    Les masques trompeurs de la bipolarité: étude de 100 cas

    Le trouble bipolaire (TB) est une pathologie dont la prévalence est estimée à 1-2 %. Le diagnostic précoce du trouble constitue un enjeu thérapeutique majeur. L'objectif de ce travail est de déterminer les différents diagnostiques attribués aux patients bipolaires avant de recevoir le diagnostic adéquat et de préciser le délai moyen du retard diagnostique. C'est une étude descriptive transversale portant sur 100 patients atteintsde TB, inclus selon les critÚres du DSM V, qui ont été vus en consultation ou bien hospitalisés dans le service de psychiatrie de l'hÎpital Militaire Avicenne de Marrakech, durant une période de deux ans. L'ùge moyen des patients était de 29,5 ans avec une prédominance masculine (80%). 40% de nos patients ont reçu au début un autre diagnostic que celui du TB et le premier diagnostic retenu était celui de l'épisode dépressif majeur dans 36% des cas, suivi de l'accÚs psychotique aigu dans 28% des cas, la schizophrénie dans 16,8% et le trouble de personnalité dans 10,2%. Le délai moyen du retard diagnostic était de 64 mois. 50% des patients ayant reçu un autre diagnostic avaient consulté au moins un psychiatre et 60% des patients avaient été hospitalisés au moins une fois avant le diagnostic du TB.Les errances diagnostiques du TB sont bien établies, conduisant forcément à un retard de prise en charge adéquate .Ces données devraient alerter les psychiatres pour favoriser un meilleur dépistage de la manie et de l'hypomanie qui restent les éléments clé du diagnostic du TB

    Independent Component Analysis for Brain fMRI Does Indeed Select for Maximal Independence

    A recent paper by Daubechies et al. claims that two independent component analysis (ICA) algorithms, Infomax and FastICA, which are widely used for functional magnetic resonance imaging (fMRI) analysis, select for sparsity rather than independence. The argument was supported by a series of experiments on synthetic data. We show that these experiments fall short of proving this claim and that the ICA algorithms are indeed doing what they are designed to do: identify maximally independent sources

    An MPEG-7 scheme for semantic content modelling and filtering of digital video

    Abstract Part 5 of the MPEG-7 standard specifies Multimedia Description Schemes (MDS); that is, the format multimedia content models should conform to in order to ensure interoperability across multiple platforms and applications. However, the standard does not specify how the content or the associated model may be filtered. This paper proposes an MPEG-7 scheme which can be deployed for digital video content modelling and filtering. The proposed scheme, COSMOS-7, produces rich and multi-faceted semantic content models and supports a content-based filtering approach that only analyses content relating directly to the preferred content requirements of the user.     A qualitative investigation of the impact of peer to peer online support for women living with Polycystic Ovary Syndrome

    Background: Polycystic Ovary Syndrome is a common, chronic condition which affects women living with the condition both physically and psychologically. Social support may be beneficial to sufferers in coping with chronic conditions and the Internet is becoming a common place for accessing social support and information. The aim of this study was to consider the experiences of women living with Polycystic Ovary Syndrome who access and participate in an online support group discussion forum dedicated to issues surrounding this condition. Methods: Fifty participants responded to a series of open-ended questions via an online survey. Results: Thematic analysis revealed a number of empowering and disempowering experiences associated with online support group participation. The empowering processes reported by members of the group included: Connecting with others who understand; Access to information and advice; Interaction with healthcare professionals; Treatment-related decision making; Improved adjustment and management. In terms disempowering processes, only two were described by group participants: Reading about the negative experiences of others and Feeling like an outsider. Conclusions: For women living with Polycystic Ovary Syndrome, participation within an online support group may help to empower them in a range of important ways however, there may be some disempowering consequences

    Use of complementary and alternative medicines by a sample of Turkish women for infertility enhancement: a descriptive study

    <p>Abstract</p> <p>Background</p> <p>Infertility patients are a vulnerable group that often seeks a non-medical solution for their failure to conceive. World-wide, women use CAM for productive health, but only a limited number of studies report on CAM use to enhance fertility. Little is known about traditional and religious forms of therapies that are used in relation to conventional medicine in Turkey. We investigated the prevalence and types of complementary and alternative medicine (CAM) used by infertile Turkish women for fertility enhancement.</p> <p>Methods</p> <p>A face-to-face questionnaire inquiring demographic information and types of CAM used for fertility enhancement were completed by hundred infertility patients admitted to a primary care family planning centre in Van, Turkey between January and July 2009.</p> <p>Results</p> <p>The vast majority of infertile women had used CAM at least once for infertility. CAM use included religious interventions, herbal products and recommendations of traditional "hodja's" (faith healers). Of these women, 87.8% were abused in the last 12 months, 36.6% felt not being supported by her partner and 80.5% had never spoken with a physician about CAM.</p> <p>Conclusions</p> <p>Infertile Turkish women use complementary medicine frequently for fertility enhancement and are in need of information about CAM. Religious and traditional therapies are used as an adjunct to, rather than a substitute for, conventional medical therapy. Physicians need to approach fertility patients with sensitivity and should be able to council their patients about CAM accordingly.</p

    Regulatory T cells (T(reg)) suppress T effector cell proliferation and maintain immune homeostasis. Autoimmune liver diseases persist despite high frequencies of T(reg) in the liver, suggesting that the local hepatic microenvironment might affect T(reg) stability, survival, and function. We hypothesized that interactions between T(reg) and endothelial cells during recruitment and then with epithelial cells within the liver affect T(reg) stability, survival, and function. To model this, we explored the function of T(reg) after migration through human hepatic sinusoidal‐endothelium (postendothelial migrated T(reg) [PEM T(reg)]) and the effect of subsequent interactions with cholangiocytes and local proinflammatory cytokines on survival and stability of T(reg). Our findings suggest that the intrahepatic microenvironment is highly enriched with proinflammatory cytokines but deficient in the T(reg) survival cytokine interleukin (IL)‐2. Migration through endothelium into a model mimicking the inflamed liver microenvironment did not affect T(reg) stability; however, functional capacity was reduced. Furthermore, the addition of exogenous IL‐2 enhanced PEM T(reg) phosphorylated STAT5 signaling compared with PEMCD8. CD4 and CD8 T cells are the main source of IL‐2 in the inflamed liver. Liver‐infiltrating T(reg) reside close to bile ducts and coculture with cholangiocytes or their supernatants induced preferential apoptosis of T(reg) compared with CD8 effector cells. T(reg) from diseased livers expressed high levels of CD95, and their apoptosis was inhibited by IL‐2 or blockade of CD95. Conclusion: Recruitment through endothelium does not impair T(reg) stability, but a proinflammatory microenvironment deficient in IL‐2 leads to impaired function and increased susceptibility of T(reg) to epithelial cell‐induced Fas‐mediated apoptosis. These results provide a mechanism to explain T(reg) dysfunction in inflamed tissues and suggest that IL‐2 supplementation, particularly if used in conjunction with T(reg) therapy, could restore immune homeostasis in inflammatory and autoimmune liver disease. Because Melanophila beetles are equipped with infrared receptors and are also attracted by hot surfaces it can be concluded that these infrared receptors are used for fire detection
