    Complementary medicine products used in pregnancy and lactation and an examination of the information sources accessed pertaining to maternal health literacy: A systematic review of qualitative studies

    © 2018 The Author(s). Background: The prevalence of complementary medicine use in pregnancy and lactation has been increasingly noted internationally. This systematic review aimed to determine the complementary medicine products (CMPs) used in pregnancy and/or lactation for the benefit of the mother, the pregnancy, child and/or the breastfeeding process. Additionally, it aimed to explore the resources women used, and to examine the role of maternal health literacy in this process. Methods: Seven databases were comprehensively searched to identify studies published in peer-reviewed journals (1995-2017). Relevant data were extracted and thematic analysis undertaken to identify key themes related to the review objectives. Results: A total of 4574 articles were identified; 28 qualitative studies met the inclusion criteria. Quantitative studies were removed for a separate, concurrent review. Herbal medicines were the main CMPs identified (n=21 papers) in the qualitative studies, with a smaller number examining vitamin and mineral supplements together with herbal medicines (n=3), and micronutrient supplements (n=3). Shared cultural knowledge and traditions, followed by women elders and health care professionals were the information sources most accessed by women when choosing to use CMPs. Women used CMPs for perceived physical, mental-emotional, spiritual and cultural benefits for their pregnancies, their own health, the health of their unborn or breastfeeding babies, and/or the breastfeeding process. Two over-arching motives were identified: 1) to protect themselves or their babies from adverse events; 2) to facilitate the normal physiological processes of pregnancy, birth and lactation. Decisions to use CMPs were made within the context of their own cultures, reflected in the locus of control regarding decision-making in pregnancy and lactation, and in the health literacy environment. Medical pluralism was very common and women navigated through and between different health care services and systems throughout their pregnancies and breastfeeding journeys. Conclusions: Pregnant and breastfeeding women use herbal medicines and micronutrient supplements for a variety of perceived benefits to their babies' and their own holistic health. Women access a range of CMP-related information sources with shared cultural knowledge and women elders the most frequently accessed sources, followed by HCPs. Culture influences maternal health literacy and thus women's health care choices including CMP use

    Positive and Negative Evidence Accumulation Clustering for Sensor Fusion: An Application to Heartbeat Clustering

    In this work, a new clustering algorithm especially geared towards merging data arising from multiple sensors is presented. The algorithm, called PN-EAC, is based on the ensemble clustering paradigm and it introduces the novel concept of negative evidence. PN-EAC combines both positive evidence, to gather information about the elements that should be grouped together in the final partition, and negative evidence, which has information about the elements that should not be grouped together. The algorithm has been validated in the electrocardiographic domain for heartbeat clustering, extracting positive evidence from the heartbeat morphology and negative evidence from the distances between heartbeats. The best result obtained on the MIT-BIH Arrhythmia database yielded an error of 1.44%. In the St. Petersburg Institute of Cardiological Technics 12-Lead Arrhythmia Database database (INCARTDB), an error of 0.601% was obtained when using two electrocardiogram (ECG) leads. When increasing the number of leads to 4, 6, 8, 10 and 12, the algorithm obtains better results (statistically significant) than with the previous number of leads, reaching an error of 0.338%. To the best of our knowledge, this is the first clustering algorithm that is able to process simultaneously any number of ECG leads.     Performance evaluation of cooperation strategies for m-health services and applications

    Health telematics are becoming a major improvement for patients’ lives, especially for disabled, elderly, and chronically ill people. Information and communication technologies have rapidly grown along with the mobile Internet concept of anywhere and anytime connection. In this context, Mobile Health (m-Health) proposes healthcare services delivering, overcoming geographical, temporal and even organizational barriers. Pervasive and m-Health services aim to respond several emerging problems in health services, including the increasing number of chronic diseases related to lifestyle, high costs in existing national health services, the need to empower patients and families to self-care and manage their own healthcare, and the need to provide direct access to health services, regardless the time and place. Mobile Health (m- Health) systems include the use of mobile devices and applications that interact with patients and caretakers. However, mobile devices have several constraints (such as, processor, energy, and storage resource limitations), affecting the quality of service and user experience. Architectures based on mobile devices and wireless communications presents several challenged issues and constraints, such as, battery and storage capacity, broadcast constraints, interferences, disconnections, noises, limited bandwidths, and network delays. In this sense, cooperation-based approaches are presented as a solution to solve such limitations, focusing on increasing network connectivity, communication rates, and reliability. Cooperation is an important research topic that has been growing in recent years. With the advent of wireless networks, several recent studies present cooperation mechanisms and algorithms as a solution to improve wireless networks performance. In the absence of a stable network infrastructure, mobile nodes cooperate with each other performing all networking functionalities. For example, it can support intermediate nodes forwarding packets between two distant nodes. This Thesis proposes a novel cooperation strategy for m-Health services and applications. This reputation-based scheme uses a Web-service to handle all the nodes reputation and networking permissions. Its main goal is to provide Internet services to mobile devices without network connectivity through cooperation with neighbor devices. Therefore resolving the above mentioned network problems and resulting in a major improvement for m-Health network architectures performances. A performance evaluation of this proposal through a real network scenario demonstrating and validating this cooperative scheme using a real m-Health application is presented. A cryptography solution for m-Health applications under cooperative environments, called DE4MHA, is also proposed and evaluated using the same real network scenario and the same m-Health application. Finally, this work proposes, a generalized cooperative application framework, called MobiCoop, that extends the incentive-based cooperative scheme for m-Health applications for all mobile applications. Its performance evaluation is also presented through a real network scenario demonstrating and validating MobiCoop using different mobile applications

    Developing artificial intelligence and machine learning to support primary care research and practice

    This thesis was motivated by the potential to use everyday data , especially that collected in electronic health records (EHRs) as part of healthcare delivery, to improve primary care for clients facing complex clinical and/or social situations. Artificial intelligence (AI) techniques can identify patterns or make predictions with these data, producing information to learn about and inform care delivery. Our first objective was to understand and critique the body of literature on AI and primary care. This was achieved through a scoping review wherein we found the field was at an early stage of maturity, primarily focused on clinical decision support for chronic conditions in high-income countries, with low levels of primary care involvement and model evaluation in real-world settings. Our second objective was to demonstrate how AI methods can be applied to problems in descriptive epidemiology. To achieve this, we collaborated with the Alliance for Healthier Communities, which provides team-based primary health care through Community Health Centres (CHCs) across Ontario to clients who experience barriers to regular care. We described sociodemographic, clinical, and healthcare use characteristics of their adult primary care population using EHR data from 2009-2019. We used both simple statistical and unsupervised learning techniques, applied with an epidemiological lens. In addition to substantive findings, we identified potential avenues for future learning initiatives, including the development of decision support tools, and methodological considerations therein. Our third objective was to advance interpretable AI methodology that is well-suited for heterogeneous data, and is applicable in clinical epidemiology as well as other settings. To achieve this, we developed a new hybrid feature- and similarity-based model for supervised learning. There are two versions, fit by convex optimization with a sparsity-inducing penalty on the kernel (similarity) portion of the model. We compared our hybrid models with solely feature- and similarity-based approaches using synthetic data and using CHC data to predict future loneliness or social isolation. We also proposed a new strategy for kernel construction with indicator-coded data. Altogether, this thesis progressed AI for primary care in general and for a particular health care organization, while making research contributions to epidemiology and to computer science

    Wearable Movement Sensors for Rehabilitation: From Technology to Clinical Practice

    This Special Issue shows a range of potential opportunities for the application of wearable movement sensors in motor rehabilitation. However, the papers surely do not cover the whole field of physical behavior monitoring in motor rehabilitation. Most studies in this Special Issue focused on the technical validation of wearable sensors and the development of algorithms. Clinical validation studies, studies applying wearable sensors for the monitoring of physical behavior in daily life conditions, and papers about the implementation of wearable sensors in motor rehabilitation are under-represented in this Special Issue. Studies investigating the usability and feasibility of wearable movement sensors in clinical populations were lacking. We encourage researchers to investigate the usability, acceptance, feasibility, reliability, and clinical validity of wearable sensors in clinical populations to facilitate the application of wearable movement sensors in motor rehabilitation
