    Tarihi İpek Yolu üzerinde eski bir ipekçilik merkezi olan Bursa’nın tarihi kent dokusu içinde yer alan onbir ipek fabrikası, kentin ipekçilikle ilgili geçmişinin 19. ve 20. yüzyıldan kalan izleridir. Kentteki ipekçilik faaliyetinin 1980’lerden sonra terk edilmesiyle özgün işlevlerini yitiren bu fabrikalar bakımsızlık, ihmal, bilinçsiz onarım ve yenileme gibi nedenlerle harap olmakta, değişmektedir. Bu çalışmada, Bursa’daki ipek fabrikalarının mevcut durumları belgelenerek, ipekçilikle ilgili endüstri mirası olarak tanınmaları ve yaşatılmaları için olanaklar araştırılmıştır. Bursa’nın ipekçilik tarihi ve fiziksel dokusunun gelişimi ile ilgili birer belge niteliği taşıyan  fabrikaların özgün değerlerini koruyarak geleceğe aktarılabilmeleri için öneriler geliştirilmiştir. Anahtar Kelimeler: Bursa, ipekçilik, ipek fabrikaları, endüstri mirası, koruma. Sericulture, was an universal culture, improved and expanded by training and practice. The Silk Road, which connected Asia and Europe, is an outcome of this culture. The legendary road created a lively commercial and cultural development at its stopping points. Bursa, once the capital of the Ottoman Empire, became an important sericulture center on the Silk Road during the fourteenth century and preserved its importance until mid twentieth century. During the nineteenth century, Ottomans started to provide raw silk for the silk weaving industry in Europe. The demand for semi-finished products urged the import of technology from Europe, resulting in the establishment of many steam-powered silk factories in Bursa. In the early years of the Turkish Republic, the weaving sector in the city was revived. With this new impetus, new units were added to steam-powered silk factories of the nineteenth century. The sericulture activities in the city came to an end about 1980 with the import of cheaper silk from the Far East. There are eleven redundant silk factories within the historic city of Bursa. These factories are under the risk of total extinction due to neglect or modifications. The aim of this study is to explore the possibilities for the preservation of the factories. The surviving structures have been surveyed and proposals have been developed to define the methodological framework for the conservation of the industrial heritage related to sericulture in Bursa.  Keywords: Bursa, sericulture, silk factories, industrial heritage, conservation

    Can we predict agitation in patients with suicide attempts in the emergency department?

    Background: The agitation in patients presenting to the emergency department (ED) after suicide attempts is common and an important problem.Objective: To establish whether we can predict agitated patients among suicide attempt patients in ED.Methods: This is a cross-sectional observational study of adult suicide attempt events in ED. Information was collected prospectively on a specially designed data-collection form. Patients aged 16 years old and above who presented to the ED for care due to suicide attempts were included in the study. Suicide attempts were grouped as aggressive and non-aggressive attempts.Results: A total of 533 patients were included. Forty-three of these patients had agitation in ED (8%). Non-aggressive suicide attempts were referred to psychiatry services more than aggressive ones (73.6%, n=345 vs 32.8%, n=21, P<0.0001). Agitation in ED and being male increased aggressive suicide attempt risk 3.5 (95% CI:1.6-7.6) and 3.2 times (95% CI:1.8-5.5), respectively. Agitation was statistically more frequent among these patients: those on antidepressant  overdose, with previous suicide attempt; with aggressive suicide attempt; and those with confusion; and unconsciousness (P<0.05).Conclusion: Patients who attempted suicide and whose risk of harm to others included those with: antidepressant overdose, aggressive suicide attempt and the unconscious.     A call from early‑career Turkish scientists: seismic resilience is only feasible with “earthquake culture”

    Despite significant scientific advances in earthquake research and building codes, Türkiye remains vulnerable to earthquakes, as demonstrated by the tragic Kahramanmaraş earthquake in February 2023. In contrast, countries such as Chile and Japan have successfully reduced earthquake damage through strict enforcement of building codes and effective public awareness campaigns. This paper highlights the need for seismic resilience in Türkiye and proposes actionable guidelines to bridge the gap between science and society. These guidelines include comprehensive geoscience education, the establishment of local earthquake centers, effective science communication, preparation for future earthquakes through scenario modeling, and development of an earthquake culture. Geoscience education should be integrated into the education system, and opportunities for geoscientists should be increased. Local earthquake centers can improve seismic monitoring, research, and public outreach. Geoscientists should prioritize science communication training to engage the public and combat misinformation. Scenario modeling and annual preparedness exercises can improve earthquake preparedness across the country, and promoting earthquake memory and awareness initiatives will build a collective consciousness about earthquakes. By implementing these guidelines, Türkiye can build earthquake resilience and mitigate the impact of future earthquakes; however, the active engagement of scientists, institutions, and the public is essential to achieve earthquake resilience

    Diagnosis of acquired generalized lipodystrophy in a single patient with T-cell lymphoma and no exposure to Metreleptin

    Abstract Background Metreleptin, a recombinant methionyl -human -leptin, was approved to treat patients with generalized lipodystrophy (GL) in February 2014. However, leptin therapy has been associated with the development of lymphoma. We present a unique case of a patient with prior history of T cell lymphoma in remission, who was diagnosed with Acquired Generalized Lipodystrophy (AGL) during the following year after a clinical remission of her lymphoma without receiving leptin therapy. Case presentation A 33-year-old woman with a diagnosis of stage IV subcutaneous panniculitis like T-cell lymphoma in 2011, underwent chemotherapy. Shortly after completion therapy, she had a relapse and required more chemotherapy with complete response, followed by allogenic stem cell transplant on June 28, 2012. Since that time, she has been on observation with no evidence of disease recurrence. Subsequent to the treatment, she was found to have high triglycerides, loss of fat tissue from her entire body and diagnosis of diabetes. Constellation of these findings led to the diagnosis of AGL in 2013. Her leptin level was low at 3.4 ng/mL (182 pmol/mL). She is currently not receiving any treatment with Metreleptin for her AGL. Conclusions Causal association between exogenous leptin therapy and T-cell lymphoma still remains unclear. We hereby present a case of a young woman who was diagnosed with AGL after going into remission from T-cell lymphoma and who has never been treated with Metreleptin. Steroid therapy and chemotherapy might have masked the diagnosis of AGL in this patient.     Phytochemical profile and some biological activities of three Centaurea species from Turkey

    Purpose: To characterise the phytochemical profile of whole plants of Centaurea balsamita, C. depressa and C. lycopifolia with LC-ESI-MS/MS, and as well as their antioxidant, anticholinesterase and antimicrobial activities.Methods: Organic and aqueous extracts of the three Centaurea species were evaluated for DPPH free radical, ABTS cation radical scavenging and cupric reducing antioxidant capacity (CUPRAC). Acetyland butyryl-cholinesterase enzyme inhibition abilities of the extracts using petroleum ether, acetone, methanol and water were studied to determine anticholinesterase activity, while antimicrobial activity was determined by disc diffusion method using appropriate antimicrobial standards and organisms. The phytochemical components of the methanol extracts were assessed by LC-MS/MS.Results: The methanol extract of C. balsamita exhibited much higher DPPH free and ABTS cation radicals scavenging activities (with IC50 of 62.65 ± 0.97 and 24.21 ± 0.70 mg/ml, respectively) than the other extracts. The petroleum ether extracts of the plant species exhibited moderate inhibitory activity against butyrylcholinesterase enzymes while the acetone extract of C. balsamita showed good antifungal activity against Candida albicans. Quinic acid (17513 ± 813 μg/g, 63874 ± 3066 μg/g and 108234 ± 5195 μg/g) was the major compound found in the methanol extracts of C. balsamita, C. depressa and C. Lycopifolia, respectively.Conclusion: These results indicate quinic acid is the major compound in the three plant species and that Centaurea balsamita has significant antioxidant, anticholinesterase and antimicrobial properties. Further studies to identify the compounds in the extracts responsible for the activities are required.Keywords: Centaurea, LC-ESI-MS/MS, Anticholinesterase, Antioxidant, Antimicrobia

    A subcutaneous adipose tissue-liver signalling axis controls hepatic gluconeogenesis.

    The search for effective treatments for obesity and its comorbidities is of prime importance. We previously identified IKK-ε and TBK1 as promising therapeutic targets for the treatment of obesity and associated insulin resistance. Here we show that acute inhibition of IKK-ε and TBK1 with amlexanox treatment increases cAMP levels in subcutaneous adipose depots of obese mice, promoting the synthesis and secretion of the cytokine IL-6 from adipocytes and preadipocytes, but not from macrophages. IL-6, in turn, stimulates the phosphorylation of hepatic Stat3 to suppress expression of genes involved in gluconeogenesis, in the process improving glucose handling in obese mice. Preliminary data in a small cohort of obese patients show a similar association. These data support an important role for a subcutaneous adipose tissue-liver axis in mediating the acute metabolic benefits of amlexanox on glucose metabolism, and point to a new therapeutic pathway for type 2 diabetes

    Does intravenous lidocaine added to nonsteroidal anti-inflammatory drugs reduce pain during colposcopy? A prospective randomized double-blind study

    Objectives: In recent years, lidocaine infusion for pain management during long operations is becoming more widespread in anesthesiology practice. However, only a limited number of studies have reported the intravenous use of lidocaine for short-term interventions. The aim of this study was to investigate the effectiveness of intravenous lidocaine use in pain management during colposcopic cervical biopsy and endocervical curettage (ECC). Material and methods: Patients between the ages of 18 and 65 years with abnormal cytological findings or who were determined to be human papillomavirus (HPV)-positive were included in this randomized double-blind study. The lidocaine group (Group L, n = 30) was intravenously administered 50 mg dexketoprofen + 1.5 mg/kg lidocaine in 10 mL saline for 3 min 30 min before the procedure. The control group (Group C, n = 30) was intravenously administered 50 mg dexketoprofen in 10 mL saline for 3 min, 30 min before the procedure. During the procedure, pain scores were assessed using the visual analogue scale (VAS). In addition, patients, operator satisfaction and duration of procedure were assessed Results: There were no differences in the demographic data of the groups. Pain scores during biopsy and ECC were significantly lower in Group L than in Group C (p < 0.001). The duration of the procedure was significantly shorter in Group L than in Group C (5.00 ± 0.78 vs 6.12 ± 1.16, respectively; p < 0.001). Patient and operator satisfaction were significantly higher in Group L than in Group C (p < 0.001). Conclusions: Intravenous lidocaine administration can be used as an alternative approach to reduce pain and increase operator and patient satisfaction during colposcopy-directed biopsy and ECC procedures in office settings
