    Applicability of in vivo staging of regional amyloid burden in a cognitively normal cohort with subjective memory complaints: the INSIGHT-preAD study.

    BACKGROUND:Current methods of amyloid PET interpretation based on the binary classification of global amyloid signal fail to identify early phases of amyloid deposition. A recent analysis of 18F-florbetapir PET data from the Alzheimer's disease Neuroimaging Initiative cohort suggested a hierarchical four-stage model of regional amyloid deposition that resembles neuropathologic estimates and can be used to stage an individual's amyloid burden in vivo. Here, we evaluated the validity of this in vivo amyloid staging model in an independent cohort of older people with subjective memory complaints (SMC). We further examined its potential association with subtle cognitive impairments in this population at elevated risk for Alzheimer's disease (AD). METHODS:The monocentric INSIGHT-preAD cohort includes 318 cognitively intact older individuals with SMC. All individuals underwent 18F-florbetapir PET scanning and extensive neuropsychological testing. We projected the regional amyloid uptake signal into the previously proposed hierarchical staging model of in vivo amyloid progression. We determined the adherence to this model across all cases and tested the association between increasing in vivo amyloid stage and cognitive performance using ANCOVA models. RESULTS:In total, 156 participants (49%) showed evidence of regional amyloid deposition, and all but 2 of these (99%) adhered to the hierarchical regional pattern implied by the in vivo amyloid progression model. According to a conventional binary classification based on global signal (SUVRCereb = 1.10), individuals in stages III and IV were classified as amyloid-positive (except one in stage III), but 99% of individuals in stage I and even 28% of individuals in stage II were classified as amyloid-negative. Neither in vivo amyloid stage nor conventional binary amyloid status was significantly associated with cognitive performance in this preclinical cohort. CONCLUSIONS:The proposed hierarchical staging scheme of PET-evidenced amyloid deposition generalizes well to data from an independent cohort of older people at elevated risk for AD. Future studies will determine the prognostic value of the staging approach for predicting longitudinal cognitive decline in older individuals at increased risk for AD

    A GPU-based Implementation for Improved Online Rebinning Performance in Clinical 3-D PET

    Online rebinning is an important and well-established technique for reducing the time required to process Positron Emission Tomography data. However, the need for efficient data processing in a clinical setting is growing rapidly and is beginning to exceed the capability of traditional online processing methods. High-count rate applications such as Rubidium 3-D PET studies can easily saturate current online rebinning technology. Realtime processing at these high-count rates is essential to avoid significant data loss. In addition, the emergence of time-of-flight (TOF) scanners is producing very large data sets for processing. TOF applications require efficient online Rebinning methods so as to maintain high patient throughput. Currently, new hardware architectures such as Graphics Processing Units (GPUs) are available to speedup data parallel and number crunching algorithms. In comparison to the usual parallel systems, such as multiprocessor or clustered machines, GPU hardware can be much faster and above all, it is significantly cheaper. The GPUs have been primarily delivered for graphics for video games but are now being used for High Performance computing across many domains.     Novel PET Systems and Image Reconstruction with Actively Controlled Geometry

    Positron Emission Tomography (PET) provides in vivo measurement of imaging ligands that are labeled with positron emitting radionuclide. Since its invention, most PET scanners have been designed to have a group of gamma ray detectors arranged in a ring geometry, accommodating the whole patient body. Virtual Pinhole PET incorporates higher resolution detectors being placed close to the Region-of-Interest (ROI) within the imaging Field-of-View (FOV) of the whole-body scanner, providing better image resolution and contrast recover. To further adapt this technology to a wider range of diseases, we proposed a second generation of virtual pinhole PET using actively controlled high resolution detectors integrated on a robotic arm. When the whole system is integrated to a commercial PET scanner, we achieved positioning repeatability within 0.5 mm. Monte Carlo simulation shows that by focusing the high-resolution detectors to a specific organ of interest, we can achieve better resolution, sensitivity and contrast recovery. In another direction, we proposed a portable, versatile and low cost PET imaging system for Point-of-Care (POC) applications. It consists of one or more movable detectors in coincidence with a detector array behind a patient. The movable detectors make it possible for the operator to control the scanning trajectory freely to achieve optimal coverage and sensitivity for patient specific imaging tasks. Since this system does not require a conventional full ring geometry, it can be built portable and low cost for bed-side or intraoperative use. We developed a proof-of-principle prototype that consists of a compact high resolution silicon photomultiplier detector mounted on a hand-held probe and a half ring of conventional detectors. The probe is attached to a MicroScribe device, which tracks the location and orientation of the probe as it moves. We also performed Monte Carlo simulations for two POC PET geometries with Time-of-Flight (TOF) capability. To support the development of such PET systems with unconventional geometries, a fully 3D image reconstruction framework has been developed for PET systems with arbitrary geometry. For POC PET and the second generation robotic Virtual Pinhole PET, new challenges emerge and our targeted applications require more efficiently image reconstruction that provides imaging results in near real time. Inspired by the previous work, we developed a list mode GPU-based image reconstruction framework with the capability to model dynamically changing geometry. Ordered-Subset MAP-EM algorithm is implemented on multi-GPU platform to achieve fast reconstruction in the order of seconds per iteration, under practical data rate. We tested this using both experimental and simulation data, for whole body PET scanner and unconventional PET scanners. Future application of adaptive imaging requires near real time performance for large statistics, which requires additional acceleration of this framework

    Analysis of 3D Cone-Beam CT Image Reconstruction Performance on a FPGA

    Efficient and accurate tomographic image reconstruction has been an intensive topic of research due to the increasing everyday usage in areas such as radiology, biology, and materials science. Computed tomography (CT) scans are used to analyze internal structures through capture of x-ray images. Cone-beam CT scans project a cone-shaped x-ray to capture 2D image data from a single focal point, rotating around the object. CT scans are prone to multiple artifacts, including motion blur, streaks, and pixel irregularities, therefore must be run through image reconstruction software to reduce visual artifacts. The most common algorithm used is the Feldkamp, Davis, and Kress (FDK) backprojection algorithm. The algorithm is computationally intensive due to the O(n4) backprojection step, running slowly with large CT data files on CPUs, but exceptionally well on GPUs due to the parallel nature of the algorithm.
