    Transportation, Terrorism and Crime: Deterrence, Disruption and Resilience

    Abstract: Terrorists likely have adopted vehicle ramming as a tactic because it can be carried out by an individual (or “lone wolf terrorist”), and because the skills required are minimal (e.g. the ability to drive a car and determine locations for creating maximum carnage). Studies of terrorist activities against transportation assets have been conducted to help law enforcement agencies prepare their communities, create mitigation measures, conduct effective surveillance and respond quickly to attacks. This study reviews current research on terrorist tactics against transportation assets, with an emphasis on vehicle ramming attacks. It evaluates some of the current attack strategies, and the possible mitigation or response tactics that may be effective in deterring attacks or saving lives in the event of an attack. It includes case studies that can be used as educational tools for understanding terrorist methodologies, as well as ordinary emergencies that might become a terrorist’s blueprint

    Emergency preparedness for tunnel fires – A systems-oriented approach

    Efficient emergency response is key to preventing major losses in tunnel fires. Our general concern in this paper is the degree to which tunnel systems are prepared and the means by which we can be prepared for a major fire in a single-tube road tunnel. Conformance to prescriptive regulations dominates existing practice in the area of emergency preparedness. Risk-based approaches exist but have little influence on emergency preparedness designs for tunnel systems. A successful emergency response to tunnel fires is dependent on many actors collaborating under serious time constraints. Safety becomes a matter of controlling critical processes necessary to keep the system in a safe state. Efficient decision-making in situations of major uncertainty is vital, to achieve safety goals. This essentially means that efficient emergency preparedness for road tunnels is a matter that needs attention in the early design phases and continuous improvements during the operational phase. To achieve high-performance emergency preparedness against tunnel fires, there is a need for radical changes to the design and operation of tunnels. In this paper, it is claimed that a system-theoretic approach is appropriate to deal with the tunnel system’s complexity and to drive the design of appropriate control structures for critical processes, from the design phase to the actual emergency. It is shown how system theoretic approaches will change the safety management practices for tunnels and how this will increase consistency between potential fire scenarios and associated control actions.publishedVersio

    Ambulatory surgery in orthopedics: experience of over 10,000 patients

    PurposeThe concept of day surgery is becoming an increasingly important part of elective surgery worldwide. Relentless pressure to cut costs may constrain clinical judgment regarding the most appropriate location for a patient’s surgical care. The aim of this study was to determine clinical and quality indicators relating to our experience in orthopedic day durgery, mainly in relation to unplanned overnight admission and readmission rates. Additionally, we focused on describing the main characteristics of the patients that experienced complications, and compared the patient satisfaction rates following ambulatory and non-ambulatory procedures.MethodsWe evaluated 10,032 patients who underwent surgical orthopedic procedures according to the protocols of our Ambulatory Surgery Unit. All complications that occurred were noted. A quality-of-life assessment (SF-36 test) was carried out both pre- and postoperatively. Ambulatory substitution rates and quality indicators for orthopedic procedures were also determined.ResultsThe major complication rate was minimal, with no mortal cases, and there was a high rate of ambulatory substitution for the procedures studied. Outcomes of the SF-36 questionnaire showed significant improvement postoperatively. An unplanned overnight admission rate of 0.14 % was achieved.ConclusionsOur institution has shown that it is possible to provide good-quality ambulatory orthopedic surgery. There still appears to be the potential to increase the proportion of these procedures. Surgeons and anesthesiologists must strongly adhere to strict patient selection criteria for ambulatory orthopedic surgery in order to reduce complications in the immediate postoperative term
