    Turkish Telemedical Assistance Service: last four years of activity

    Background: Telemedical Assistance Service (TMAS) is very important for seafarers and passengers who are far from full-fledged health services. This study aimed to determine the frequency of using Turkish TMAS by seafarers and other patients and to compare the given services. Materials and methods: The data obtained from calls to Turkish TMAS in between 2014 and 2017 are composed of 5080 calls. In the study, they were examined in two groups as calls regarding seafarers and other patients. Also, the diseases or injuries related to these calls were evaluated in 22 main groups according to ICD-10 code. All of these cases were analysed by using descriptive statistics according to types of callers, type of diseases, types of given services and methods of contacts. Results: There were 5080 contacts to Turkish TMAS from January 2014 to December 2017. Contacts were 92.3% for seafarers and 7.7% for other patients. The most common reason of contacts was similar for both seafarers and other patients and it was injury and poisoning situations. While the most common service given to seafarers was giving a medicine or other treatment, the most common service given to other patients was medical evacuation. Conclusions: This study laid weight on diseases or injuries of seafarers and other patients at sea. According to this situation, TMAS can improve and maintain its service quality and also in the light of the findings, a guide for seafarers and other patients can be prepared in order to lessen such diseases and injuries

    Reviewing the Defense Spendings’ Effects on Foreign Trade of Turkey

    In order to ensure military security in a country, defense spending must be done for having a trained army with modern weapon systems. The effects of defense spending, which are not completed once and must be made continuously, are inevitable. This study sought to answer the question of how to influence Turkey's foreign trade balance in defense spending in the years 2000-2018. The Granger causality tests in the E-Views, a Windows-based econometric software program have been used. Defense spending has emerged from the analysis did not affect the foreign trade balance in Turkey. It is observed that the gross domestic product has an impact on the export and import of defense industry products

    Service-aware multi-resource allocation in software-defined next generation cellular networks

    Şefik Şuayb Arslan (MEF Author)Network slicing is one of the major solutions needed to meet the requirements of next generation cellular networks, under one common network infrastructure, in supporting multiple vertical services provided by mobile network operators. Network slicing makes one shared physical network infrastructure appear as multiple logically isolated virtual networks dedicated to different service types where each Network Slice (NS) benefits from on-demand allocated resources. Typically, the available resources distributed among NSs are correlated and one needs to allocate them judiciously in order to guarantee the service, MNO, and overall system qualities. In this paper, we consider a joint resource allocation strategy that weights the significance of the resources per a given NS by leveraging the correlation structure of different quality-of-service (QoS) requirements of the services. After defining the joint resource allocation problem including the correlation structure, we propose three novel scheduling mechanisms that allocate available network resources to the generated NSs based on different type of services with different QoS requirements. Performance of the proposed schedulers are then investigated through Monte-Carlo simulations and compared with each other as well as against a traditional max-min fairness algorithm benchmark.     Probiotics in irritable bowel syndrome: An up-to-date systematic review

    Irritable bowel syndrome (IBS) is a frequent functional gastrointestinal disorder, and alterations in the gut microbiota composition contributes to symptom generation. The exact mechanisms of probiotics in the human body are not fully understood, but probiotic supplements are thought to improve IBS symptoms through manipulation of the gut microbiota. The aim of this systematic review was to assess the latest randomized controlled trials (RCTs) evaluating the effect of probiotic supplementation on symptoms in IBS patients. A literature search was conducted in Medline (PubMed) until March 2019. RCTs published within the last five years evaluating effects of probiotic supplements on IBS symptoms were eligible. The search identified in total 35 studies, of which 11 met the inclusion criteria and were included in the systematic review. Seven studies (63.6%) reported that supplementation with probiotics in IBS patients significantly improved symptoms compared to placebo, whereas the remaining four studies (36.4%) did not report any significant improvement in symptoms after probiotic supplementation. Of note, three studies evaluated the effect of a mono-strain supplement, whereas the remaining eight trials used a multi-strain probiotic. Overall, the beneficial effects were more distinct in the trials using multi-strain supplements with an intervention of 8 weeks or more, suggesting that multi-strain probiotics supplemented over a period of time have the potential to improve IBS symptoms.publishedVersio

    Geçici Hiperglisemi anında Sevofluran ve Desfluranın Eritrosit Deformabilitesi Üzerine Etkilerinin Araştırılması

    Aim: Micro and macrovascular complications due to long-term hyperglycemia are associated with increased mortality and morbidity. Erythrocytes exposed to hyperglycemia for a long time may cause morphological changes in erythrocytes such as decreased deformability and development of aggregation. As a result, complications such as shortening life span of erythrocytes, impairment of oxygen carrying capacity, tissue hypoxia may occur. In our study, we would like to investigate the effects of Sevoflurane and Desflurane on erythrocyte deformability during transient hyperglycemia. Materials and Methods: In this study, 30 male Wistar albino rats were used. The animals were randomly divided into five groups, each contained 6 rats: Diabetic control (group DC), diabetic hyperglycemia group (group DH), diabetic hyperglycemia group with desflurane (group DH-D), and diabetic hyperglycemia group with sevoflurane (group DH-S) groups. Another 6 rats without diabetes were assigned as control group (group C). Streptozotocin- induced diabetic rats were kept 6 weeks, then transient hyperglycemia was created, and the administration of sevoflurane and desflurane were performed. After 24 hours blood samples were obtained and deformability measurements were performed in erythrocyte suspensions containing Htc 5% in a PBS buffer. Results: Diabetes mellitus was found to increase relative resistance in the control group (p <0.0001). Acute hyperglycemia increased relative resistance in diabetes control, relatively. Group DH, Group DH-D and Group DH-S deformability index were significantly different when compared to Group DC (p=0.007, p=0.025, p=0.016, respectively). It was found that administration of desflurane or sevoflurane did not alter erythrocyte deformability during acute hyperglycemia (p = 0.591, p = 0.739). Conclusion: As a consequence, we think that we can safely use inhalation anesthetics such as Desflurane and Sevoflurane during acute hyperglycemia attacks. But, it needs further investigation as both experimental and clinicalAmaç: Uzun süreli hipergliseminin oluşturduğu mikro ve makrovasküler komplikasyonlar mortalite ve morbidite artışı ile birliktedir. Eritrositlerin ise yaşam süreleri boyunca uzun süre hiperglisemiye maruz kalmaları morfolojik olarak eritrositlerde deformabilitede azalma ve agregasyon gelişmesi gibi bir takım değişikliklere neden olmaktadır. Bunun sonucunda eritrositlerin yaşam sürelerinde kısalma, oksijen taşıma kapasitesinde bozukluk, doku hipoksisi gibi komplikasyonlar meydana gelmektedir. Sevofluran ve desfluranın ise eritrosit deformabilitesi üzerine etkileri ile ilgili çeşitli çalışmalar mevcuttur. Biz de bu çalışmamızda geçici olarak oluşturulan hiperglisemi sırasında sevofluran ve desfluranın eritrosit deformabilitesi üzerine etkilerini araştırmayı amaçladık. Yöntem: Çalışma Gazi Üniversitesi Deneysel ve Klinik Araştırma Merkezi'nde Gazi Üniversitesi Deney Hayvanları Etik Kurulu onayı ile yapıldı. Çalışmaya 30 rat dahil edildi. Ratlar; kontrol grubu, diyabetik-kontrol, diyabetik hiperglisemi, diyabetik–hiperglisemi-sevofluran, diyabetik-hiperglisemi- desfluran olmak üzere 5 gruba ayrıldı. Streptozosin ile diyabet oluşturulan ratlar 6 hafta yaşatıldıktan sonra geçici hipergilisemi oluşturuldu ve sevofluran ve desfluran anestezisi uygulandı. 24 saat sonra kan örnekleri alındı ve santrifüj edildi. Eritrosit deformabilitesi sabit akım filtrometre sistemleri kullanılarak ölçüldü. Rölatif rezistansının artması eritrosit deformabilitesinin azalması olarak yorumlandı. Bulgular: Diyabet oluşturulmasının kontrol grubuna göre rölatif rezistansı arttırdığı bulundu (p<0.0001). Akut hiperglisemi, diyabet kontrol grubuna göre rölatif rezistansı arttırdı. Grup DH, Grup DH-D ve Grup DH-S deformabilite indeksi Grup DK ile karşılaştırıldığında anlamlı olarak farklı bulundu (p=0.007, p=0.025, p=0.016). Akut hiperglisemi sırasında Desfluran veya Sevofluran uygulanmasının ise eritrosit deformabilitesini değiştirmediği tespit edildi (p=0.591, p=0.739). Sonuç: Sevofluran ve Desfluran gibi inhalasyon ajanların akut hiperglisemi sırasında güvenle kullanılabileceğini düşünmekteyiz. Ancak bu bulguların daha detaylı ve geniş serilerde yapılacak klinik ve deneysel çalışmalarla desteklenmesi gerekmektedir

    Polyclonal outbreak of bacteremia caused by Burkholderia cepacia in the intensive care unit

    Aim: Burkholderia cepacia is a multidrug-resistant, opportunistic pathogen of humans and outbreaks of infection in hospitals have been described. In this study, we aimed to report an outbreak in patients without cystic fibrosis or chronic granulomatous disease involving different species of Burkholderia cepacia. Material and Methods: A small outbreak of nosocomial Burkholderia cepacia complex occurred in a 6-bed intensive care unit. We isolated Burkholderia cepacia from blood cultures of the patients admitted to our intensive care unit. All isolates from patients and the environment were identified by standard microbiological techniques and VITEK system. Antibiotic susceptibility testing was performed using Kirby Bauer's disk diffusion method and the VITEK system Results: All isolates exhibited identical patterns of antibiotic susceptibility and all isolates were sensitive to trimethoprim-sulfamethoxazole, ceftazidime and meropenem. The isolates were typed using pulsed-field gel electrophoresis using the restriction enzymes XbaI and SpeI. Accordingly, while 4 strains were similar, one was different. Discussion: The experience from this outbreak reminded us of the importance of outbreak investigation in such small outbreaks and keeping the health care workers educated and constant attention on this issue. The results of this study emphasized once again the necessity to maintain our sensitivity to the basic principles of sanitation and to raise our awareness of such outbreaks

    Orbital Complications of Acute Sinusitis: Evaluation, Management, and Results

    Objective: Even though acute rhinosinusitis orbital complications are not very common, they can appear as very severe complications with high mortality rates, due to infection spreading to orbital and intracranial tissues. The objective of this study is to assess the treatment and its results for patients treated in our clinic due to rhinosinusitis complications.Methods:Patients who were admitted to our clinic due to acute rhinosinusitis complications between January 2010 and March 2012 were examined. We retrospectively evaluated 11 patients 8 (73%) males and 3 (23%) females with a mean age 12.36 years (range 6-20 years)who were treated for orbital complications of acute rhinosinusitis. Cases were evaluated according to age, sex, etiologic factors, localization, treatment, and outcome.Results: Preseptal cellulitis was observed in 9 patients (82%), orbital abscess was observed in 1 patient (9%), and subperiosteal abscess was observed in 1 patient (9%). Ten of 11 patients achieved full recovery through medical treatment, whereas 1 patient was treated with right subperiosteal abscess drainage and right functional endoscopic sinus surgery due to subperiosteal abscess. The patients’ average admission duration was 7 days.Conclusion: The orbita is the area with the most common sinusitis complications, due to its proximity to the paranasal sinuses and especially to ethmoid cells. Early diagnosis and aggressive treatment are important for the reduction of unwanted manifestations. Computed tomography with contrast remains the optimal imaging study for orbital inflammation. Surgical treatment is indicated when subperiosteal orbital abscess and orbital abscess are scenes
