    Experimental Assessment of a Skyhook Semiactive Strategy for Seismic Vibration Control of a Steel Structure

    Sky-hook damping is one of the most promising techniques for feedback control of structural vibrations. It is based on the idea of connecting the structure to an ideal fixed point of the space through passive dissipative devices. Herein the benefit of semiactive (SA) sky-hook (SH) damping is investigated for seismic protection of a two-storey steel frame via shaking table tests. This kind of SA control is achieved implementing a continuous monitoring of selected structural response parameters and using variable dampers. The damping properties of the latter are changed in real-time so as to make the force provided by the damper match the desired SH damping force as closely as possible. To this aim, two prototype magnetorheological dampers have been installed at the first level of the frame and remotely driven by a SH controller. The effectiveness of the control strategy is measured as response to reduction in terms of floor accelerations and interstory drift in respect to the uncontrolled configuration. Two different calibrations of the SH controller have been tested. The experimental results are deeply discussed in order to identify the optimal one and understand the motivations of its better performance

    Experimental characterization of tensile strength of steel and fibre rovings also under environmental conditioning

    Abstract The efficiency of the strengthening techniques by externally applied materials can be improved enhancing the debonding strength of the reinforcement from the support by the use of connectors (anchor spikes) consisting of unidirectional bundles of fibres embedded in concrete or masonry by means of organic or inorganic matrices. The use of connectors is suggested in various codes and guidelines of strengthening techniques by composite materials and provisions for their application are given, but currently there are no details for the qualification of the material. In order to investigate anchor spikes made of glass, basalt, aramid, carbon, PBO and steel, a large experimental campaign was carried out at the Materials and Structures Laboratory of the University of Sannio. The tests allowed to evaluate the mechanical characteristics (tensile strength, modulus of elasticity, deformation at the maximum load) of the anchor spikes constituted by only dry fibres, not impregnated, also as a result of environmental conditioning such as freezing and thawing, controlled humidity, alkaline and saline environment

    Residual Strength and Durability of Glass Fiber FRCM and CRM Systems Aged in Alkaline Environments

    Fabric Reinforced Cementitious Matrix (FRCM) and Composite Reinforced Mortar (CRM) systems are used as Externally Bonded Reinforcements (EBR) in civil and historical construction. These materials are made by fibrous reinforcement, in forms of dry (FRCM) or cured (CRM) meshes embedded in a cementitious/hydraulic lime matrix. At present, this technology is considered very promising in the field of structural strengthening, retrofitting and repair existing structures. This is true especially in those cases of masonry and historical buildings, due to the specific criteria of conservation and compatibility with the substrate that need to be fulfilled. These materials, in fact, results more compatible with masonry substrate because of the inorganic matrix, instead of polymeric resin used for the well-known FRP systems (Fiber Reinforced Polymers). The recent use of these new materials in civil engineering needs appropriate and complete guidelines, that regard not only the design aspects but also the durability features. This paper presents the results of a large experimental program focused on the durability of FRCM and CRM systems and their single components, in different alkaline environments. For the whole experimental campaign, the samples have been immersed into three different alkaline solutions, for four exposure times (500, 1000, 2000 and 3000 hrs). In addition, in order to study the different accelerating effects due to temperature, three different temperatures were maintained during the ageing periods: 23°C, 40°C and 70°C. The results about the mechanical characterization of residual properties are discussed in order to highlight the influence of alkaline environments on the mechanical properties of single elements and the whole strengthening systems that were tested herein

    On the phase structure of five-dimensional SU(2) gauge theories with anisotropic couplings

    The phase diagram of five-dimensional SU(2) gauge theories is explored using Monte Carlo simulations of the theory discretized on a Euclidean lattice using the Wilson plaquette action and periodic boundary conditions. We simulate anisotropic gauge couplings which correspond to different lattice spacings a_4 in the four dimensions and a_5 along the extra dimension. In particular we study the case where a_5>a_4. We identify a line of first order phase transitions which separate the confined from the deconfined phase. We perform simulations in large volume at the bulk phase transition staying in the confined vacuum. The static potential measured in the hyperplanes orthogonal to the extra dimension hint at dimensional reduction. We also locate and analyze second order phase transitions related to breaking of the center along one direction.Comment: 28 pages, 22 figures, 4 tables; few explanations and references added; version accepted for publication in Nucl.     Emergency surgery in a tertiary hospital: The covid-19 experience

    Objective: Evaluate the impact of Covid-19 in a critical area and analyze the changes in the daily activities in an Emergency Department of a tertiary COVID-Hospital. Methods: We reported and compared the surgical procedures performed for acute appendicitis and acute cholecystitis between two periods (March and April 2019 and March and April 2020) at Emergency Surgery Department of Parma University Hospital, a tertiary COVID-Hospital. Results: A total of 72 patients underwent surgery between March and April 2019 and 36 between March and April 2020 for acute appendicitis and cholecystitis. The average length of stay was comparable in the two considered years for LA (4.23±1.69 days in 2019 versus 4.5±2.33 days in 2020). The average length of stay in patients with acute cholecystitis was 5.9±3.8 days in 2019 and 8.23±5.5 days in 2020(P=0.038). The average hospitalization was comparable with 2019 data (5.62±3.77 days)n March 2020;whereas, April 2020 was statistically significantly longer 10.5±6 days (P= 0.023). Conclusions: Half of the emergency centers reported a drop in the overall number of urgent cases as confirmed in our department, where the number of total appendicectomy and cholecystectomy was halved comparing the two months in 2019 and 2020, 72 operations in 2019 versus 36 operations in 2020.During the pandemic, the confidence of the population to the healthcare systems was poor, and this can explain the delayed access to the emergency department of patients who suffered from an acute illness.     MEK1/2 regulate normal BCR and ABL1 tumor-suppressor functions to dictate ATO response in TKI-resistant Ph+ leukemia

    Resistance to tyrosine kinase inhibitors (TKIs) remains a clinical challenge in Ph-positive variants of chronic myeloid leukemia. We provide mechanistic insights into a previously undisclosed MEK1/2/BCR::ABL1/BCR/ABL1-driven signaling loop that may determine the efficacy of arsenic trioxide (ATO) in TKI-resistant leukemic patients. We find that activated MEK1/2 assemble into a pentameric complex with BCR::ABL1, BCR and ABL1 to induce phosphorylation of BCR and BCR::ABL1 at Tyr360 and Tyr177, and ABL1, at Thr735 and Tyr412 residues thus provoking loss of BCR's tumor-suppression functions, enhanced oncogenic activity of BCR::ABL1, cytoplasmic retention of ABL1 and consequently drug resistance. Coherently, pharmacological blockade of MEK1/2 induces dissociation of the pentameric MEK1/2/BCR::ABL1/BCR/ABL1 complex and causes a concurrent BCRY360/Y177, BCR::ABL1Y360/Y177 and cytoplasmic ABL1Y412/T735 dephosphorylation thereby provoking the rescue of the BCR's anti-oncogenic activities, nuclear accumulation of ABL1 with tumor-suppressive functions and consequently, growth inhibition of the leukemic cells and an ATO sensitization via BCR-MYC and ABL1-p73 signaling axes activation. Additionally, the allosteric activation of nuclear ABL1 was consistently found to enhance the anti-leukemic effects of the MEK1/2 inhibitor Mirdametinib, which when combined with ATO, significantly prolonged the survival of mice bearing BCR::ABL1-T315I-induced leukemia. These findings highlight the therapeutic potential of MEK1/2-inhibitors/ATO combination for the treatment of TKI-resistant leukemia

    Long-term home ventilation of children in Italy: A national survey.

    BACKGROUND: Improved technology, as well as professional and parental awareness, enable many ventilator-dependent children to live at home. However, the profile of this growing population, the quality and adequacy of home care, and patients' needs still require thorough assessment. OBJECTIVES: To define the characteristics of Italian children receiving long-term home mechanical ventilation (HMV) in Italy. METHODS: A detailed questionnaire was sent to 302 National Health Service hospitals potentially involved in the care of HVM in children (aged <17 years). Information was collected on patient characteristics, type of ventilation, and home respiratory care. RESULTS: A total of 362 HMV children was identified. The prevalence was 4.2 per 100,000 (95% CI: 3.8-4.6), median age was 8 years (interquartile range 4-14), median age at starting mechanical ventilation was 4 years (1-11), and 56% were male. The most frequent diagnostic categories were neuromuscular disorders (49%), lung and upper respiratory tract diseases (18%), hypoxic (ischemic) encephalopathy (13%), and abnormal ventilation control (12%). Medical professionals with nurses (for 62% of children) and physiotherapists (20%) participated in the patients' discharge from hospital, though parents were the primary care giver, and in 47% of cases, the sole care giver. Invasive ventilation was used in 41% and was significantly related to young age, southern regional residence, longer time spent under mechanical ventilation, neuromuscular disorders, or hypoxic (ischemic) encephalopathy. CONCLUSIONS: Care and technical assistance of long-term HMV children need assessment, planning, and resources. A wide variability in pattern of HMV was found throughout Italy. An Italian national ventilation program, as well as a national registry, could be useful in improving the care of these often critically ill children

    Antidepressant and antipsychotic use in an Italian pediatric population

    <p>Abstract</p> <p>Background</p> <p>The safety and effectiveness of psychotropic drug use in the paediatric population is widely debated, in particular because of the lack of data concerning long term effects.</p> <p>In Italy the prevalence of psychotropic drug prescriptions increased in the early 2000s and decreased afterwards. In such a context, a study with the aim to estimate the incidence and prevalence of psychotropic drug prescription in the paediatric population and to describe diagnostic and therapeutic approaches was performed.</p> <p>Methods</p> <p>The study population was composed of 76,000 youths less than 18 years and living in the area covered by the local health unit of Verona, Italy. The data source was the Verona local health unit's administrative prescription database. Prevalence and incidence of antidepressant and/or antipsychotic drug prescriptions in the 2004-2008 period were estimated. Children and adolescents receiving antidepressant and/or antipsychotic drug prescriptions between 1 January 2005 and 31 December 2006 were identified and questionnaires were sent to the prescribers with the aim to collect data concerning diagnostic and therapeutic approaches, and care strategies.</p> <p>Results</p> <p>The prevalence of psychotropic drug prescriptions did not change in the 2004-2008 period, while incidence slightly increased (from 7.0 in 2005 to 8.3 per 10,000 in 2008). Between 1 January 2005 and 31 December 2006, 111 youths received at least one psychotropic drug prescription, 91 of whom received antidepressants. Only 28 patients attended child and adolescent psychiatry services. Information concerning diagnostic and therapeutic approaches, and care strategies was collected for 52 patients (47%). Anxiety-depressive syndrome and attention disorders were the diseases for which psychotropic drugs were most commonly prescribed. In all, 75% youths also received psychological support and 20% were prescribed drugs for 2 or more years.</p> <p>Conclusions</p> <p>Despite the low drug prescription prevalence, the finding that most children were not cared for by child and adolescent psychiatric services is of concern and calls for a systematic, continuous monitoring of psychopharmacological treatments.</p

    Ocular medicines in children: the regulatory situation related to clinical research

    <p>Abstract</p> <p>Background</p> <p>Many ocular medications are prescribed for paediatric patients, but the evidence for their rational use is very scant. This study was planned to compare the availability and the licensing status of ocular medications marketed in Italy, the United Kingdom (UK), and the United States of America (USA) related to the amount of published and un-published RCTs testing these drugs in the paediatric population.</p> <p>Methods</p> <p>A quantitative analysis was performed to evaluate the number of ocular medications with a paediatric license in Italy, the UK, and the USA. A literature search was also performed in MEDLINE, EMBASE, and The Cochrane Central Register of Controlled Trials for randomized controlled trials (RCTs) on ophthalmic pharmacological therapy in children aged < 18 years, published up to December 2010. A search in the international clinical trial registries, the list of paediatric investigation plans (PIPs) approved by European Medicines Agency (EMA), and the table of medicines with new paediatric information approved by Food and Drug Administration (FDA) was also performed.</p> <p>Results</p> <p>In all, of 197 drugs identified, 68 (35%) single drugs are licensed for paediatric use at least in one considered country, while 23 (12%) were marketed in all three countries. More specifically, in Italy 43 single drugs (48% of those marketed) had a paediatric license, while 39 (64%) did in the UK and 22 (54%) did in the USA. Only 13 drugs were marketed with a paediatric license in all countries.</p> <p>The percentage of drugs licensed for paediatric use and for which at least one RCT had been performed ranged between 51% in Italy and 55% in the USA. No published RCTs were found for 11 (48%) drugs licensed for paediatric use in all three countries. In all, 74 (35%) of the retrieved RCTs involved mydriatic/cycloplegic medications.</p> <p>A total of 62 RCTs (56% completed) on 46 drugs were found in the international clinical trial registries. Cyclosporin and bevacizumab were being studied in many ongoing trials. Twenty-six drugs had new paediatric information approved by FDA based on new paediatric clinical trials, while only 4 PIPs were approved by EMA.</p> <p>Conclusions</p> <p>There is a pressing need for further research and clinical development in the pediatric ophthalmic area, where effective up-to-date treatments, and additional research and education on use in children, remain priorities.</p
