    To burn-out or not to burn-out: A cross-sectional study in healthcare professionals in Spain during COVID-19 pandemic

    Objective To assess the prevalence of burn-out syndrome in healthcare workers working on the front line (FL) in Spain during COVID-19. Design Cross-sectional, online survey-based study. Settings Sampling was performed between 21st April and 3rd May 2020. The survey collected demographic data and questions regarding participants' working position since pandemic outbreak. Participants Spanish healthcare workers working on the FL or usual ward were eligible. A total of 674 healthcare professionals answered the survey. Main outcomes and measures Burn-out syndrome was assessed by the Maslach Burnout Inventory-Medical Personnel. Results Of the 643 eligible responding participants, 408 (63.5%) were physicians, 172 (26.8%) were nurses and 63 (9.8%) other technical occupations. 377 (58.6%) worked on the FL. Most participants were women (472 (73.4%)), aged 31-40 years (163 (25.3%)) and worked in tertiary hospitals (>600 beds) (260 (40.4%)). Prevalence of burn-out syndrome was 43.4% (95% CI 39.5% to 47.2%), higher in COVID-19 FL workers (49.6%, p<0.001) than in non-COVID-19 FL workers (34.6%, p<0.001). Women felt more burn-out (60.8%, p=0.016), were more afraid of self-infection (61.9%, p=0.021) and of their performance and quality of care provided to the patients (75.8%, p=0.015) than men. More burn-out were those between 20 and 30 years old (65.2%, p=0.026) and those with more than 15 years of experience (53.7%, p=0.035). Multivariable logistic regression analysis revealed that, working on COVID-19 FL (OR 1.93; 95% CI 1.37 to 2.71, p<0.001), being a woman (OR 1.56; 95% CI 1.06 to 2.29, p=0.022), being under 30 years old (OR 1.75; 95% CI 1.06 to 2.89, p=0.028) and being a physician (OR 1.64; 95% CI 1.11 to 2.41, p=0.011) were associated with high risk of burn-out syndrome. Conclusions This survey study of healthcare professionals reported high rates of burn-out syndrome. Interventions to promote mental well-being in healthcare workers exposed to COVID-19 need to be immediately implemented.publishersversionpublishe

    Resumo: A proposta desse estudo, é discutir a importância de se construir uma real transformação na educação do campo, sendo esta voltada para a formação humana e social de um sujeito crítico transformador de sua própria realidade e, nessa perspectiva, sugerimos a aproximação entre o debate dessa educação e a noção de “vida activa”, trabalhada pela filósofa Hannah Arendt, onde faz-se uma contextualização de alguns princípios da história da educação do campo apresentando a noção de labor, trabalho e ação. Ao término deste estudo, foi possível fazer uma análise e compreender que a educação é ferramenta essencial na formação de um homem, pois fica claro que a escola, dá espaço para a ação, que na luz de Hannah Arendt, é a ferramenta perfeita para essa ação. Palavras-chave: Educação no Campo; Labor, Trabalho e Ação; Vida Activa. 

    Variações em atributos biofísicos e fisiológicos em nível de individuo e de parcela ao longo de um gradiente de densidade arbórea no Cerrado

    O objetivo deste estudo foi caracterizar e compreender as variações morfológicas e fisiológicas em atributos relacionados à economia da água, em nível de indivíduo e de parcela, que ocorrem ao longo de um gradiente de densidade arbórea em uma savana neotropical (cerrado). Parcelas de vegetação de cerrado submetidas a um experimento de adubação de longo prazo e matas de galeria também foram incluídas na análise, a fim de expandir os eixos de variação do ecossistema. Mudanças consistentes em atributos biofísicos em nível de parcela foram observadas ao longo do gradiente de densidade crescente de árvores. Estas incluíram uma diminuição na densidade média ponderada da madeira, aumentos na área foliar por planta, na condutividade hidráulica específica da folha, na área foliar específica e na condutância estomática. Um modelo conceitual das interações entre as características biofísicas, morfológicas e fisiológicas foi desenvolvido em uma tentativa de explicar os determinantes da arquitetura hidráulica e variações na economia de água de árvores do Cerrado. Consistente com o modelo, os valores mínimos de potencial hídrico durante a estação seca foram maiores nas árvores de baixa densidade de madeira, em comparação com árvores de alta densidade de madeira, enquanto os maiores valores de potencial hídrico foliar estiveram associados com maior condutância estomática. Por outro lado, ao longo do gradiente de densidade arbórea, as variações em nível de parcela das características morfológicas e fisiológicas não puderam ser explicadas por variações de um único fator ambiental. Alguns dos fatores contribuintes potenciais são: maior concentração e disponibilidade de nutrientes do solo na porção superior do gradiente de densidade arbórea onde há um maior número de árvores e restrições no estabelecimento e crescimento pela inundação periódica e pela presença de concreções no perfil do solo onde a densidade de árvores é mais baixa.The objective of this study was to characterize and understand morphological and physiological variation in traits related to water economy, at the plant and stand level, along a gradient of tree density in a Neotropical savanna (Cerrado). Cerrado plots subjected to long-term fertilization and gallery forests were also included in the analysis to expand the axes of ecosystem variation. Consistent changes in stand level biophysical traits observed along the gradient of increasing tree density included a decrease in weighted-average wood density, and increases in leaf surface area per plant, leaf specific hydraulic conductivity, specific leaf area and stomatal conductance. A conceptual model of biophysical, morphological and physiological trait interactions was developed in an attempt to explain determinants of hydraulic architecture and variations in water economy of Cerrado trees. Consistent with the model minimum leaf water potentials were higher, during the dry season, in low wood density trees compared to high wood density trees, and higher leaf water potential was associated with higher stomatal conductance. On the other hand, variations in stand level physiological and morphological traits along the tree density gradient could not be explained by variations in single environmental factors. Some of the potential contributing factors are: higher concentration and availability of soil nutrients in the upper portion of the gradient where tree density is greatest and constrains to tree establishment and growth by waterlogging and presence of concretions in the soil profile where tree density is lowest

    Hybrid Closed-Loop System Achieves Optimal Perioperative Glycemia in a Boy With Type 1 Diabetes: A Case Report

    The goal in type 1 diabetes (T1D) therapy is to maintain optimal glycemic control under any circumstance. Diabetes technology is in continuous development to achieve this goal. The most advanced Food and Drug Administration- and European Medicines Agency-approved devices are hybrid closed-loop (HCL) systems, which deliver insulin subcutaneously in response to glucose levels according to an automated algorithm. T1D is frequently encountered in the perioperative period. The latest international guidelines for the management of children with diabetes undergoing surgery include specific adjustments to the patient's insulin therapy, hourly blood glucose monitoring, and intravenous (IV) insulin infusion. However, these guidelines were published while the HCL systems were still marginal. We present a case of a 9-year-old boy with long-standing T1D, under HCL system therapy for the last 9 months, and needing surgery for an appendectomy. We agreed with the family, the surgical team, and the anesthesiologists to continue HCL insulin infusion, without further adjustments, hourly blood glucose checks or IV insulin, while monitoring closely. The HCL system was able to keep glycemia within range for the total duration of the overnight fast, the surgery, and the initial recovery, without any external intervention or correction bolus. This is, to the best of our knowledge, the first reported pediatric case to undergo major surgery using a HCL system, and the results were absolutely satisfactory for the patient, his family, and the medical team. We believe that technology is ripe enough to advocate for a "take your pump to surgery" message, minimizing the impact and our interventions. The medical team may discuss this possibility with the family and patients

    The synthesis of hybrid materials, combining the properties of organic and inorganic components, results in composites with unique physical and chemical features. Polyoxometalates (POMs), i.e. inorganic anionic molecular metal oxides, are considered as promising future metallodrugs due to their antiviral, antitumoral and antibacterial activities. The combination of bulky organic cations with POMs results in composite ionic liquids (IL; melting point below 100°C) which combine the unique properties of both components.     Phenotypic characteristics of the p.Asn215Ser (p.N215S) GLA mutation in male and female patients with Fabry disease: A multicenter Fabry Registry study.

    BackgroundThe p.Asn215Ser or p.N215S GLA variant has been associated with late-onset cardiac variant of Fabry disease.MethodsTo expand on the scarce phenotype data, we analyzed natural history data from 125 p.N215S patients (66 females, 59 males) enrolled in the Fabry Registry (NCT00196742) and compared it with data from 401 patients (237 females, 164 males) harboring mutations associated with classic Fabry disease. We evaluated interventricular septum thickness (IVST), left ventricular posterior wall thickness (LVPWT), estimated glomerular filtration rate and severe clinical events.ResultsIn p.N215S males, mildly abnormal mean IVST and LVPWT values were observed in patients aged 25-34 years, and values gradually increased with advancing age. Mean values were similar to those of classic males. In p.N215S females, these abnormalities occurred primarily in patients aged 55-64 years. Severe clinical events in p.N215S patients were mainly cardiac (males 31%, females 8%) while renal and cerebrovascular events were rare. Renal impairment occurred in 17% of p.N215S males (mostly in patients aged 65-74 years), and rarely in females (3%).Conclusionp.N215S is a disease-causing mutation with severe clinical manifestations found primarily in the heart. Cardiac involvement may become as severe as in classic Fabry patients, especially in males

    The COMPASS Setup for Physics with Hadron Beams

    The main characteristics of the COMPASS experimental setup for physics with hadron beams are described. This setup was designed to perform exclusive measurements of processes with several charged and/or neutral particles in the final state. Making use of a large part of the apparatus that was previously built for spin structure studies with a muon beam, it also features a new target system as well as new or upgraded detectors. The hadron setup is able to operate at the high incident hadron flux available at CERN. It is characterised by large angular and momentum coverages, large and nearly flat acceptances, and good two and three-particle mass resolutions. In 2008 and 2009 it was successfully used with positive and negative hadron beams and with liquid hydrogen and solid nuclear targets. This paper describes the new and upgraded detectors and auxiliary equipment, outlines the reconstruction procedures used, and summarises the general performance of the setup.Peer Reviewe

    Odd and even partial waves of ηπηπ^− and ηπη′π^− in πpη()πpπ^−p → η^{(′)}π^−p at 191 GeV/c

    Exclusive production of ηπ− and η′π− has been studied with a 191 GeV/c π− beam impinging on a hydrogen target at COMPASS (CERN). Partial-wave analyses reveal different odd/even angular momentum ( L ) characteristics in the inspected invariant mass range up to 3 GeV/c2 . A striking similarity between the two systems is observed for the L=2,4,6 intensities (scaled by kinematical factors) and the relative phases. The known resonances a2(1320) and a4(2040) are in line with this similarity. In contrast, a strong enhancement of η′π− over ηπ− is found for the L=1,3,5 waves, which carry non- qq¯ quantum numbers. The L=1 intensity peaks at 1.7 GeV/c2 in η′π− and at 1.4 GeV/c2 in ηπ− , the corresponding phase motions with respect to L=2 are different.Peer Reviewe
