    Qualitative Exploration of the Suitability of Capability Based Instruments to Measure Quality of Life in Family Carers of People with Dementia

    Background. In an ageing population, many individuals find themselves becoming a carer for an elderly relative. This qualitative study explores aspects of quality of life affected by caring for a person with dementia, with the aim of identifying whether capability based questionnaires are suitable for measuring carer quality of life. Methods. Semistructured interviews lasting up to an hour were conducted, November 2010-July 2011, with eight family carers of people with dementia. Interviews typically took place at the participants' homes and were recorded and transcribed verbatim. Framework analysis was used to code and analyse data. Domains from three capability based questionnaires (ICECAP-O, Carer Experience Scale, and ASCOT) were used as initial codes. Similar codes were grouped into categories, and broader themes were developed from these categories. Results. Four themes were identified: social network and relationships; interactions with agencies; recognition of role; and time for oneself. Conclusions. By identifying what affects carers' quality of life, an appropriate choice can be made when selecting instruments for future carer research. The themes identified had a high degree of overlap with the capability instruments, suggesting that the capabilities approach would be suitable for future research involving carers of people with dementia

    Health economics research into supporting carers of people with dementia: A systematic review of outcome measures

    Advisory bodies, such as the National Institute for Health and Clinical Excellence (NICE) in the UK, advocate using preference based instruments to measure the quality of life (QoL) component of the quality-adjusted life year (QALY). Cost per QALY is used to determine cost-effectiveness, and hence funding, of interventions. QALYs allow policy makers to compare the effects of different interventions across different patient groups. Generic measures may not be sensitive enough to fully capture the QoL effects for certain populations, such as carers, so there is a need to consider additional outcome measures, which are preference based where possible to enable cost-effectiveness analysis to be undertaken. This paper reviews outcome measures commonly used in health services research and health economics research involving carers of people with dementia. An electronic database search was conducted in PubMed, Medline, the Cumulative Index to Nursing and Allied Health Literature (CINAHL), PsycINFO, the National Health Service Economic Evaluation Database (NHS EED), Database of Abstracts of Reviews of Effects (DARE) and Health Technology Assessment database. Studies were eligible for inclusion if they included an outcome measure for carers of people with dementia. 2262 articles were identified. 455 articles describing 361 studies remained after exclusion criteria were applied. 228 outcome measures were extracted from the studies. Measures were categorised into 44 burden measures, 43 mastery measures, 61 mood measures, 32 QoL measures, 27 social support and relationships measures and 21 staff competency and morale measures. The choice of instrument has implications on funding decisions; therefore, researchers need to choose appropriate instruments for the population being measured and the type of intervention undertaken. If an instrument is not sensitive enough to detect changes in certain populations, the effect of an intervention may be underestimated, and hence interventions which may appear to be beneficial to participants are not deemed cost-effective and are not funded. If this is the case, it is essential that additional outcome measures which detect changes in broader QoL are included, whilst still retaining preference based utility measures such as EQ-5D to allow QALY calculation for comparability with other interventions

    Designing a mobile augmented memory system for people with traumatic brain injuries

    Augmented memory systems help people remember events in their lives. Individuals with Traumatic Brain Injury (TBI) often have memory impairments. We conducted a user study to learn about strategies individuals with TBI use to remember events in their lives. We explored what characteristics individuals with TBI expect of an augmented memory system. We then investigated these aspects in an initial mobile app design, and propose here a concept for a rehearsal application that addresses the issues found in our studies

    ISOLOK VALVE ACCEPTANCE TESTING FOR DWPF SME SAMPLING PROCESS

    Evaluation of the Defense Waste Processing Facility (DWPF) Chemical Process Cell (CPC) cycle time identified several opportunities to improve the CPC processing time. Of the opportunities, a focus area related to optimizing the equipment and efficiency of the sample turnaround time for DWPF Analytical Laboratory was identified. The Mechanical Systems & Custom Equipment Development (MS&CED) Section of the Savannah River National Laboratory (SRNL) evaluated the possibility of using an Isolok{reg_sign} sampling valve as an alternative to the Hydragard{reg_sign} valve for taking process samples. Previous viability testing was conducted with favorable results using the Isolok sampler and reported in SRNL-STI-2010-00749 (1). This task has the potential to improve operability, reduce maintenance time and decrease CPC cycle time. This report summarizes the results from acceptance testing which was requested in Task Technical Request (TTR) HLW-DWPF-TTR-2010-0036 (2) and which was conducted as outlined in Task Technical and Quality Assurance Plan (TTQAP) SRNL-RP-2011-00145 (3). The Isolok to be tested is the same model which was tested, qualified, and installed in the Sludge Receipt Adjustment Tank (SRAT) sample system. RW-0333P QA requirements apply to this task. This task was to qualify the Isolok sampler for use in the DWPF Slurry Mix Evaporator (SME) sampling process. The Hydragard, which is the current baseline sampling method, was used for comparison to the Isolok sampling data. The Isolok sampler is an air powered grab sampler used to 'pull' a sample volume from a process line. The operation of the sampler is shown in Figure 1. The image on the left shows the Isolok's spool extended into the process line and the image on the right shows the sampler retracted and then dispensing the liquid into the sampling container. To determine tank homogeneity, a Coliwasa sampler was used to grab samples at a high and low location within the mixing tank. Data from the two locations were compared to determine if the contents of the tank were well mixed. The Coliwasa sampler is a tube with a stopper at the bottom and is designed to obtain grab samples from specific locations within the drum contents. A position paper (4) was issued to address the prototypic flow loop issues and simulant selections. A statistically designed plan (5) was issued to address the total number of samples each sampler needed to pull, to provide the random order in which samples were pulled and to group samples for elemental analysis. The TTR required that the Isolok sampler perform as well as the Hydragard sampler during these tests to ensure the acceptability of the Isolok sampler for use in the DWPF sampling cells.     The Fastest Relativistic Jets: VLBA Observations of Blazars with Apparent Speeds Exceeding 25c

    We have measured peak apparent speeds of 25.6c+/-7.0c, 25.6c+/-4.4c, and 28.2c+/-6.6c in the jets of 0235+164, 0827+243, and 1406-076, respectively, based on six epochs of high-sensitivity VLBA observations at 22 and 43 GHz during 2002 and 2003 (H0=71 km s-1 Mpc-1, Ωm=0.27, and ΩΛ=0.73). These blazars had been identified as potentially having apparent speeds exceeding 40c in an earlier VLBA survey of EGRET blazars by Jorstad and coworkers. We therefore confirm (with high confidence in 0827+243, and lower confidence in 0235+164 and 1406-076) the presence of highly relativistic pattern speeds in these three jets, although not at the \u3e40c levels reported by Jorstad and coworkers. The lower limit to the bulk Lorentz factor implied by the observed apparent speeds is Γ\u3e~25-30 in these three sources, if the pattern speeds are equal to or slower than the bulk flow speed

    Local adaptation constrains drought tolerance in a tropical foundation tree

    Plant species with broad climatic ranges might be more vulnerable to climate change than previously appreciated due to intraspecific variation in climatic stress tolerance. In tropical forests, drought is increasingly frequent and severe, causing widespread declines and altering community dynamics. Yet, little is known about whether foundation tropical trees vary in drought tolerance throughout their distributions, and how intraspecific variation in drought tolerance might contribute to their vulnerability to climate changE. We tested for local adaptation in seedling emergence and establishment with a full-factorial reciprocal transplant experiment including 27 populations and 109,350 seeds along a 3,500 mm precipitation gradient for a widespread tropical foundation tree, Metrosideros polymorpha, in Hawaii. To more precisely relate seedling performance to soil moisture, we conducted a complementary greenhouse experiment to test responses of the same focal populations to simulated drought. In the reciprocal transplant experiment, we observed significant variation among populations and sites in germination and seedling establishment rates. Overall, there was a significant link between historical rainfall of populations and their performance under current rainfall at the study sites consistent with local adaptation. In particular, populations from historically wet sites demonstrated lower germination rates in currently dry sites compared to wet field sites, while populations from historically dry sites germinated well across all sites, with particularly high germination in dry sites. In the greenhouse, seedlings from wet populations survived fewer days without water, and succumbed at wetter soil conditions than populations from historically dry sites, corroborating results from the field experiment. Synthesis. While climate change models project the greatest drying trends for historically dry areas in Hawaii, even moderate drying of wet sites could significantly reduce Metrosideros polymorpha recruitment given the sensitivity of seedlings to very slight changes in water regimes. Thus, although M. polymorpha demonstrates high seedling drought tolerance in some populations, providing evidence of resilience at the species-scale, there are nonetheless vulnerable populations that will likely decline under climate change. Our approach demonstrates that even trees with high dispersal abilities can show significant clines in drought tolerance, and suggests that similar intraspecific variation might be an important consideration for other tropical foundational tree species

    The salience of sport in cross-race friendship selection

    In response to growing racial concerns, stakeholders have called for programs that facilitate positive cross-race interactions. Adolescence has been targeted as the most effective life stage to initiate programs promoting relationships. Since schools are one of the primary social influences on adolescent development, they have been emphasized as a particularly important setting for promoting intergroup contact. Sport plays an important role in the experiences of students, and contributes to intergroup relations within a school. The purpose of this study is to examine the unique effect of sport participation on cross-race friendship selection. Using data from Wave I of The National Longitudinal Study of Adolescent Health (Add Health), mixed-effects regression models and analysis of variance (ANOVA) were used to assess the effects of sport participation on friendship heterogeneity. Overall, participation in sport activities did not significantly influence the racial heterogeneity of adolescent’s friendship networks

    Seismic data reveal eastern Black Sea Basin structure

    Rifted continental margins are formed by progressive extension of the lithosphere. The development of these margins plays an integral role in the plate tectonic cycle, and an understanding of the extensional process underpins much hydrocarbon exploration. A key issue is whether the lithosphere extends uniformly, or whether extension varies\ud with depth. Crustal extension may be determined using seismic techniques. Lithospheric extension may be inferred from the waterloaded subsidence history, determined from\ud the pattern of sedimentation during and after rifting. Unfortunately, however, many rifted margins are sediment-starved, so the subsidence history is poorly known.\ud To test whether extension varies between the crust and the mantle, a major seismic experiment was conducted in February–March 2005 in the eastern Black Sea Basin (Figure 1), a deep basin where the subsidence history is recorded\ud by a thick, post-rift sedimentary sequence. The seismic data from the experiment indicate the presence of a thick, low-velocity zone, possibly representing overpressured sediments. They also indicate that the basement and\ud Moho in the center of the basin are both several kilometers shallower than previously inferred. These initial observations may have considerable impact on thermal models of the petroleum system in the basin. Understanding\ud the thermal history of potential source rocks is key to reducing hydrocarbon exploration risk. The experiment, which involved collaboration between university groups in the United Kingdom, Ireland, and Turkey, and BP and\ud Turkish Petroleum (TPAO), formed part of a larger project that also is using deep seismic reflection and other geophysical data held by the industry partners to determine the subsidence history and hence the strain evolution of\ud the basin

    Identifying wildlife observations on twitter

    Despite the potential of social media for environmental monitoring, concerns remain about the quality and reliability of the information automatically extracted. Notably there are many observations of wildlife on Twitter, but their automated detection is a challenge due to the frequent use of wildlife related words in messages that have no connection with wildlife observation. We investigate whether and what type of supervised machine learning methods can be used to create a fully automated text classification model to identify genuine wildlife observations on Twitter, irrespective of species type or whether Tweets are geo-tagged. We perform experiments with various techniques for building feature vectors that serve as input to the classifiers, and consider how they affect classification performance. We compare three classification approaches and perform an analysis of the types of features that are indicative for genuine wildlife observations on Twitter. In particular, we compare some classical machine learning algorithms, widely used in ecology studies, with state-of-the-art neural network models. Results showed that the neural network-based model Bidirectional Encoder Representations from Transformers (BERT) outperformed the classical methods. Notably this was the case for a relatively small training corpus, consisting of less than 3000 instances. This reflects that fact that the BERT classifier uses a transfer learning approach that benefits from prior learning on a very much larger collection of generic text. BERT performed particularly well even for Tweets that employed specialised language relating to wildlife observations. The analysis of possible indicative features for wildlife Tweets revealed interesting trends in the usage of hashtags that are unrelated to official citizen science campaigns. The findings from this study facilitate more accurate identification of wildlife-related data on social media which can in turn be used for enriching citizen science data collections

    Ellipticity and Deviations from Orthogonality in the Polarization Modes of PSR B0329+54

    We report on an analysis of the polarization of single pulses of PSR B0329+54 at 328 MHz. We find that the distribution of polarization orientations in the central component diverges strongly from the standard picture of orthogonal polarization modes (OPMs), making a remarkable partial annulus on the Poincare sphere. A second, tightly clustered region of density appears in the opposite hemisphere, at a point antipodal to the centre of the annulus. We argue that this can be understood in terms of birefringent alterations in the relative phase of two elliptically polarized propagation modes in the pulsar magnetosphere (i.e. generalised Faraday rotation). The ellipticity of the modes implies a significant charge density in the plasma, while the presence of both senses of circular polarization, and the fact that only one mode shows the effect, supports the view that refracted ordinary-mode rays are involved in the production of the annulus. At other pulse longitudes the polarization (including the circular component) is broadly consistent with an origin in elliptical OPMs, shown here quantitatively for the first time, however considerable non-orthogonal contributions serve to broaden the orientation distribution in an isotropic manner.Comment: 13 pages, 5 figures, to appear in A&
