22.4% had a perceived risk of dying from COVID-19. Being a woman, working in healthcare-related sectors, contracting pulmonary disease, knowing people in the immediate social environment who are or have been infected with COVID-19, as well as seeing or reading about individuals infected with COVID-19 on social media or TV were all associated with a higher perceived risk of becoming infected with COVID-19. In addition, being a woman, elderly, having heart disease and pulmonary disease, knowing people in the immediate social environment who are or have been infected with COVID-19, and seeing or reading about individuals infected with COVID-19 on social media or TV had a higher perceived risk of dying from COVID-19. Conclusions: The perceived risk of infection and death due to COVID-19 are relatively low among respondents; this suggests the need to conduct health campaigns to disseminate knowledge and information on the ongoing pandemic

    The E-ELT first light spectrograph HARMONI: capabilities and modes

    Trabajo presentado en SPIE Astronomical Telescopes, celebrado en San Diego (California), del 26 de junio al 1 de julio de 2016HARMONI is the E-ELT's first light visible and near-infrared integral field spectrograph. It will provide four different spatial scales, ranging from coarse spaxels of 60 × 30 mas best suited for seeing limited observations, to 4 mas spaxels that Nyquist sample the diffraction limited point spread function of the E-ELT at near-infrared wavelengths. Each spaxel scale may be combined with eleven spectral settings, that provide a range of spectral resolving powers (R 3500, 7500 and 20000) and instantaneous wavelength coverage spanning the 0.5 - 2.4 ¿m wavelength range of the instrument. In autumn 2015, the HARMONI project started the Preliminary Design Phase, following signature of the contract to design, build, test and commission the instrument, signed between the European Southern Observatory and the UK Science and Technology Facilities Council. Crucially, the contract also includes the preliminary design of the HARMONI Laser Tomographic Adaptive Optics system. The instrument's technical specifications were finalized in the period leading up to contract signature. In this paper, we report on the first activity carried out during preliminary design, defining the baseline architecture for the system, and the trade-off studies leading up to the choice of baseline

    Shiga Toxin 1 Induces on Lipopolysaccharide-Treated Astrocytes the Release of Tumor Necrosis Factor-alpha that Alter Brain-Like Endothelium Integrity

    The hemolytic uremic syndrome (HUS) is characterized by hemolytic anemia, thrombocytopenia and renal dysfunction. The typical form of HUS is generally associated with infections by Gram-negative Shiga toxin (Stx)-producing Escherichia coli (STEC). Endothelial dysfunction induced by Stx is central, but bacterial lipopolysaccharide (LPS) and neutrophils (PMN) contribute to the pathophysiology. Although renal failure is characteristic of this syndrome, neurological complications occur in severe cases and is usually associated with death. Impaired blood-brain barrier (BBB) is associated with damage to cerebral endothelial cells (ECs) that comprise the BBB. Astrocytes (ASTs) are inflammatory cells in the brain and determine the BBB function. ASTs are in close proximity to ECs, hence the study of the effects of Stx1 and LPS on ASTs, and the influence of their response on ECs is essential. We have previously demonstrated that Stx1 and LPS induced activation of rat ASTs and the release of inflammatory factors such as TNF-α, nitric oxide and chemokines. Here, we demonstrate that rat ASTs-derived factors alter permeability of ECs with brain properties (HUVECd); suggesting that functional properties of BBB could also be affected. Additionally, these factors activate HUVECd and render them into a proagregant state promoting PMN and platelets adhesion. Moreover, these effects were dependent on ASTs secreted-TNF-α. Stx1 and LPS-induced ASTs response could influence brain ECs integrity and BBB function once Stx and factors associated to the STEC infection reach the brain parenchyma and therefore contribute to the development of the neuropathology observed in HUS
