    Fast electrophoresis in conventional capillaries by employing a rapid injection device and contactless conductivity detection

    A purpose-made set-up featuring an automated fast injector allowed the easy optimization of the injected amount and the adjustment of the separation length of conventional capillaries from a minimum of 5cm upward. It was found that a compromise in capillary length for separation efficiency and analysis time also has to take into account the injected amount, which in turn affects the sensitivity and hence the detection limit. The versatility of the system was demonstrated by the analysis of the major cations and anions in natural water samples in less than 1min, the concurrent determination of a mixture of amino acids and carbohydrates in 160s, and of three active ingredients in a pharmaceutical preparation in 40s. Plate numbers were typically around 50,000 and detection limits down to 1μM could be achieve

    Improper Supplementation Habits of Folic Acid Intake by Hungarian Pregnant Women: Improper Recommendations

    Background: Neural tube defects (NTDs) are some of the most common congenital anomalies. Proper folic acid supplementation is a dominant risk factor, which has been shown to decrease the incidence of NTDs. In Canada, the incidence of neuroblastoma has presented a considerable decrease of 60% as a result of enrichment cereal grain flours with synthetic folic acid. The aim of this study was to investigate the effect of folic acid intake by pregnant women on the incidence of NTDs and neuroblastoma. Methods: Regular folic acid intake has been recommended to pregnant women in Hungary since the eighties of the last century by health visitors eventually raking effect as an official protocol which had been released in 1997. During 2001, 2002 and 2003. folic acid intake habits of pregnant women were evaluated by health visitors, proving to be successful in collecting data front 95.06% of the pregnant women. The incidence of NTDs has been registered by the Hungarian National Centre of Epidemiology, Department of Human Genetics and Teratology. The Pediatric Cancer Registry provided the incidence of neuroblastoma in children. Results: Consistent findings revealed a regular intake of supplementary folic acid products by 68.71% of the pregnant women. Out of these. 93.13% of pregnant women who were taking folic acid, started the supplementation after their 7 weeks of pregnancies, a time designated as the completion period of the development of the neural tube. The dose of folic acid supplementation was evaluated as less than 5 mg/day in 84.75% of the pregnant women. In Hungary, the incidence of NTDs has remained constant, while the incidence of neuroblastoma has shown constant slight increase in spite of the introduction of folic acid supplementation in 1997. Conclusions: Based on our experience, folic acid supplementation was initiated after the recognition of pregnancy and its application in a dose of lower than 5 mg/day neither decreased the incidence of NTDs nor did it have an effect on the neuroblastoma incidence. It is implicated that proper folic acid supplementation, which is started front the conception. can be achieved only with the enrichment of cereal grain flours

    Magyarországon évente kb. 7-9 új Ewing szarkómás gyermeket diagnosztizálunk. Jelen munkánk célja az volt, hogy az Országos Gyermektumor Regiszter adatait felhasználva megvizsgáljuk a magyarországi Ewing szarkómás gyermekek prezentációs tüneteit, a klinikai paramétereket, a prognosztikai faktorokat, a terápiás és a túlélési eredményeket 1992 és 2002 közötti 11 éves periódusban. A fenti időszakban 88 új beteg került diagnosztizálásra, a fiú – leány arány 1,05 : 1 volt, az átlagéletkor 11 év 7 hónapnak bizonyult. A két leggyakoribb prezentációs tünet a lokális fájdalom és duzzanat voltak. 38 betegünkben hosszú csöves csontra lokalizálódott a megbetegedés, 29 gyermekben a Ewing szarkóma a csípő tájékról indult ki és 21 esetben valamely egyéb testtájékról. A betegeink közel egyharmadánál (29/88) már a diagnózis felállításakor áttétek voltak kimutathatóak. A fent nevezett időszakban három kemoterápiás protokollt alkalmaztunk Magyarországon: a CWS, az EICESS/CESS és 1999. decembere óta az Euro-EWING 99 protokollt. A 88 beteg 37,5%-nál (33/88) észleltünk recidívát átlagosan 22,4 hónappal a primer diagnózis felállítása után. A 88 betegből 45 jelenleg is él, az átlag követési idő 28,6 hónap. Az összes beteg 5-éves kumulatív túlélési valószínűsége 48,06±5,9%, a 10 éves 42,91±6,3%. A metasztázissal rendelkező betegek 5 és 8 éves túlélése 19,91±9,4%, a metasztázis nélküliek 5 éves túlélése 60,23±6,9%, míg a 10 éves 52,82±7,8%. A hazai eredmények megközelítik a nemzetközi adatokat, azonban törekednünk kell a diagnózis korai felállítására és ezáltal a kimondottan rossz prognózisú primer metasztatikus esetek számának csökkentésére. The number of newly diagnosed children in a year with Ewing’s sarcoma is 7-9 in Hungary. The aim of our study was to evaluate the presenting symptoms, clinical features, prognostic risk factors and treatment results of children’s Ewing’s sarcoma in Hungary using data from the National Childhood Cancer Registry in a 11 years-long period between 1992 and 2002. In this period, 88 new patients were diagnosed, the male-female ratio was 1,05:1 and the mean age was 11 years 7 months. The two most common presenting symptoms were local pain and swallowing. Tumor was located in the pelvis area in 29 patients, in the extremities in 38 and other sites in 21 cases. Almost one third of our patients (29/88) had metastasis at the time of the diagnosis. In this time period, three different protocols were used for treatment: CWS, EICESS/CESS and since December 1999 Euro-EWING 99. 37,5% of our patients (33/88) had relapse with a mean of 22,4 months after the diagnosis. 45 children are still alive, the median follow-up time is 28,6 months. The overall survival (OS) of all patients (n=88) was 48,06±5,9% at 5 years and 42,91±6,3% at 10 years. Patients with metastasis had OS 19,91±9,4% at 5 and at 8 years. The 5-year OS of children without metastasis was 60,23±6,9%, and 52,82±7,8% at 10 years. The Hungarian data are similar to international results, but we have to try to decrease the number of the primary metastatic cases with early diagnosis

    Dissection of the Bradyrhizobium japonicum NifA+σ54 regulon, and identification of a ferredoxin gene ( fdxN ) for symbiotic nitrogen fixation

    Hierarchically organized regulatory proteins form a complex network for expression control of symbiotic and accessory genes in the nitrogen-fixing soybean symbiont Bradyrhizobium japonicum. A genome-wide survey of regulatory interactions was made possible with the design of a custom-made gene chip. Here, we report the first use of the microarray in a comprehensive and complete characterization of the B. japonicum NifA+σ54 regulon which forms an important node in the entire network. Comparative transcript profiles of anaerobically grown wild-type, nifA, and rpoN 1/2 mutant cells were complemented with a position-specific frequency matrix-based search for NifA- and σ54-binding sites plus a simple operon definition. One of the newly identified NifA+σ54-dependent genes, fdxN, encodes a ferredoxin required for efficient symbiotic nitrogen fixation, which makes it a candidate for being a direct electron donor to nitrogenase. The fdxN gene has an unconventional, albeit functional σ54 promoter with the dinucleotide GA instead of the consensus GC motif at position −12. A GC-containing mutant promoter and the atypical GA-containing promoter of the wild type were disparately activated. Expression analyses were also carried out with two other NifA+σ54 targets (ectC; ahpC). Incidentally, the tiling-like design of the microarray has helped to arrive at completely revised annotations of the ectC- and ahpC-upstream DNA regions, which are now compatible with promoter locations. Taken together, the approaches used here led to a substantial expansion of the NifA+σ54 regulon size, culminating in a total of 65 genes for nitrogen fixation and diverse other processe

    Hematological Changes in Women and Infants Exposed to an AZT-Containing Regimen for Prevention of Mother-to-child-transmission of HIV in Tanzania.

    Tanzanian guidelines for prevention of mother-to-child-transmission of HIV (PMTCT) recommend an antiretroviral combination regimen involving zidovudine (AZT) during pregnancy, single-dosed nevirapine at labor onset, AZT plus Lamivudine (3TC) during delivery, and AZT/3TC for 1-4 weeks postpartum. As drug toxicities are a relevant concern, we assessed hematological alterations in AZT-exposed women and their infants. A cohort of HIV-positive women, either with AZT intake (n = 82, group 1) or without AZT intake (n = 62, group 2) for PMTCT during pregnancy, was established at Kyela District Hospital, Tanzania. The cohort also included the infants of group 1 with an in-utero AZT exposure ≥4 weeks, receiving AZT for 1 week postpartum (n = 41), and infants of group 2 without in-utero AZT exposure, receiving a prolonged 4-week AZT tail (n = 58). Complete blood counts were evaluated during pregnancy, birth, weeks 4-6 and 12. For women of group 1 with antenatal AZT intake, we found a statistically significant decrease in hemoglobin level, red blood cells, white blood cells, granulocytes, as well as an increase in red cell distribution width and platelet count. At delivery, the median red blood cell count was significantly lower and the median platelet count was significantly higher in women of group 1 compared to group 2. At birth, infants from group 1 showed a lower median hemoglobin level and granulocyte count and a higher frequency of anemia and granulocytopenia. At 4-6 weeks postpartum, the mean neutrophil granulocyte count was significantly lower and neutropenia was significantly more frequent in infants of group 2. AZT exposure during pregnancy as well as after birth resulted in significant hematological alterations for women and their newborns, although these changes were mostly mild and transient in nature. Research involving larger cohorts is needed to further analyze the impact of AZT-containing regimens on maternal and infant health

    Spirosoma linguale Migula 1894 is the type species of the genus. S. linguale is a free-living and non-pathogenic organism, known for its peculiar ringlike and horseshoe-shaped cell morphology. Here we describe the features of this organism, together with the complete genome sequence and annotation. This is only the third completed genome sequence of a member of the family Cytophagaceae. The 8,491,258 bp long genome with its eight plasmids, 7,069 protein-coding and 60 RNA genes is part of the Genomic Encyclopedia of Bacteria and Archaea project

    MHC Class I+/II− Dendritic Cells Induce Hapten-Specific Immune Responses In Vitro and In Vivo

    Activation requirements and biologic properties of hapten-specific, major histocompatibility complex class I-restricted CD8+ T lymphocytes are not fully understood. To address this issue, a novel CD45+/major histocompatibility complex class I+(H-2k)/II−/CD80+ dendritic cell line, termed 80/1, which is capable of stimulating naïve, allogeneic CD8+ but not CD4+ T cells in vitro, was derivatized with trinitrobenzenesulfonic acid and co-cultured for 4 d with syngeneic, naïve CD8+ T cells. Results obtained showed that trinitrophenyl-derivatized, but not underivatized 80/1 dendritic cells, can induce vigorous proliferation of CD8+ T cells. T-cell blasts generated in this fashion were able to lyse syngeneic, trinitrophenyl-derivatized targets but failed to lyse underivatized or trinitrophenyl-derivatized syngeneic, major histocompatibility complex class I− mutant cells or allogeneic targets. The ability of 80/1 dendritic cells to prime naïve, syngeneic T cells in vivo was tested in a contact hypersensitivity model. C3H/HeN mice were injected subcutaneously with identical numbers of (i) trinitro-phenyl-derivatized 80/1 dendritic cells; (ii) trinitro-phenyl-derivatized 80/1 dendritic cells fragmented by freeze-thawing cycles; (iii) trinitrophenyl-derivatized fibrosarcoma L929; and (iv) trinitrophenyl-derivatized lymphoma R1.1 cells. Whereas live trinitrophenyl-derivatized 80/1 dendritic cells were able to sensitize for contact hypersensitivity, killed hapten-derivatized 80/1 dendritic cells or control cells failed to do so. Thus, we conclude that 80/1 dedritic cells, when compared with major histocompatibility complex class I+ non-dendritic cells, can effectively prime naive, syngeneic CD8+ T cells for hapten-specific responses, probably due to their better costimulatory and migratory properties

    Implementation in nursing and midwifery. A scoping review

    Hintergrund: Evidenzbasiertes Wissen steht im Pflege- und Hebammenbereich zunehmend zur Verfügung, wird aber nicht automatisch in die Praxis umgesetzt. Implementationsforschung gewinnt daher immer grössere Bedeutung. Die international verfügbaren Forschungsergebnisse zu den Faktoren, die zum Gelingen einer Implementation beitragen, sind bislang noch nicht genügend aufbereitet worden. Gegenstand des Artikels: Im vorliegenden Scoping Review wird der Frage nachgegangen, welche Arbeiten zur gelungenen Vorbereitung, Durchführung und Aufrechterhaltung von Interventionen im Bereich der Pflege- und Hebammenarbeit vorliegen. Methode: In der für Scoping Reviews vorgesehenen, systematischen Vorgehensweise wurden einschlägige Datenbanken durchsucht, um Reviews und Studien zur Implementationsforschung im Bereich Pflege und Hebammenarbeit aufzufinden. Der Auswahlprozess, der zur Trefferliste führte, und die Charakteristika der eingeschlossenen Studien werden in Abbildungen und Tabellen strukturiert aufbereitet. Ergebnis: In vier Reviews und 38 Studien, die den Einschlusskriterien entsprechen, werden verschiedene Faktoren mit gelungener Implementation verbunden. Für die eingeschlossenen Studien werden folgende vier Kategorien aufgestellt: (1) Arbeitsplatzkultur mit 16 Nennungen, (2) Leitungskultur: 28 Nennungen, (3) Ressourcen: 4 Nennungen und (4) Training mit 22 Nennungen. Je nach Phase der Implementation (Vorbereitung, Durchführung oder Aufrechterhaltung) kommt ihnen eine etwas andere Bedeutung zu. Zusammenfassung: Das Review bietet eine Orientierung im Feld der Implementationsforschung und zeigt Design, Thema und Ergebnis der eingeschlossenen Studien auf. Nur wenige Studien berücksichtigen relevante Theorien und zeigen den Einfluss sowohl der Forschenden als auch der Zielgruppe auf den Implementationsprozess oder die genaue Beschreibung der Umstände, in denen die Implementation stattfindet, auf. Background: Evidence-based expertise in nursing and midwifery is growing but is not automatically turned into practice. The importance of implementation research is therefore increasing. International research results on factors facilitating implementation have not yet been sufficiently presented. Objective: This scoping review will explore the findings on successful preparation, realisation and maintenance of interventions in nursing and midwifery. Method: Based on an existing systematic approach, relevant data bases were accessed to identify original studies relating to implementation research in nursing and midwifery. The process of study selection and the characteristics of the included studies were charted. Findings: In the four reviews and 38 studies which met the inclusion criteria, various factors leading to successful implementation are identified. Based on the included studies, four facilitating factors can be pinpointed: (1) workplace culture (16 entries), (2) leadership culture (28 entries), (3) resources (4 entries), and (4) training (22 entries). Depending on the specific phase of the implementation process (preparation, realisation, or maintenance), these factors will vary in importance. Conclusion: This scoping review provides an orientation for the field of implementation research and it maps the design, themes and results of the studies included. It should be noted that few studies take into consideration the relevant theories, as well as the influence that the researchers and the target group may have on the implementation process, or provide an exact description of the setting in which the implementation takes place
