    En el ámbito del derecho constitucional y legal mexicano no está prevista la figura de la Responsabilidad Patrimonial del Estado en el funcionamiento de la Administración de Justicia por los daños causados por los órganos encargados de impartirla, quienes al interpretar las normas jurídicas para dirimir las controversias planteadas por los particulares pueden llegar a causar daños como el error judicial o el funcionamiento anormal de dicha administración, y ante el vacío legal para promover una acción indemnizatoria se hace necesaria y patente la plena adecuación mediante una reforma al marco jurídico constitucional que regule la Responsabilidad Patrimonial del Estado para garantizar la indemnización patrimonial derivada por error judicial

    Dado el aumento de las técnicas de reproducción humana y las alternativas médicas para combatir los problemas de infertilidad, es necesario evaluar las consecuencias legales de su regulación en México, especialmente cuando viola la diversidad de derechos fundamentales de las familias modernas o contemporáneas. En este artículo se hace un estudio sobre la inconstitucionalidad de la regulación de la maternidad subrogada en la entidad federativa de Tabasco, porque hasta ahora, el procedimiento es exclusivamente para la familia tradicional. Se propone que los creadores de la norma jurídica comiencen a legislar dentro de la modernidad ante la evolución de la institución de la familia y a romper paradigmas no sólo en los cambios estructurales del sistema jurídico

    Equipo de Respuesta Inmediata obstétrico en el Instituto Mexicano del Seguro Social, factores facilitadores

    ResumenAntecedentesLos Equipos de Respuesta Inmediata hospitalarios tienen factores obstaculizadores y facilitadores. Se determinó la participación de estos últimos en el Instituto Mexicano del Seguro Social (IMSS).Material y métodosEstudio observacional, retrospectivo de reportes de urgencia obstétrica enviados por tecnología móvil y correo electrónico a la Unidad de Atención Médica del IMSS durante 2013. Se utilizó el programa Excel 2010 para estadística descriptiva.ResultadosUn total de 164,250 mensajes se recibieron (promedio 425 mensajes/día); el 32.2% correspondió a urgencia obstétrica que requirió apoyo de los Equipos de Respuesta Inmediata Directivo-Delegacional-Central. Por correo electrónico, hubo 73,452 casos de morbilidad obstétrica extrema (promedio de 6 casos/día). Se realizaron en total 480 simulacros (uno/mes) en los hospitales.Los factores facilitadores fueron la oportuna sincronización de los mensajes entre los directivos de los diferentes niveles de mando, el registro preciso de la urgencia obstétrica y los simulacros realizados por el personal operativo.La urgencia obstétrica más común fue preeclampsia-eclampsia con 3,351 reportes, y hemorragia obstétrica con 2,982 casos.DiscusiónLa sincronización adecuada de comunicación por mensajes entre el equipo de respuesta inmediata de los diferentes niveles de mando permitió mayor rapidez en la gestión médica-administrativa así como la participación del equipo médico hospitalario en el proceso de la atención de la urgencia obstétrica.ConclusiónEl uso de la tecnología móvil en el funcionamiento del equipo de respuesta inmediata en el IMSS, ha permitido mayor rapidez en la gestión médico-administrativa en la atención de la urgencia obstétrica; sin embargo, es necesario realizar estudios comparativos para determinar la significación estadística.AbstractBackgroundThere are barriers and enablers for the implementation of Rapid Response Teams in obstetric hospitals. The enabling factors were determined at Instituto Mexicano del Seguro Social (IMSS)Material and methodsAn observational, retrospective study was conducted by analysing the emergency obstetric reports sent by mobile technology and e-mail to the Medical Care Unit of the IMSS in 2013. Frequency and mean was obtained using the Excel 2010 program for descriptive statistics.ResultsA total of 164,250 emergency obstetric cases were reported, and there was a mean of 425 messages per day, of which 32.2% were true obstetric emergencies and required the Rapid Response team. By e-mail, there were 73,452 life threatening cases (a mean of 6 cases per day). A monthly simulation was performed in hospitals (480 in total).Enabling factors were messagés synchronisation among the participating personnel,the accurate record of the obstetrics, as well as the simulations performed by the operational staff.The most common emergency was pre-eclampsia-eclampsia with 3,351 reports, followed by obstetric haemorrhage with 2,982 cases.DiscussionThe enabling factors for the implementation of a rapid response team at IMSS were properly timed communication between the central delegation teams, as they allowed faster medical and administrative management and participation of hospital medical teams in the process.ConclusionMobile technology has increased the speed of medical and administrative management in emergency obstetric care.     Introducción: El Tumor de Pancoast se produce por crecimiento local de un tumor del vértice pulmonar que penetra fácilmente en el canal neural y destruye las raíces nerviosas octava cervical y primera y segunda torácicas. Su causa más frecuente es el cáncer de pulmón.Objetivo: Al presentar este caso, nuestro objetivo es mostrar de forma ilustrativa una de las formas neurológicas de presentación del cáncer de pulmóny señalar una vez más cómo este tipo de paciente casi siempre acude por Ortopedia y rara vez por Neumología o Medicina Interna.Presentación del caso: Se presentó una paciente que aquejó primeramente dolor en  hombro derecho, acude al ortopédico y al reumatólogo, este último  al verle las manos (uñas en vidrio de reloj y dedos hipocráticos), la remite a Neumología, se realiza radiografía de tórax, se ingresa en Medicina Interna, donde se le practica una  serie de estudios, incluida cirugía de mínimo acceso por el Grupo de TóraxConclusiones: El estudio histológico corroboró  el diagnóstico de carcinoma bronquial epidermoide, así como demostró cómo estos casos entran por Ortopedia, lo cual atrasa el  accionar médico.Motivación: Tuvimos a bien presentar este caso, que ilustra cómo los pacientes con esta patología, en muchas ocasiones, entran por Ortopedia, lo cual demora el diagnóstico y, por ende, la conducta.Palabras clave: tumor de pancoast, uñas en vidrio de reloj, dedos hipocráticos. carcinoma bronquial epidermoide.</p

    Characteristics of Serve, Reception and Set That Determine the Setting Efficacy in Men’s Volleyball

    This study belongs to the doctoral thesis called Analysis of the variables that affect the reception and the setting in volleyball, made by JG-S at the University of Extremadura (González-Silva, 2019). This study was made possible thanks to the contribution of the Consejería de Economía e Infraestructuras de la Junta de Extremadura (Spain) through the European Regional Development fund: A way to make Europe (GR18129).The aim of this investigation was to establish the criteria of service, reception and set that determine setting efficacy in world-class top-level volleyball. The study sample consisted of 4.113 gaming actions (1.371 serve actions, 1.371 reception actions, and 1.371 set actions), corresponding to the observation of four matches for each of the 12 best ranked teams in the Volleyball World Championship - a total of 23 matches. The criteria were: in-game role of the server, serve zone, type of serve, striking technique and serve direction; receiver player, reception zone, and reception efficacy; setting zone, type of set, setting technique, setting efficacy, a set’s area, and set tempo. Multinomial logistic regression showed that criteria related to reception (reception efficacy) and to set (setting zone, type of set, a set’s area, and set tempo) determined set efficacy. Specifically, positive and negative receptions and settings from acceptable and nonacceptable zones reduced perfect setting. In contrast, the jump set toward zones three and six and the first and second tempo increased perfect setting. Serve criteria did not determine set efficacy.     Six parasitoid species from three families of Hymenoptera were identified attacking whiteflies and armored scales on orange crops in the Chontalpa region of Tabasco, Mexico. These were: Arrhenophagus chionaspidis (Auvr.), Encarsia citrina (Craw), E. haitiensis Dozier, E. perplexa (Huang &amp; Polaszek), Eretmocerus sp. and Signiphora sp., all of which are new records for the region and the state.

    Maternal serum adipokines and inflammatory markers at late gestation and newborn weight in mothers with and without gestational diabetes mellitus

    Objectives: Maternal obesity increases the risk of gestational diabetes mellitus (GDM) and is positively correlated with neonatal obesity increasing the risk of adiposity in both young and adult offspring. Maternal secreted factors from adipose tissue such as adipokines and inflammatory cytokines may regulate fetal growth. This study investigated associations between maternal adipokines and inflammatory markers at late gestation, and neonatal anthropometric characteristics in mothers with and without GDM. Material and methods: The study included 65 women with GDM and 65 pregnant women with normal glucose tolerance evaluated at the time of term elective Caesarean section. Adiponectin, leptin, resistin, adipsin, neutrophil gelatinase-associated lipocalin (NGAL), nerve growth factor (NGF), monocyte chemotactic protein-1 (MCP-1) and tumor necrosis factor-alpha (TNF-alpha) concentrations were measured in maternal serum by the multiplex immunoassay using Magpix technology. C-reactive protein (CRP) was measured with a particle-enhanced turbidimetric immunoassay and neonatal anthropometric variables were assessed. The association of birthweight with individual biomarkers was analyzed using multivariate logistic regression adjusted for maternal factors. Results: Adiponectin, leptin, resistin, adipsin, NGAL and NGF were not significantly associated with higher birthweight. The maternal factors in association with higher birthweight observed in GDM were CRP, MCP-1 and TNF-alpha levels. Regression analysis showed that TNF-alpha was an independent risk factor for higher birthweight (p = 0.046). Conclusions: These results suggest an involvement of maternal inflammatory markers at late gestation and fetal growth in mothers with GDM, and that TNF-alpha could play a major role

    Anti-IL-6 Receptor Tocilizumab in Refractory Graves? Orbitopathy: National Multicenter Observational Study of 48 Patients

    Graves’ orbitopathy (GO) is the most common extrathyroidal manifestation of Graves’ disease (GD). Our aim was to assess the e cacy and safety of Tocilizumab (TCZ) in GO refractory to conventional therapy. This was an open-label multicenter study of glucocorticoid-resistant GO treated with TCZ. The main outcomes were the best-corrected visual acuity (BVCA), Clinical Activity Score (CAS) and intraocular pressure (IOP). These outcome variables were assessed at baseline, 1st, 3rd, 6th and 12th month after TCZ therapy onset. The severity of GO was assessed according to the European Group on Graves’ Orbitopathy (EUGOGO). We studied 48 (38 women and 10 men) patients (95 eyes); mean age standard deviation 51 11.8 years. Before TCZ and besides oral glucocorticoids, they had received IV methylprednisolone (n = 43), or selenium (n = 11). GO disease was moderate (n =29) or severe (n = 19) and dysthyroid optic neuropathy (DON) (n = 7). TCZ was used in monotherapy (n = 45) or combined (n = 3) at a dose of 8 mg/kg IV every four weeks (n = 43) or 162 mg/s.c. every week (n = 5). TCZ yielded a significant improvement in all of the main outcomes at the 1st month that was maintained at one year. Comparing the baseline with data at 1 year all of the variables improved; BCVA (0.78 0.25 vs. 0.9 0.16; p = 0.0001), CAS (4.64 1.5 vs. 1.05 1.27; p = 0.0001) and intraocular pressure (IOP) (19.05 4.1 vs. 16.73 3.4 mmHg; p = 0.007). After a mean follow-up of 16.1 2.1 months, low disease activity (CAS 3), was achieved in 88 eyes (92.6%) and TCZ was withdrawn in 29 cases due to low disease activity (n = 25) or ine cacy (n = 4). No serious adverse events were observed.
