Wage losses in the year after breast cancer: Extent and determinants among Canadian women

Their assessment is made more urgent because more women now participate in the workforce and because breast cancer is managed using multiple treatment modalities that could lead to long work absences. We evaluated wage losses, their determinants, and the associations between wage losses and changes for the worse in the family's financial situation among Canadian women over the first 12 months after diagnosis of early breast cancer.
Methods - We conducted a prospective cohort study among women with breast cancer from eight hospitals throughout the province of Quebec. Information that permitted the calculation of wage losses and information on potential determinants of wage losses were collected by three pretested telephone interviews conducted over the year following the start of treatment. Information on medical characteristics was obtained from medical records. The main outcome was the proportion of annual wages lost because of breast cancer. Multivariable analysis of variance using the general linear model was used to identify personal, medical, and employment characteristics associated with the proportion of wages lost. All statistical tests were two-sided.
Results - Among 962 eligible breast cancer patients, 800 completed all three interviews. Of these, 459 had a paying job during the month before diagnosis. On average, these working women lost 27% of their projected usual annual wages (median = 19%) after compensation received had been taken into account. Multivariable analysis showed that a higher percentage of lost wages was statistically significantly associated with a lower level of education (Ptrend = .0018), living 50 km or more from the hospital where surgery was performed (P = .070), lower social support (P = .012), having invasive disease (P = .086), receipt of chemotherapy (P < .001), self-employment (P < .001), shorter tenure in the job (Ptrend < .001), and part-time work (P < .001).

The Sloan Digital Sky Survey Reverberation Mapping Project: Velocity Shifts of Quasar Emission Lines
Quasar emission lines are often shifted from the systemic velocity due to
various dynamical and radiative processes in the line-emitting region. The
level of these velocity shifts depends both on the line species and on quasar
properties. We study velocity shifts for the line peaks of various narrow and
broad quasar emission lines relative to systemic using a sample of 849 quasars
from the Sloan Digital Sky Survey Reverberation Mapping (SDSS-RM) project. The
coadded (from 32 epochs) spectra of individual quasars have sufficient
signal-to-noise ratio (SNR) to measure stellar absorption lines to provide
reliable systemic velocity estimates, as well as weak narrow emission lines.
The sample also covers a large dynamic range in quasar luminosity (~2 dex),
allowing us to explore potential luminosity dependence of the velocity shifts.
We derive average line peak velocity shifts as a function of quasar luminosity
for different lines, and quantify their intrinsic scatter. We further quantify
how well the peak velocity can be measured for various lines as a function of
continuum SNR, and demonstrate there is no systematic bias in the line peak
measurements when the spectral quality is degraded to as low as SNR~3 per SDSS
pixel. Based on the observed line shifts, we provide empirical guidelines on
redshift estimation from [OII]3728, [OIII]5008, [NeV]3426, MgII, CIII],
HeII1640, broad Hbeta, CIV, and SiIV, which are calibrated to provide unbiased
systemic redshifts in the mean, but with increasing intrinsic uncertainties of
46, 56, 119, 205, 233, 242, 400, 415, and 477 km/s, in addition to the
measurement uncertainties. These more realistic redshift uncertainties are
generally much larger than the formal uncertainties reported by the redshift
pipelines for spectroscopic quasar surveys, and demonstrate the infeasibility
of measuring quasar redshifts to better than ~200 km/s with only broad lines.Comment: matched to the published version; minor changes and conclusions

Stability of stationary states in the cubic nonlinear Schroedinger equation: applications to the Bose-Einstein condensate
The stability properties and perturbation-induced dynamics of the full set of
stationary states of the nonlinear Schroedinger equation are investigated
numerically in two physical contexts: periodic solutions on a ring and
confinement by a harmonic potential. Our comprehensive studies emphasize
physical interpretations useful to experimentalists. Perturbation by stochastic
white noise, phase engineering, and higher order nonlinearity are considered.
We treat both attractive and repulsive nonlinearity and illustrate the

Crowdsourced assessment of surgical skill proficiency in cataract surgery
OBJECTIVE: To test whether crowdsourced lay raters can accurately assess cataract surgical skills.
DESIGN: Two-armed study: independent cross-sectional and longitudinal cohorts.
SETTING: Washington University Department of Ophthalmology.
PARTICIPANTS AND METHODS: Sixteen cataract surgeons with varying experience levels submitted cataract surgery videos to be graded by 5 experts and 300+ crowdworkers masked to surgeon experience. Cross-sectional study: 50 videos from surgeons ranging from first-year resident to attending physician, pooled by years of training. Longitudinal study: 28 videos obtained at regular intervals as residents progressed through 180 cases. Surgical skill was graded using the modified Objective Structured Assessment of Technical Skill (mOSATS). Main outcome measures were overall technical performance, reliability indices, and correlation between expert and crowd mean scores.
RESULTS: Experts demonstrated high interrater reliability and accurately predicted training level, establishing construct validity for the modified OSATS. Crowd scores were correlated with (r = 0.865, p \u3c 0.0001) but consistently higher than expert scores for first, second, and third-year residents (p \u3c 0.0001, paired t-test). Longer surgery duration negatively correlated with training level (r = -0.855, p \u3c 0.0001) and expert score (r = -0.927, p \u3c 0.0001). The longitudinal dataset reproduced cross-sectional study findings for crowd and expert comparisons. CONCLUSIONS: Crowdsourced rankings correlated with expert scores, but were not equivalent; crowd scores overestimated technical competency, especially for novice surgeons. A novel approach of adjusting crowd scores with surgery duration generated a more accurate predictive model for surgical skill. More studies are needed before crowdsourcing can be reliably used for assessing surgical proficiency
RPPA Space: An R Package for Normalization and Quantitation of Reverse-Phase Protein Array Data
SUMMARY: Reverse-Phase Protein Array (RPPA) is a robust high-throughput, cost-effective platform for quantitatively measuring proteins in biological specimens. However, converting raw RPPA data into normalized, analysis-ready data remains a challenging task. Here, we present the RPPA SPACE (RPPA Superposition Analysis and Concentration Evaluation) R package, a substantially improved successor to SuperCurve, to meet that challenge. SuperCurve has been used to normalize over 170 000 samples to date. RPPA SPACE allows exclusion of poor-quality samples from the normalization process to improve the quality of the remaining samples. It also features a novel quality-control metric, \u27noise\u27, that estimates the level of random errors present in each RPPA slide. The noise metric can help to determine the quality and reliability of the data. In addition, RPPA SPACE has simpler input requirements and is more flexible than SuperCurve, it is much faster with greatly improved error reporting.

Explaining productivity in a poor productivity region
© 2017, © The Author(s) 2017. Productivity is the preferred measure of firm-level efficiency and perceived to reflect resource use rates. Semi-structured interviews with restaurant managers in a tourism-dominated low productivity rural area reveal that they are motivated to supply products that they believe in and to sustain a quality of life that meets their needs rather than striving to achieve higher productivity. Pricing strategies, managerial objectives and local market characteristics are found to radically influence the area’s productivity value. An area’s productivity value might not be an indicator of resource use rates or productive efficiency, and could instead reflect resident managers’ motivations towards money and the presence of opportunities to achieve scale economies
The Sloan Digital Sky Survey Reverberation Mapping Project: Technical Overview
The Sloan Digital Sky Survey Reverberation Mapping project (SDSS-RM) is a
dedicated multi-object RM experiment that has spectroscopically monitored a
sample of 849 broad-line quasars in a single 7 deg field with the SDSS-III
BOSS spectrograph. The RM quasar sample is flux-limited to i_psf=21.7 mag, and
covers a redshift range of 0.1<z<4.5. Optical spectroscopy was performed during
2014 Jan-Jul dark/grey time, with an average cadence of ~4 days, totaling more
than 30 epochs. Supporting photometric monitoring in the g and i bands was
conducted at multiple facilities including the CFHT and the Steward Observatory
Bok telescopes in 2014, with a cadence of ~2 days and covering all lunar
phases. The RM field (RA, DEC=14:14:49.00, +53:05:00.0) lies within the CFHT-LS
W3 field, and coincides with the Pan-STARRS 1 (PS1) Medium Deep Field MD07,
with three prior years of multi-band PS1 light curves. The SDSS-RM 6-month
baseline program aims to detect time lags between the quasar continuum and
broad line region (BLR) variability on timescales of up to several months (in
the observed frame) for ~10% of the sample, and to anchor the time baseline for
continued monitoring in the future to detect lags on longer timescales and at
higher redshift. SDSS-RM is the first major program to systematically explore
the potential of RM for broad-line quasars at z>0.3, and will investigate the
prospects of RM with all major broad lines covered in optical spectroscopy.
SDSS-RM will provide guidance on future multi-object RM campaigns on larger
scales, and is aiming to deliver more than tens of BLR lag detections for a
homogeneous sample of quasars. We describe the motivation, design and
implementation of this program, and outline the science impact expected from

HIV-1 Infection Results in Sphingosine-1-Phosphate Receptor 1 Dysregulation in the Human Thymus
Regeneration of functional naïve T lymphocytes following the onset of human immunodeficiency virus (HIV) infection remains a crucial issue for people living with HIV (PLWH), even when adhering to antiretroviral therapy (ART). Thus far, reports on the impact of HIV-1 infection on the entry of thymic precursors and the egress of functional naïve T lymphocytes to and from the thymus are limited. We examined the impact of HIV-1 on Sphingosine-1-phosphate (S1P) signaling, which governs the egress of functional naïve thymocytes from the thymus to the periphery. Using in vitro experiments with primary human thymocytes and in vivo and ex vivo studies with humanized mice, we show that HIV-1 infection results in upregulation of the expression of S1P receptor 1 (S1PR1) in the human thymus. Intriguingly, this upregulation occurs during intrathymic infection (direct infection of the human thymic implant) as well as systemic infection in humanized mice. Moreover, considering the dysregulation of pro- and anti-inflammatory cytokines in infected thymi, the increased expression of S1PR1 in response to in vitro exposure to Interferon-Beta (IFN-β) and Tumor Necrosis Factor-Alpha (TNF-α) indicates that cytokine dysregulation following HIV infection may contribute to upregulation of S1PR1. Finally, an increased presence of CD3hiCD69− (fully mature) as well as CD3hiCD69+ (less mature) T cells in the spleen during HIV infection in humanized mice, combined with earlier expression of S1PR1 during thymocyte development, suggests that upregulation of S1PR1 may translate to increased or accelerated egress from the thymus. The egress of thymocytes that are not functionally mature from the thymus to peripheral blood and lymphoid organs may have implications for the immune function of PLWH
Possible realization of Josephson charge qubits in two coupled Bose-Einstein condensates
We demonstrate that two coupled Bose-Einstein condensates (BEC) at zero
temperature can be used to realize a qubit which is the counterpart of
Josephson charge qubits. The two BEC are weakly coupled and confined in an
asymmetric double-well trap. When the "charging energy" of the system is much
larger than the Josephson energy and the system is biased near a degeneracy
point, the two BEC represent a qubit with two states differing only by one
atom. The realization of the BEC qubits in realistic BEC experiments is briefly

Socioeconomic inequalities in the rate of stillbirths by cause: a population-based study.
OBJECTIVE: To assess time trends in socioeconomic inequalities in overall and cause-specific stillbirth rates in England. DESIGN: Population-based retrospective study. SETTING: England. PARTICIPANTS: Stillbirths occurring among singleton infants born between 1 January 2000 and 31 December 2007. MAIN OUTCOME MEASURE: Cause-specific stillbirth rate per 10 000 births by deprivation tenth and year of birth. Deprivation measured using the UK index of multiple deprivation at Super Output Area level. METHODS: Poisson regression models were used to estimate the relative deprivation gap (comparing the most and least deprived tenths) in rates of stillbirths (overall and cause-specific). Excess mortality was calculated by applying the rates seen in the least deprived tenth to the entire population at risk. Discussions with our local NHS multicentre ethics committee deemed that this analysis of national non-identifiable data did not require separate ethics approval. RESULTS: There were 44 stillbirths per 10 000 births, with no evidence of a change in rates over time. Rates were twice as high in the most deprived tenth compared with the least (rate ratio (RR) 2.1, 95% CI 2.0 to 2.2) with no evidence of a change over time. There was a significant deprivation gap for all specific causes except mechanical events (RR 1.2, 95% CI 0.9 to 1.5). The widest gap was seen for stillbirths due to antepartum haemorrhages (RR 3.1, 95% CI 2.8 to 3.5). No evidence of a change in the rate of stillbirth or deprivation gap over time was seen for any specific cause. CONCLUSION: A wide deprivation gap exists in stillbirth rates for most causes and is not diminishing. Unexplained antepartum stillbirths accounted for 50% of the deprivation gap, and a better understanding of these stillbirths is necessary to reduce socioeconomic inequalities
