Retrospective and comparative study of forty-three eyes with acute hydrops among five-hundred-sixty-seven keratoconus cases
PURPOSE: To compare clinical characteristics and surgical out comes between keratoconus cases with and without acute hydrops (AH). METHODS: All keratoconus cases followed up at the Hospital Universitário Antônio Pedro - UFF and at Clinica Oftamológica Souza Pena between 1982 and 2000 were reviewed. Main analyzed outcome parameters were: visual acuity, follow-up time since diagnosis, type of visual correction, morphological and keratometric classification, and surgical results. RESULTS: The overall acute hydrops incidence was 5.8%. Gender, keratoconus family history, atopy did not differ significantly between the two groups. Among acute hydrops cases, 72.7% were classified as peripheral cones, against 21.4% of the control cases (p < 0.05). Overall, 19% of the patients needed penetrating keratoplasty (PK). Out of 1062 eyes that did not develop acute hydrops, 8.4% underwent PK, compared to 88.4 % of the acute hydrops eyes (p < 0.05). Surgical outcomes showed no significant difference between the two groups. CONCLUSION: Peripheral cones were more frequently related to acute hydrops, and consequently, to PK. Keratoconus surgical treatment results were similar in cases with and without acute hydrops.OBJETIVO: Comparar as caracterÃsticas clÃnicas e os resultados do tratamento cirúrgico entre casos com e sem hidropisia aguda, do total de 567 pacientes com ceratocone. MÉTODOS: Foram revistos os prontuários de todos os casos de ceratocone acompanhados entre 1982 e 2000 no Hospital Universitário Antônio Pedro - UFF e na ClÃnica Oftalmológica Souza Pena, em Niterói. Acuidade visual, tempo de evolução, tipo de correção ótica, morfologia do ceratocone, classificação ceratométrica e resultados cirúrgicos foram as principais variáveis comparadas entre os dois grupos. RESULTADOS: A incidência de hidropisia aguda foi 5,8%. Não houve diferença estatisticamente significativa entre os grupos quanto ao sexo, história familiar e atopia. No grupo da hidropisia aguda, 72,7% dos casos foram classificados como periféricos, contrastando com 21,4% do grupo controle (p < 0,05). Dezenove por cento dos pacientes foram submetidos à ceratoplastia penetrante. Dentre os 1062 olhos que não desenvolveram hidropisia aguda, 8,4% foram operados, ao passo que 88,4% dos olhos com hidropisia submeteram-se ao transplante de córnea (p < 0,05). O resultado pós-operatório não diferiu entre os dois grupos. CONCLUSÃO: O ceratocone periférico esteve mais associado à hidropisia aguda e, conseqüentemente, à indicação de ceratoplastia penetrante. Estudo retrospectivo e comparativo de quarenta e três olhos com hidropisia aguda em quinhentos e sessenta e sete casos de ceratocone
Assessment of the auditory handicap in adults with unilateral hearing loss
A deficiência auditiva (DA) unilateral é caracterizada pela diminuição da audição em apenas uma orelha podendo acarretar dificuldade acadêmica, alterações de linguagem e dificuldade sócio-emocionais. OBJETIVO: Avaliar a autopercepção do handicap auditivo de sujeitos adultos, portadores de DA sensório-neural unilateral, não usuários de Aparelho de Amplificação Sonora Individual. MATERIAL E MÉTODO: Estudo Prospectivo com 52 sujeitos adultos, com média de 34.5 anos e de ambos os gêneros (26 do gênero feminino, 26 do masculino) com deficiência auditiva: tipo neurossensorial unilateral, de graus variados; responderam a um questionário para a avaliação do handicap sendo utilizado o "Hearing Handicap Inventory for Adults" (HHIA). RESULTADOS: Foram pontuadas as subescalas dos aspectos emocionais e sociais/situacionais sendo encontrados 73,1% de presença do handicap entre os graus leve, moderado e significativo e em maior porcentagem (88,5%) no gênero feminino. CONCLUSÕES: A aplicação do questionário mostrou-se um procedimento eficiente, pois a DA unilateral pode, não raramente, comprometer aspectos sociais e emocionais do sujeito adulto e o mesmo grau de DA podem reagir de forma diferente, indicando que a grande variabilidade na autopercepção do handicap auditivo está associada a aspectos não audiológicos.Hearing impairment (HI) is characterized by unilateral hearing loss in one ear and can result in learning difficulties, language impairment and socio-emotional difficulties. To assess the perception of hearing handicap in adult subjects, patients with unilateral sensorineural HI, non-users of individual hearing aids. MATERIALS AND METHODS: Prospective study with 52 adult subjects with a mean of 34.5 years of age, from both genders (26 females, 26 males) with hearing loss: sensorineural unilateral, in varying degrees, responded to a questionnaire for assessing hearing handicap, and for that we used the Hearing Handicap Inventory for Adults (HHIA). RESULTS: We scored the subscales of the emotional and social/situational aspects, and we found 73.1% of the handicap present being mild, moderate and significant, but at a higher percentage (88.5%) in females. CONCLUSIONS: The use of the questionnaire proved to be an effective procedure, because the unilateral HI may, not infrequently, compromise social and emotional aspects of the adult subject and the same degree of HI who can react differently, indicating that the wide variability in the perception of the hearing handicap is associated with non-audiological aspects
Control postural dinámico y estático de jugadores de vóley con amputación transfemoral
Introdução: Uma diminuição no controle postural e propriocepção é um resultado comum da amputação dos membros inferiores. No entanto, a adaptação do controle postural nos jogadores paralÃmpicos de vôlei sentado com amputação de membros inferiores ainda não é compreendida. Objetivo: O propósito desse estudo transversal controlado consistia em investigar o controle postural estático e dinâmico em jogadores de vôlei sentado com amputação transfemoral unilateral. Métodos: Dezesseis indivÃduos participaram do estudo, oito deles eram amputados com amputação transfemoral unilateral (grupo amputado: idade: 33,5 ± 5,6 anos, peso: 77,7 ± 5,3 kg, altura: 179,4 ± 5,3 cm) e oito eram fisicamente ativos não-amputados (idade do grupo controle: 27,2 ± 8,4 anos, peso: 82,7 ± 6,6 kg, altura: 178,7 ± 6,1 cm). O controle postural estático e dinâmico foi avaliado usando a plataforma Neurocom® Balance Master System (Teste ClÃnico Modificado de Interação Sensorial no EquilÃbrio, Limites de Estabilidade, Variação RÃtmica de Carga, Sentado para de Pé, Marcha na Plataforma e Passo com Inversão Rápida). As análises estatÃsticas foram realizadas com o teste Shapiro-Wilk, o teste de Levene e o teste t de Student para amostras pareadas (p 0,05). Conclusão: Apesar do treinamento no vôlei sentado, nossos resultados demonstraram que os amputados apresentam um controle postural mais precário tanto em tarefas estáticas como dinâmicas, quando comparados com os não-amputados fisicamente ativos. However, postural control adaptation in Paralympic sitting volleyball players with lower limb amputation is not yet understood. Objective: The purpose of this cross-sectional controlled study was to investigate static and dynamic postural control in sitting volleyball players with unilateral transfemoral amputation. Methods: Sixteen subjects participated in the study, eight of whom were amputees with unilateral transfemoral amputation (amputee group: age: 33.5 ± 5.6 years, weight: 77.7 ± 5.3 kg, height: 179.4 ± 5.3 cm) and eight physically active non-amputees (control group age: 27.2 ± 8.4 years, weight: 82.7 ± 6.6 kg, height: 178.7 ± 6.1 cm). Static and dynamic postural control was evaluated using the Neurocom® Balance Master System platform (Modified Clinical Test of Sensory Interaction on Balance, Limits of Stability, Rhythmic Weight Shift, Sit-to-Stand, Walk Across, and Step and Quick Turn). Statistical analyses were performed with the Shapiro-Wilk test, Levene's test, and Student's t-test for paired samples (p .05). Conclusion: Despite training in sitting volleyball, our results demonstrated that amputees have poorer postural control in both static and dynamic tasks when compared to physically active non-amputees. Imidazole derivatives as promising agents for the treatment of Chagas disease
More than 100 years later after being firstly described, Chagas disease remains endemic in 21 Latin American countries and has spread to other continents. Indeed, this disease, caused by the protozoan parasite Trypanosoma cruzi, is no longer just a problem for the American continent but has become a global health threat. Current therapies, nifurtimox and benznidazole (Bz), are far from being adequate due to undesirable effects and their lack of efficacy in the chronic phases of the disease. In this work, we present an in-depth phenotypical evaluation in T.cruzi of a new class of imidazole compounds, discovered in a previous phenotypic screening against different trypanosomatids and designed as potential inhibitors of cAMP phosphodiesterases (PDEs). The confirmation of several activities similar or superior to Bz prompted a synthesis program of hit optimization and extended SAR, aimed at improving drug-like properties such as aqueous solubility, resulting in additional hits with IC50 similar to Bz. The cellular effects of one representative hit, compound 9, on bloodstream trypomastigotes were further investigated. Transmission electron microscopy revealed cellular changes, after just 2 h of incubation with the IC50 concentration, that were consistent with induced autophagy and osmotic stress - mechanisms previously linked to cAMP signaling. Compound 9 induced highly significant increases in both cellular and medium cAMP, confirming that inhibition of T.cruzi PDE(s) is part of its mechanism of action. The potent and selective activity of this imidazole-based PDE inhibitor class against T.cruzi constitutes a successful repurposing of research into inhibitors of mammalian PDEs
ASSOCIAÇÃO ENTRE A AUTOPERCEPÇÃO DO ENVELHECIMENTO E A AUTOPERCEPÇÃO DA SAÚDE
A autopercepção da saúde é um determinante multidimensional e subjetivo do indivÃduo para análise de sua própria saúde, considerado um fator para evidências de declÃnio funcional e de mortalidade. O objetivo deste estudo foi verificar a associação da autopercepção do envelhecimento e autopercepção da saúde. Trata-se de um estudo com delineamento transversal, descritivo e de associação. A amostra foi constituÃda de 224 indivÃduos com idade igual ou superior a 60 anos, de ambos os sexos, sendo 64,4% indivÃduos femininos e 35,5% masculinos, todos residentes no Distrito Federal. A coleta de dados foi realizada por meio de questionário sociodemográfico e pelo Questionário de Autopercepção do Envelhecimento (QAPE), versão brasileira. Foram realizadas análises descritivas, comparativas de grupo e correlações. Verificou-se que não ocorreu diferença estatÃstica significativa entre a autopercepção do envelhecimento e a autopercepção da saúde, contudo, os dados demonstram que quanto melhor a autopercepção da saúde, melhor a autopercepção do envelhecimento
Can clinical and physical-functional factors predict falls in cognitively impaired older adults?
Objetivo: Investigar a frequência de quedas, os fatores clÃnicos e fÃsico-funcionais associados
às quedas e à acurácia desses fatores para identificar risco de cair em idosos com deficit
cognitivo. Método: Estudo transversal com idosos com deficit cognitivo identificado no
Mini-Exame do Estado Mental. Variável dependente: histórico de queda nos 6 meses
anteriores. Variáveis independentes: autorrelato de confusão mental, deficiência auditiva
e visual, cansaço fÃsico, fraqueza muscular, tontura, desequilÃbrio corporal, insegurança
para caminhar, diagnóstico de depressão, internações, quantidade de medicamentos de
uso contÃnuo (formulário de pesquisa), massa muscular (circunferência de panturrilha),
força de preensão palmar (dinamometria), capacidade funcional (Pfeffer) e mobilidade
corporal (SAM-Br). Foi realizada estatÃstica descritiva. Os grupos foram comparados
com teste U Mann Whitney, os fatores de risco identificados por regressão logÃstica uni
e multivariada e a área sob a curva ROC (AUC) foi calculada para fatores associados.
Resultados: 216 idosos com deficit cognitivo foram incluÃdos nas análises, dos quais 41,7%
eram caidores. Análises de regressão multivariada indicaram que a queixa de deficit visual
(OR=2,8; p=0,015) e de desequilÃbrio corporal (OR=2,7; p=0,004) e a maior quantidade
de medicamentos (OR=1,1; p=0,038) associaram-se às quedas. A AUC verificou fraca
acurácia da quantidade de medicamentos para rastrear caidores (AUC=0,6 [0,5; 0,7];
p=0,028). Conclusão: os idosos com deficit cognitivo apresentaram alta frequência de quedas.
As queixas de deficit visual, desequilÃbrio corporal e polifarmácia foram preditoras de
quedas. A rápida investigação desses fatores pode contribuir para a identificação de risco
de cair de idosos com deficit cognitivo na prática clÃnica e em pesquisas.Objective: to investigate the frequency of falls and clinical and physical-functional factors
associated with falls, and the accuracy of such factors to identify the risk of falling
in cognitively impaired older adults. Method: a cross-sectional study with cognitively
impaired older adults was carried out using the Mini-Mental State Examination. The
dependent variable was a history of falls in the previous six months. The independent
variables were self-reported mental confusion, hearing and visual impairment, physical
fatigue, muscle weakness, dizziness, body imbalance, insecurity when walking, diagnosed
depression, hospitalizations, continuous use medications (form from study used), muscle
mass (calf circumference) and handgrip strength (dynamometry), functional capacity
(Pfeffer) and mobility (SAM-Br). Descriptive statistics were applied. The groups were
compared using the Mann Whitney U test, the risk factors were identified by univariate
and multivariate logistic regression, and the area under the ROC curve (AUC) was
calculated for the associated factors. Results: 216 cognitively impaired older adults were
included in the analysis, 41.7% of whom were fallers. Multivariate regression analyzes
indicated that complaints of visual impairment (OR=2.8; p=0.015) and body imbalance
(OR=2.7; p=0.004), and greater medication use (OR=1.1; p=0.038) were associated with a
history of falls. The AUC found poor accuracy for quantity of medications as a screening
tool for fallers (AUC=0.6 [0.5; 0.7]; p=0.028). Conclusion: cognitively impaired older
adults had a high frequency of falls. Complaints of visual impairment, body imbalance
and polypharmacy were predictors of falls. The early assessment of these factors can
contribute to the identification of cognitively-impaired older adults at risk of falling in
clinical practice and research
Safety issues of raw milk: evaluation of bacteriological and physicochemical characteristics of human milk from a bank in a teaching hospital, focusing on Staphylococcus species
Many infants are nurtured with milk supplied by human banks, whose bacteriological and physical-chemical profiles are a major issue. We investigated the bacteriological and physical-chemical characteristics, as well as genotypic and phenotypic and profiles of Staphylococcus species isolated from 240 samples of breast milk from a bank in a teaching hospital. Dornic acidity of milk revealed that 95.4% (229/240) had acceptable limits (< 8.0 oD). Caloric intake showed a wide variation in cream content (4%), fat (4%) and energy values (559.81 Kcal/L). Staphylococcus (105/186 or 56.5%) and Enterobacter (25/186 or 13.4%) were the most prevalent genera, although other microorganisms were identified, including Klebsiella pneumoniae and Pseudomonas aeruginosa. Amoxicillin/clavulanic acid (125/157 or 79.6%), vancomycin (115/157 or 73.2%), and cephalexin (112/157 or 71.3%) were the most effective antimicrobials. High resistance rates of isolates were found to penicillin G (141/157 or 89.8%), ampicillin (135/157 or 86%), and oxacillin (118/157 or 75.2%). Multidrug resistance to ≥ 3 antimicrobials occurred in 66.2% (123/186) of the isolates. Residues of microbial multiplication inhibitory substances were found in 85% (204/240) of samples. Among the coagulase-positive-CPS and negative-CoNS staphylococci, the mecA gene was detected in 53.3% (8/15) and 75% (30/40), respectively. Genes sea, seb and sec were detected in 20% (3/15) of CPS, while tsst-1 was detected in 13.34% (2/15). In addition, 13.3% (2/15) of S. aureus were toxin-producers. Genes sea, seb and sec were detected in 90% (36/40), 5% (2/40) and 15% (6/40) CoNS, respectively. Enterotoxin production was identified in 5% (2/40) of CoNS. The identification of multidrug-resistant bacteria, staphylococci species toxin-producers harboring methicillin-resistance genes, and residues of microbial multiplication inhibitory substances reinforce the need for a continuous vigilance of milk quality offered to infant consumption by human banks
