    Questionnaires do not discriminate motor imagery ability of people with different motor expertise

    Questionnaires are presented as reliable measure of motor imagery (MI), i.e. the ability to mentally simulate a movement in an internal perspective. Although there is some evidence that MI is domain-specific (i.e., i.e., higher scores for motor imagery may be generated by people with extensive real-world experience and practice), MI studies have typically employed fixed and generic movements as items. Thus, we investigated the content validity of the movement items of the Vividness of Movement Imagery Questionnaire-2 (VMIQ-2). Sixty participants were divided in groups of athletes (competitive and not-competitive, with an extensive motor experience) and not-athletes (with a reduced motor experience) and analysed by means of a mixed factorial MANOVA. The three MI modalities, external visual, internal visual and kinesthetic imagery, did not result in significantly different scores between the groups. We recommend caution in using MI generic questionnaires in studies that compare people with different motor experiences. Moreover, we suggest that the structure of the questionnaires should be redesigned, in order to make them adaptable to the specific needs of professionals and researchers.Los cuestionarios han sido considerados como medidas fiables y válidas de imaginación motora (IM), entendida como la habilidad de un sujeto de simular mentalmente un movimiento desde su perspectiva interna. Aunque hay evidencia que la IM es específica de dominio (e.g. puntuaciones más altas de IM se generan en aquello sujetos con mayor práctica y experiencia en el mundo real). En este estudio, hemos investigado la validez de contenido para los items de movimientos de la escala VMIQ-2 ("Vividness of Movement Imagery Questionnaire-2"). Sesenta participantes fueron divididos en 2 grupos mediante MANOVA factorial mixto: un grupo de "atletas" (con mayor experiencia motora, participación competitiva y no competitiva) y un grupo de "no-atletas" (con una experiencia motora reducida). Como esperábamos, los grupos no difirieron en ninguna de las puntuaciones de las tres modalidades de la IM (visual externa, visual interna y cinestésico). Por ello, recomendamos ser cuidadosos en la utilización e interpretación de los cuestionarios de IM en estudios que comparan personas con distintas habilidades motoras.     ComEDA: A new tool for stress assessment based on electrodermal activity

    Non-specific sympathetic arousal responses to different stressful elicitations can be easily recognized from the analysis of physiological signals. However, neural patterns of sympathetic arousal during physical and mental fatigue are clearly not unitary. In the context of physiological monitoring through wearable and non-invasive devices, electrodermal activity (EDA) is the most effective and widely used marker of sympathetic activation. This study presents ComEDA, a novel approach for the characterization of complex dynamics of EDA. ComEDA overcomes the methodological limitations related to the application of nonlinear analysis to EDA dynamics, is not parameter-sensitive and is suitable for the analysis of ultra-short time series. We validated the proposed algorithm using synthetic series of white noise and 1/f noise, varying the number of samples from 50 to 5000. By applying our approach, we were able to discriminate a statistically significant increase of complexity in the 1/f noise with respect to white noise, obtaining p-values in the range [4.35 × 10−6, 0.03] after the Mann–Whitney test. Then, we tested ComEDA on both EDA signal and its tonic and phasic components, acquired from healthy subjects during four experimental protocols: two inducing a sympathetic activation through physical efforts and two based on mentally stressful tasks. Results are encouraging and promising, outperforming state of the art metrics such as the Sample Entropy. ComEDA shows good performance not only in discriminating between stressful tasks and resting state (p-value < 0.01 after the Wilcoxon non-parametric statistical test applied to EDA signals of all the four datasets), but also in differentiating different trends of complexity of EDA dynamics when induced by physical and mental stressors. These findings suggest future applications to automatically detect and selectively identify threats due to overwhelming stress impacting both physical and mental health or in the field of telemedicine to monitor autonomic diseases correlated to atypical sympathetic activation.     Is Hypnotic Induction Necessary to Experience Hypnosis and Responsible for Changes in Brain Activity?

    The relevance of formal hypnotic induction to the experience of trance and its neural correlates is not clear, in that hypnotizability, beliefs and expectation of hypnosis may play a major role. The aim of the study was assessing the EEG brain activity of participants with high (highs) or low hypnotizability scores (lows), aware of their hypnotizability level and informed that the session will include simple relaxation, formal hypnotic induction and neutral hypnosis. A total of 16 highs and 15 lows (according to the Stanford Hypnotic Susceptibility Scale, form A) were enrolled. Their EEGs were recorded during consecutive conditions of open/closed-eyes relaxation, hypnotic induction, neutral hypnosis and post hypnosis not interrupted by interviews. The studied variables were theta, alpha and gamma power spectral density (PSD), and the Determinism (DET) and Entropy (ENT) of the EEG signal Multidimensional Recurrence Plot (mRP). Highs reported significantly greater changes in their state of consciousness than lows across the session. The theta, alpha and gamma PSD did not exhibit condition-related changes in both groups. The Alpha PSD was larger in highs than in lows on midline sites, and the different sides/regions’ theta and gamma PSD were observed in the two groups independently from conditions. ENT showed no correlation with hypnotizability, while DET positively correlated with hypnotizability during hypnosis. In conclusion, the relevance of formal hypnotic induction to the experience of trance may be scarce in highs, as they are aware of their hypnotizability scores and expecting hypnosis.     Visuospatial imagery in healthy individuals with different hypnotizability levels

    Hypnotizability is a psychophysiological trait associated with morpho-functional brain differences. Since also cerebellar peculiarities have been reported in individuals with different hypnotizability levels and the cerebellar function is relevant to spatial imagery, the present study was aimed at investigating possible hypnotizability-related differences in the ability of spatial imagery. Highly (highs, N=31), low (lows, N=17) and medium (mediums, N=16) hypnotizable participants (classified by Stanford Hypnotic Susceptibility Scale, form A) of both genders were submitted to a test of mental rotation, which requires the integrity of both executive and cerebellar structures. In order to disentangle the role of the cerebellum from that of executive circuits as much as possible, visuospatial and verbal working memory tests, which mainly reflect executive processes, were also performed. Healthy highs exhibited higher scores of mental rotation ability compared to mediums in the absence of significant differences in visual-spatial and verbal working memory. Lows reported intermediate scores not significantly different from both highs’ and mediums’. Different cognitive strategies were observed in the three groups as the correlations between mental rotation, visuospatial and verbal working memory were different in highs, mediums and lows. In conclusion, present findings represent the first report of hypnotizability-related differences in a mental rotation task, which is relevant to several cognitive functions

    Acceptability and Adherence in a Chemoprevention Trial among Women at Increased Risk for Breast Cancer Attending the Modena Familial Breast and Ovarian Cancer Center (Italy)

    Chemoprevention for women at risk for breast cancer has been shown to be effective, but in actual practice, women's uptake of chemoprevention has been poor. We explored factors that influence acceptability, adherence, and dropout in the International Breast (Prevention) Intervention Study during our first 3 years of activity at the Modena Familial Breast and Ovarian Cancer Center. We evaluated socio-demographic characteristics, health status, adherence, and side effect intensity. Semi-structured interviews analyzed reasons for accepting/refusing/stopping the trial. A total of 471 postmenopausal women were invited to participate, of which 319 declined to participate (68%), 137 accepted to participate (29%), and 15 participants did not make a final decision (3%). Breast cancer-related worries and trust in our preventive and surveillance programs were the most frequent reasons for accepting. Side effect-related worry was the most frequent reason for refusing. General practitioners' and family members' opinions played an important role in the decision-making process. Adherence significantly decreased after a 12-month follow-up, but it remained unchanged after 24- and 36-month follow-ups. Mild/moderate side effects reported by women did not change after 12 months of treatment. Forty percent of women withdrew from the study due to complaints of side effects. We concluded that chemoprevention trials are difficult medical experiments and that the process of deciding about whether or not to participate is based mainly on beliefs and values. This study has important clinical implications. During counselling with prospective participants, it is important to emphasize the potential benefits and to promote an informed choice. How participants make decisions, their belief systems, and their perception of risk are all factors that should be investigated in future research

    Multi-Omics Integrative Approach of Extracellular Vesicles: A Future Challenging Milestone

    In the era of multi-omic sciences, dogma on singular cause-effect in physio-pathological processes is overcome and system biology approaches have been providing new perspectives to see through. In this context, extracellular vesicles (EVs) are offering a new level of complexity, given their role in cellular communication and their activity as mediators of specific signals to target cells or tissues. Indeed, their heterogeneity in terms of content, function, origin and potentiality contribute to the cross-interaction of almost every molecular process occurring in a complex system. Such features make EVs proper biological systems being, therefore, optimal targets of omic sciences. Currently, most studies focus on dissecting EVs content in order to either characterize it or to explore its role in various pathogenic processes at transcriptomic, proteomic, metabolomic, lipidomic and genomic levels. Despite valuable results being provided by individual omic studies, the categorization of EVs biological data might represent a limit to be overcome. For this reason, a multi-omic integrative approach might contribute to explore EVs function, their tissue-specific origin and their potentiality.     Treatment of HCV-related mixed cryoglobulinemia

    Mixed cryoglobulinemia syndrome (MCs) is a systemic vasculitis, involving skin, joints, peripheral nerves, and several internal organs. Hepatitis C virus (HCV) is recognized as the etiologic agent for the majority of MCs patients, as well as of number of autoimmune, lymphoproliferative, and neoplastic disorders. In this context, HCV-related MCs represents an important model autoimmune/neoplastic disease triggered by a virus in humans. With regard the therapeutic strategies of MCs, we can treat these patients at different steps by means of etiological (antivirals), pathogenetic, symptomatic drugs (mainly immunosuppressors, corticosteroids, plasmapheresis). In the majority of individuals, MCs shows a mild, slow-progressive clinical course needing only symptomatic treatments, generally low doses of corticosteroids. Considering the etiopathogenesis of MCs, the eradication of HCV should be considered the gold standard in the treatment of MCs. The use of combined peg-interferon-α/ribavirin and/or novel antiviral drugs may lead to HCV eradication in a significant percentage of cases with possible remission of MCs. On the other hand, the presence of rapidly progressive, diffuse vasculitis with multiple organ involvement may be successfully treated with aggressive immunosuppressive and anti-inflammatory therapies, mainly based on cyclophosphamide or rituximab, high dose corticosteroids, and plasma aphaeresis. Moreover, sequential/combined antiviral or immunosuppressive treatments could represent an useful therapeutic strategy particularly in MCs patients with major clinical manifestations. The treatment of MCs should be decided for every patient according to the severity of clinical picture. Thus, a careful follow-up of the disease is necessary, with particular attention to the possibility of cancers onset, such as B-cell lymphoma. The present review focuses on the different therapeutic strategies in patients with MCs, including the treatment of cryoglobulinemic skin ulcers, which represents one of the most discouraging complications of the disease

    Reduced miR-184-3p expression protects pancreatic β-cells from lipotoxic and proinflammatory apoptosis in type 2 diabetes via CRTC1 upregulation

    The loss of functional beta-cell mass in type 2 diabetes (T2D) is associated with molecular events that include beta-cell apoptosis, dysfunction and/or dedifferentiation. MicroRNA miR-184-3p has been shown to be involved in several beta-cell functions, including insulin secretion, proliferation and survival. However, the downstream targets and upstream regulators of miR-184-3p have not been fully elucidated. Here, we show reduced miR-184-3p levels in human T2D pancreatic islets, whereas its direct target CREB regulated transcription coactivator 1 (CRTC1) was increased and protects beta-cells from lipotoxicity- and inflammation-induced apoptosis. Downregulation of miR-184-3p in beta-cells leads to upregulation of CRTC1 at both the mRNA and protein levels. Remarkably, the protective effect of miR-184-3p is dependent on CRTC1, as its silencing in human beta-cells abrogates the protective mechanism mediated by inhibition of miR-184-3p. Furthermore, in accordance with miR-184-3p downregulation, we also found that the beta-cell-specific transcription factor NKX6.1, DNA-binding sites of which are predicted in the promoter sequence of human and mouse MIR184 gene, is reduced in human pancreatic T2D islets. Using chromatin immunoprecipitation analysis and mRNA silencing experiments, we demonstrated that NKX6.1 directly controls both human and murine miR-184 expression. In summary, we provide evidence that the decrease in NKX6.1 expression is accompanied by a significant reduction in miR-184-3p expression and that reduction of miR-184-3p protects beta-cells from apoptosis through a CRTC1-dependent mechanism
