    Factors Contributing to Medication Adherence in Patients with a Chronic Condition : A Scoping Review of Qualitative Research

    Introduction: Medication adherence continues to be a significant challenge in healthcare, and there is a shortage of effective interventions in this area. This scoping review studied the patient-related factors of medication adherence. Methods: We searched Medline Ovid, Scopus, and Cochrane Library from January 2009 to June 2021 to find the most recent original qualitative studies or systematic reviews that addressed the patient-related factors of medication adherence in treating chronic conditions. We used the PRISMA-ScR checklist to ensure the quality of the study. Results: The initial search revealed 4404 studies, of which we included 89 qualitative studies in the scoping review. We inductively organized the patient-related factors causing barriers, as well as the facilitators to medication adherence. The studies more often dealt with barriers than facilitators. We classified the factors as patient-specific, illness-specific, medication-related, healthcare and system-related, sociocultural, as well as logistical and financial factors. Information and knowledge of diseases and their treatment, communication, trust in patient-provider relationships, support, and adequate resources appeared to be the critical facilitators in medication adherence from the patient perspective. Discussion and conclusions: Patients are willing to discuss their concerns about medications. Better communication and better information on medicines appear to be among the critical factors for patients. The findings of this scoping review may help those who plan further interventions to improve medication adherence.Peer reviewe

    The Economic Burden and Non-Adherence Tuberculosis Treatment in Indonesia: Systematic Review

    Until today, Tuberculosis (TB) is still taking part as a world health problem, especially in The South East Asia Region. TB in South East Asia contributes almost 50% of the worldwide burden. Indonesia is one of the regional countries that provide to the statistics, and the World Health Organization (WHO) enlisted it in the 30 High-TBBurden Countries. The high number of TB in Indonesia mostly caused by a lack of case registration, non-adherence of treatment, and lost to follow-up. This study conducted an overview of systematic review based on Prisma Protocol to systematically and objectively focus on the economic burden as a risk factor of non-adherence of TB treatment in Indonesia. Relevant literature was searched from PubMed, Science Direct,Google Scholar and reviewed abstracts and full-text article based on eligibility criteria. Seven studies included in this review after selected using inclusion and exclusion criteria. The studies revealed a consistent association between economic burden as a risk factor of non-adherence of TB treatment. In conclusion, financial burden was significant with non-adherence of TB treatment and most of TB patient who stopped the treatment should spare their family spending into TB treatment and this often constitutes to more considerable financial risk for them.     Improving Prolonged Antibiotics Education in a Pediatric Unit

    Problem Children’s Hospital A\u27s incidence report tool, iCares, demonstrates that 50% of the issues related to prolonged intravenous (IV) antibiotics are categorized as administration issues. This leads to the medication being delivered at the wrong time or rate. Having these issues increases patient length of stay, increasing hospital costs. Context The microsystems for this quality improvement project include inpatient pediatric patient care units (PCU) 300, 360, and 400. Children’s Hospital A is a 361-bed San Francisco Bay Area hospital. Interventions The Clinical Nurse Leader students implemented a presentation defining prolonged IV antibiotics, their rationale, and a video demonstrating the correct pump setup. This presentation was shown at an in-person staff meeting on April 11, 2024, to 90 nurses and nurse managers. Measures To assess nurses’ baseline knowledge, a pre-video quiz was deployed before the intervention was presented. A post-video quiz was utilized to assess the impact of the video on nurses’ knowledge. Results When comparing baseline knowledge to post-intervention knowledge, the results went from 86.9% to 95.5%, reflecting an 8.6% improvement. Conclusions This project comprehensively assessed the microsystem using a needs assessment tool, identified the problem, and conducted a root-cause analysis. The students analyzed data to implement an educational tool that increased nurse knowledge regarding prolonged IV antibiotics. The tool is cost-effective, and easily attained ensuring sustainability. The video will be evaluated biannually utilizing iCares data to assess long-term impacts

    Potential therapeutic effects of pigment epithelium-derived factor for treatment of diabetic retinopathy

    Diabetic retinopathy (DR), a major micro-vascular complication of diabetes, has emerged as a leading cause of visual impairment and blindness among working adults in the worldwide. The pathobiology of DR involves multiple molecular pathways and is characterized chronic neurovascular degeneration. Current approaches to prevent or to treat DR are still far from satisfactory. Therefore, it is important to develop new therapeutic strategies for the prevention and treatment to DR. Pigment epithelium-derived factor (PEDF), a 50-kDa secreted glycoprotein, has been described as a multi-functional protein. Some emerging evidences indicate that PEDF are able to target multiple pathways exerting neurotropic, neuroprotective, anti-angiogenic, antivasopermeability, anti-inflammation, anti-thrombogenic and anti-oxidative effects in DR.     Pijat bayi sejak lama telah diketahui bermanfaat dalam membantu tumbuh kembang bayi dan merupakan tradisi turun temurun. Manfaat pijat bayi telah ditunjukkan dalam berbagai penelitian, namun kurangnya pengetahuan ibu tentang cara memijat bayi yang benar turut mendorong para ibu mencari layanan pijat bayi baik ke dukun maupun baby spa. Faktor – faktor tersebut memungkinkan RSU Bhakti Rahayu untuk mengembangkan layanan Baby Care pada tahun 2020 sebagai wujud kontinuitas layanan yang diberikan kepada ibu pasca bersalin. Tujuan penelitian ini adalah untuk menganalisis strategi pemasaran layanan Baby Care RSU Bhakti Rahayu Denpasar tahun 2020. Desain penelitian yang digunakan adalah deskriptif dengan pendekatan potong lintang. Analisis dilakukan dengan melakukan segmentation, targeting, dan positioning dari layanan Baby Care, dilanjutkan menentukan bauran pemasaran 4P (Product, Price, Place, dan Promotion). Layanan Baby Care memiliki delapan produk dengan harga yang terjangkau dan kompetitif, serta kegiatan yang dilakukan di rumah akan mempermudah ibu dalam mendapatkan layanan. Promosi layanan Baby Care perlu diperluas dan evaluasi lebih lanjut perlu dilakukan sehingga dapat diketahui mengenai pemanfaatan layanan Baby Care di RSU Bhakti Rahayu Denpasar

    Distance and Similarity Measurements of P Waves Before and After Pulmonary Vein Isolation in Patients with Atrial Fibrillation

    [EN] This study aimed to assess electric markers obtained from the surface electrocardiogram in order to analyse significant differences before and after pulmonary vein isolation in patients who suffered from paroxysmal atrial fibrillation. 30 patients who underwent catheter ablation (in order to permanently restore sinus rhythm and stop atrial fibrillation episodes) were included in the study. Both surface electrocardiogram and intracavitary recordings were simultaneously acquired starting some minutes before catheter ablation began until the whole procedure successfully ended. P-waves have been delineated on V1 lead, and measurements of distances and similarities between them have been obtained to compare the recordings. It has been found that distances between Pwaves significatively decrease (about 14%) whereas similarities significatively increase (about 3%) after catheter ablation. The use of these features would help to identify the success of the catheter ablation procedure, which is the main objective of this preliminary study: the non-invasive identification of spontaneous reconnection of pulmonary veins, the main cause of the arrhythmia recurrences.N.     Can Desarda Technique Repair all Adult Inguinal Hernia?

    Objectives: This paper focuses on reporting the results of the daily use of the Desarda technique for the treatment of adult inguinal hernias without selection of patients at national hospital of Niamey. Patients and Method: This was a prospective and descriptive study from December 2012 to August 2014. It included 76 patients aged over 15 years who underwent inguinal hernia repair by Desarda technique without any selection. The variables studied include the type of inguinal hernia, the Nyhus class of inguinal hernia, the operating time, the postoperative pain, and the hernia recurrences. Results: There were 74 men and 2 women. The average age of the patients was 43.6 years [± 17.6]. Inguinal hernia was found in 57.9% of patients. There were 76.31% simple hernias, 15.80% recurrent hernias, 2.63% intractable hernias by loss of domain, and 5.26% strangulated hernias. 90.79% of the patients were operated under spinal anesthesia. The most frequent types of inguinal hernia were Nyhus type 2 found in 35.52% of patients, followed by Nyhus type 3b found in 30.26% of patients. The mean operative time was 46.67 [± 11.5] minutes. The immediate operative follow-up was marked by the occurrence of a hematoma of the scrotum in two patients. There was no recurrence of chronic postoperative pain after a median follow up of 40 months. Conclusion: The Desarda technique could be a good alternative for the cure of all inguinal hernias of adults

    A Domain Model for Transparency in Portuguese Cooperatives

    The aim of this chapter is to present a domain model that represents the informational needs of transparency (governance structure and accountability dimensions) in Portuguese cooperatives. A domain model is an abstract representation of a reality and a milestone in the development of a metadata application profile (MAP). A community of practice publishes linked open MAP-based data for these data to be interoperable; this means intelligent software/agents can aggregate these data, provide different types of visualizations, infer from the data, and ultimately provide new discoveries. This model was developed having as basis the information obtained from the accomplishment of a focus group, and the analysis of financial reports and websites of seven Portuguese cooperatives. The authors will continue to work on the domain model to include 1) other dimensions that also contribute for transparency in the organizations and 2) other types of entities of the social economy (SE).
