    The project for the work of a new bridge over Pisuerga river is described in this article. Considering how peculiar the topography of the place is, the idea of a truss-bridge is suggested, planning its construction thinking of the bearings that have to support the bridge and its constructive system, intending first its durability.Se describe el proyecto de obra del nuevo puente sobre el río Pisuerga.     A software based DVB-T2 time frequency slicing evaluation framework

    DVB-T2 is the second generation terrestrial broadcast system developed within the DVB consortium. Based on multi-carrier transmission OFDM, DVB-T2 introduces many innovative tools offering high flexibility to broadcasters. Among many other new techniques DVB-T2 defines an optional Time-Frequency-Slicing (TFS) profile. In TFS slices of a PLP are sent over multiple and non necessarily adjacent RF frequency channels. This technique provides a gain associated to propagation diversity, as well as an intrinsic multiplexation gain. Following a software defined radio approach this paper presents a flexible DVB-T2 Time Frequency Slicing evaluation framework built to study the practical performance of DVB-T2 Time Frequency Slicing configurations under realistic channel scenarios.     Ocular Decompression Retinopathy with Macular Edema after Deep Sclerectomy with Intrascleral Implantation

    We report a case of ocular decompression retinopathy (ODR) with macular edema, diagnosed by optical coherence tomography, after a deep sclerectomy (DS) with an intrascleral implant, resolved with medical therapy 6 months later. The medical literature reports that in 14% of patients suffering from ODR, a pars plana vitrectomy was required, and 15% of patients had a poor final visual acuity. An otherwise healthy 75-year-old man with high myopia and a primary open-angle glaucoma, with previous intraocular pressure (IOP) of 24 mm Hg, underwent a DS with intrascleral implant without complications. The patient suffered postoperatively from ODR with macular edema that required medical therapy with nonsteroidal anti-inflammatory eye drops (Nepafenac® 0.3%) for a period of 6 months. ODR is an infrequent complication that may occur after any surgical or medical procedure that causes a sudden IOP decrease. The presence of macular edema is only reported in 5% of cases and can occur in patients who report a decreased visual acuity, commonly associated with a retinal hemorrhage. We have described a case of ODR with macular edema after DS with intrascleral implant. Although ODR is considered to cause a low level of morbidity, in some patients this may not be the case

    Clinical and Economic Impact of Community-Onset Urinary Tract Infections Caused by ESBL-Producing Klebsiella pneumoniae Requiring Hospitalization in Spain : An Observational Cohort Study

    Objective : To analyze the clinical and economic impact of community-onset urinary tract infections (UTIs) caused by extended-spectrum beta-lactamase (ESBL)-producing Klebsiella pneumoniae requiring hospitalization. Methods : A retrospective cohort study that included all adults with a UTI caused by K. pneumoniae that were admitted to a tertiary care hospital in Barcelona, Spain, between 2011 and 2015. Demographic, clinical, and economic data were analyzed. Results : One hundred and seventy-three episodes of UTIs caused by K. pneumoniae were studied; 112 were non-ESBL-producing and 61 were ESBL-producing. Multivariate analysis identified ESBL production, acute confusional state associated with UTI, shock, and the time taken to obtain adequate treatment as risk factors for clinical failure during the first seven days. An economic analysis showed differences between ESBL-producing and non-ESBL-producing K. pneumoniae for the total cost of hospitalization per episode (mean EUR 6718 vs EUR 3688, respectively). Multivariate analysis of the higher costs of UTI episodes found statistically significant differences for ESBL production and the time taken to obtain adequate treatment. Conclusion : UTIs caused by ESBL-producing K. pneumoniae requiring hospitalization and the time taken to obtain adequate antimicrobial therapy are associated with worse clinical and economic outcomes

    Publicación de las Jornadas que celebró el mes de marzo de 2016, en el Parque Nacional Las Tablas de Daimiel, la asociación Cultura de Ribera con el objetivo de hacer de la cultura y el arte elementos para la conservación de nuestros Espacios Naturales Protegidos. Además de dar unas nociones sobre el paisaje y el territorio de La Mancha Húmeda, en la obra, se traza un panorama histórico sobre Las Tablas y su importancia como el primer Parque Nacional que fue declarado, en gran medida, gracias a la presión ciudadana y del, por entonces, incipiente movimiento ecologista. También se puede encontrar una visión ética de la conservación medioambiental, los antecedentes históricos y experiencias actuales del arte como vector para la conservación de los Espacios Naturales Protegidos y una reflexión sobre el papel del arte y las humanidades en el contexto de la crisis ecológica global. La obra se cierra con un artículo sobre la figura de Ignacio Meco, artista que en su obra aunaba arte y conservación del medio y que tuvo su casa taller en la Zona de Protección de Las Tablas

    RESUMEN. Desde el verano de 2007 está en servicio el nuevo puente atirantado “Puerta de las Rozas” que vuela sobre la A-6 creando un nuevo acceso a la ciudad de las Rozas. Esta nueva y reconocible imagen para los muchos usuarios de la carretera de la Coruña en su entrada a Madrid es el fruto tres años de trabajo en sus distintas fases de encaje, concurso, proyecto y ejecución. Se trata de un puente atirantado asimétrico de 102 m de luz con un ancho de tablero de 20 m, suspendido por 9 parejas de tirantes de un único mástil formado por dos células triangulares que incluyen los tirantes rígidos de contrarresto anclados a contrapeso.ABSTRACT. The new “Puerta de las Rozas cable-stayed bridge, set over the A-6 motorway has been in service since the summer of 2007 and creates a new access to the suburb of Las Rozas.     Resveratrol inhibits nonalcoholic fatty liver disease in rats

    Es reproducción del documenteo publicado en http://dx.doi.org/10.1186/1471-230X-8-40Background: The prevalence of nonalcoholic fatty liver disease (NAFLD) is high. NAFLD is linked to obesity, diabetes mellitus, and hypertriglyceridemia. Approximately 20% of patients with NAFLD will eventually develop cirrhosis. Our purpose was to investigate whether resveratrol decreased hepatic steatosis in an animal model of steatosis, and whether this therapeutic approach resulted in a decrease in tumor necrosis factor alpha (TNF-alpha) production, lipid peroxidation and oxidative stress. Methods: Male Wistar CRL: Wi (Han) (225 g) rats were randomized into three groups. A control group (n = 12) was given free access to regular dry rat chow for 4 weeks. The steatosis (n = 12) and resveratrol (n = 12) groups were given free access to feed (a high carbohydrate-fat free modified diet) and water 4 days per week, and fasted for the remaining 3 days for 4 weeks. Rats in the resveratrol group were given resveratrol 10 mg daily by the oral route. All rats were killed at 4 weeks and assessed for fatty infiltration and bacterial translocation. Levels of TNF-alpha in serum, hepatic malondialdehyde (MDA), oxidative stress (superoxide dismutase, glutathione peroxidase, catalase and nitric oxide synthase) and biochemical parameters were measured. Results: Fat deposition was decreased in the resveratrol group as compared to the steatosis group (Grade 1 vs Grade 3, P < 0.05). TNF-alpha and MDA levels were significantly increased in the steatosis group (TNF-alpha; 33.4 +/- 5.2 vs 26.24 +/- 3.47 pg/ml and MDA; 9.08 +/- 0.8 vs 3.17 +/- 1.45 mu M respectively, P < 0.05). This was accompanied by increased superoxide dismutase, glutathione peroxidase and catalase and decreased nitric oxide synthase in the liver of resveratrol group significantly (P < 0.05 vs steatosis group). Bacterial translocation was not found in any of the groups. Glucose levels were decreased in the group of rats given resveratrol (P < 0.05). Conclusion: Resveratrol decreased NAFLD severity in rats. This effect was mediated, at least in part, by TNF-alpha inhibition and antioxidant activities

    Metabolomics analysis of type 2 diabetes remission identifies 12 metabolites with predictive capacity: a CORDIOPREV clinical trial study.

    Type 2 diabetes mellitus (T2DM) is one of the most widely spread diseases, affecting around 90% of the patients with diabetes. Metabolomics has proven useful in diabetes research discovering new biomarkers to assist in therapeutical studies and elucidating pathways of interest. However, this technique has not yet been applied to a cohort of patients that have remitted from T2DM. All patients with a newly diagnosed T2DM at baseline (n = 190) were included. An untargeted metabolomics approach was employed to identify metabolic differences between individuals who remitted (RE), and those who did not (non-RE) from T2DM, during a 5-year study of dietary intervention. The biostatistical pipeline consisted of an orthogonal projection on the latent structure discriminant analysis (O-PLS DA), a generalized linear model (GLM), a receiver operating characteristic (ROC), a DeLong test, a Cox regression, and pathway analyses. The model identified a significant increase in 12 metabolites in the non-RE group compared to the RE group. Cox proportional hazard models, calculated using these 12 metabolites, showed that patients in the high-score tercile had significantly (p-value < 0.001) higher remission probabilities (Hazard Ratio, HR, high versus low = 2.70) than those in the lowest tercile. The predictive power of these metabolites was further studied using GLMs and ROCs. The area under the curve (AUC) of the clinical variables alone is 0.61, but this increases up to 0.72 if the 12 metabolites are considered. A DeLong test shows that this difference is statistically significant (p-value = 0.01). Our study identified 12 endogenous metabolites with the potential to predict T2DM remission following a dietary intervention. These metabolites, combined with clinical variables, can be used to provide, in clinical practice, a more precise therapy.
