Primary adrenal insufficiency in adults: 150 years after Addison
Thomas Addison first described, 150 years ago, a clinical syndrome characterized by salt-wasting and skin hyperpigmentation, associated with a destruction of the adrenal gland. Even today, over a century after Addison's report, primary adrenal insufficiency can present as a life-threatening condition, since it frequently goes unrecognized in its early stages. In the 1850 s, tuberculous adrenalitis was present in the majority of patients, but nowadays, autoimmune Addison's disease is the most common cause of primary adrenal insufficiency. In the present report, we show the prevalence of different etiologies, clinical manifestations and laboratorial findings, including the adrenal cortex autoantibody, and 21-hydroxylase antibody in a Brazilian series of patients with primary adrenal insufficiency followed at Divisão de Endocrinologia da Universidade Federal de São Paulo (UNIFESP) and at Faculdade de Medicina de Ribeirão Preto - USP (FMRP-USP).Thomas Addison descreveu pela primeira vez, há 150 anos, uma síndrome clínica de perda de sal em indivíduos com hiperpigmentação cutânea, associada à destruição da glândula adrenal. A adrenalite tuberculosa era a causa mais freqüente na maioria dos casos descritos inicialmente, mas, na atualidade, a doença de Addison auto-imune está presente em uma grande porcentagem de pacientes com insuficiência adrenal primária. Dynamical models for sand ripples beneath surface waves
We introduce order parameter models for describing the dynamics of sand
ripple patterns under oscillatory flow. A crucial ingredient of these models is
the mass transport between adjacent ripples, which we obtain from detailed
numerical simulations for a range of ripple sizes. Using this mass transport
function, our models predict the existence of a stable band of wavenumbers
limited by secondary instabilities. Small ripples coarsen in our models and
this process leads to a sharply selected final wavenumber, in agreement with

Uncertainty in the Representation of Orography in Weather and Climate Models and Implications for Parameterized Drag
The representation of orographic drag remains a major source of uncertainty for numerical weather prediction (NWP) and climate models. Its accuracy depends on contributions from both the model grid‐scale orography (GSO) and the subgrid‐scale orography (SSO). Different models use different source orography datasets and different methodologies to derive these orography fields. This study presents the first comparison of orography fields across several operational global NWP models. It also investigates the sensitivity of an orographic drag parameterisation to the inter‐model spread in SSO fields and the resulting implications for representing the northern hemisphere winter circulation in a NWP model. The inter‐model spread in both the GSO and the SSO fields is found to be considerable. This is due to differences in the underlying source dataset employed and in the manner in which this dataset is processed (in particular how it is smoothed and interpolated) to generate the model fields. The sensitivity of parameterised orographic drag to the inter‐model variability in SSO fields is shown to be considerable and dominated by the influence of two SSO fields: the standard deviation and the mean gradient of the SSO. NWP model sensitivity experiments demonstrate that the inter‐model spread in these fields is of first‐order importance to the inter‐model spread in parameterised surface stress, and to current known systematic model biases. The revealed importance of the SSO fields supports careful reconsideration of how these fields are generated, guiding future development of orographic drag parameterisations and re‐evaluation of the resolved impacts of orography on the flow
Clinical, microbiological, and genomic characteristics of clade-III Candida auris colonization and infection in southern California, 2019-2022.
BackgroundCandida auris is an emerging fungal pathogen causing outbreaks in healthcare facilities. Five distinctive genomic clades exhibit clade-unique characteristics, highlighting the importance of real-time genomic surveillance and incorporating genotypic information to inform infection prevention practices and treatment algorithms.MethodsBoth active and passive surveillance were used to screen hospitalized patients. C. auris polymerase chain reaction (PCR) assay on inguinal-axillary swabs was performed on high-risk patients upon admission. All clinical yeast isolates were identified to the species level. C. auris isolates were characterized by both phenotypic antifungal susceptibility tests and whole-genome sequencing.ResultsFrom late 2019 to early 2022, we identified 45 patients with C. auris. Most had a tracheostomy or were from a facility with a known outbreak. Moreover, 7 patients (15%) were only identified through passive surveillance. Also, 8 (18%) of the patients had a history of severe COVID-19. The overall mortality was 18%. Invasive C. auris infections were identified in 13 patients (29%), 9 (69%) of whom had bloodstream infections. Patients with invasive infection were more likely to have a central line. All C. auris isolates were resistant to fluconazole but susceptible to echinocandins. Genomic analysis showed that 1 dominant clade-III lineage is circulating in Los Angeles, with very limited intrahost and interhost genetic diversity.ConclusionsWe have demonstrated that a robust C. auris surveillance program can be established using both active and passive surveillance, with multidisciplinary efforts involving the microbiology laboratory and the hospital epidemiology team. In Los Angeles County, C. auris strains are highly related and echinocandins should be used for empiric therapy

Carrier dynamics investigated by time resolved optical spectroscopy
We have investigated the transport of carriers in GaAs using time resolved optical spectroscopy with picosecond resolution. Carriers are optically created to the sample surface by an ultra-fast laser pulse. They diffuse and drift throught a thick GaAs layer, until they are captured by an InGaAs quantum well, where they recombine with holes from a p-type doped layer at an inner InGaP barrier. Our study was performed with a set of samples with different GaAs layer thickness. As the GaAs thickness increases, the emission from the quantum well is delayed and its decay slows down significantly. We have investigated the effect of an applied DC field between the surface and the InGaAs quantum well. The transient of the quantum well emission is mostly independent of the applied DC voltage up to field of the order of 20 KV/cm, including both polarities. This is a clear indication that the carrier transport is dominated by ambipolar diffusion due to the Coulomb interaction that strongly couples photoinjected electrons and holes. On the other hand, the decay of the GaAs emission varies signi-cantly when a DC gate voltage is applied such as a current appears at the structure.35335
Gonadotropin-dependent pubertal disorders are common in patients with virilizing adrenocortical tumors in childhood
Objective: To investigate the impact of early exposure to androgen excess on gonadotropin-dependent puberty (GDP) and final height (FH) of patients with androgen-secreting adrenocortical tumors (ACT) in childhood.
Methods: Retrospective cohort study. Occurrence of GDP and achievement of FH were evaluated. Central precocious puberty (CPP) and early fast puberty (EFP) were considered pubertal disorders. Patients with normal puberty and pubertal disorders were compared.
Results: The study included 63 patients (44F), followed in a single institution from 1975 until 2017. At diagnosis of ACT, median age was 25.8 month s; duration of signs, 6 months; stature SDS, 0.5 (−3.6 to 3.9) and bone age advancement, 14.7 months (−27.9 to 85.4). To date, 37 patients developed GDP: 26 had normal puberty; one, precocious thelarche; seven, CPP and three, EFP. GnRHa effectively treated CPP/EFP. Tall stature and older age at diagnosis of ACT were associated with risk of CPP alone (RR 4.17 (95% CI 1.17–14.80)) and CPP/EFP (RR 3.0 (95% CI 1.04–8.65)). Recurrence/metastasis during follow-up were associated with risk of CPP alone (RR 4.17 (95% CI 1.17–14.80)) and CPP/EFP (RR 3.0 (95% CI 1.12–8.02)). Among the 19 patients that reached FH, stature SDS dropped from 1.4 to −0.02 since diagnosis of ACT (P = 0.01). Seventeen achieved normal FH. There was no difference in FH SDS between patients with norm al puberty and pubertal disorders (P = 0.75).
Conclusions: Gonadotropin-dependent pubertal disorders are common in patients with androgen-secreting ACT in childhood. FH is usually not impaired. The study reinforces the importance of close follow-up after surgery to identify and treat consequences of early exposure to androgen excess
