    Reducing emissions from deforestation and forest degradation in developing countries (REDD+) has opened up a new global discussion on forest monitoring and carbon accounting in developing countries. We analyze and compare the extent to which the concept of measurement, reporting, and verification (MRV) for REDD+ has become institutionalized in terms of new policy discourses, actors, resources, and rules in Indonesia, Peru, and Tanzania. To do so, we draw on discursive institutionalism and the policy arrangement approach. A qualitative scale that distinguishes between "shallow" institutionalization on the one end, and "deep" institutionalization on the other, is developed to structure the analysis and comparison. Results show that in all countries MRV has become institutionalized in new or revised aims, scope, and strategies for forest monitoring, and development of new agencies and mobilization of new actors and resources. New legislations to anchor forest monitoring in law and procedures to institutionalize the roles of the various agencies are being developed. Nevertheless, the extent to which MRV has been institutionalized varies across countries, with Indonesia experiencing "deep" institutionalization, Peru "shallow-intermediate" institutionalization, and Tanzania "intermediate-deep" institutionalization.     A practice based approach is new to studies of forest and nature governance and fairly new to governance studies in general. In this chapter, we outline the promise of such an approach for such studies. The chapter is in two parts. Firstly, a number of conclusions are drawn from the preceding individual chapters. They relate to: (1) the types of forest and nature governance practices that can be empirically distinguished; (2) the way the sensitising concepts of logic of practice, situated agency, and performativity have been used to move beyond mainstream governance approaches; and (3) the specific characteristics of a practice based approach to forest and nature governance. The second part of the chapter discusses the academic and societal value of the practice based approach as offered in this book, firstly by comparing this approach to an interpretative approach in governance studies and addressing similarities and differences, and then by discussing whether the practice based approach can contribute to policy making and steering social change. We conclude that a practice based approach can convincingly address some points that mainstream accounts of governance cannot, but only if certain long-held convictions about what governance really is are abandoned

    Does minimally invasive lumbar disc surgery result in less muscle injury than conventional surgery? A randomized controlled trial

    The concept of minimally invasive lumbar disc surgery comprises reduced muscle injury. The aim of this study was to evaluate creatine phosphokinase (CPK) in serum and the cross-sectional area (CSA) of the multifidus muscle on magnetic resonance imaging as indicators of muscle injury. We present the results of a double-blind randomized trial on patients with lumbar disc herniation, in which tubular discectomy and conventional microdiscectomy were compared. In 216 patients, CPK was measured before surgery and at day 1 after surgery. In 140 patients, the CSA of the multifidus muscle was measured at the affected disc level before surgery and at 1 year after surgery. The ratios (i.e. post surgery/pre surgery) of CPK and CSA were used as outcome measures. The multifidus atrophy was classified into three grades ranging from 0 (normal) to 3 (severe atrophy), and the difference between post and pre surgery was used as an outcome. Patients’ low-back pain scores on the visual analogue scale (VAS) were documented before surgery and at various moments during follow-up. Tubular discectomy compared with conventional microdiscectomy resulted in a nonsignificant difference in CPK ratio, although the CSA ratio was significantly lower in tubular discectomy. At 1 year, there was no difference in atrophy grade between both groups nor in the percentage of patients showing an increased atrophy grade (14% tubular vs. 18% conventional). The postoperative low-back pain scores on the VAS improved in both groups, although the 1-year between-group mean difference of improvement was 3.5 mm (95% CI; 1.4–5.7 mm) in favour of conventional microdiscectomy. In conclusion, tubular discectomy compared with conventional microdiscectomy did not result in reduced muscle injury. Postoperative evaluation of CPK and the multifidus muscle showed similar results in both groups, although patients who underwent tubular discectomy reported more low-back pain during the first year after surgery

    Climate-smart land use requires local solutions, transdisciplinary research, policy coherence and transparency

    Successfully meeting the mitigation and adaptation targets of the Paris Climate Agreement (PA) will depend on strengthening the ties between forests and agriculture. Climate-smart land use can be achieved by integrating climate-smart agriculture (CSA) and REDD+. The focus on agriculture for food security within a changing climate, and on forests for climate change mitigation and adaptation, can be achieved simultaneously with a transformational change in the land-use sector. Striving for both independently will lead to competition for land, inefficiencies in monitoring and conflicting agendas. Practical solutions exist for specific contexts that can lead to increased agricultural output and forest protection. Landscape-level emissions accounting can be used to identify these practices. Transdisciplinary research agendas can identify and prioritize solutions and targets for integrated mitigation and adaptation interventions. Policy coherence must be achieved at a number of levels, from international to local, to avoid conflicting incentives. Transparency must lastly be integrated, through collaborative design of projects, and open data and methods. Climate-smart land use requires all these elements, and will increase the likelihood of successful REDD+ and CSA interventions. This will support the PA as well as other initiatives as part of the Sustainable Development Goals

    Flavanols and Anthocyanins in Cardiovascular Health: A Review of Current Evidence

    Nowadays it is accepted that natural flavonoids present in fruits and plant-derived-foods are relevant, not only for technological reasons and organoleptic properties, but also because of their potential health-promoting effects, as suggested by the available experimental and epidemiological evidence. The beneficial biological effects of these food bioactives may be driven by two of their characteristic properties: their affinity for proteins and their antioxidant activity. Over the last 15 years, numerous publications have demonstrated that besides their in vitro antioxidant capacity, certain phenolic compounds, such as anthocyanins, catechins, proanthocyanidins, and other non coloured flavonoids, may regulate different signaling pathways involved in cell survival, growth and differentiation. In this review we will update the knowledge on the cardiovascular effects of anthocyanins, catechins and proanthocyanidins, as implied by the in vitro and clinical studies on these compounds. We also review the available information on the structure, distribution and bioavailability of flavanols (monomeric catechins and proanthocyanidins) and anthocyanins, data necessary in order to understand their role in reducing risk factors and preventing cardiovascular health problems through different aspects of their bioefficacy on vascular parameters (platelet agregation, atherosclerosis, blood pressure, antioxidant status, inflammation-related markers, etc.), myocardial conditions, and whole-body metabolism (serum biochemistry, lipid profile), highlighting the need for better-designed clinical studies to improve the current knowledge on the potential health benefits of these flavonoids to cardiovascular and metabolic health
