    Effectiveness of a “Whole of Chain” Approach in Linking Farmers to Market: A Case of Pakistan Mango Market

    AbstractMango is the second major fruit crop in Pakistan. The domestic retail market for mango in Pakistan is dominated by small retail shops, street hawkers, and road side stalls. The fruit sold in these retail outlets is prescribed by the traditional quality standards of size, appearance and price. However the growth of superior outlets/supermarkets chain especially in the major cities are setting additional quality standards such as blemish free, improved packaging, prestige, convenience to deliver premium quality mangoes. Similarly the export market is mainly targeted to the expatriate Pakistani consumers rather than quality conscious foreign consumers. This is because of inadequate market information and understanding all along the chain. A whole of chain approach is undertaken to improve the market understanding in an ACIAR project. Since the approach is new, a conceptual framework is developed in order to asses the effectiveness of the approach. The results indicate that the participants all along the chain would change their practices if they find the compelling reason to change in their existing businesses

    Preparation and quality characterization of soy milk based non-dairy ice cream

    Soy milk made from soybean has prospective to be used as a substitute of milk due to its health benefits. It is a rich source of iso-flavones, omega-3-fatty acid, dietary fiber, vitamin C, carotenoids, protein and oligosaccharides. The current study was designed to examine the effects of galacto-manan on ice cream by using commercially available (silk) and locally prepared soy milk. Galacto-mannan (guar gum) was used in different concentration (0.3, 0.4, 0.5 and 0.6%) for the preparation of ice cream. Ice cream was analyzed for physico-chemical and sensory characteristics at 0, 30 and 60 days of storage interval. Overrun, meltdown, viscosity, total solids, pH and acidity were affected significantly by ice cream samples as well as storage. While non-significant effects of stabilizer and storage were found on fat, protein, and ash contents of ice cream. On organoleptic evaluation, the highest scores were awarded to the ice cream sample prepared with 0.5% of guar gum. Ice cream manufactured with locally prepared soy milk and guar gum revealed comparable quality with lower cost

    Genetics, environmental stress, and amino acid supplementation affect lactational performance via mTOR signaling pathway in bovine mammary epithelial cells

    Mammary glands are known for their ability to convert nutrients present in the blood into milk contents. In cows, milk synthesis and the proliferation of cow mammary epithelial cells (CMECs) are regulated by various factors, including nutrients such as amino acids and glucose, hormones, and environmental stress. Amino acids, in particular, play a crucial role in regulating cell proliferation and casein synthesis in mammalian epithelial cells, apart from being building blocks for protein synthesis. Studies have shown that environmental factors, particularly heat stress, can negatively impact milk production performance in dairy cattle. The mammalian target of rapamycin complex 1 (mTORC1) pathway is considered the primary signaling pathway involved in regulating cell proliferation and milk protein and fat synthesis in cow mammary epithelial cells in response to amino acids and heat stress. Given the significant role played by the mTORC signaling pathway in milk synthesis and cell proliferation, this article briefly discusses the main regulatory genes, the impact of amino acids and heat stress on milk production performance, and the regulation of mTORC signaling pathway in cow mammary epithelial cells

    Evolutionary Integrated Heuristic with Gudermannian Neural Networks for Second Kind of Lane–Emden Nonlinear Singular Models

    In this work, a new heuristic computing design is presented with an artificial intelligence approach to exploit the models with feed-forward (FF) Gudermannian neural networks (GNN) accomplished with global search capability of genetic algorithms (GA) combined with local convergence aptitude of active-set method (ASM), i.e., FF-GNN-GAASM to solve the second kind of Lane–Emden nonlinear singular models (LE-NSM). The proposed method based on the computing intelligent Gudermannian kernel is incorporated with the hidden layer configuration of FF-GNN models of differential operatives of the LE-NSM, which are arbitrarily associated with presenting an error-based objective function that is used to optimize by the hybrid heuristics of GAASM. Three LE-NSM-based examples are numerically solved to authenticate the effectiveness, accurateness, and efficiency of the suggested FF-GNN-GAASM. The reliability of the scheme via statistical valuations is verified in order to authenticate the stability, accuracy, and convergence

    Effects of a high-dose 24-h infusion of tranexamic acid on death and thromboembolic events in patients with acute gastrointestinal bleeding (HALT-IT): an international randomised, double-blind, placebo-controlled trial

    Background: Tranexamic acid reduces surgical bleeding and reduces death due to bleeding in patients with trauma. Meta-analyses of small trials show that tranexamic acid might decrease deaths from gastrointestinal bleeding. We aimed to assess the effects of tranexamic acid in patients with gastrointestinal bleeding. Methods: We did an international, multicentre, randomised, placebo-controlled trial in 164 hospitals in 15 countries. Patients were enrolled if the responsible clinician was uncertain whether to use tranexamic acid, were aged above the minimum age considered an adult in their country (either aged 16 years and older or aged 18 years and older), and had significant (defined as at risk of bleeding to death) upper or lower gastrointestinal bleeding. Patients were randomly assigned by selection of a numbered treatment pack from a box containing eight packs that were identical apart from the pack number. Patients received either a loading dose of 1 g tranexamic acid, which was added to 100 mL infusion bag of 0·9% sodium chloride and infused by slow intravenous injection over 10 min, followed by a maintenance dose of 3 g tranexamic acid added to 1 L of any isotonic intravenous solution and infused at 125 mg/h for 24 h, or placebo (sodium chloride 0·9%). Patients, caregivers, and those assessing outcomes were masked to allocation. The primary outcome was death due to bleeding within 5 days of randomisation; analysis excluded patients who received neither dose of the allocated treatment and those for whom outcome data on death were unavailable. This trial was registered with Current Controlled Trials, ISRCTN11225767, and ClinicalTrials.gov, NCT01658124. Findings: Between July 4, 2013, and June 21, 2019, we randomly allocated 12 009 patients to receive tranexamic acid (5994, 49·9%) or matching placebo (6015, 50·1%), of whom 11 952 (99·5%) received the first dose of the allocated treatment. Death due to bleeding within 5 days of randomisation occurred in 222 (4%) of 5956 patients in the tranexamic acid group and in 226 (4%) of 5981 patients in the placebo group (risk ratio [RR] 0·99, 95% CI 0·82–1·18). Arterial thromboembolic events (myocardial infarction or stroke) were similar in the tranexamic acid group and placebo group (42 [0·7%] of 5952 vs 46 [0·8%] of 5977; 0·92; 0·60 to 1·39). Venous thromboembolic events (deep vein thrombosis or pulmonary embolism) were higher in tranexamic acid group than in the placebo group (48 [0·8%] of 5952 vs 26 [0·4%] of 5977; RR 1·85; 95% CI 1·15 to 2·98). Interpretation: We found that tranexamic acid did not reduce death from gastrointestinal bleeding. On the basis of our results, tranexamic acid should not be used for the treatment of gastrointestinal bleeding outside the context of a randomised trial

    Tocilizumab in patients admitted to hospital with COVID-19 (RECOVERY): a randomised, controlled, open-label, platform trial

    Background: In this study, we aimed to evaluate the effects of tocilizumab in adult patients admitted to hospital with COVID-19 with both hypoxia and systemic inflammation. Methods: This randomised, controlled, open-label, platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]), is assessing several possible treatments in patients hospitalised with COVID-19 in the UK. Those trial participants with hypoxia (oxygen saturation <92% on air or requiring oxygen therapy) and evidence of systemic inflammation (C-reactive protein ≥75 mg/L) were eligible for random assignment in a 1:1 ratio to usual standard of care alone versus usual standard of care plus tocilizumab at a dose of 400 mg–800 mg (depending on weight) given intravenously. A second dose could be given 12–24 h later if the patient's condition had not improved. The primary outcome was 28-day mortality, assessed in the intention-to-treat population. The trial is registered with ISRCTN (50189673) and ClinicalTrials.gov (NCT04381936). Findings: Between April 23, 2020, and Jan 24, 2021, 4116 adults of 21 550 patients enrolled into the RECOVERY trial were included in the assessment of tocilizumab, including 3385 (82%) patients receiving systemic corticosteroids. Overall, 621 (31%) of the 2022 patients allocated tocilizumab and 729 (35%) of the 2094 patients allocated to usual care died within 28 days (rate ratio 0·85; 95% CI 0·76–0·94; p=0·0028). Consistent results were seen in all prespecified subgroups of patients, including those receiving systemic corticosteroids. Patients allocated to tocilizumab were more likely to be discharged from hospital within 28 days (57% vs 50%; rate ratio 1·22; 1·12–1·33; p<0·0001). Among those not receiving invasive mechanical ventilation at baseline, patients allocated tocilizumab were less likely to reach the composite endpoint of invasive mechanical ventilation or death (35% vs 42%; risk ratio 0·84; 95% CI 0·77–0·92; p<0·0001). Interpretation: In hospitalised COVID-19 patients with hypoxia and systemic inflammation, tocilizumab improved survival and other clinical outcomes. These benefits were seen regardless of the amount of respiratory support and were additional to the benefits of systemic corticosteroids.     Convalescent plasma in patients admitted to hospital with COVID-19 (RECOVERY): a randomised controlled, open-label, platform trial

    Background: Many patients with COVID-19 have been treated with plasma containing anti-SARS-CoV-2 antibodies. We aimed to evaluate the safety and efficacy of convalescent plasma therapy in patients admitted to hospital with COVID-19. Methods: This randomised, controlled, open-label, platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]) is assessing several possible treatments in patients hospitalised with COVID-19 in the UK. The trial is underway at 177 NHS hospitals from across the UK. Eligible and consenting patients were randomly assigned (1:1) to receive either usual care alone (usual care group) or usual care plus high-titre convalescent plasma (convalescent plasma group). The primary outcome was 28-day mortality, analysed on an intention-to-treat basis. The trial is registered with ISRCTN, 50189673, and ClinicalTrials.gov, NCT04381936. Findings: Between May 28, 2020, and Jan 15, 2021, 11558 (71%) of 16287 patients enrolled in RECOVERY were eligible to receive convalescent plasma and were assigned to either the convalescent plasma group or the usual care group. There was no significant difference in 28-day mortality between the two groups: 1399 (24%) of 5795 patients in the convalescent plasma group and 1408 (24%) of 5763 patients in the usual care group died within 28 days (rate ratio 1·00, 95% CI 0·93–1·07; p=0·95). The 28-day mortality rate ratio was similar in all prespecified subgroups of patients, including in those patients without detectable SARS-CoV-2 antibodies at randomisation. Allocation to convalescent plasma had no significant effect on the proportion of patients discharged from hospital within 28 days (3832 [66%] patients in the convalescent plasma group vs 3822 [66%] patients in the usual care group; rate ratio 0·99, 95% CI 0·94–1·03; p=0·57). Among those not on invasive mechanical ventilation at randomisation, there was no significant difference in the proportion of patients meeting the composite endpoint of progression to invasive mechanical ventilation or death (1568 [29%] of 5493 patients in the convalescent plasma group vs 1568 [29%] of 5448 patients in the usual care group; rate ratio 0·99, 95% CI 0·93–1·05; p=0·79). Interpretation: In patients hospitalised with COVID-19, high-titre convalescent plasma did not improve survival or other prespecified clinical outcomes.     Factors Influencing the Selection of Kids’ Preliminary School in Pakistan: A Qualitative Approach

    Purpose: The study aimed at exploring those factors by following the procedures of qualitative research as well as exploration of rich sources of information contemplated by parents in order to get a better idea of schooling institutions. Methodology: The data was collected by interviewing the parents who had lately admitted their kids to the schools and had the selection process fresh in their memory. To provide more valuable insights to the readers and by keeping in view the importance of parents’ mindfulness in the decision making, only those participants were considered who had at least 14 years of education themselves; about half of them are working as faculty members in the education sector in Pakistan. An in-depth qualitative analysis was done using NVIVO 20 software by utilizing the thematic analysis technique. Findings: The findings revealed that numerous factors were considered by the parents such as the affordability of the educational expenses, the cultural values, the transportation issues, involvement of school in extra-curricular activities, the distance from home, the environment, the infrastructure of the school, the faculty and education level as well as the skills and creativity enhancement of kids. Although the educated participants talked about affordability issue but most of them were of the view that quality education of children should not be compromised because of this issue and other expenses could be lessened accordingly. The participants mostly relied on personal visits and consultation with educated people in their access for the collection of information about the institutes. Implications: The readers of the paper could benefit from these decision-making insights of educated citizens as well as of those involved with education sector professionally

    Antecedents and Precedents of On-Job Bullying: A Qualitative Approach

    Purpose: The purpose of this study is to identify the antecedents and precedents of on-job bullying regardless of the gender. Lower the Job stress and on-job bullying, higher the effectiveness and productivity of the employees. Employee engagement has also been identified as a mediator in some studies between on-job bullying and job performance of employees. Somewhere there is a lack of information stating the antecedents and precedents of on-job bullying in a comprehensive manner covering the versatile combination of both manufacturing and service sector. Methodology: This face-to-face interview based qualitative study is conducted on employees of manufacturing firms and banks of Faisalabad and Lahore. Interview Protocol was designed, consisting of four main questions along with multiple probing questions. It has been observed after five interviews that the saturation of data floated among all. The data was thus observed using NVivo 10 software to analyze the data deeply. Multiple techniques like Nodes, Word Tag Clouds, and Word Trees validate the collected data. Findings: This study is vital as it acts as a beacon of light to deprive the concept of this harmful norm of our society. Although this is significant to uproot the belief that women are bullied on job only. This study also provided the general recommendations based upon the suggestions of both the respondents and researcher. Implications: This study paves a path for its audience to identify and mitigate the factors that unbalances their job environment
