Endometrial Carcinoma Presenting as Vasculitic Sensorimotor Polyneuropathy
Paraneoplastic syndromes (PNS) are a heterogeneous group of symptoms which are indirectly caused by primary or metastatic tumor. Paraneoplastic polyneuropathy (PNP) is mostly related to small cell lung cancer (5%), prostate, gastric, and breast cancer. Only sporadic cases have been reported to be associated with endometrial cancer. We present a case of a premenopausal woman with severe vasculitic, asymmetric sensorimotor polyneuropathy that developed in conjunction with an endometrial carcinoma responding to surgical therapy of primary tumor combined to steroid therapy. Neurological symptoms such as asymmetrical sensorimotor deficits and painful paresthesias are suspicious when they occur in otherwise healthy women with no medical history. The phenomenon of a paraneoplastic syndrome can point to an underlying malignancy and can be used as marker of progression or regression of the tumor. Due to the rarity of PNP, there is no standard treatment. Recommended therapy is stage-adjusted treatment of the primary tumor

Für den naturwissenschaftlichen Unterricht bieten mobile Endgeräte, Computer und digitale Videotechnik eine echte Alternative zu den klassischen Verfahren der Erfassung und Analyse von Bewegungsdaten. Seit einigen Jahren hat sich daher die (digitale) Videoanalyse im Unterricht etabliert. Ein realer Bewegungsvorgang wird dabei per Digital-, Handy oder Tablet-Kamera aufgezeichnet. Der digitale Videoclip der Bewegung besteht aus einer Reihe von Einzelbildern, über die sich die Bewegung eines Objektes verfolgen lässt – manuell ‚per Hand’ (mit Maus, Finger, Eingabestift) oder auch automatisiert.In den letzten Jahrzehnten wurden dazu zahlreiche Videoanalysesysteme vorgestellt. Neben den kommerziellen Lösungen existieren auch Freewarelösungen wie z. B. Viana (für Windows Viana 3, Viana.NET).Viana wurde nun auch als sog. Mucinous Cystic Neoplasm of Pancreas in a Pregnant Woman Presenting with Severe Anemia and Gastric Bleeding: Case Report and Review of the Literature
Mucinous cystic neoplasms of the pancreas are uncommon and especially their occurrence during pregnancy is an extremely rare event which necessitates an individualized and interdisciplinary management. A 33-year old woman was referred to our department during her third trimester of pregnancy (34th week of gestation) with severe anemia and tarry stools. Based on gastroscopic findings, our interdisciplinary team suspected a gastrointestinal stromal tumor and therefore indicated a prompt delivery via cesarean section completed with an oncological resection of the neoplasm. Histological examination subsequently showed a mucinous cystic neoplasm of the pancreas with no evidence of malignancy. To review the prevalence of mucinous cystic neoplasms and to discuss diagnosis and treatment during pregnancy. Moreover, we critically value the indication of preterm delivery and the oncological procedure in the perspective of outcome for mother and infant. A bleeding gastrointestinal tumor during pregnancy represents a life-threatening risk for mother and infant and requires an immediate interdisciplinary treatment. The urgency and radicality of the therapy should be adapted according to individual findings. As our patient's tumor was suspected of having a malignant progression, an extensive surgical intervention was necessary

Tumor Mutation Burden Forecasts Outcome in Ovarian Cancer with BRCA1 or BRCA2 Mutations
Background: Increased number of single nucleotide substitutions is seen in breast and ovarian cancer genomes carrying disease-associated mutations in BRCA1 or BRCA2. The significance of these genome-wide mutations is unknown. We hypothesize genome-wide mutation burden mirrors deficiencies in DNA repair and is associated with treatment outcome in ovarian cancer. Methods and Results: The total number of synonymous and non-synonymous exome mutations (Nmut), and the presence of germline or somatic mutation in BRCA1 or BRCA2 (mBRCA) were extracted from whole-exome sequences of high-grade serous ovarian cancers from The Cancer Genome Atlas (TCGA). Cox regression and Kaplan-Meier methods were used to correlate Nmut with chemotherapy response and outcome. Higher Nmut correlated with a better response to chemotherapy after surgery. In patients with mBRCA-associated cancer, low Nmut was associated with shorter progression-free survival (PFS) and overall survival (OS), independent of other prognostic factors in multivariate analysis. Patients with mBRCA-associated cancers and a high Nmut had remarkably favorable PFS and OS. The association with survival was similar in cancers with either BRCA1 or BRCA2 mutations. In cancers with wild-type BRCA, tumor Nmut was associated with treatment response in patients with no residual disease after surgery. Conclusions: Tumor Nmut was associated with treatment response and with both PFS and OS in patients with high-grade serous ovarian cancer carrying BRCA1 or BRCA2 mutations. In the TCGA cohort, low Nmut predicted resistance to chemotherapy, and for shorter PFS and OS, while high Nmut forecasts a remarkably favorable outcome in mBRCA-associated ovarian cancer. Our observations suggest that the total mutation burden coupled with BRCA1 or BRCA2 mutations in ovarian cancer is a genomic marker of prognosis and predictor of treatment response. This marker may reflect the degree of deficiency in BRCA-mediated pathways, or the extent of compensation for the deficiency by alternative mechanisms
Experimental quantum speed-up in reinforcement learning agents
Increasing demand for algorithms that can learn quickly and efficiently has
led to a surge of development within the field of artificial intelligence (AI).
An important paradigm within AI is reinforcement learning (RL), where agents
interact with environments by exchanging signals via a communication channel.
Agents can learn by updating their behaviour based on obtained feedback. The
crucial question for practical applications is how fast agents can learn to
respond correctly. An essential figure of merit is therefore the learning time.
While various works have made use of quantum mechanics to speed up the agent's
decision-making process, a reduction in learning time has not been demonstrated
yet. Here we present a RL experiment where the learning of an agent is boosted
by utilizing a quantum communication channel with the environment. We further
show that the combination with classical communication enables the evaluation
of such an improvement, and additionally allows for optimal control of the
learning progress. This novel scenario is therefore demonstrated by considering
hybrid agents, that alternate between rounds of quantum and classical
communication. We implement this learning protocol on a compact and fully
tunable integrated nanophotonic processor. The device interfaces with
telecom-wavelength photons and features a fast active feedback mechanism,
allowing us to demonstrate the agent's systematic quantum advantage in a setup
that could be readily integrated within future large-scale quantum
