Coronariografía por tomografía computarizada de doble fuente
La tomografía computarizada multicorte (TCMC) permite estudiar las
arterias coronarias de forma no invasiva. A pesar de la elevada resolución
espacial y temporal de los equipos de sesenta y cuatro cortes (TCMC-
64) esta técnica no se encuentra exenta de limitaciones. La tomografía
computarizada de doble fuente (TCDF) permite estudiar las arterias
coronarias con gran calidad diagnóstica en todos los sujetos independien-
temente de su frecuencia cardiaca sin necesidad, por tanto, de emplear
betabloqueantes. En este trabajo se describen los estudios por TCDF de
tres pacientes con elevada frecuencia cardiaca y gran variabilidad del
ritmo. Se demuestra la utilidad de esta técnica para obtener estudios
de excelente calidad diagnóstica en casos en que la coronariografía por
TCMC-64 convencional presenta limitaciones. INGLÉS: Multidetector computed tomography (MDCT) has been demonstrated to be a very useful technique to non-invasively study coronary arteries. Despite the high spatial and temporal resolution of 64-slice MDCT scanners, this technique has several limitations. Dual-source computed tomography (DSCT) allows to study coronary arteries with excellent diagnostic quality in all subjects independent of the heart rate, thus avoiding the use of beta-blockers. In this article DSCT studies from three subjects with elevated heart rate and irregular heart rhythm are described. In routine clinical practice surveillance of heart
transplant recipients is usually performed using echocardiography
and conventional coronary angiography. The
latter permits diagnosis and follow-up of coronary allograft
vasculopathy. However, this procedure is invasive
and is not free of complications. Conventional multislice
computed tomography (MSCT) has been shown to be a
useful non-invasive tool for ruling out coronary artery
disease and evaluating cardiac function. However, due
to its limited temporal resolution betablocker administration
is required, and its usefulness in certain patient
populations with restricted response to this medication,
such as heart transplant recipients, may therefore be
limited. Dual-source CT (DSCT) allows evaluation of the
coronary arteries in all individuals independent of their
heart rate. In the case presented here, we demonstrate
that DSCT may be useful for evaluating cardiac function
and ruling out coronary allograft vasculopathy in heart
transplant recipients

Dual-source CT for visualization of the coronary arteries in heart transplant patients with high heart rates
OBJECTIVE. The purpose of this study was to evaluate the quality of dual-source CT images of the coronary arteries in heart transplant recipients with high heart rates.
SUBJECTS AND METHODS. Contrast-enhanced dual-source CT coronary angiography was performed on 23 heart transplant recipients (20 men, three women; mean age, 61.1 ± 12.8 years). Data sets were reconstructed in 5% steps from 30% to 80% of the R-R interval. Two blinded independent readers using a 5-point scale (0, not evaluative; 4, excellent quality) assessed the quality of images of coronary segments.
RESULTS. The mean heart rate during scanning was 89.2 ± 10.4 beats/min. Interobserver agreement on the quality of images of the whole coronary tree was a kappa value of 0.78 and for selection of the optimal reconstruction interval was a kappa value of 0.82. The optimal reconstruction interval was systole in 17 (74%) of the 23 of heart transplant recipients. At the best reconstruction interval, diagnostic image quality (score ≥ 2) was obtained in 92.1% (303 of 329) of the coronary artery segments. The mean image quality score for the whole coronary tree was 3.1 ± 1.01. No significant correlation between mean heart rate (ρ = 0.31) or heart rate variability (ρ = 0.23) and overall image quality score was observed (p = not significant).
CONCLUSION. Dual-source CT acquisition yields coronary angiograms of diagnostic quality in heart transplant recipients. Mean heart rate and heart rate variability during scanning do not have a negative effect on the overall quality of images of the coronary arteries
Recomendaciones basadas en la evidencia del grupo andaluz para la reflexión e investigación en nutrición (Garin) para el manejo del paciente con síndrome de intestino corto
In order to develop evidence-based recommendations and expert consensus for the nutritional management of patients with short bowel syndrome (SBS), we conducted a systematic literature search using the PRISMA methodology plus a critical appraisal following the GRADE scale procedures. Pharmacological treatment with antisecretory drugs, antidiarrheal drugs, and somatostatin contributes to reducing intestinal losses. Nutritional support is based on parenteral nutrition; however, oral intake and/or enteral nutrition should be introduced as soon as possible. In the chronic phase, the diet should have as few restrictions as possible, and be adapted to the SBS type. Home parenteral nutrition (HPN) should be individualized. Single-lumen catheters are recommended and taurolidine should be used for locking the catheter. The HPN’s lipid content must be greater than 1 g/kg per week but not exceed 1 g/kg per day, and omega-6 fatty acids (ω6 FAs) should be reduced. Trace element vials with low doses of manganese should be used. Patients with chronic SBS who require long-term HPN/fluid therapy despite optimized treatment should be considered for teduglutide treatment. All patients require a multidisciplinary approach and specialized follow-up. These recommendations and suggestions regarding nutritional management in SBS patients have direct clinical applicability
Regular insulin added to total parenteral nutrition vs subcutaneous glargine in non-critically ill diabetic inpatients, a multicenter randomized clinical trial: INSUPAR trial
Background: There is no established insulin regimen in T2DM patients receiving parenteral nutrition. Aims: To compare the effectiveness (metabolic control) and safety of two insulin regimens in patients with diabetes receiving TPN. Design: Prospective, open-label, multicenter, clinical trial on adult inpatients with type 2 diabetes on a non-critical setting with indication for TPN. Patients were randomized on one of these two regimens: 100% of RI on TPN or 50% of Regular insulin added to TPN bag and 50% subcutaneous Gl. Data were analyzed according to intention-to-treat principle. Results: 81 patients were on RI and 80 on GI. No differences were observed in neither average total daily dose of insulin, programmed or correction, nor in capillary mean blood glucose during TPN infusion (165.3 +/- 35.4 in RI vs 172.5 +/- 43.6 mg/dL in GI; p = 0.25). Mean capillary glucose was significantly lower in the GI group within two days after TPN interruption (160.3 +/- 45.1 in RI vs 141.7 +/- 43.8 mg/dL in GI; p = 0.024). The percentage of capillary glucose above 180 mg/dL was similar in both groups. The rate of capillary glucose <= 70 mg/dL, the number of hypoglycemic episodes per 100 days of TPN, and the percentage of patients with non-severe hypoglycemia were significantly higher on GI group. No severe hypoglycemia was detected. No differences were observed in length of stay, infectious complications, or hospital mortality. Conclusion: Effectiveness of both regimens was similar. GI group achieved better metabolic control after TPN interruption but non-severe hypoglycemia rate was higher in the GI group. (C) 2019 The Author(s). Published by Elsevier Ltd
Recommendations of the GARIN group for dietary managing of patient with chronic kidney disease
[ES] Introducción y objetivos: en el tratamiento dietético de los pacientes con enfermedad renal crónica (ERC) existen muchas áreas de incertidumbre.
El grupo de trabajo GARIN tiene como objetivo defi nir su posición en este campo.
Material y métodos: revisión bibliográfi ca previa y reunión presencial en la que se discutieron y contestaron preguntas específi cas sobre el tema.
Resultados: la actuación terapéutica debe ser individualizada y atendiendo al grado de enfermedad renal que presente el paciente y a sus comorbilidades.
En cuanto a la terapia médica nutricional, nuestro grupo propone tres niveles diferentes de actuación, en los que las recomendaciones
de ingesta proteica, fi bra, ácidos grasos o potasio son distintas. Además, sugerimos utilizar el concepto ratio fósforo/proteína en el ajuste de la
dieta del paciente con ERC. Damos recomendaciones en cuanto al tratamiento en diabetes y en suplementación artifi cial.
Conclusiones: estas recomendaciones aportan respuestas concretas sobre cuestiones comunes en la asistencia a pacientes con ERC.[EN] Background and objectives: by means of this update, the GARIN working group aims to defi ne its position regarding the dietary treatment of
patients with chronic kidney disease (CKD). In this area there are many aspects of uncertainty.
Material and methods: bibliographical review and specifi c questions in advance were discussed and answered at a meeting in the form of conclusions.
Results: the therapeutic action must be individualized and taking into account the degree of renal failure that the patient presents and their
comorbidities. Regarding nutritional medical therapy, our group proposes three different levels of action, in which the recommendations of protein
intake, fi ber, fatty acids or potassium are different. In addition, we suggest using the phosphorus/protein ratio concept in adjusting the diet of the
patient with CKD. We give recommendations regarding treatment in diabetes and artifi cial supplementation.

Nutrición parenteral domiciliaria en España, 2019: informe del Grupo de Nutrición Artificial Domiciliaria y Ambulatoria NADYA
RESUMEN Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadyasenpe.com) del año 2019. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2019. Resultados: se registraron 283 pacientes (51,9 %, mujeres), 31 niños y 252 adultos procedentes de 47 hospitales españoles, lo que representa una tasa de prevalencia de 6,01 pacientes/millón de habitantes/año 2019. El diagnóstico más frecuente en los adultos fue “oncológico paliativo” y “otros” (21,0 %). En los niños fue la enfermedad de Hirschsprung junto a la enterocolitis necrotizante, las alteraciones de la motilidad intestinal y la pseudoobstrucción intestinal crónica, con 4 casos cada uno (12,9 %). El primer motivo de indicación fue el síndrome del intestino corto tanto en los niños (51,6 %) como en los adultos (37,3 %). El tipo de catéter más utilizado fue el tunelizado tanto en los niños (75,9 %) como en los adultos (40,8 %). Finalizaron 68 episodios, todos en adultos: la causa más frecuente fue el fallecimiento (54,4 %). Pasaron a la vía oral el 38,2 %. Conclusiones: el número de centros y profesionales colaboradores con el registro NADYA va incrementándose. Se mantienen estables las principales indicaciones y los motivos de finalización de la NPD
