    165 profiles of seventy-one beaches along the Gulf of Cadiz (SW, Spain) were studied to improve the formulation of the beach profile in tidal seas. Maritime climate, degree of energy exposure and size of the sand grains were taken into account to study the two sections of the biparabolic profile. The objective of the study was the determination of more accurate formulations of the design parameters for the equilibrium profile that involves tidal seas. These formulations were modelled and validated based on existing profiles to quantify the error existing between the real profile and the modelling. This comparative analysis was extended by considering the formulations proposed by other authors. The best results were obtained with the proposal presented herein

    Atraumatic maxillary sinus elevation using threaded bone dilators for immediate implants. A three-year clinical study

    Objective: The aim of this study was to evaluate the efficacy of sinus floor elevation using sequential bone dilators. Materials and Methods: Thirty patients took part in the study (18 women and 12 men) with ages ranging between thirty-six and sixty-three years, selected according to inclusion and exclusion criteria, who showed a bone deficit in the upper posterior alveolar margin of 5-8 mm in height. Sixty expanded platform internal connection implants were placed with diameters of 4/5/4 mm and lengths varying between 10 (n=10) and 11.5 mm (n= 50). Results: Data obtained were analyzed using SPSS 15.0 software. The average intra-sinus bone gain with MP3 biomaterial of porcine origin was 4.13 +/- 0.97 SD mm at the moment of implant placement, 3.90 +/- 1.15 SD mm after twelve months, 3.74 +/- 1.05 SD mm after 24 months and 3.62 +/- 1.75 SD mm after 36 months. Two implants were lost at the moment of prosthesis placement. Conclusions: Alveolar lifting technique in the upper maxilla using bone dilators achieved a 96.6 % implant success rate after a three-year follow-up. Intra-sinus bone biomaterial remodeling was 0.51 +/- 0.08 mm from day zero to the thirty-six-month follow-up. This is a procedure that reduces the amount of surgery necessary and is of both aesthetic and functional benefit to the patient

    A Design Parameter for Reef Beach Profiles—A Methodology Applied to Cadiz, Spain

    The southwestern coast of Spain is in a tidal zone (mesotidal) which causes the equilibrium profile to be developed in two different sections: the breakage section and the swash section. These two sections give rise to the typical bi-parabolic profile existing in tidal seas. The existence of areas with reefs/rocks which interrupt the normal development of the typical bi-parabolic profile causes different types of beach profiles. The objective of this article is designing an easy methodology for determining new formulations for the design parameters of the equilibrium profile of beaches with reefs in tidal seas. These formulations are applied on 16 profiles to quantify the error between the real profile data and the modelling results. A comparative analysis is extended to the formulations proposed by other authors, from which it is found that better results are obtained with the new formulations

    Immediate loading and immediate restoration in 105 expanded-platform implants via the Diem System after a 16-month follow-up period

    Objective: The aim of this study was to report on our experience and outcomes with Certain Prevail Implants and immediate loading via the Diem System after a 16-month follow-up period. Materials and Methods: Over a 16- month period, 105 (14 maxilla, 91 mandible) expanded-platform implants were placed in 18 patients (15 females, 3 males; 55.97 ±7.25 SD years). Resonance frecuency analysis (RFA) was measured on the day of placement and at 3, 12 and 16 months. All prostheses were screw mounted on IOL DIEM standard abutments. The follow-up time varied between 3 up to 16 months. Results: One implant (0.9%) failed during final prothetic placement (3 months). The RFA (ISQ) measurements at 16 months evalution for 4 mm- and 5 mm diameter implants were: 76.13 ± 5.0 SD and 76 ±7.7 SD respectively. The RFA data related to mandible and maxilla, the day 0 mean was 76.47 ± 4.75 SD and 70.13 ± 1.35 SD, at 3 months 70.53 ± 4.0 SD and 66.07 ±2.9 SD, at 12 months 68.23± 3.6 SD and 66.40 ±2.7 SD and 16 months 68.23± 3.6 SD and 66.60 ± 2.5 SD respectively. The crestal bone resorption was 0.6 mm at 16 months in platform switching implants.Conclusions: The cumulative implant survival rate during the follow-up period was 99.1% Immediate loading on IOL Diem abutments is a reliable and effective technique for edentulous patients in the maxilla and mandible

    About 1154 the Arab chorographer az-Zuhri followed the high course of the Segura, describing admiringly the gorge into which the river ran anci the mouth from which emerged a fountain which he called "Fuente Oscura" (Black Fountain). As he himself said, it was from this fountain that '%he Romans of Ello took water to irrigate their fertile plains". Archaeological surveys, which are being conducted from Almadenes (Cieza), are discovering the remains of Roman "villae" that run along the irrigation channels of botgh banks of the river. On the right hand side of the river near "The Gold Mountains" and dominating this landscape, are two "oppida" made with materials from the late roman period. The second of these, around which the water returns to the river is important because it marks the point where the town "La Cieza Vieja" began in the tenth century. It was said of this "oppidum" in 1579 ". .. it must have been important as can be deduced from the channel, which was taken in ancient times from the river Segura at the point that is called Almadenes and which terminated under the town"

    Biofunctional and Tribo-mechanical Behavior of Porous Titanium Substrates Coated with a Bioactive Glass Bilayer (45S5 ‒ 1393)

    Porous substrates of commercially pure titanium have been coated with a novel bilayer of bioactive glasses, 45S5 and 1393, to improve the osseointegration and solve the stress-shielding phenomenon of titanium partial implants. The porosity of the substrates, the scratch resistance and bioactivity of the coating have been evaluated. Results are discussed in terms of stiffness and yield strength of the substrates, as well as the chemical composition, thickness and design of the bioglass coating (monolithic vs. bilayer). The role of the pores was a crucial issue in the anchoring of the coating, both in porosity percentage (30 and 60 vol. %) and pore range size (100 – 200 and 355 ‒ 500 μm). The study was focused on the adhesion and infiltration of a 1393 bioglass layer (in contact with a porous titanium substrate), in combination with the biofunctionality of the 45S5 bioglass layer (surrounded by the host bone tissue), as 1393 bioglass enhances the adherence, while 45S5 bioglass promotes higher bioactivity. This bioactivity of the raw powder was initially estimated by nuclear magnetic resonance, through the evaluation of the chemical environments, and confirmed by the formation of hydroxyapatite, when immersed in simulated body fluid. Results revealed that the substrate with 30 vol. % of porosity and a range of 355 ‒ 500 μm pore size, coated with this novel bioactive glass bilayer, presented the best combination in terms of mechanical and biofunctional properties.Junta de Andalucía–FEDER (Spain) Project Ref.     Porous titanium substrates coated with a bilayer of bioactive glasses

    Porous titanium substrates coated by dripping-sedimentation technique with a novel bilayer of (45S5 / 1393) bioactive glasses are proposed to overcome some limitations of the use of titanium for implants, such as the stress shielding and the poor osseointegration. Composition, thickness, roughness and micromechanical behavior (P-h curves) of the coating and the influence of the porous titanium substrates have been characterized. Best results were found for the substrate with 30 vol.% of porosity and a range size of 355 ‒ 500 μm, since it enhanced the mechanical and biofunctional behavior, due to the good adhesion of the 1393 bioglass to the substrate and the greater bioactivity of the 45S5 bioglass, which would be in contact with the bone.Junta de Andalucía–FEDER (Spain) US-1259771M.E.C.     Developmental Dental Defects in Permanent Teeth Resulting from Trauma in Primary Dentition: A Systematic Review

    The objective was to determine whether trauma in primary dentition causes alterations in the development of permanent dentition. Searches were made in May 2020 using PubMed, MEDLINE, MEDES, Scopus, Lilacs, and Embase. Papers in English, German, and Spanish, without restrictions in the year of publication, were included. The quality of the studies was analyzed using the NOS Scale. The search retrieved 537 references, and seven studies were included for a qualitative analysis. The results showed that trauma to a deciduous tooth can damage the bud of the permanent tooth. Enamel discoloration and/or hypoplasia were the most common sequelae in the permanent teeth after trauma to the primary predecessor. The type and severity of sequelae in the permanent tooth are associated with the development phase of the bud. Children with trauma of their primary teeth should receive checkups until the eruption of the permanent teeth for the early diagnosis and treatment of possible sequelae. Intrusion of the primary tooth was the trauma that caused the most damage and enamel alterations the most frequent sequelae

    Effects of Fluoride and Calcium Phosphate-Based Varnishes in Children at High Risk of Tooth Decay: A Randomized Clinical Trial

    Background: The aim of this study was to investigate the effect of the application of two varnishes—MI Varnish (5% sodium fluoride with CPP-ACP) and Clinpro White Varnish (5% sodium fluoride with fTCP)—applied every three months in children with high caries risk for 12 months on plaque indexes, salivary pH, salivary lactic acid and chemical elements concentrations. Methods: We included 58 children aged 4–12 years, assigned to control (placebo), Clinpro and MI groups. Baseline and three-month saliva samples were taken. We assessed changes in pH, lactic acid concentrations and chemical elements in saliva. Results: At 12 months, all groups showed a nonsignificant increase in pH levels and a reduction in lactic acid, which was greatest in the placebo group. There was a significant reduction in 24Mg (p = <0.001), 31P (p = 0.033) and 66Zn (p = 0.005) levels in the placebo group (p ≤ 0.05), but not in the other elements studied: 23Na, 27Al, 39K, 44Ca, 52Cr, 55Mn, 57Fe, 59Co, 63Cu, 75As, 111Cd, 137Ba, 208Pb and 19F. Conclusions: Neither pH, lactic acid concentrations or most salivary chemical elements were useful in defining patients at high risk of caries or in monitoring the effect of MI Varnish and Clinpro White Varnish after three-month application for 12 months. However, the appearance of new cavities was stopped, and the hygiene index improved, probably due to hygienic and dietary measures and the use of fluoridated toothpaste.
