    The safe insertion of peripheral intravenous catheters : a mixed methods descriptive study of the availability of the equipment needed

    Background: Intravenous cannulation is undertaken in a high proportion of hospitalised patients. Much international attention has been given to the use of care bundles to reduce the incidence of infection in these patients. However, less attention has been given to the systems required to ensure availability of the equipment needed to support these care bundles. Our objectives were to assess how reliably the equipment recommended for a peripheral intravenous care bundle was available for use, and to explore factors which contributed to its non-availability. Methods: We studied 350 peripheral cannula insertions in three NHS hospital organisations across the UK. Staff inserting cannulae were asked to report details of all equipment problems. Key staff were then interviewed to identify the causes of problems with equipment availability, using semi-structured qualitative interviews and a standard coding frame. Results: 47 equipment problems were recorded during 46 of 350 cannulations, corresponding to a reliability of 87%, or 94% if problems with sharps disposal were excluded. Overall reliability was similar in all three organisations, but the types of problem varied. Interviews revealed a variety of causes including issues associated with purchasing policies, storage facilities, and lack of teamwork and communication in relation to reordering. The many human factors related to the supply chain were highlighted. Often staff had adopted work-arounds to deal with these problems. Conclusions: Overall, 87% of cannulations had the correct and functional equipment available. Different problems were identified in different organisations, suggesting that each had resolved some issues. Supply chain management principles may be useful to support best practice in care bundle delivery.     STOL Simulation Requirements for Development of Integrated Flight/propulsion Control Systems

    The role and use of simulation as a design tool in developing integrated systems where design criteria is largely unavailable is well known. This paper addresses additional simulation needs for the development of Integrated Flight/Propulsion Control Systems (IFPCS) which will improve the probability of properly interpreting simulation results. These needs are based on recent experience with power approach flying qualities evaluations of an advanced fighter configuration which incorporated Short Takeoff and Landing (STOL) technologies and earlier experiences with power approach flying qualities evaluations on the AFTI/F-16 program. The use of motion base platforms with axial and normal degrees of freedom will help in evaluating pilot coupling and workload in the presence of high frequency low amplitude axial accelerations produced by high bandwidth airspeed controllers in a gusty environment

    Many developing countries have recognized the need for comprehensive national reforms and comprehensive prevention, treatment, and care and support initiatives to reduce future transmission of and to meet the growing demand for HIV/AIDS services. As a part of these national health reform initiatives, governments are exploring ways to allocate resources in the most efficient and effective way to mitigate the HIV/AIDS epidemic. However, many countries lack information on the level and nature of the costs of HIV/AIDS programs. This document provides an introduction to the procedure for calculating and analyzing the costs of HIV/AIDS programs and describes how to measure directly the actual costs of a program that is up and running. The step-by-step guide is intended to provide project managers in the field with a framework for how to do measure costs for a single, recent year in the life of an HIV/AIDS program. An illustrative activities list in the report annex will assist the user to develop an activities-based framework. The information gleaned from the costing framework will enable policymakers and program managers to make informed resource allocation decisions

    Eavesdropping on GSM: state-of-affairs

    In the almost 20 years since GSM was deployed several security problems have been found, both in the protocols and in the - originally secret - cryptography. However, practical exploits of these weaknesses are complicated because of all the signal processing involved and have not been seen much outside of their use by law enforcement agencies. This could change due to recently developed open-source equipment and software that can capture and digitize signals from the GSM frequencies. This might make practical attacks against GSM much simpler to perform. Indeed, several claims have recently appeared in the media on successfully eavesdropping on GSM. When looking at these claims in depth the conclusion is often that more is claimed than what they are actually capable of. However, it is undeniable that these claims herald the possibilities to eavesdrop on GSM using publicly available equipment. This paper evaluates the claims and practical possibilities when it comes to eavesdropping on GSM, using relatively cheap hardware and open source initiatives which have generated many headlines over the past year.     Computers for real time flight simulation: A market survey

    An extensive computer market survey was made to determine those available systems suitable for current and future flight simulation studies at Ames Research Center. The primary requirement is for the computation of relatively high frequency content (5 Hz) math models representing powered lift flight vehicles. The Rotor Systems Research Aircraft (RSRA) was used as a benchmark vehicle for computation comparison studies. The general nature of helicopter simulations and a description of the benchmark model are presented, and some of the sources of simulation difficulties are examined. A description of various applicable computer architectures is presented, along with detailed discussions of leading candidate systems and comparisons between them

    Energy efficiency in discrete-manufacturing systems: insights, trends, and control strategies

    Since the depletion of fossil energy sources, rising energy prices, and governmental regulation restrictions, the current manufacturing industry is shifting towards more efficient and sustainable systems. This transformation has promoted the identification of energy saving opportunities and the development of new technologies and strategies oriented to improve the energy efficiency of such systems. This paper outlines and discusses most of the research reported during the last decade regarding energy efficiency in manufacturing systems, the current technologies and strategies to improve that efficiency, identifying and remarking those related to the design of management/control strategies. Based on this fact, this paper aims to provide a review of strategies for reducing energy consumption and optimizing the use of resources within a plant into the context of discrete manufacturing.
