    Emerging product-process archetypes in oncology: informing the sustainable provision of next-generation medicines

    The emergence of more targeted molecular therapies has contributed to accelerated growth within the oncology market. Projected to become the leading therapeutic area by 2017, forecast spends are expected to be in the range of $74-84 billion. Coupled with its many specificities around pricing, insurance implications, and ethics, we argue that the oncology segment may best inform future pharmaceutical value network design characteristics - in supporting the sustainable manufacture and supply of next-generation medicines. Through exploration of future state scenarios and opportunities areas, driven by the adoption of emerging process and digital technologies, a base framework is extended to enable a systematic assessment of a series of candidates representative of the wider oncology market. These include niche, low volume drugs on-patent with high QALYs (quality-adjusted life years), through to higher volume generics with a history of supply shortages. A series of emerging product-process ‘archetypes’ in oncology are proposed – classified as ‘New Niche’, ‘Old Niche’ and ‘Established Generics’ – with associated models for reconfiguration, based on the clustering of potential supply benefits. A key application of this systems approach is the potential of informing economies of drug ‘repurposing’, through its extension from commercial to drug discovery, development and clinical trial contexts, and in matching emerging process capabilities to future adaptive supply requirements – for the sustainable provision of next-generation medicines

    Pharmaceuticals supply chain management:buffering & bridging response strategies in shortage management

    There exists a growing and tenacious challenge in the medicine supply chain, to manage the consequences of supply chain disruptions. Hospitals experience frequent shortages, in a European based study, 45% of respondents indicated that life preserving drugs were affected by shortages (Pauwels 2015). This research seeks to augment theoretical understanding underlying this significant issue and provide advice in the management of shortages which is a critical issue within the pharmaceutical industry. Previous research has focused on the antecedents of supply chain disruptions; what happens after a disruption has received scant attention. This research acknowledges that gap and seeks to build on the Bode et al. (2011), model of organisational responses to supply chain disruptions. This study uses a research onion design as a stencil to guide the pluralistic methodological approach chosen to release the multifaceted dimensions of the problem under consideration. Data collection instruments were interviews, an online survey, and focus groups. It was based on 318 acute care hospitals in the UK and Ireland. Findings indicate there are patterns of responses, and shortage performances vary depending on the length of time since the disruption and response mixture chosen. This research gives new insights into the impact of supply chain disruption response behaviour and supply chain performance in shortage management. It provides additional empirical tests to the Bode et al. (2011) normative model, providing insights into the underlying balanced theories, through examination of three constructs: supply chain disruption orientation; supply chain disruption performance and organisational response. It extends the concept of supply chain disruption orientation and highlights how response behaviour can feed back into the dynamic decision-making process, augmenting the Bode et al. (2011) framework. It provides insights to managers taking a system-wide view on drug delivery performance

    Integrated Functional Sanitation Value Chain

    The value chain (VC) system is a key way to address important sanitation technological and institutional gaps in production and service delivery and could constitute a natural platform for development actions and also serve as a market systems approach to improve access to safely-managed sanitation. It has been suggested that sanitation could boost local and national economies and global interconnections with a growing recognition that the private sector can play a bigger role in delivering the Sustainable Development Goal for sanitation, and help businesses understand value-added and product opportunities. This book proposes a pathway towards re-thinking the sanitation value chain (SVC) and suggests that it should cover all processes, activities and products of enterprises/actors in the sanitation supply chain that provide value-added services within each stage. Following the Regenerative Sanitation Principles, this book presents a new perspective to the SVC known as the 'integrated functional sanitation value chain' (IFSVC) to address operational functions within sanitation systems in combination with sanitation enterprises, operators and external actors that support the growth of the sanitation economy. The underlying premise of this book is that the IFSVC represents a new perspective that would have major social, environmental and economic implications for local, national, regional and global sanitation service delivery. It is hoped that researchers, business leaders, entrepreneurs, government officials and funders will find this book valuable, and be inspired and enabled to carry sanitation work forward in their own spheres of operation. The book gives several examples of encouraging developments, particularly in technical and business model innovation. It is our hope that this book will provide the stimulus for new learning and its application, particularly through cross-disciplinary and cross-sector partnerships that bring together all the skills and capabilities needed to deliver a fully effective IFSVC

    A systems thinking approach for modelling supply chain risk propagation

    Supply Chain Risk Management (SCRM) is rapidly becoming a most sought after research area due to the influence of recent supply chain disruptions on global economy. The thesis begins with a systematic literature review of the developments within the broad domain of SCRM over the past decade. Thematic and descriptive analysis supported with modern knowledge management techniques brings forward seven distinctive research gaps for future research in SCRM. Overlapping research findings from an industry perspective, coupled with SCRM research gaps from the systematic literature review has helped to define the research problem for this study. The thesis focuses on a holistic and systematic approach to modelling risks within supply chain and logistics networks. The systems thinking approach followed conceptualises the phenomenon of risk propagation utilising several recent case studies, workshop findings and focus studies. Risk propagation is multidimensional and propagates beyond goods, finance and information resource. It cascades into technology, human resource and socio-ecological dimensions. Three risk propagation zones are identified that build the fundamentals for modelling risk behaviour in terms of cost and delay. The development of a structured framework for SCRM, a holistic supply chain risk model and a quantitative research design for risk assessment are the major contributions of this research. The developed risk assessment platform has the ability to capture the fracture points and cascading impact within a supply chain and logistics network. A reputed aerospace and defence organisation in UK was used to test the experimental modelling set up for its viability and for bridging the gap between theory and practice. The combined statistical and simulation modelling approach provides a new perspective to assessing the complex behavioural performance of risks during multiple interactions within network
