Objectives: This study aims to investigate whether or not radial deviation developing after wrist involvement of rheumatoid arthritis (RA) is a cause of median nerve swelling. Patients and methods: The study included 51 RA patients (12 males, 39 females; mean age 50.9±8.9 years; range, 18 to 65 years) without carpal tunnel syndrome (CTS) detected by electroneuromyography. Duruöz hand index, visual analog scale, and painDETECT questionnaire were performed in clinical assessment. Radiographic measurements including radial inclination (RI) angle were performed. Using ultrasonography, the median nerve cross-sectional areas (CSAs) were measured from the four levels of the distal one third of the forearm, radioulnar joint, pisiform bone, and hook of hamate, while the ulnar nerve CSAs were measured from the pisiform bone. Results: The study was completed with 102 hands of 51 patients. A negative correlation was found between the RI and the median CSAs measured from the radioulnar joint (R=-0.49; p=0.00), the pisiform bone (R=-0.45; p=0.00), and hook of hamate (R=-0.60, p=0.00). When the hands were divided into three groups according to the ranges of RI specified in the literature, the median nerve CSA was found to be significantly higher in the group with low RI at these levels (p<0.001). Conclusion: In patients with RA without CTS, the increase in the median nerve CSAs may be associated with radiographic measures such as radial deviation. ©2019 Turkish League Against Rheumatism. To evaluate long- term effects of Ahmed glaucoma valve (AGV) implantation in refractory glaucoma cases on intraocular pressure, visual acuity and cornea. The cases who had undergone AGV implantation between the years 2003 to 2014 with the diagnosis of refractory glaucoma were evaluated retrospectively. The cases with minimal light perception visual acuity and followed up for longer than 12 months were selected. Preoperative and postoperative corrected distance visual acuity levels (CDVA), intraocular pressure (IOP), number of medications and central corneal thickness (CCT) were recorded. Complications and additional surgeries were noted. The complete surgical cure was based on following considerations: no need for additional surgery, achievement of mean IOP level between 5-22 mmHg, stability of visual acuity and corneal transparency. Forty eight eyes of 47 refractory glaucoma cases with neovascular (n=13), aphasic (n=13), secondary to penetrating keratoplasty (PK) (n=7), congenital (n=4), traumatic (n=5) and primary refractory glaucoma (n=6) were evaluated. Mean age (50.40±22.80 years), mean follow-up period ( 32.33±20.78 (12-84) months), preoperative 1.50±0.61 LogMAR, and postoperative CDVA 1.60±0.57 LogMAR were determined. IOP values ( 39.22±9.47 vs 17.02±4.29 mmHg), number of medications (3.47±0.92 vs 1.52±1.09), CCT values (590.00±88.86 vs 655.25 ±170.88µm (p<0.05 were also estimated. Additional surgeries were performed including drainage of suprachoroidal hemorrhage (n=1), cleaning of AGV tip (n=5), anterior vitrectomy (n=5), AGV reposition (n=3), AGV tip shortening (n=6), corneal graft insufficiency related PK (n=3), and additional cyclodestructive procedures (n=5). Perfect surgical, total surgical success rates were detected as 46 % (n=22), and 89.4% (n=43),respectively. However surgery failed in 5 (10.6%) cases AGV was a considerable successful surgical option for decreasing the IOP in refractory glaucoma cases. However even there is not a significant AGV endothelial touch, endothelial insufficiency related corneal edema and eventual visual loss was seen. © 2018, Logos Medical Publishing. Origin and geodynamic environments of the metamorphic sole rocks from the İzmir-Ankara-Erzincan suture zone (Tokat, Northern Turkey)

The Late Cretaceous accretionary complex of the İzmir–Ankara–Erzincan suture zone, near Artova, is composed mainly of peridotites (variably serpentinized), amphibolite, garnet-micaschist, calc-schist, marble, basalt, sandstones, neritic limestones. The metamorphic rocks were interpreted as the metamorphic sole rocks occurring at the base of mantle tectonites, because: (i) amphibolites were observed together with the serpentinized peridotites suggesting their occurrences in the oceanic environment; (ii) foliation in amphibolites and serpentinized peridotites run subparallel to each other; (iii) all these metamorphic rocks and serpentinized peridotites are cross-cut by the unmetamorphosed dolerite dikes with island arc tholeiite-like chemistry. Geochemical characteristics of the amphibolites display enriched mid-ocean ridge basalt (E-MORB)- and ocean island basalt (OIB)-like signatures. The dolerite dikes, on the other hand, yield an island arc tholeiite-like composition. Geothermobarometric investigations of the metamorphic sole rocks suggest that the metamorphic temperature was ~650 ± 30°C and the pressure condition was less than 0.5 GPa. Dating of hornblende grains from amphibolite yielded age values ranging from 139 ± 11 to 157 ± 3.6 Ma (2σ). The oldest weighted average age value is regarded as approximating the timing of the intra-oceanic subduction. These cooling ages were interpreted to be the intra-oceanic subduction/thrusting time of the İzmir–Ankara–Erzincan oceanic domain

Aim This study aimed to test the validity and reliability of the Turkish version of the Nursing Teamwork Survey, which was developed to measure the concept of nursing teamwork. Method Performed as a methodological study with a cross-sectional design, the study included 486 bedside nurses in three different types of hospitals in Istanbul. The data were collected using the Nursing Teamwork Survey between October and December 2018. The scale includes five subscales and 33 items. The data were analysed using SPSS 22.0 and LISREL 8.7. Results Nurses' mean age was 29.82 (SD= 7.33). The content validity index of the scale was 0.94. Confirmatory factor analysis conducted on the original form showed that all items fell within the subscales of the original scale and that their factor loads were between 0.48 and 0.91. Following factor analysis, the factor structure of the original scale was valid; however, suggested modifications were applied to improve the results. The obtained goodness-of-fit indices were close to the excellent range according to literature. Cronbach's alpha was 0.95 for the scale. Conclusion The scale is a valid and reliable tool for measuring teamwork among nurses

PubMed ID: 27511346Background/aim: Broad-spectrum antibiotics have become available for use only with the approval of infectious disease specialists (IDSs) since 2003 in Turkey. This study aimed to analyze the tendencies of doctors who are not disease specialists (non-IDSs) towards the restriction of antibiotics. Materials and methods: A questionnaire form was prepared, which included a total of 22 questions about the impact of antibiotic restriction (AR) policy, the role of IDSs in the restriction, and the perception of this change in antibiotic consumption. The questionnaire was completed by each participating physician. Results: A total of 1906 specialists from 20 cities in Turkey participated in the study. Of those who participated, 1271 (67.5%) had ?5 years of occupational experience (junior specialists = JSs) and 942 (49.4%) of them were physicians. Specialists having >5 years of occupational experience in their branch expressed that they followed the antibiotic guidelines more strictly than the JSs (P < 0.05) and 755 of physicians (88%) and 720 of surgeons (84.6%) thought that the AR policy was necessary and useful (P < 0.05). Conclusion: This study indicated that the AR policy was supported by most of the specialists. Physicians supported this restriction policy more so than surgeons did. © TÜBITAK
