    Assessment of a novel patient-specific 3D printed multi-material simulator for endoscopic sinus surgery

    Background: Three-dimensional (3D) printing is an emerging tool in the creation of anatomical models for surgical training. Its use in endoscopic sinus surgery (ESS) has been limited because of the difficulty in replicating the anatomical details. Aim: To describe the development of a patient-specific 3D printed multi-material simulator for use in ESS, and to validate it as a training tool among a group of residents and experts in ear-nose-throat (ENT) surgery. Methods: Advanced material jetting 3D printing technology was used to produce both soft tissues and bony structures of the simulator to increase anatomical realism and tactile feedback of the model. A total of 3 ENT residents and 9 ENT specialists were recruited to perform both non-destructive tasks and ESS steps on the model. The anatomical fidelity and the usefulness of the simulator in ESS training were evaluated through specific questionnaires. Results: The tasks were accomplished by 100% of participants and the survey showed overall high scores both for anatomy fidelity and usefulness in training. Dacryocystorhinostomy, medial antrostomy, and turbinectomy were rated as accurately replicable on the simulator by 75% of participants. Positive scores were obtained also for ethmoidectomy and DRAF procedures, while the replication of sphenoidotomy received neutral ratings by half of the participants. Conclusion: This study demonstrates that a 3D printed multi-material model of the sino-nasal anatomy can be generated with a high level of anatomical accuracy and haptic response. This technology has the potential to be useful in surgical training as an alternative or complementary tool to cadaveric dissection

    A Review of Virtual Reality Based Training Simulators for Orthopaedic Surgery

    This review presents current virtual reality based training simulators for hip, knee and other orthopaedic surgery, including elective and trauma surgical procedures. There have not been any reviews focussing on hip and knee orthopaedic simulators. A comparison of existing simulator features is provided to identify what is missing and what is required to improve upon current simulators. In total 11 total hip replacement pre-operative planning tools were analysed, plus 9 hip trauma fracture training simulators. Additionally 9 knee arthroscopy simulators and 8 other orthopaedic simulators were included for comparison. The findings are that for orthopaedic surgery simulators in general, there is increasing use of patient-specific virtual models which reduce the learning curve. Modelling is also being used for patient-specific implant design and manufacture. Simulators are being increasingly validated for assessment as well as training. There are very few training simulators available for hip replacement, yet more advanced virtual reality is being used for other procedures such as hip trauma and drilling. Training simulators for hip replacement and orthopaedic surgery in general lag behind other surgical procedures for which virtual reality has become more common. Further developments are required to bring hip replacement training simulation up to date with other procedures. This suggests there is a gap in the market for a new high fidelity hip replacement and resurfacing training simulator

    Simulation training approaches in intracranial aneurysm surgery-a systematic review.

    BACKGROUND With the increasing complexity and decreasing exposure to intracranial aneurysm surgery, training and maintenance of the surgical skills have become challenging. This review elaborated on simulation training for intracranial aneurysm clipping. METHODS A systematic review was performed according to the PRISMA guidelines to identify studies on aneurysm clipping training using models and simulators. The primary outcome was the identification of the predominant modes of the simulation process, models, and training methods associated with a microsurgical learning curve. The secondary outcomes included assessments of the validation of such simulators and the learning capability from the use of such simulators. RESULTS Of the 2068 articles screened, 26 studies met the inclusion criteria. The chosen reports used a wide range of simulation approaches including ex vivo methods (n = 6); virtual reality (VR) platforms (n = 11); and static (n = 6) and dynamic (n = 3) 3D-printed aneurysm models (n = 6). The ex vivo training methods have limited availability, VR simulators lack haptics and tactility, while 3D static models lack important microanatomical components and the simulation of blood flow. 3D dynamic models including pulsatile flow are reusable and cost-effective but miss microanatomical components. CONCLUSIONS The existing training methods are heterogenous and do not realistically simulate the complete microsurgical workflow. The current simulations lack certain anatomical features and crucial surgical steps. Future research should focus on developing and validating a reusable, cost-effective training platform. No systematic validation method exists for the different training models, so there is a need to build homogenous assessment tools and validate the role of simulation in education and patient safety

    Entering a new era of surgical training : developing 3-dimensional print models for hands-on surgical training and its introduction into the congenital cardiac surgical curriculum

    Congenital heart surgery is a technically challenging subspecialty of cardiothoracic surgery. This is due to a combination of factors including the rarity and variety of pathology and the small patient size. This coupled with the increasing public scrutiny and the expectation of excellent patient outcomes for even the most complex pathologies has led to limitations for surgical trainees to develop their surgical competencies in an efficient manner. Simulation has been used successfully to develop technical skills in other surgical specialities but is limited in congenital heart surgery. The objectives of this work were to develop and integrate hands-on simulation methods into the training of congenital heart surgeons using anatomically accurate 3D-printed heart models and to use validated, objective assessment methods to measure performance. The simulation programme was successfully developed and integrated into the regular training of congenital heart surgeons. The objective assessments demonstrated that there was an improvement in procedural performance and time across multiple complex procedures following deliberate practice and rehearsal. Furthermore, surgeons who had participated in the programme retained their technical skills following a prolonged delay supporting the value of simulation. Overall, there is value in the incorporation of hands-on simulation training into congenital heart surgery and it has the potential to be integrated into training programmes globally

    Manufacture of Anatomical Synthetic Demonstrating Models

    The practical demonstration session is usually conducted using wet dissected cadaver specimens, plastinated specimens and sometimes plastic anatomical models. Computer-aided teaching means are added nowadays. Due to high formalin toxicity, high cost of plastinated specimens, high cost of computer-aided teaching programs, the synthetic specimens or plastic models are still good substitutes for practical teaching and learning, especially in developing countries like Egypt. Since imported synthetic models are relatively expensive, especially the magnified or those of high quality, the present trial aims at finding a simplified designing idea for manufacture of anatomical models. This simplified methodology helps in changing the two dimensional (2D) pictures to 3D synthetic anatomical local models. These models can be of great help in illustrations and teaching purposes especially for minute parts like natural ear ossicles, which can never be displayed in practical lessons except as pictures. In the present study, different human middle ear ossicles were obtained by repeated dissections of multiple wet cadavers to make an average dimensions for each ossicle. Two dimensional photographs for the ossicles on 1 mm-squared background sheet were taken to confirm measurements and the desired magnification was determined according to the reasonable demonstration distance. The third dimension is deduced by repeated reading of different sources and by reviewing their pictures in different aspects. Three-dimensional primitive magnified models made of sculpting clay were made. Permanent models were manufactured by using synthetic liquid polyester. All models contain wire core skeleton. These manufactured demonstrating anatomical models were found to be durable and easily portable in addition to their relatively simplified manufacturing methodology. They can also offer the basis for manufacturing of implants and prosthesis needed for rehabilitation purposes

    Performance Factors in Neurosurgical Simulation and Augmented Reality Image Guidance

    Virtual reality surgical simulators have seen widespread adoption in an effort to provide safe, cost-effective and realistic practice of surgical skills. However, the majority of these simulators focus on training low-level technical skills, providing only prototypical surgical cases. For many complex procedures, this approach is deficient in representing anatomical variations that present clinically, failing to challenge users’ higher-level cognitive skills important for navigation and targeting. Surgical simulators offer the means to not only simulate any case conceivable, but to test novel approaches and examine factors that influence performance. Unfortunately, there is a void in the literature surrounding these questions. This thesis was motivated by the need to expand the role of surgical simulators to provide users with clinically relevant scenarios and evaluate human performance in relation to image guidance technologies, patient-specific anatomy, and cognitive abilities. To this end, various tools and methodologies were developed to examine cognitive abilities and knowledge, simulate procedures, and guide complex interventions all within a neurosurgical context. The first chapter provides an introduction to the material. The second chapter describes the development and evaluation of a virtual anatomical training and examination tool. The results suggest that learning occurs and that spatial reasoning ability is an important performance predictor, but subordinate to anatomical knowledge. The third chapter outlines development of automation tools to enable efficient simulation studies and data management. In the fourth chapter, subjects perform abstract targeting tasks on ellipsoid targets with and without augmented reality guidance. While the guidance tool improved accuracy, performance with the tool was strongly tied to target depth estimation – an important consideration for implementation and training with similar guidance tools. In the fifth chapter, neurosurgically experienced subjects were recruited to perform simulated ventriculostomies. Results showed anatomical variations influence performance and could impact outcome. Augmented reality guidance showed no marked improvement in performance, but exhibited a mild learning curve, indicating that additional training may be warranted. The final chapter summarizes the work presented. Our results and novel evaluative methodologies lay the groundwork for further investigation into simulators as versatile research tools to explore performance factors in simulated surgical procedures

    Intraoperative Navigation Systems for Image-Guided Surgery

    Recent technological advancements in medical imaging equipment have resulted in a dramatic improvement of image accuracy, now capable of providing useful information previously not available to clinicians. In the surgical context, intraoperative imaging provides a crucial value for the success of the operation. Many nontrivial scientific and technical problems need to be addressed in order to efficiently exploit the different information sources nowadays available in advanced operating rooms. In particular, it is necessary to provide: (i) accurate tracking of surgical instruments, (ii) real-time matching of images from different modalities, and (iii) reliable guidance toward the surgical target. Satisfying all of these requisites is needed to realize effective intraoperative navigation systems for image-guided surgery. Various solutions have been proposed and successfully tested in the field of image navigation systems in the last ten years; nevertheless several problems still arise in most of the applications regarding precision, usability and capabilities of the existing systems. Identifying and solving these issues represents an urgent scientific challenge. This thesis investigates the current state of the art in the field of intraoperative navigation systems, focusing in particular on the challenges related to efficient and effective usage of ultrasound imaging during surgery. The main contribution of this thesis to the state of the art are related to: Techniques for automatic motion compensation and therapy monitoring applied to a novel ultrasound-guided surgical robotic platform in the context of abdominal tumor thermoablation. Novel image-fusion based navigation systems for ultrasound-guided neurosurgery in the context of brain tumor resection, highlighting their applicability as off-line surgical training instruments. The proposed systems, which were designed and developed in the framework of two international research projects, have been tested in real or simulated surgical scenarios, showing promising results toward their application in clinical practice
