    Bajo la hipótesis de que los parámetros nutricionales influyen en el desarrollo perioperatorio de la cirugía de cadera realizamos un estudio prospectivo observacional de tipo clínico sobre 100 pacientes con una edad media de 76 años. Realizamos una evaluación clínica al ingreso: talla, peso, pliegue cutáneo, circunferencia braquial y muscular. Medición de proteínas plasmáticas y linfocitos totales al ingreso, en el postoperatorio y cada 7 días. Definimos los pacientes según los valores nutricionales analíticos y clínicos. El 51% de nuestros pacientes presentaban desnutrición caloricoproteica. La media transfusional en malnutridos fue 600 c.c. frente a 350 c.c. del grupo nutrido (p < 0,05). El 73% de las complicaciones se presentaron en pacientes malnutridos. No hallamos diferencias en los períodos de sedestación y deambulación, aunque sí en la estancia hospitalaria, 15 días frente a 18,5 días (p < 0,01). La prealbúmina mostró variaciones de forma más precoz ante el acto quirúrgico.A prospective observation was conducted with the hypothesis that the nutritional situation has influence in postoperative development of hip surgery [n = 100; age 76 years (55-95)]. A nutritional evaluation was performed before surgery through the evolution of height, body weight, skin fold and braquial and muscular circunference. Serie proteins and lymphocytes levels were measured during hospital admission, in postoperative and each seven days. A 51% of patients showed protein-energy malnutrition (PEM). Transfusional haematic requirements were 600 c c . in malnutrition patients and 350 c.c. in the rest (p < 0.05). Complications appeared in PEM patients in 73% cases. The hospital stay was longer in those patients who presented PEM upon admittance (p < 0.01). Postoperatively, the prealbumin was the protein whose serie level had faster changes

    Effectiveness of the 23-valent polysaccharide pneumococcal vaccine against invasive pneumococcal disease in people 60 years or older

    <p>Abstract</p> <p>Background</p> <p>The 23-valent polysaccharide pneumococcal vaccine (PPV) is currently recommended in elderly and high-risk adults. However, its efficacy in preventing pneumococcal infections remains controversial. This study assessed the clinical effectiveness of vaccination against invasive pneumococcal disease (IPD) among people over 60 years.</p> <p>Methods</p> <p>Population-based case-control study that included 88 case patients over 60 years-old with a laboratory-confirmed IPD (bacteraemic pneumonia, meningitis or sepsis) and 176 outpatient control subjects who were matched by primary care centre, age, sex and risk stratum. Adjusted odds ratios (ORs) for vaccination were calculated using conditional logistic regression, controlling for underlying conditions. Vaccine effectiveness was estimated as (1 - OR) ×100.</p> <p>Results</p> <p>Pneumococcal vaccination rate was significantly lower in cases than in control subjects (38.6% <it>vs </it>59.1%; p = 0.002). The adjusted vaccine effectiveness was 72% (OR: 0.28; 95% CI: 0.15-0.54) against all IPD and 77% (OR: 0.23; 95% CI: 0.08-0.60) against vaccine-type IPD. Vaccination was significantly effective against all IPD in both age groups: 60-79 years-old (OR 0.32; 95% CI: 0.14-0.74) and people 80 years or older (OR: 0.29; 95% CI: 0.09-0.91). Vaccination appears significantly effective as for high-risk immunocompetent subjects (OR: 0.29; 95% CI: 0.11-0.79) as well as for immunocompromised subjects (OR: 0.12; 95% CI: 0.03-0.53).</p> <p>Conclusion</p> <p>These findings confirm the effectiveness of the 23-valent PPV against IPD, and they also support the benefit of vaccination in preventing invasive infections among high-risk and older people.</p

    Relationship between solar radiation on watercore on apple fruit assessed with MRI

    This work is a preliminary studio of the possibility of assess a relationship between solar radiation and watercore development on apple fruit, during maturation, using a non destructive method such as Magnetic Resonance Imaging (MRI). For such purpose, several low cost solar radiation sensors were designed for the trial and placed at 2 different heights (1.5 and 2.5 m) on 6 adult ?Esperiega? apple trees, in a commercial orchard in Ademuz (Valencia). Sensors were connected along 27 days, during the end of the growth period and start of the fruit maturation process, and radiation measurements of the a-Si sensors were recorded every 1 minute. At the end of this period, fruits from the upper and the lower part of the canopy of each tree were harvested. In all, 152 apples were collected and images with MRI. A Principal Component Analysis, perfomed over the histograms of the images, as well as segmentation methods were performed on the MR images in order to find a pattern involving solar radiation and watercore incidence

    Assessment of watercore development in apples with MRI: Effect offruit location in the canopy

    tWatercore distribution inside apple fruit (block or radial), and its incidence (% of tissue) were relatedto the effect of solar radiation inside the canopy as measured by a set of low-cost irradiation sensors.221 samples were harvested in two seasons from the top and the bottom of the canopy and submittedto the non-invasive and non-destructive technique of magnetic resonance imaging (MRI) in order toobtain 20 inner tomography slices from each fruit and analyze the damaged areas using an interactive3D segmentation method. The number of fruit corresponding to each type of damage and the relevantpercentage were calculated and it was found that apples from the top of the tree were mainly of the radialtype (84%) and had more watercore (approx. 5% more) than apples from the bottom (65% radial). From theimage segmentation, the Euler number, a morphometric parameter, was extracted from the segmentedimages and related to the type of watercore symptoms. Apples with block watercore were grouped inEuler numbers between −400 and 400 with a small evolution. For apples with radial development, theEuler number was highly negative: up to −1439. Significant differences were also found regarding sugarcomposition, with higher fructose and total sugar contents in apples from the upper canopy, compared tothose in the lower canopy location. In the seasons studied (2011 and 2012), significantly higher sorbitoland lower sucrose and fructose contents were found in watercore-affected tissue compared to the healthytissue of affected apples and also compared to healthy apples

    Ghrelin induces clock gene expression in the liver of goldfish in vitro via protein kinase C and protein kinase A pathways

    International audienceThe liver is the most important link between the circadian system and metabolism. As a food-entrainable oscillator, the hepatic clock needs to be entrained by food-related signals. The objective of the present study was to investigate the possible role of ghrelin (an orexigenic peptide mainly synthesized in the gastrointestinal tract) as an endogenous synchronizer of the liver oscillator in teleosts. To achieve this aim, we first examined the presence of ghrelin receptors in the liver of goldfish. Then, the ghrelin regulation of clock gene expression in the goldfish liver was studied. Finally, the possible involvement of the phospholipase C/ protein kinase C (PLC/ PKC) and adenylate cyclase/protein kinase A (AC/PKA) intracellular signalling pathways was investigated. Ghrelin receptor transcripts, ghs-r1a, are present in the majority of goldfish hepatic cells. Ghrelin induced the mRNA expression of the positive (gbmal1a, gclock1a) and negative (gper genes) elements of the main loop of the molecular clock machinery, as well as grev-erba (auxiliary loop) in cultured liver. These effects were blocked, at least in part, by a ghrelin antagonist. Incubation of liver with a PLC inhibitor (U73122), a PKC activator (phorbol 12-myristate 13-acetate) and a PKC inhibitor (chelerythrine chloride) demonstrated that the PLC/ PKC pathway mediates such ghrelin actions. Experiments with an AC activator (forskolin) and a PKA inhibitor (H89) showed that grev-erba regulation could be due to activation of PKA. Taken together, the present results show for the first time in vertebrates a direct action of ghrelin on hepatic clock genes and support a role for this hormone as a temporal messenger in the entrainment of liver circadian functions

    10 kg scaled-up preparation of Al/Fe-pillared clay CWPO catalysts from concentrated precursors

    In this work, the significant intensification of a bentonite pillaring process was achieved by using a novel methodological approach, leading to an intercalating Al/Fe mixed oligomeric precursor, around 100 times more concentrated than usually reported. In addition, the intercalating step was achieved directly on the clay with no previous swelling of the mineral being required; this allowed the successful scaled-up preparation of the Al/Fe-PILC, by a factor of one thousand, from the lab (10 g) to the pilot scale (10 kg). Intercalating solutions prepared under either oncentrated (13 cm3) or diluted (widely reported, 2.0 dm3) conditions for lab-scale preparations were both translucent, displaying similar final pH values (close to 4.0) typical of highly oligomerized Al-pillaring solutions. The clay modified from concentrated precursors at the 10 g scale reached a high basal spacing (18.3 Å) and specific surface area (198 m2 g−1 ) with very comparable fractions of Fe forming truly mixed Al/Fe pillars in comparison to a reference material (H2-TPR analyses). This promoted high performance in the catalytic wet peroxide oxidation of phenol in aqueous solution as a toxic model organic molecule at very mild temperature (25.0 °C ± 1.0 °C) and pressure (76 kPa), exhibiting the highest catalytic efficiency as a function of both parameters (full conversion of phenol together with 45.2% of TOC mineralization) with low iron leaching using a very low catalyst concentration (0.25 g dm−3). Particle size refining of the starting clay, the speed of stirring and conditions for the final washing of the interlayered precursor are the main factors influencing successful pillaring at scales higher than 1.0 kg

    Burden of multimorbidity, socioeconomic status and use of health services across stages of life in urban areas: a cross-sectional study

Please cite the published version which is available via the DOI link in this record.Background The burden of chronic conditions and multimorbidity is a growing health problem in developed countries. The study aimed to determine the estimated prevalence and patterns of multimorbidity in urban areas of Catalonia, stratified by sex and adult age groups, and to assess whether socioeconomic status and use of primary health care services were associated with multimorbidity. Methods A cross-sectional study was conducted in Catalonia. Participants were adults (19+ years) living in urban areas, assigned to 251 primary care teams. Main outcome: multimorbidity (≥2 chronic conditions). Other variables: sex (male/female), age (19–24; 25–44; 45–64; 65–79; 80+ years), socioeconomic status (quintiles), number of health care visits during the study. Results We included 1,356,761 patients; mean age, 47.4 years (SD: 17.8), 51.0% women. Multimorbidity was present in 47.6% (95% CI 47.5-47.7) of the sample, increasing with age in both sexes but significantly higher in women (53.3%) than in men (41.7%). Prevalence of multimorbidity in each quintile of the deprivation index was higher in women than in men (except oldest group). In women, multimorbidity prevalence increased with quintile of the deprivation index. Overall, the median (interquartile range) number of primary care visits was 8 (4–14) in multimorbidity vs 1 (0–4) in non-multimorbidity patients. The most prevalent multimorbidity pattern beyond 45 years of age was uncomplicated hypertension and lipid disorder. Compared with the least deprived group, women in other quintiles of the deprivation index were more likely to have multimorbidity than men until 65 years of age. The odds of multimorbidity increased with number of visits in all strata. Conclusions When all chronic conditions were included in the analysis, almost 50% of the adult urban population had multimorbidity. The prevalence of multimorbidity differed by sex, age group and socioeconomic status. Multimorbidity patterns varied by life-stage and sex; however, circulatory-endocrine-metabolic patterns were the most prevalent multimorbidity pattern after 45 years of age. Women younger than 80 years had greater prevalence of multimorbidity than men, and women’s multimorbidity prevalence increased as socioeconomic status declined in all age groups.     Simulations of micro-sphere/shell 2D silica photonic crystals for radiative cooling

One technique to increase the radiative cooling performance is to engineer the surface of a polar dielectric material to enhance its emittance atwavelengths in the atmospheric infrared transparency window (8-13 ìm) by outcoupling surface-phonon polaritons (SPhPs) into free-space. Here we present a theoretical investigation of new surface morphologies based upon self-assembled silica photonic crystals (PCs) using an in-house built rigorous coupled-wave analysis (RCWA) code. Simulations predict that silica micro-sphere PCs can reach up to 73 K below ambient temperature, when solar absorption and conductive/convective losses can be neglected. Micro-shell structures are studied to explore the direct outcoupling of the SPhP, resulting in near-unity emittance between 8 and 10 ìm. Additionally, the effect of material composition is explored by simulating soda-lime glass micro-shells, which, in turn, exhibit a temperature reduction of 61 K below ambient temperature.
