    Fins de vie en réanimation et religions monothéistes

    Le 22 avril 2005, l Assemblée Nationale votait à l unanimité la loi n 2005-370 relative aux droits des malades et à la fin de vie. Elle faisait suite aux débats provoqués par l affaire Vincent Humbert, jeune homme devenu tétraplégique à la suite d un accident de voiture et qui s était vu refuser un droit de mourir. Cependant le contexte n était pas suffisamment propice à l élaboration d une nouvelle loi sur la problématique des fins de vie et la difficile question d une éventuelle légalisation de l euthanasie. Dans un esprit de modération, ce texte a pu voir le jour grâce notamment au recul pris par M. Jean Leonetti et les parlementaires du groupe de travail, de manière non partisane. Pour autant, une mission d évaluation de cette loi, déclenchée suite à une autre affaire fortement médiatisée en 2008, a pointé du doigt la méconnaissance de la loi par les professionnels de santé et l insuffisance de l offre de soins dans le domaine des soins palliatifs.Dans un premier temps, nous nous sommes attachés à replacer le débat dans son contexte : définitions utilisées et historique de la législation française. Par la suite, nous avons cherché à évaluer cette loi au travers du prisme de la religion. Nous avons auditionné quatre personnalités issues de sphères religieuses différentes et dont la compétence en la matière est reconnue. Après avoir explicité les grands concepts de ce débat au regard des Ecritures Saintes, chacun de nos intervenants a été invité à se prononcer sur les avancées ou les carences de la loi du 22 avril 2005, sur les conditions de sa promulgation, sur les arguments des militants en faveur de l euthanasie. Enfin, nous leur avons soumis un cas clinique identique, représentatif d une fin de vie en réanimation, tiré des communiqués de presse du Vatican dans les derniers instants de la vie du Pape Jean-Paul II. Il va sans dire que toutes les religions monothéistes condamnent fermement l euthanasie et abondent dans le sens du texte de loi. Pour autant, la réflexion qui mène à ces opinions est variable. Il existe des points de convergences solides entre le judaïsme, le catholicisme, et l islam en raison de leurs sources écrites communes. En revanche, le protestantisme se démarque par une approche plus humaniste dans ce débat. Il se refuse à chercher des réponses modernes dans des textes anciens et assume l hétérogénéité des avis de ses coreligionnaires. La place est faite au raisonnement de l Homme qui reste seul juge de ce qui est bon pour lui et pour la société qui l entoure, raisonnement proche de la philosophie. Ainsi la demande d euthanasie ne doit pas être méprisée : elle doit être entendue et analysée sans pour autant que l on y accède. Enfin, le traitement, trop souvent simpliste et superficiel de cette problématique par les médias français, abonde dans le sens d associations militantes en faveur de l euthanasie, entretenant une confusion sémantique propice à la manipulation des émotions. Pour conclure, nous avons cherché à travers ce travail à comprendre comment cette loi était perçue par des hommes de foi plutôt que de nous faire l écho de l avis officiel de chacune de ces religions. A cette fin, notre exposé s est fondé sur la transcription des interventions que nous avons auditionnées. Il ne prétend pas apporter de réponses définitives à ce problème si sensible qu est la fin de vie, toutefois nous espérons que des professionnels de santé confrontés à des cas litigieux, pourront y trouver des outils qui les aideront à gérer la fin de vie de patients qui demanderaient que leur religion soit prise en compte.PARIS6-Bibl.Pitié-Salpêtrie (751132101) / SudocSudocFranceF

    Analysis of the medical response to November 2015 Paris terrorist attacks: resource utilization according to the cause of injury

    International audiencePURPOSE: The majority of terrorist acts are carried out by explosion or shooting. The objective of this study was first, to describe the management implemented to treat a large number of casualties and their flow together with the injuries observed, and second, to compare these resources according to the mechanism of trauma.METHODS: This retrospective cohort study collected medical data from all casualties of the attacks on November 13th 2015 in Paris, France, with physical injuries, who arrived alive at any hospital within the first 24 h after the events. Casualties were divided into two groups: explosion injuries and gunshot wounds.RESULTS: 337 casualties were admitted to hospital, 286 (85%) from gunshot wounds and 51 (15%) from explosions. Gunshot casualties had more severe injuries and required more in-hospital resources than explosion casualties. Emergency surgery was required in 181 (54%) casualties and was more frequent for gunshot wounds than explosion injuries (57% vs. 35%, p < 0·01). The types of main surgery needed and their delay following hospital admission were as follows: orthopedic [n = 107 (57%); median 744 min]; general [n = 27 (15%); 90 min]; vascular [n = 19 (10%); median 53 min]; thoracic [n = 19 (10%); 646 min]; and neurosurgery [n = 4 (2%); 198 min].CONCLUSION: The resources required to deal with a terrorist attack vary according to the mechanism of trauma. Our study provides a template to estimate the proportion of various types of surgical resources needed overall, as well as their time frame in a terrorist multisite and multitype attack

    Blood product needs and transfusion timelines for the multisite massive Paris 2015 terrorist attack: A retrospective analysis

    International audienceObjective: Hemorrhage is the leading cause of death after terrorist attack, and the immediacy of labile blood product (LBP) administration has a decisive impact on patients' outcome. The main objective of this study was to evaluate the transfusion patterns of the Paris terrorist attack victims, November 13, 2015.Methods: We performed a retrospective analysis including all casualties admitted to hospital, aiming to describe the transfusion patterns from admission to the first week after the attack.Results: Sixty-eight of 337 admitted patients were transfused. More than three quarters of blood products were consumed in the initial phase (until November 14, 11:59 PM), where 282 packed red blood cell (pRBC) units were transfused along with 201 plasma and 25 platelet units, to 55 patients (16% of casualties). Almost 40% of these LBPs (134 pRBC, 73 plasma, 8 platelet units) were transfused within the first 6 hours after the attack. These early transfusions were massive transfusion (MT) for 20 (6%) of 337 patients, and the average plasma/red blood cell ratio was 0.8 for MT patients who received 366 (72%) of 508 LBPs.The median time from admission to pRBC transfusion was 57 (25-108) minutes and 208 (52-430) minutes for MT and non-MT patients, respectively. These same time intervals were 119 (66-202) minutes and 222 (87-381) minutes for plasma and 225 (131-289) minutes and 198 (167-230) minutes for platelets.Conclusion: Our data suggest that improving transfusion procedures in mass casualty setting should rely more on shortening the time to bring LBP to the bedside than in increasing the stockpile

    Predicting 90-day survival of patients with COVID-19: Survival of Severely Ill COVID (SOSIC) scores

    International audienceBackground Predicting outcomes of critically ill intensive care unit (ICU) patients with coronavirus-19 disease (COVID-19) is a major challenge to avoid futile, and prolonged ICU stays. Methods The objective was to develop predictive survival models for patients with COVID-19 after 1-to-2 weeks in ICU. Based on the COVID–ICU cohort, which prospectively collected characteristics, management, and outcomes of critically ill patients with COVID-19. Machine learning was used to develop dynamic, clinically useful models able to predict 90-day mortality using ICU data collected on day (D) 1, D7 or D14. Results Survival of Severely Ill COVID (SOSIC)-1, SOSIC-7, and SOSIC-14 scores were constructed with 4244, 2877, and 1349 patients, respectively, randomly assigned to development or test datasets. The three models selected 15 ICU-entry variables recorded on D1, D7, or D14. Cardiovascular, renal, and pulmonary functions on prediction D7 or D14 were among the most heavily weighted inputs for both models. For the test dataset, SOSIC-7’s area under the ROC curve was slightly higher (0.80 [0.74–0.86]) than those for SOSIC-1 (0.76 [0.71–0.81]) and SOSIC-14 (0.76 [0.68–0.83]). Similarly, SOSIC-1 and SOSIC-7 had excellent calibration curves, with similar Brier scores for the three models. Conclusion The SOSIC scores showed that entering 15 to 27 baseline and dynamic clinical parameters into an automatable XGBoost algorithm can potentially accurately predict the likely 90-day mortality post-ICU admission (sosic.shinyapps.io/shiny). Although external SOSIC-score validation is still needed, it is an additional tool to strengthen decisions about life-sustaining treatments and informing family members of likely prognosis

    Characteristics, management, and prognosis of elderly patients with COVID-19 admitted in the ICU during the first wave: insights from the COVID-ICU study

    International audienceBackground: The COVID-19 pandemic is a heavy burden in terms of health care resources. Future decision-making policies require consistent data on the management and prognosis of the older patients (&gt; 70 years old) with COVID-19 admitted in the intensive care unit (ICU). Methods: Characteristics, management, and prognosis of critically ill old patients (&gt; 70 years) were extracted from the international prospective COVID-ICU database. A propensity score weighted-comparison evaluated the impact of intubation upon admission on Day-90 mortality. Results: The analysis included 1199 (28% of the COVID-ICU cohort) patients (median [interquartile] age 74 [72–78] years). Fifty-three percent, 31%, and 16% were 70–74, 75–79, and over 80 years old, respectively. The most frequent comorbidities were chronic hypertension (62%), diabetes (30%), and chronic respiratory disease (25%). Median Clinical Frailty Scale was 3 (2–3). Upon admission, the PaO2/FiO2 ratio was 154 (105–222). 740 (62%) patients were intubated on Day-1 and eventually 938 (78%) during their ICU stay. Overall Day-90 mortality was 46% and reached 67% among the 193 patients over 80 years old. Mortality was higher in older patients, diabetics, and those with a lower PaO2/FiO2 ratio upon admission, cardiovascular dysfunction, and a shorter time between first symptoms and ICU admission. In propensity analysis, early intubation at ICU admission was associated with a significantly higher Day-90 mortality (42% vs 28%; hazard ratio 1.68; 95% CI 1.24–2.27; p &lt; 0·001). Conclusion: Patients over 70 years old represented more than a quarter of the COVID-19 population admitted in the participating ICUs during the first wave. Day-90 mortality was 46%, with dismal outcomes reported for patients older than 80 years or those intubated upon ICU admission

    Benefits and risks of noninvasive oxygenation strategy in COVID-19: a multicenter, prospective cohort study (COVID-ICU) in 137 hospitals

    International audienceAbstract Rational To evaluate the respective impact of standard oxygen, high-flow nasal cannula (HFNC) and noninvasive ventilation (NIV) on oxygenation failure rate and mortality in COVID-19 patients admitted to intensive care units (ICUs). Methods Multicenter, prospective cohort study (COVID-ICU) in 137 hospitals in France, Belgium, and Switzerland. Demographic, clinical, respiratory support, oxygenation failure, and survival data were collected. Oxygenation failure was defined as either intubation or death in the ICU without intubation. Variables independently associated with oxygenation failure and Day-90 mortality were assessed using multivariate logistic regression. Results From February 25 to May 4, 2020, 4754 patients were admitted in ICU. Of these, 1491 patients were not intubated on the day of ICU admission and received standard oxygen therapy (51%), HFNC (38%), or NIV (11%) ( P < 0.001). Oxygenation failure occurred in 739 (50%) patients (678 intubation and 61 death). For standard oxygen, HFNC, and NIV, oxygenation failure rate was 49%, 48%, and 60% ( P < 0.001). By multivariate analysis, HFNC (odds ratio [OR] 0.60, 95% confidence interval [CI] 0.36–0.99, P = 0.013) but not NIV (OR 1.57, 95% CI 0.78–3.21) was associated with a reduction in oxygenation failure). Overall 90-day mortality was 21%. By multivariable analysis, HFNC was not associated with a change in mortality (OR 0.90, 95% CI 0.61–1.33), while NIV was associated with increased mortality (OR 2.75, 95% CI 1.79–4.21, P < 0.001). Conclusion In patients with COVID-19, HFNC was associated with a reduction in oxygenation failure without improvement in 90-day mortality, whereas NIV was associated with a higher mortality in these patients.
