En la presente investigación a través de la revisión de los datos obtenidos de la PCT 5 de los establecimientos pertenecientes a la primera fase del Plan Piloto Atención Integral TB/DM se estableció la asociación entre estas dos patologÃas, lo cual se logró a través de la caracterización de la población, el establecimiento de la prevalencia de DM en pacientes con TB y la comparación de la probabilidad de padecer TB entre las personas diabéticas y no diabéticas.Diplomado en Investigación con énfasis en Tuberculosi
Exploring the contribution of dietary protein from poultry by-product meal and fish meal to the growth of catfish Ictalurus punctatus by means of nitrogen stable isotopes
The natural nitrogen stable isotope signatures (δ
15N) found in poultry by-product meal (PBM) and fish meal (FM) were used to estimate the relative contribution of dietary nitrogen supplied by both ingredients to the somatic growth of juvenile channel catfish Ictalurus punctatus. Six isonitrogenous and isoenergetic experimental diets were formulated using FM and PBM. Two of these diets consisted of isotopic controls having only one ingredient supplying dietary nitrogen, either FM or PBM. Four combined diets were formulated with varying proportions of these ingredients in order to supply high proportions of PBM (FM:PBM, 50:50, 35:65 20:80 and 5:95) on a nitrogen basis. There were significant differences in mean final weight of fish at the end of the trial. Lower growth was observed as the dietary level of PBM increased. In order to determine the relative contributions of the dietary nitrogen supplied by FM and PBM to catfish growth, an isotopic mixing model was applied. Results indicated that the incorporation of dietary nitrogen supplied by PBM was equivalent to the die tary proportions. The dietary nitrogen available in combined diets containing 50, 65 and 80% of PBM was incorporated in fish bodies as 50, 62 and 81%, respectively. However, high incorporation of dietary nitrogen from PBM was not always reflected in higher growth rates. Results demonstrate the viable use of stable isotopes to determine the allocation of dietary nitrogen and indicate that practical diets for catfish can be formulated with levels of PBM as high as 65% without affecting growth and survival
Molecular detection and parasite load of Trypanosoma cruzi in digestive tract tissue of Chagas disease patients affected by megacolon
Chagas disease, caused by the Trypanosoma cruzi parasite in the Americas affects similar to 7 million people, 30% with cardiac tissue damage and 10-15% with digestive disorders. In this study, we have developed a protocol to detect the presence of the parasite and estimate its load in resected dysfunctional tissue segments of chronically infected patients with digestive megacolon. We have included samples from 43 individuals, 38/5 with positive/negative serology for Chagas disease and digestive syndromes. Samples of 1.5 to 2.0 cm(2) were taken from different points of the dysfunctional digestive tract in specialized centres in Cochabamba, Bolivia. T. cruzi cultures were performed by inoculation with NNN-LIT culture medium, and genomic material was obtained from the samples for multiplex qPCR with TaqMan probes targeting satellite nuclear DNA. Cultures failed to isolate T. cruzi but qPCR reached a sensitivity of 42.1% (16/38) with all three spots and in triplicate. A new quantification methodology using synthetic satellite DNA as quantitation standard revealed parasite loads ranging from 2.2 x 10(2) to 1.0 x 10(6) satellite DNA copies/mu l. Positive samples from the distal end showed a higher parasite load. The results of the present study strengthen and add further evidence to previous findings in an experimental mouse model of chronic T. cruzi infection, providing a valuable tool to improve scientific knowledge on the relevance of the digestive tract in parasite persistence, and underline the need of a better understanding of host-pathogen interaction in digestive tissues, considering pathophysiology, disease immunology and response to treatment
Slow oscillatory activity and levodopa-induced dyskinesias in Parkinson’s disease
The pathophysiology of levodopa-induced dyskinesias (LID) in Parkinson’s disease is not well understood.
We have recorded local field potentials (LFP) from macroelectrodes implanted in the subthalamic nucleus
(STN) of 14 patients with Parkinson’s disease following surgical treatment with deep brain stimulation. Patients
were studied in the ‘Off’ medication state and in the ‘On’ motor state after administration of levodopa–
carbidopa (po) or apomorphine (sc) that elicited dyskinesias in 11 patients. The logarithm of the power
spectrum of the LFP in selected frequency bands (4–10, 11–30 and 60–80 Hz) was compared between the
‘Off’ and ‘On’ medication states. A peak in the 11–30 Hz band was recorded in the ‘Off’ medication state
and reduced by 45.2% (P < 0.001) in the ‘On’ state. The ‘On’ was also associated with an increment of 77. 6%
(P < 0.001) in the 4–10 Hz band in all patients who showed dyskinesias and of 17.8% (P < 0.001) in the 60–80 Hz
band in the majority of patients. When dyskinesias were only present in one limb (n = 2), the 4–10 Hz peak was
only recorded in the contralateralSTN. These findings suggest that the 4–10 Hz oscillation is associated with the
expression of LID in Parkinson’s disease
Coinfección por patógenos respiratorios virales y bacterianos detectados por métodos moleculares en pacientes hospitalizados por COVID-19 y su impacto en la mortalidad y desenlaces desfavorables
Resumen del Proyecto:
Antecedentes y Justificación
La mortalidad de los pacientes infectados con COVID-19 suele estar alrededor del 5%,
dependiendo de las poblaciones reportadas y los métodos diagnósticos usados. Sin
embargo, en pacientes hospitalizados, la mortalidad puede llegar al 30%.
La variabilidad de este y otros indicadores pueden estar supeditados a la presencia de
otras condiciones como las coinfecciones respiratorias. Se han reportado la presencia
de coinfecciones en alrededor del 10 a 12,5% de pacientes hospitalizados por COVID19 siendo estos mayormente detectados con métodos de baja sensibilidad, por lo que
su prevalencia es probablemente mayor. Aunque en otras infecciones respiratorias el
rol de la coinfección con patógenos respiratorios como neumococo o estafilococo es
alta, su rol en el pronóstico de la enfermedad por COVID-19 no está claro aún.
Objetivo principal
Evaluar la frecuencia de coinfección con patógenos respiratorios bacterianos y virales
detectados mediante métodos moleculares y su asociación con desenlaces
desfavorables incluyendo la necesidad de ventilación mecánica y muerte en pacientes
hospitalizados por COVID-19.
MetodologÃa
Estudio longitudinal analÃtico de cohorte prospectiva que analizará muestras de esputo
o lavado bronquial obtenidas de pacientes hospitalizados con sospecha de COVID-19
en el Hospital Nacional Hipólito Unanue entre abril y octubre 2020, para evaluar la
coinfección de patógenos respiratorios bacterianos y virales y su asociación con
mortalidad y desenlaces desfavorables. El tamaño muestral requerido es de 196
participantes
Las muestras de esputo y lavado bronco alveolar evaluadas para la identificación de
agentes etiológicos virales y bacterianos mediante la plataforma molecular FilmArray.
Se analizarán 33 patógenos respiratorios incluyendo 18 bacterias, 9 virus y 7 genes
asociados a resistencia antimicrobiana.
Se considerará como desenlace primario la mortalidad. Como desenlaces secundarios
se incluirán la necesidad de ventilación mecánica, ingreso a unidad de cuidados
intensivos, estancia hospitalaria y el indicador combinado de mortalidad e ingreso a
cuidados intensivos.
Resultados esperados (corto y mediano plazo)
Nuestros datos serán útiles para establecer recomendaciones basadas en evidencia
nacional para determinar la necesidad y posible beneficio de terapia antimicrobiana. De
encontrarse asociación entre la coinfección bacteriana o viral y la mortalidad o
complicaciones se justificarÃa la implementación de diagnóstico molecular en estas
muestras a fin de intervenir tempranamente y evitar desenlaces desfavorables.Trabajo academic
Knowledge to Serve the City: Insights from an Emerging Knowledge-Action Network to Address Vulnerability and Sustainability in San Juan, Puerto Rico
This paper presents initial efforts to establish the San Juan Urban Long-Term Research Area Exploratory (ULTRA-Ex), a long-term program aimed at developing transdisciplinary social-ecological system (SES) research to address vulnerability and sustainability for the municipality of San Juan. Transdisciplinary approaches involve the collaborations between researchers, stakeholders, and citizens to produce socially-relevant knowledge and support decision-making. We characterize the transdisciplinary arrangement emerging in San Juan ULTRA-Ex as a knowledge-action network composed of multiple formal and informal actors (e.g., scientists, policymakers, civic organizations and other stakeholders) where knowledge, ideas, and strategies for sustainability are being produced, evaluated, and validated. We describe in this paper the on-the-ground social practices and dynamics that emerged from developing a knowledge-action network in our local context. Specifically, we present six social practices that were crucial to the development of our knowledge-action network: 1) understanding local framings; 2) analyzing existing knowledge-action systems in the city; 3) framing the social-ecological research agenda; 4) collaborative knowledge production and integration; 5) boundary objects and practices; and 6) synthesis, application, and adaptation. We discuss key challenges and ways to move forward in building knowledge-action networks for sustainability. Our hope is that the insights learned from this process will stimulate broader discussions on how to develop knowledge for urban sustainability, especially in tropical cities where these issues are under-explored
