Estudo de parâmetros clĂnicos e metabĂłlicos em pacientes com epidermĂłlise bolhosa hereditária e sua relação com a densidade mineral Ăłssea
Dissertação (mestrado)—Universidade de BrasĂlia, Faculdade de CiĂŞncias da SaĂşde, Programa de PĂłs-Graduação em CiĂŞncias da SaĂşde, 2017.Introdução: Epidermolise bolhosa hereditaria (EBH) corresponde a um grupo de doencas clinicamente caracterizadas por fragilidade da pele e das mucosas, com consequente formacao de bolhas aos minimos traumas. Baixa densidade mineral ossea (DMO) e fraturas sao potenciais complicacoes das formas graves de EBH e podem ter etiologia multifatorial. Objetivos: Avaliar variaveis clinico-antropometricas e metabolicas de pacientes com EBH acompanhados no Hospital Universitario de Brasilia e estudar a relacao dessas variaveis com a densidade mineral ossea areal (aDMO) dos pacientes. MĂ©todos: Trata-se de um estudo observacional transversal envolvendo oito individuos com EBH, sendo seis do sexo feminino, tres com EB simples (EBS), cinco com EB distrofica recessiva (EBDR), media de idade 16,4 } 8,3 anos. Realizou-se historia clinica, exame fisico, avaliacao antropometrica, avaliacoes metabolicas e densitometria ossea areal (DXA) da coluna lombar (L1-L4) e do corpo total nas criancas e adolescentes. Nos individuos com mais de 18 anos, alem dos exames anteriores, tambem foi realizada a DXA do femur proximal. A aDMO de individuos com baixa estatura foi ajustada para idade-estatura. As possiveis correlacoes das variaveis clinicas e metabolicas com a aDMO foram avaliadas. Resultados: Nenhum paciente com EBS apresentou baixa estatura ou baixo indice de massa corporal (IMC). Quatro pacientes (tres EBS) apresentaram mobilidade sem restricoes. Apenas os pacientes com EBDR apresentaram anemia (media de hemoglobina 7,9 ± 2,1 g/dL), insuficiencia de 25(OH)D (media 15,8 ± 2,6 ng/mL) e proteina C reativa (PCR) elevada (media 5,8 ± 1,5 mg/dL) . A media da interleucina (IL) - 6 dos pacientes com EBDR (38,9 ± 19,9 pg/mL) foi significativamente maior que a dos pacientes com EBS (3,1 ± 3,4 pg/mL), p = 0,014. Tres pacientes com EBDR apresentaram baixa massa ossea na coluna lombar corrigida para idade-estatura e nenhum paciente apresentou baixa massa ossea na avaliacao do corpo total corrigida para idade-estatura. A aDMO de L1-L4 apresentou correlacao fortemente positiva e significativa com o grau de mobilidade dos pacientes (r = 0,86 e p = 0,013), quando um outlier foi removido. Houve correlacao forte e significativa entre a aDMO da coluna lombar com o IMC (r = 0,79749; p = 0,0317), concentracoes sericas de 25(OH)D (r = 0,80474; p = 0,0290) e de hemoglobina (r = 0,76887; p = 0,0434). Houve correlacao fortemente positiva e significativa entre as concentracoes sericas de IL-6 e PCR (r = 0,86; p = 0,005). Entretanto esses marcadores inflamatorios nao apresentaram correlacoes estatisticamente significativas com a aDMO. Nenhuma correlacao estatisticamente significativa foi encontrada entre a aDMO de corpo total corrigida para idade-estatura e as variaveis estudadas. ConclusĂŁo: Em relacao aos pacientes com EBS, os pacientes com EBDR apresentam maior comprometimento da estatura e do estado nutricional, menores concentracoes de 25(OH)D e de hemoglobina, alem de valores mais elevados dos parametros inflamatorios. No grupo estudado, encontrou-se correlacao positiva e significativa da aDMO da coluna lombar com o grau de mobilidade do paciente, o padrao nutricional, as concentracoes sericas de vitamina D e hemoglobina.Introduction: Hereditary epidermolysis bullosa (HEB) corresponds to a group of diseases clinically characterized by fragility of the skin and mucosa, with the consequent formation of blisters after minimal traumas. Low bone mineral density (BMD) and fractures are possible complications of the severe forms of HEB and may have multifactorial etiology. Objectives: To evaluate clinical, anthropometric and metabolic variables of patients with HEB followed at the Brasilia University Hospital and to study the correlation among those variables and the areal bone mineral density (aBMD) of the patients. Methods: This was a cross-sectional observational study involving eight individuals with HEB, consisting of six females, three with EB simplex (EBS), five with recessive dystrophic EB (RDEB), mean age 16.4 } 8.3 years . A clinical history, physical examination, anthropometric evaluation, metabolic assessments and DXA of the lumbar spine (L1-L4) and of the total body in children and adolescents were performed. In individuals older than 18 years, in addition to the previous exams, DXA of the proximal femur was also performed. The aBMD of individuals with short stature was adjusted for age-stature. The possible correlations of clinical and metabolic variables with aBMD were evaluated. Results: No patient with EBS presented short stature or low body mass index (BMI). Four patients (three EBS) had normal mobility. Only patients with RDEB had anemia (mean hemoglobin 7.9 ± 2.1 g/dL), 25(OH)D insufficiency (mean 15.8 ± 2.6 ng/mL) and high C-reactive protein (CRP) (mean 5.8 ± 1.5 mg/dL). The mean interleukin (IL) -6 of patients with RDEB (38.9 ± 19.9 pg/mL) was significantly higher than that of patients with EBS (3.1 ± 3.4 pg/mL), p = 0.014. Three patients with RDEB presented low bone mass in the lumbar spine, corrected for age-stature, and no patient presented low bone mass when evaluating the total body corrected for age-stature. aBMD of L1-L4 showed a strongly positive and significant correlation with the degree of mobility of the patients (r = 0.86, p = 0.013), when an outlier was removed. There was a strong and significant correlation between lumbar spine aBMD and BMI (r = 0.79749, p = 0.0317), serum concentrations of 25(OH)D (r = 0.80474, p = 0.0290) and hemoglobin (r = 0.76887, p = 0.0434) There was a strong positive and significant correlation between the serum concentrations of IL-6 and CRP (r = 0.86, p = 0.005), but those inflammatory markers did not present statistically significant correlations with aBMD. Conclusion: In relation to patients with EBS, patients with RDEB presented greater impairment of height and nutritional status, lower concentrations of 25(OH)vitamin D and hemoglobin levels, and higher values of inflammatory parameters. In the studied group, positive and significant correlations were found between aBMD of the lumbar spine and the degree of mobility of the patient, nutritional status and serum concentrations of vitamin D and hemoglobin
Catastrophic Kawasaki disease unresponsive to IVIG in a 3-month-old infant : a diagnostic and therapeutic challenge
The present report describes the severe evolution of Kawasaki disease in a three-month-old infant. The ailment was
initially atypical in its presentation, with the patient exhibiting only persistent fever in association with a progressive
lethargy and maculopapular rash on the face, trunk and limbs erroneously diagnosed as roseola infantum. On the
10th
day of the condition, mainly due to the unexplained persistence of fever, the infant was admitted to a local
hospital. The typical features of KD appeared only on the 14th
day of illness with the relapse of the maculopapular
rash in association with non-purulent conjunctivitis; dry, reddish and fissured lips; tongue with reddish and
hypertrophic papillae; erythema and edema of the palms and soles. During the following days, the ailment rapidly
evolved to a catastrophic clinical picture characterized by generalized vasculitis, splenic infarction, pulmonary
thrombosis, giant right and left coronary aneurysms, dilatation of common and internal iliac arteries and
progressive ischemia of the distal third of the feet resulting in necrotic lesions of both halluces. Appropriate therapy
was initiated, but repeated administration of intravenous immunoglobulin G (IVIG) followed by three days of
administration of methylprednisolone did not abate the intense inflammatory activity. The remission of
inflammation and regression of vascular lesions were only achieved during the following five weeks after the
introduction of methotrexate associated with etanercept. The report of this case aims to draw attention to severe
forms of KD that exhibit an unfavorable evolution and can be extremely refractory to the conventional therapy
Prospective study of Kawasaki Disease complications : review of 115 cases
Objetivo: Chamar a atenção para as complicações, que podem surgir em qualquer fase da Doença de Kawasaki (DK), para os fatores de risco que contribuem para o parecimento dessas complicações e para as possĂveis sequelas da doença, sejam elas transitĂłrias ou permanentes. MĂ©todos: Estudo prospectivo (coorte clĂnica) realizado entre abril de 2002 e abril de 2009 de 115 pacientes com DK internados no serviço de Reumatologia Pediátrica do Hospital Geral do Distrito Federal. Todos os pacientes foram sequencialmente avaliados com exames clĂnicos e laboratoriais, ecocardiogramas com Doppler, imitanciometria, potenciais evocados auditivos, avaliação psicolĂłgica, exame oftalmolĂłgico e, em um paciente com coreia, angiorressonância magnĂ©tica cerebral. Em todos os pacientes foram aplicados questionários avaliando a possĂvel presença de dificuldades cognitivas, emocionais, comportamentais e sociais. Resultados: Vinte e cinco pacientes (21,7%) apresentaram aneurismas de coronárias. Trinta e oito pacientes (33%) apresentaram perda auditiva neurossensorial durante a doença aguda e subaguda, e 13 pacientes (11,3%) mantiveram a perda auditiva seis meses apĂłs a primeira avaliação. Outras complicações observadas foram: paralisia facial em um paciente (0,9%), ataxia na fase aguda e subaguda em 11 pacientes (9,5%), complicações oftalmolĂłgicas em 15 pacientes (13,2%), constatando-se uveĂte em 13, edema de papila em um paciente e hemorragia conjuntival em outro. Um paciente apresentou coreia (0,9%) sendo que a angioressonância magnĂ©tica evidenciou alterações compatĂveis com isquemia cerebral. Em um paciente constatou-se a presença de aneurisma de aorta torácica (0,9%), e outro apresentou vasculite necrosante que evoluiu com gangrena perifĂ©rica e perda da ponta da lĂngua (0,9%). Alterações de comportamento durante a convalescença (20%) foi observada em 23 crianças. ConclusĂŁo: A DK pode evoluir com complicações diversas, mesmo meses apĂłs a fase aguda da doença, eventualmente resultando em sequelas permanentes. Quanto mais precoce forem o diagnĂłstico e a intervenção terapĂŞutica com a administração de IgG IV, menor será a ocorrĂŞncia de complicações. Presença de trombocitose, anemia e de atividade inflamatĂłria elevada e por tempo prolongado sĂŁo fatores de risco para o aparecimento de complicações.Objective: To draw attention to complications that might arise in any Kawasaki disease (KD) stage, risk factors contributing to the onset of complications and possible transient or permanent disease sequelae. Methods: Prospective study (clinical cohort) conducted between April 2002 and April 2009 of 115 patients with KD admitted to the Pediatric Rheumatology Clinic of the General Hospital of the Federal District, Brazil. All patients were sequentially assessed with clinical and laboratory examinations, Doppler echocardiography, imitanciometry, auditory evoked potentials, psychological evaluation, ophthalmologic examination and, in one patient with chorea, cerebral magnetic resonance angiography. In all patients, a questionnaire assessing the possible presence of cognitive, emotional, behavioral and social disorders was applied. Results: Twenty-five patients (21.7%) had coronary aneurisms. Thirty-eight patients (33%) had a sensorineural auditory loss during the acute and subacute phases of the disease and 13 patients (11.3%) maintained the auditory loss six months after the first assessment. Other complications observed were as follows: facial palsy in one patient (0.9%), ataxia in acute and subacute phases in 11 (9.5%); 15 patients had ophthalmologic complications (13.2%), with uveitis in 13, papilledema in one patient, and conjunctival hemorrhage in another patient. One patient experienced chorea (0.9%), with a magnetic resonance angiography showing changes consistent with cerebral ischemia. In one patient, a thoracic aorta aneurism was found (0.9%) and another patient had a necrotizing vasculitis progressing to peripheral gangrene and tongue tip loss (0.9%). Behavioral changes over convalescence were observed in 23 children. Conclusion: KD may progress with several complications even within months of the disease acute phase, eventually resulting in permanent sequelae. The earlier the diagnosis and therapeutic intervention with IV IgG administration are, the lower will be the occurrence of complications; the presence of thrombocytosis, anemia and elevated and extended inflammatory activity are risk factors for complication arising
Pineocitoma causando Hidrocefalia: Pineocytoma causing Hydrocephalus
Introdução: O Pineocitoma Ă© um tumor do sistema nervoso central de caráter benigno. Esse tipo histolĂłgico Ă© infrequente correspondendo menos de 1% dos tumores intracranianos. A clĂnica apresenta desde hipertensĂŁo intracraniana atĂ© compressĂŁo de estruturas nervosas, associado a uma altĂssima taxa de morbidade e mortalidade. Apresentação do caso: Paciente com 23anos, sexo feminino, procurou serviço de emergĂŞncia mĂ©dica referindo cefaleia intensa de escala de dor 8/10, sem fatores precipitantes e sem melhora com uso de analgĂ©sicos ou anti-inflamatĂłrio, negou comorbidades e uso de medicações contĂnuas. Apresenta-se com muita dor, administrando medicação endovenosa para melhorar os sintomas. Foi solicitada uma Tomografia de crânio que evidenciou uma lesĂŁo sugestiva de meningioma, paciente foi internada e conduzida para cirurgia para retirada da lesĂŁo a qual foi constatada no histopatolĂłgico que se tratava de um Pineocitoma. DiscussĂŁo: o diagnĂłstico Ă© baseado na clĂnica, exames de neuroimagem, marcadores tumorais no lĂquor cefalorraquidiano, o histopatolĂłgico já Ă© um exame definitivo, com a tomografia computadorizada de crânio Ă© possĂvel visualizar, massa com contorno bem delimitado, em torno de 3 cm de diâmetro, sendo possĂvel encontrar hidrocefalia associada. O tratamento padrĂŁo ouro Ă© ressecção cirĂşrgica completa, associando a radioterapia e quimioterapia. ConclusĂŁo: o tratamento mostra resultado de cura, visto que o Pineocitoma raramente evolui para metástase, resultando em um bom prognĂłstico devido a evolução tecnolĂłgica em tratamento e mĂ©todos diagnĂłsticos
SĂndrome de Mccleery: Mccleery Syndrome
Introdução: A SĂndrome de Mccleery foi descrita detalhadamente a primeira vez em 1951, por Mccleery. É um subtipo da SĂndrome do Desfiladeiro Torácico, causada pela obstrução nĂŁo trombĂłtica da veia subclávia, em que acomete adultos jovens (principalmente do sexo masculino) e atletas, em razĂŁo dos esforços repetitivos e da hipertrofia muscular da regiĂŁo torácica. Apresentação do caso: Paciente sexo masculino, 25 anos, atleta, apresenta quadro de dor intensa de inĂcio sĂşbito em membro superior direito, seguida de edema e cianose. Relata piora do quadro ao movimentar o membro e melhora parcial com uso de analgĂ©sicos e repouso. A ultrassonografia com Doppler identifica compressĂŁo de veia subclávia. Realizada venografia que apresentou obstrução de veia subclávia no espaço costoclavicular. DiscussĂŁo: Nesta sĂndrome, o espaço costoclavicular, especificamente, ganha destaque. Delimitado entre a primeira costela e a clavĂcula, onde passam os vasos subclávios e o plexo braquial. Seu diagnĂłstico Ă© clĂnico, contudo deve ser associado a exames de imagem. O tratamento Ă© direcionado de acordo com a etiologia e a presença ou nĂŁo de complicações. Frequentemente a conduta inicial Ă© o tratamento clĂnico, procurando aliviar os sintomas. No entanto, estudiosos vĂŞm advogando intervenções cirĂşrgicas precoces, no intuito de evitar eventos trombĂłticos, limitações funcionais e restrições de mobilidade. ConclusĂŁo: O presente relato de caso faz-se importante como contribuição para o literatura cientĂfica, de forma que haja melhor acesso e maior discussĂŁo acerca do tema nas plataformas acadĂŞmicas
Pervasive gaps in Amazonian ecological research
Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4
While the increasing availability of global databases on ecological communities has advanced our knowledge
of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In
the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of
Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus
crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced
environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian
Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by
2050. This means that unless we take immediate action, we will not be able to establish their current status,
