The effects of suppressing inflammation by tofacitinib may simultaneously improve glycaemic parameters and inflammatory markers in rheumatoid arthritis patients with comorbid type 2 diabetes: a proof-of-concept, open, prospective, clinical study
Background: A consistent connection has been increasingly reported between rheumatoid arthritis (RA), insulin resistance (IR), and type 2 diabetes (T2D). The β-cell apoptosis induced by pro-inflammatory cytokines, which could be exaggerated in the context of RA, is associated with increased expression pro-apoptotic proteins, which is dependent on JAnus Kinase/Signal Transducer and Activator of Transcription (JAK/STAT) activation. On these bases, we aimed to evaluate if the administration of tofacitinib, a potent and selective JAK inhibitor, could simultaneously improve glycaemic parameters and inflammatory markers in patients with RA and comorbid T2D. Methods: The primary endpoint was the change in the 1998-updated homeostatic model assessment of IR (HOMA2-IR) after 6 months of treatment with tofacitinib in RA patients with T2D. Consecutive RA patients with T2D diagnosis were included in this proof-of-concept, open, prospective, clinical study, which was planned before the recent emergence of safety signals about tofacitinib. Additional endpoints were also assessed regarding RA disease activity and metabolic parameters. Results: Forty consecutive RA patients with T2D were included (female sex 68.9%, mean age of 63.4 ± 9.9 years). During 6-month follow-up, a progressive reduction of HOMA2-IR was observed in RA patients with T2D treated with tofacitinib. Specifically, a significant effect of tofacitinib was shown on the overall reduction of HOMA2-IR (β = − 1.1, p = 0.019, 95%CI − 1.5 to − 0.76). Also, HOMA2-β enhanced in these patients highlighting an improvement of insulin sensitivity. Furthermore, although a longer follow-up is required, a trend in glycated haemoglobin reduction was also recorded. The administration of tofacitinib induced an improvement in RA disease activity, and a significant reduction of DAS28-CRP and SDAI was observed; 76.8% of patients achieved a good clinical response. In this study, no major adverse events (AEs) were retrieved without the identification of new safety signals. Specifically, no life-threatening AEs and cardiovascular and/or thromboembolic events were recorded. Conclusions: The administration of tofacitinib in RA with T2D led to a simultaneous improvement of IR and inflammatory disease activity, inducing a “bidirectional” benefit in these patients. However, further specific designed and powered studies are warranted to entirely evaluate the metabolic effects of tofacitinib in RA patients with T2D
POS1344 EVALUATING THE MULTIVISCERAL INVOLVEMENT ON ADULT-ONSET STILL'S DISEASE TO RETRIEVE IMAGING-BASED DIFFERENCES IN PATIENTS WITH AND WITHOUT MACROPHAGE ACTIVATION SYNDROME; RESULTS FROM A SINGLE-CENTRE OBSERVATIONAL STUDY
Background:Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder usually affecting young adults, burdened by life-threatening complications, mainly macrophage activation syndrome (MAS), a secondary form of hemophagocytic lymphohistiocytosis [1]. In this context, the importance of an accurate assessment of AOSD is suggested to promptly recognise the multivisceral involvement of the disease which is associated with life-threatening complications. The assessment of the most aggressive subsets of the disease could guide the clinicians when to apply additional resources but avoiding unnecessary expenditures in patients with a less severe clinical picture.Objectives:In this study, we aimed at describing the multivisceral involvement of the disease to retrieve imaging-based differences in AOSD patients with and without MAS.Methods:The present evaluation has been designed as a cross-sectional study to descriptively compare the multivisceral involvement in AOSD patients with and without MAS. Patients admitted to our Institution, who underwent a total body CT scan, were selected from our historical cohort and assessed. Clinical and CT scan characteristics of AOSD patients with and without MAS were compared. Clinical and CT scan characteristics of AOSD patients with and without MAS were analysed by parametric or non-parametric t tests for all continuous variables, and chi squared test was used for categorical ones, as appropriate. Furthermore, possible correlations among radiological outcomes with laboratory markers and systemic score were estimated by using a point-biserial coefficient correlation.Results:This study evaluated 39 AOSD patients (men 64.1%), mean age of 48.8±16.6 years). Out of those, 14 patients (35.9%) were complicated by MAS. These patients showed higher values of ferritin [AOSD: 770.0 (1306.5) ng/mL vs MAS: 2926.3 (4918.5) ng/mL p=0.003] and systemic score (AOSD: 4.6±1.4 vs MAS: 6.9±1.7, p<0.0001). AOSD patients with MAS presented a higher prevalence of lung disease than others (AOSD: 56.0% vs MAS 85.7% p=0.048). Lung disease correlated with the systemic score (coefficient 0.491, p=0.003). AOSD patients with MAS were more frequently characterised by hepatomegaly (AOSD: 12.0% vs MAS: 50.0% p=0.019) and splenomegaly (AOSD: 16.0% vs MAS 50.0% p=0.033), respectively, than others. Hepatomegaly correlated with CRP (coefficient 0.421, p=0.016), ferritin (coefficient 0.397, p=0.020), and systemic score (coefficient 0.391, p=0.022). Furthermore, the presence of splenomegaly correlated with the systemic score (coefficient 0.439, p=0.009). CT scan features of abdominal effusions were more frequently observed in AOSD patients with MAS than those without this complication (AOSD: 12.0% vs 57.1% p=0.007). Finally, a higher percentage of AOSD patients with MAS showed a significant lymph node enlargement, either mediastinal or abdominal, than others on CT scan (AOSD: 36.0% vs MAS 71.4% p=0.048). The presence of lymphadenomegaly correlated with the systemic score (coefficient 0.368, p=0.032).Conclusion:Our findings showed a higher prevalence of multiorgan involvement in AOSD patients with MAS, suggesting imaging-based differences, although other studies are needed to fully assess this issue. Pulmonary disease, hepatomegaly, splenomegaly, lymph nodes enlargement, and abdominal effusions were associated with these more aggressive patients.References:[1]Giacomelli R, Ruscitti P, Shoenfeld Y. Changes of statistical structural fluctuations unveils an early compacted degraded stage of PNS myelin
Degradation of the myelin sheath is a common pathology underlying
demyelinating neurological diseases from Multiple Sclerosis to
Leukodistrophies. Although large malformations of myelin ultrastructure in the
advanced stages of Wallerian degradation is known, its subtle structural
variations at early stages of demyelination remains poorly characterized. This
is partly due to the lack of suitable and non-invasive experimental probes
possessing sufficient resolution to detect the degradation. Here we report the
feasibility of the application of an innovative non-invasive local structure
experimental approach for imaging the changes of statistical structural
fluctuations in the first stage of myelin degeneration. Scanning micro X-ray
diffraction, using advances in synchrotron x-ray beam focusing, fast data
collection, paired with spatial statistical analysis, has been used to unveil
temporal changes in the myelin structure of dissected nerves following
extraction of the Xenopus laevis sciatic nerve. The early myelin degeneration
is a specific ordered compacted phase preceding the swollen myelin phase of
Wallerian degradation. Our demonstration of the feasibility of the statistical
analysis of SmXRD measurements using biological tissue paves the way for
further structural investigations of degradation and death of neurons and other
cells and tissues in diverse pathological states where nanoscale structural

BEYOND WAIST CIRCUMFERENCE IN AN ADULT MALE POPULATION OF SOUTHERN ITALY: IS THERE ANY ROLE FOR SUBSCAPULAR SKINFOLD THICKNESS IN THE RELATIONSHIP BETWEEN INSULIN-LIKE GROWTH FACTOR-1 SYSTEM AND METABOLIC PARAMETERS ?
ABSTRACT
Background: Apart from waist circumference, other adiposity measures, such as subscapular skin fold (SST), arouse growing interest due to their relationship to metabolic complications and cardiovascular risk. The Insulin-like Growth Factor (IGF)-1 system is deregulated in obese subjects in proportion to their degree of visceral adiposity. Aim : To examine the association among IGF-1, IGF-Binding Protein (BP)1 and 3 levels and different measures of adiposity in a sample of adult male population in Southern Italy. Materials and Methods: A complete database for this analysis was available for 229 (age range 50–82 years) participating at 2002-2004 Olivetti Heart Study follow-up. Results: After adjustment for age, IGF-1 was inversely associated with BMI and waist circumference (p<0.05). IGFBP1 was inversely associated with BMI, waist circumference, SST, Homeostasis Model Assessment (HOMA) index, Fat Mass (FM). HOMA index, age and SST significantly predicted the IGFBP1 plasma levels, with 24% of IGFPB-1 variability explained at a linear regression analysis. Conclusions: IGFBP1 inversely correlated to adiposity and HOMA index. Among adiposity indexes, SST was the best predictor of IGFPB-1 levels. The evaluation of some components of the IGFs system, and simple measures of body adiposity, such as SST, may represent a further tool to better evidence phenotype profiles associated to the pathogenetic mechanism of cardiovascular risk factor clustering in male adults
The N-terminal ricin propeptide influences the fate of ricin A-chain in tobacco protoplasts.
The plant toxin ricin is synthesized in castor bean seeds as an endoplasmic reticulum (ER)-targeted precursor. Removal of the signal peptide generates proricin in which the mature A- and B-chains are joined by an intervening propeptide and a 9-residue propeptide persists at the N terminus. The two propeptides are ultimately removed in protein storage vacuoles, where ricin accumulates. Here we have demonstrated that the N-terminal propeptide of proricin acts as a nonspecific spacer to ensure efficient ER import and glycosylation. Indeed, when absent from the N terminus of ricin A-chain, the non-imported material remained tethered to the cytosolic face of the ER membrane, presumably by the signal peptide. This species appeared toxic to ribosomes. The propeptide does not, however, influence catalytic activity per se or the vacuolar targeting of proricin or the rate of retrotranslocation/degradation of A-chain in the cytosol. The likely implications of these findings to the survival of the toxin-producing tissue are discussed

The loss of energy and environmental efficiency of passenger mobility in the Big La Plata, between 1993 and 2013
La forma de crecimiento de las ciudades argentinas en los últimos años ha incrementado la demanda de movilidad de pasajeros y la ineficiencia energética y ambiental de cada viaje. En este trabajo se presenta una evaluación energética y ambiental del sistema de transporte de pasajeros de la ciudad de La Plata entre 1993 y 2013, a efectos de dimensionar la pérdida de eficiencia de dicho sistema. La información de transporte se basa en encuestas de movilidad y la construcción de indicadores a partir de los valores del modelo Leap adaptados por la fundación Bariloche al caso argentino. Los resultados permiten establecer niveles de criticidad a las variables involucradas siendo la primera el incremento de las tasas de generación de viajes en automóvil, incluyendo los particulares y los autos de alquiler, y la segunda es el incremento de la distancia media de viajes por modo, especialmente por los modos individuales. Las conclusiones sugieren puntos focales para la aplicación de medidas de eficiencia energética y ambiental en el binomio integrado del desarrollo urbano y la movilidad de pasajeros.The form of growth of the Argentine cities in recent years has increased the demand for passenger mobility and energy and environmental inefficiency of each trip. In this paper an energy and environmental assessment passenger transport system of the city of La Plata between 1993 and 2013, in order to gauge the loss of efficiency of the system we are presented. Transportation information is based on surveys of mobility and construction of indicators based on the values of LEAP model adapted by the Argentine Bariloche Foundation case. The results indicate levels of criticality to the variables involved being the first increased generation rates of car trips, including individuals and rental cars, and the second is the increase in the average distance of travel by mode, especially for the individual modes. The findings suggest focal points for the implementation of measures of energy and environmental efficiency binomial integrated urban development and mobility of passengers.Instituto de Investigaciones y Políticas del Ambiente Construid
The role of CDC48 in the retro-translocation of non-ubiquitinated toxin substrates in plant cells
When the catalytic A subunits of the castor
bean toxins ricin and Ricinus communis
agglutinin (denoted as RTA and RCA A,
respectively) are delivered into the
endoplasmic reticulum (ER) of tobacco
protoplasts, they become substrates for ER-associated
protein degradation (ERAD). As
such, these orphan polypeptides are retro-translocated
to the cytosol, where a significant
proportion of each protein is degraded by
proteasomes. Here we begin to characterise
the ERAD pathway in plant cells, showing
that retro-translocation of these lysine-deficient
glycoproteins requires the ATPase
activity of cytosolic CDC48. Lysine
polyubiquitination is not obligatory for this
step. We also show that while RCA A is found
in a mannose-untrimmed form prior to its
retro-translocation, a significant proportion of
newly synthesised RTA cycles via the Golgi
and becomes modified by downstream
glycosylation enzymes. Despite these
