    Thoracic myopericytoma in an older adult, rare but possible: A case report

    Myopericytoma is a rare tumor generally arising from skin and soft tissues of extremities, trunk, head, and neck regions, rarely from visceral sites. An intrathoracic visceral localization may carry a broad differential diagnosis including primary lung, pleura and chest wall lesions, or metastatic lesions. To date, any radiological features have been recognized and diagnosis of myopericytoma with intrathoracic localization remains still challenging. Here, we describe the case of a subpleural lesion incidentally diagnosed in an older adult affected by gastric cancer. Radiological features did not allow a differential diagnosis between a benign lesion, a primary tumor, or a metastasis. After resection, the histological examination showed histopathological features congruent with the diagnosis of myopericytoma. This unusual presentation reflects the need to share clinical, radiological, and histopathological data about this uncommon but frequently misdiagnosed disease

    On the modulation instability development in optical fiber systems

    Extensive numerical simulations were performed to investigate all stages of modulation instability development from the initial pulse of pico-second duration in photonic crystal fiber: quasi-solitons and dispersive waves formation, their interaction stage and the further propagation. Comparison between 4 different NLS-like systems was made: the classical NLS equation, NLS system plus higher dispersion terms, NLS plus higher dispersion and self-steepening and also fully generalized NLS equation with Raman scattering taken into account. For the latter case a mechanism of energy transfer from smaller quasi-solitons to the bigger ones is proposed to explain the dramatical increase of rogue waves appearance frequency in comparison to the systems when the Raman scattering is not taken into account.Comment: 9 pages, 54 figure

    Extreme events in two dimensional disordered nonlinear lattices

    Spatiotemporal complexity is induced in a two dimensional nonlinear disordered lattice through the modulational instability of an initially weakly perturbed excitation. In the course of evolution we observe the formation of transient as well as persistent localized structures, some of which have extreme magnitude. We analyze the statistics of occurrence of these extreme collective events and find that the appearance of transient extreme events is more likely in the weakly nonlinear regime. We observe a transition in the extreme events recurrence time probability from exponential, in the nonlinearity dominated regime, to power law for the disordered one.Comment: 5 figures, 5 page

    BoBafit: A copy number clustering tool designed to refit and recalibrate the baseline region of tumors’ profiles

    Human cancer arises from a population of cells that have acquired a wide range of genetic alterations, most of which are targets of therapeutic treatments or are used as prognostic factors for patient's risk stratification. Among these, copy number alterations (CNAs) are quite frequent. Currently, several molecular biology technologies, such as microarrays, NGS and single-cell approaches are used to define the genomic profile of tumor samples. Output data need to be analyzed with bioinformatic approaches and particularly by employing computational algorithms. Molecular biology tools estimate the baseline region by comparing either the mean probe signals, or the number of reads to the reference genome. However, when tumors display complex karyotypes, this type of approach could fail the baseline region estimation and consequently cause errors in the CNAs call. To overcome this issue, we designed an R-package, BoBafit, able to check and, eventually, to adjust the baseline region, according to both the tumor-specific alterations’ context and the sample-specific clustered genomic lesions. Several databases have been chosen to set up and validate the designed package, thus demonstrating the potential of BoBafit to adjust copy number (CN) data from different tumors and analysis techniques. Relevantly, the analysis highlighted that up to 25% of samples need a baseline region adjustment and a redefinition of CNAs calls, thus causing a change in the prognostic risk classification of the patients. We support the implementation of BoBafit within CN analysis bioinformatics pipelines to ensure a correct patient's stratification in risk categories, regardless of the tumor type

    Negative group delay for Dirac particles traveling through a potential well

    The properties of group delay for Dirac particles traveling through a potential well are investigated. A necessary condition is put forward for the group delay to be negative. It is shown that this negative group delay is closely related to its anomalous dependence on the width of the potential well. In order to demonstrate the validity of stationary-phase approach, numerical simulations are made for Gaussian-shaped temporal wave packets. A restriction to the potential-well's width is obtained that is necessary for the wave packet to remain distortionless in the travelling. Numerical comparison shows that the relativistic group delay is larger than its corresponding non-relativistic one.Comment: 10 pages, 5 figure

    EACTS expert consensus statement for surgical management of pleural empyema

    Pleural infection is a frequent clinical condition. Prompt treatment has been shown to reduce hospital costs, morbidity and mortality. Recent advances in treatment have been variably implemented in clinical practice. This statement reviews the latest developments and concepts to improve clinical management and stimulate further research. The European Association for Cardio-Thoracic Surgery (EACTS) Thoracic Domain and the EACTS Pleural Diseases Working Group established a team of thoracic surgeons to produce a comprehensive review of available scientific evidence with the aim to cover all aspects of surgical practice related to its treatment, in particular focusing on: surgical treatment of empyema in adults; surgical treatment of empyema in children; and surgical treatment of post-pneumonectomy empyema (PPE). In the management of Stage 1 empyema, prompt pleural space chest tube drainage is required. In patients with Stage 2 or 3 empyema who are fit enough to undergo an operative procedure, there is a demonstrated benefit of surgical debridement or decortication [possibly by video-assisted thoracoscopic surgery (VATS)] over tube thoracostomy alone in terms of treatment success and reduction in hospital stay. In children, a primary operative approach is an effective management strategy, associated with a lower mortality rate and a reduction of tube thoracostomy duration, length of antibiotic therapy, reintervention rate and hospital stay. Intrapleural fibrinolytic therapy is a reasonable alternative to primary operative management. Uncomplicated PPE [without bronchopleural fistula (BPF)] can be effectively managed with minimally invasive techniques, including fenestration, pleural space irrigation and VATS debridement. PPE associated with BPF can be effectively managed with individualized open surgical techniques, including direct repair, myoplastic and thoracoplastic techniques. Intrathoracic vacuum-assisted closure may be considered as an adjunct to the standard treatment.     On the statistical interpretation of optical rogue waves

    Numerical simulations are used to discuss various aspects of "optical rogue wave" statistics observed in noise-driven fiber supercontinuum generation associated with highly incoherent spectra. In particular, we consider how long wavelength spectral filtering influences the characteristics of the statistical distribution of peak power, and we contrast the statistics of the spectrally filtered SC with the statistics of both the peak power of the most red-shifted soliton in the SC and the maximum peak power across the full temporal field with no spectral selection. For the latter case, we show that the unfiltered statistical distribution can still exhibit a long-tail, but the extreme-events in this case correspond to collisions between solitons of different frequencies. These results confirm the importance of collision dynamics in supercontinuum generation. We also show that the collision-induced events satisfy an extended hydrodynamic definition of "rogue wave" characteristics.Comment: Paper accepted for publication in the European Physical Journal ST, Special Topics.     The Alliance for Recovery Research in Music Therapy

    The mental health recovery movement recognises the importance of expertise by experience held by service users alongside healthcare practitioners. Recovery has gained attention in music therapy but a situation prevails where practitioners and researchers set research agendas. A group of music therapists recognised the absence of service user voices in music therapy research, and in 2017 they established a network called the Alliance for Recovery Research in Music Therapy (ARRIMT). In 2020, they started to develop a multi-national platform to explore mental health recovery in relation to research. Service users and music therapists were invited from three countries including Ireland, Norway, and the United Kingdom (UK). Local meetings were held to introduce stakeholders from each country, followed by three online meetings. Music was central to each meeting and each built upon content from previous meetings. Our conversations opened up new possibilities for working together. Four priorities for practice and research were identified: Music as a connector; music between sessions; music technology; and, online music therapy. This report will share our process and what we learnt from working together. We contextualise our work within concepts of foregrounding and mattering and view this work as a crucial step towards meaningful co-production. We reflect upon the role of music in building group identity alongside the importance of careful curation. Finally, we present ideas for future music therapy and mental health research. Group DescriptionIn 2018 the Alliance for Recovery Research in Music Therapy (ARRIMT) was founded as an international group of music therapy service users, researchers and practitioners from Australia, Ireland, Norway, and the United Kingdom. Fundamental to this group is the concept of recovery where those who use and those who provide mental health services work together to share knowledge and experiences that can have a positive impact on mental health service delivery. Key to this is listening carefully to the voices of those who use music therapy so that their views and experiences influence how music therapy is offered in mental health services. The founders and coordinators of the group are Tríona McCaffrey, Hans Petter Paulen Solli, and Catherine E. Carr. Other members of the group are Cornelia Bent, Darmuid Boyle, Oda Bjørke Dypvik, Kenneth Dybdahl, Tommy Hayes, Lauren M.
