Validación del Nursing Activities Score en unidades de cuidados intensivos portuguesas
Objective: to describe the process of adaptation and validation of the Nursing Activities Score to the Portuguese context. Method:
this was a pilot study of adaptation and validation of the Nursing Activities Score with a sample consisting of 67 patients hospitalized
in the intensive care units of three Portuguese hospitals. The construct validity was assessed through factor analysis procedures and
the internal consistency of the items was measured through the Cronbach’s alpha coeffi cient. Results: a mean workload value of
63.04% (SD = 14.25; Median = 61.30) was obtained. Psychometric data revealed a Cronbach’s alpha of 0.71 in the total scale,
indicating an acceptable accuracy. Confi rmatory factor analysis suggested an appropriate adjustment between the model and the
data (χ2 (199) = 214.5, p = 0.214; CFI = 0.95; RMSA = 0.035). Conclusion: in the present study, the Portuguese version of the Nursing
Activities Score was found to be a valid instrument, enabling a safe assessment of the workload of nurses.Objetivo: descrever o processo de adaptação e validação do Nursing Activities Score para o contexto português. Método: trata-se
de um estudo-piloto de adaptação e validação do Nursing Activities Score, com amostra de 67 doentes internados em unidades de
cuidados intensivos de três hospitais portugueses. A validade de constructo avaliou-se mediante procedimentos de análise fatorial e a
consistência interna dos itens através do coefi ciente Alpha de Cronbach. Resultados: obteve-se um valor médio da carga de trabalho
de 63,04% (DP = 14,25; Mediana = 61,30). Os dados psicométricos revelaram um Alpha de Cronbach de 0,71, na escala total,
indicando uma fi delidade aceitável. A análise fatorial confi rmatória sugeriu um ajustamento adequado entre o modelo e os dados
(χ2(199) = 214,5, p = 0,214; CFI = 0,95; RMSA = 0,035). Conclusão: neste estudo, a versão portuguesa do Nursing Activities Score

Ascertainment and Verification of End-Stage Renal Disease and End-Stage Liver Disease in the North American AIDS Cohort Collaboration on Research and Design
The burden of HIV disease has shifted from traditional AIDS-defining illnesses to serious non-AIDS-defining comorbid conditions. Research aimed at improving HIV-related comorbid disease outcomes requires well-defined, verified clinical endpoints. We developed methods to ascertain and verify end-stage renal disease (ESRD) and end-stage liver disease (ESLD) and validated screening algorithms within the largest HIV cohort collaboration in North America (NA-ACCORD). Individuals who screened positive among all participants in twelve cohorts enrolled between January 1996 and December 2009 underwent medical record review to verify incident ESRD or ESLD using standardized protocols. We randomly sampled 6% of contributing cohorts to determine the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of ESLD and ESRD screening algorithms in a validation subcohort. Among 43,433 patients screened for ESRD, 822 screened positive of which 620 met clinical criteria for ESRD. The algorithm had 100% sensitivity, 99% specificity, 82% PPV, and 100% NPV for ESRD. Among 41,463 patients screened for ESLD, 2,024 screened positive of which 645 met diagnostic criteria for ESLD. The algorithm had 100% sensitivity, 95% specificity, 27% PPV, and 100% NPV for ESLD. Our methods proved robust for ascertainment of ESRD and ESLD in persons infected with HIV
Readiness for enhanced self-health management among people with diabetes mellitus Disposición para el control aumentado del régimen terapéutico entre personas con diabetes mellitus Disposição para controle aumentado do regime terapêutico entre pessoas com diabetes mellitus
OBJECTIVE: To identify the nursing diagnosis, readiness for enhanced self-health management, among people with type 2 diabetes who are adherent to pharmacological treatment. METHODS: The sample consisted of 579 subjects from 12 primary care services who received home visits. To determine adherence to the pharmacological treatment, the pill count test was conducted. Those classified as adherent were given a checklist with the defining characteristics of the nursing diagnosis, readiness for enhanced self-health management. RESULTS: Subjects with type 2 diabetes who presented with the nursing diagnosis were not alcoholics (p = 0.003), did not miss their diabetes consultations (p = 0.000), and had no difficulties / inappropriate behaviors with regard to taking their oral antidiabetic agents (p = 0.000). CONCLUSION: The identification of human responses to the clinical treatment of type 2 diabetes should be continuous, so that nurses can understand the factors involved in the readiness to enhance therapeutic control.<br>OBJETIVO: identificar el diagnóstico de enfermerÃa disposición para el control aumentado del régimen terapéutico entre personas con diabetes tipo 2 adheridas al tratamiento farmacológico. MÉTODOS: La muestra estuvo constituida por 579 sujetos de 12 servicios de atención primaria que recibieron visitas domiciliarias. Para determinar la adhesión al tratamiento farmacológico fue realizado el test del recuento de comprimidos. En aquellos clasificados como adherentes se aplicó un check list con las caracterÃsticas definidoras del diagnóstico de enfermerÃa disposición para el control aumentado del régimen terapéutico. RESULTADOS: Los sujetos con DM 2 que presentaron el diagnóstico de enfermerÃa estudiado no eran etÃlicos (p=0,003), no faltaban a las consultas para DM 2 (p=0,000) y no tenÃan dificultades/comportamientos inadecuados en la toma de los antidiabéticos orales (p=0,000). CONCLUSIÓN: La identificación de respuestas humanas frente al tratamiento clÃnico de los sujetos con diabetes tipo 2 debe ser continua para que el enfermero pueda conocer los factores involucrados en una disposición aumentada para el control terapéutico.<br>OBJETIVO: identificar o diagnóstico de enfermagem disposição para controle aumentado do regime terapêutico entre pessoas com diabetes tipo 2 aderentes ao tratamento farmacológico. MÉTODOS: A amostra foi constituÃda de 579 sujeitos de 12 serviços de atenção primária que receberam visitas domiciliares. Para determinar a adesão ao tratamento farmacológico foi realizado o teste da contagem de comprimidos. Naqueles classificados como aderentes foi aplicado um check list com as caracterÃsticas definidoras do diagnóstico de enfermagem disposição para controle aumentado do regime terapêutico. RESULTADOS: Os sujeitos com DM 2 que apresentaram o diagnóstico de enfermagem estudado não eram etilistas (p=0,003), não faltavam as consultas para DM 2 (p=0,000) e não tinham dificuldades/comportamentos inadequados na tomada dos antidiabéticos orais (p=0,000). CONCLUSÃO: A identificação de respostas humanas frente ao tratamento clÃnico dos sujeitos com diabetes tipo 2 deve ser contÃnua para que o enfermeiro possa conhecer os fatores envolvidos numa disposição aumentada para o controle terapêutico
