    The Highest Geomagnetic Storms of the Solar Cycle Observed at Ground Level

    We report two ground-level observations, of geomagnetic storms of different origins; they are among the highest geomagnetic storms, in the solar Cycle 24. The first is St. Patrick’s Day storm on March 17, 2015, originated by the impact on Earth’s atmosphere of coronal mass ejections (CMEs), the storm reaching the condition of G4 (severe) level, in the NOAA geomagnetic scale. The second included the major geomagnetic storm whose origin is attributed to the interaction with the Earth of a High-Speed Stream (HSS) ahead of a positive polarity coronal hole on October 7, 2015. This storm reached the condition G3 (strong) level. We give emphasis to observations detected by the New-Tupi muon telescopes, located at sea level in Brazil (22.53° S, 43.13° W). We present a study of these observations in correlation with observations reported by multipoint space-based measurements, such as the ACE at Lagrange Point L1 and the geostationary GOES weather satellite, including two global geomagnetic indices and several ground-based detectors.     New endoscopic procedure for bladder wall closure: results from the porcine model

    Upper urinary tract urothelial carcinomas are usually managed by radical nephroureterectomy (RNU), often followed by intravesical chemotherapy to minimize recurrence. Open surgery is the gold standard procedure for RNU, but it associates with high morbidity, and it has been increasingly replaced by minimally invasive strategies, such as laparoscopy and endoscopy. Although effective, endoscopic ureteral excision leaves the bladder unsutured, increasing the risk of tumor spillage, and precluding the immediate administration of intravesical chemotherapy. Here we describe a new method to close the bladder wall after ureteral excision, using barbed sutures via the endoscopic access. Our results in 8 female pigs demonstrate that this method is effective to close the bladder wall. The procedure was completed in a median time of 24 min, and no adverse events were registered in the follow-up or at the three-week necropsy. This technique improves a previous approach described by our group because the device is more flexible and allows to tie the knots inside the bladder. Barbed sutures have been used in the clinical practice for other types of surgeries, and therefore this method can further be adapted to human patients with no safety concerns.     Influence of Activation Parameters on the Mechanical and Microstructure Properties of an Alkali-Activated BOF Steel Slag

    ABSTRACT: Steel slag (SS) is a secondary material from steelmaking production with little commercial value. Its volumetric expansion and low reactivity limit the use of SS in Portland cement (PC)- based materials. This study investigated the potential use of basic oxygen furnace (BOF) slag as a single precursor in alkali-activated matrices (AAMs). Six AAM pastes were assessed by changing the silica modulus (0.75, 1.50 and 2.22) and the sodium concentration (4% or 6% Na2O?wt. SS). The early hydration was assessed using isothermal calorimetry (IC), followed by the assessment of the mechanical performance (compressive strength), apparent porosity, and structure and microstructure characterization (X-ray diffraction, thermogravimetric analysis and scanning electron microscopy). The results indicated that although the BOF slag may be considered a low-reactivity material, the alkaline environment effectively dissolved important crystalline phases to produce hydrates (reaction products). An optimized combination of activator sources was achieved with 4% Na2O and a silica modulus of 1.50?2.22, with a compressive strength up to 20 MPa, a significant amount of reaction products (C-S-H/C-A-S-H gels), and low initial and cumulative heat release.     Estimativa dos coeficientes especĂ­ficos de mortalidade infantil segundo peso ao nascer no municĂ­pio de SĂŁo Paulo (Brasil)

    Foi estimada a distribuição do peso ao nascer da coorte dos recém-nascidos que deu origem aos óbitos infantis estudados pela Investigação Interamericana de Mortalidade na Infùncia no projeto de São Paulo (1968-70) e determinados os riscos de óbito associados a cada intervalo de peso de nascimento. Assim, foram apurados coeficientes de mortalidade infantil de 305,5, 50,2 e 34,4 para recém-nascidos de baixo peso, peso deficiente e peso superior a 3.000 g. A comparação destes coeficientes com os registrados na årea americana incluída na investigação citada (Califórnia), revelou excesso de mortalidade, particularmente notåvel não para os recém-nascidos de baixo peso, mas para os recém-nascidos de peso superior a 3.000 g. O ajuste da mortalidade de São Paulo (Brasil) à distribuição do peso ao nascer observada na Califórnia foi capaz de explicar 15% do excesso da mortalidade infantil e 21% do excesso da mortalidade neonatal de São Paulo.By means of the birth-rate distribution found to exist during the Inter-American Investigation of Mortality in Childhood in S. Paulo (1968-70) the infant death rate association with each birth-weight interval was estimated. The infant mortality rates were 305.5, 50.2 and 34.4 respectively for low birth-weight, deficient weight and weight above 3,000 grams. Compared with the rates found in California, USA, by the same Investigation, the infant mortality rates in S. Paulo are higher, particularly for mortality associated with birth-weight of above 3,000 grams. The adjustment of infant mortality rates in S. Paulo according to the birth-weight distribution found in California led to the conclusion that, at that period, only 15% of the excess of the infant mortality of S. Paulo could be attributed exclusively to birth-weight. In the case of neonatal mortality, 21% of the mortality excess in S. Paulo could be attributed to birth-weight

    Acute left sided colonic diverticulitis is one of the most common clinical conditions encountered by surgeons in acute setting. A World Society of Emergency Surgery (WSES) Consensus Conference on acute diverticulitis was held during the 3rd World Congress of the WSES in Jerusalem, Israel, on July 7th, 2015. During this consensus conference the guidelines for the management of acute left sided colonic diverticulitis in the emergency setting were presented and discussed. This document represents the executive summary of the final guidelines approved by the consensus conference.1

    Randomized elimination and prolongation of ACE inhibitors and ARBs in coronavirus 2019 (REPLACE COVID) Trial Protocol

    Severe acute respiratory syndrome coronavirus 2 (SARS- CoV- 2), the virus responsible for coronavirus disease 2019 (COVID- 19), is associated with high incidence of multiorgan dysfunction and death. Angiotensin- converting enzyme 2 (ACE2), which facilitates SARS- CoV- 2 host cell entry, may be impacted by angiotensin- converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs), two commonly used antihypertensive classes. In a multicenter, international randomized controlled trial that began enrollment on March 31, 2020, participants are randomized to continuation vs withdrawal of their long- term outpatient ACEI or ARB upon hospitalization with COVID- 19. The primary outcome is a hierarchical global rank score incorporating time to death, duration of mechanical ventilation, duration of renal replacement or vasopressor therapy, and multiorgan dysfunction severity. Approval for the study has been obtained from the Institutional Review Board of each participating institution, and all participants will provide informed consent.
