    Testing Lorentz invariance by use of vacuum and matter filled cavity resonators

    We consider tests of Lorentz invariance for the photon and fermion sector that use vacuum and matter-filled cavities. Assumptions on the wave-function of the electrons in crystals are eliminated from the underlying theory and accurate sensitivity coefficients (including some exceptionally large ones) are calculated for various materials. We derive the Lorentz-violating shift in the index of refraction n, which leads to additional sensitivity for matter-filled cavities ; and to birefringence in initially isotropic media. Using published experimental data, we obtain improved bounds on Lorentz violation for photons and electrons at levels of 10^-15 and below. We discuss implications for future experiments and propose a new Michelson-Morley type experiment based on birefringence in matter.Comment: 15 pages, 8 table

    Ten years of ecosystem services matrix: Review of a (r)evolution

    With the Ecosystem Service (ES) concept's popularisation, the need for robust and practical methodologies for ES assessments has increased. The ES matrix approach, linking ecosystem types or other geospatial units with ES in easy-to-apply lookup tables, was first developed ten years ago and, since then, has been broadly used. Whereas detailed methodological guidelines can be found in literature, the ES matrix approach seems to be often used in a quick (and maybe even "quick and dirty”) way. Based on a reviewa of scientific publications, in which the ES matrix approach was used, we present the diversity of application contexts, highlight trends of uses and propose future recommendations for improved applications of the ES matrix. A total of 109 studies applying the ES matrix approach and one methodological study without concrete applications were considered for the review. Amongst the main patterns observed, the ES matrix approach allows the assessment of a higher number of ES than other ES assessment methods. ES can be jointly assessed with indicators for ecosystem condition and biodiversity in the ES matrix. Although the ES matrix allows us consider many data sources to achieve the assessment scores for the individual ES, in the reviewed studies, these were mainly used together with expert-based scoring (73%) and/or ES scores that were based on an already-published ES matrix or deduced by information found in related scientific publications (51%). We must acknowledge that 27% of the studies did not clearly explain their methodology. This points out a lack of method elucidation on how the data had been used and where the scores came from. Although some studies addressed the need to consider variabilities and uncertainties in ES assessments, only a minority of studies (15%) did so. Our review shows that, in 29% of the studies, an already-existing matrix was used as an initial matrix for the assessment (mainly the same matrix from one of the Burkhard et al. papers). In 16% of the reviewed studies, no other data were used for the matrix scores or no adaptation of the existing matrix used was made. However, the actual idea of the ES scores, included in the Burkhard et al.'s matrices published 10 years ago, was to provide some examples and give inspiration for one's own studies. Therefore, we recommend to use only scores assessed for a specific study or, if one wishes to use pre-existing scores from another study, to revise them in depth, taking into account the local context of the new assessment. We also recommend to systematically report and consider variabilities and uncertainties in each ES assessment. We emphasise the need for all scientific studies to describe clearly and extensively the whole methodology used to score or evaluate ES in order to be able to rate the quality of the scores obtained. In conclusion, the application of the ES matrix has to become more transparent and integrate more variability analyses. The increasing number of studies that use the ES matrix approach confirms its success, appropriability, flexibility and utility for decision-making, as well as its ability to increase awareness of ES

    A narrative review on endopancreatic interventions: an innovative access to the pancreas

    The natural connection between the duodenum and the pancreatic duct enables a minimally invasive access to the pancreas. Endoscopically this access is already regularly used, mainly for diagnostic and even for certain therapeutic purposes. With per-oral pancreatoscopy the endopancreatic approach allows the direct visualization of the pancreatic duct system potentially improving the diagnostic work-up of pancreatic cystic neoplasms, intrapancreatic strictures and removal of pancreatic duct stones. However, the endopancreatic access can equally be applied for surgical interventions. The objective of this review is to summarize endoscopic and surgical interventions using the endopancreatic access. Endopancreatic surgery stands for a further development of the endoscopic technique: a rigid endoscope is transabdominally introduced over the duodenum and the papilla to enable resections of strictures and inflamed tissue from inside the pancreas under visual control. While the orientation and localization of target structures using this minimally invasive approach is difficult, the development of an accurate image guidance system will play a key role for the clinical implementation and widespread use of endoscopic and surgical endopancreatic interventions

    A Systematic Review of the Perforated Duodenal Diverticula: Lessons Learned from the Last Decade

    Background: The perforated duodenal diverticulum remains a rare clinical entity, the optimal management of which has not been well established. Historically, primary surgery has been the preferred treatment modality. This was called into question during the last decade, with the successful application of non-operative therapy in selected patients. The aim of this systematic review is to identify cases of perforated duodenal diverticula published over the past decade and to assess any subsequent evolution in treatment. Methods: A systematic review of English and non-English articles reporting on perforated duodenal diverticula using MEDLINE (2008-2020) was performed. Only cases of perforated duodenal diverticula in adults (> 18 years) that reported on diagnosis and treatment were included. Results: Some 328 studies were identified, of which 31 articles met the inclusion criteria. These studies included a total of 47 patients with perforated duodenal diverticula. This series suggests a trend towards conservative management with 34% (16/47) of patients managed non-operatively. In 31% (5/16) patients initially managed conservatively, a step-up approach to surgical intervention was required. Conclusion: Conservative treatment of perforated duodenal diverticula appears to be an acceptable and safe treatment strategy in stable patients without signs of peritonitis under careful observation. For patients who fail to respond to conservative treatment, a step-up approach to percutaneous drainage or surgery can be applied. If surgery is required, competence in techniques ranging from simple diverticulectomy to Roux-en-Y gastric diversion or even Whipple's procedure may be required depending on tissue friability and diverticular collar size.     Right-sided ALPPS after preoperative emergency embolization of the right hepatic artery: case report with a favorable anatomy

    In patients with extensive colorectal liver metastases (CRLM) and insufficient future liver remnant (FLR) a faster and more effective FLR augmentation than portal vein embolization is the associating liver partition and portal vein ligation in staged hepatectomy (ALPPS). Before ALPPS, the presence of arterial blood supply to the subsequently resected hemiliver must be ensured. We present a case with neoadjuvant-treated CRLM and insufficient FLR who developed a large intrahepatic hematoma after liver biopsy. For continuous bleeding, the right hepatic artery was embolized. Fortunately, an accessory right hepatic artery arising from the superior mesenteric artery was present, which enabled the ALPPS procedure to be performed. After ALPPS, the patient did not experience liver failure. The case exemplifies that preoperative evaluation of the vascular supply of the liver is of paramount importance in advanced hepatic surgery such as ALPPS

    Pre-Emptive Endoluminal Negative Pressure Therapy at the Anastomotic Site in Minimally Invasive Transthoracic Esophagectomy (the preSPONGE Trial): Study Protocol for a Multicenter Randomized Controlled Trial

    Introduction: Anastomotic leakage (AL) accounts for a significant proportion of morbidity following oesophagectomy. Endoluminal negative pressure (ENP) therapy via a specifically designed polyurethane foam (EsoSponge®, B.Braun Medical, Melsungen, Germany) has become the standard of care for AL in many specialized centres. The prophylactic (pENP) application of this technique aims to reduce postoperative morbidity and is a novel approach which has not yet been investigated in a prospective study. The aim of this study is therefore to assess the effect of pENP at the anastomotic site in high-risk patients undergoing minimally invasive transthoracic Ivor Lewis oesophagectomy. Methods and analysis: The study design is a prospective, multi-centre, two-arm, parallel-group, randomised controlled trial and will be conducted in two phases. Phase one is a randomised feasibility and safety pilot trial involving 40 consecutive patients. After definitive sample size calculation, additional patients will be included accordingly during phase two. The primary outcome of the study will be the postoperative length of hospitalization until reaching previously defined “fit for discharge criteria”. Secondary outcomes will include postoperative morbidity, mortality and postoperative AL-rates based on 90-day follow-up. A confirmatory analysis based on intention-to-treat will be performed. Ethics and dissemination: The ethics committee of the University of Zurich approved this study (2019-00562), which has been registered with ClinicalTrials.gov on 14.11.2019 (NCT04162860) and the Swiss National Clinical Trials Portal (SNCTP000003524). The results of the study will be published and presented at appropriate conferences

    Submillimetre line spectrum of the Seyfert galaxy NGC1068 from the Herschel-SPIRE Fourier Transform Spectrometer

    The first complete submillimetre spectrum (190-670um) of the Seyfert 2 galaxy NGC1068 has been observed with the SPIRE Fourier Transform Spectrometer onboard the {\it Herschel} Space Observatory. The sequence of CO lines (Jup=4-13), lines from water, the fundamental rotational transition of HF, two o-H_2O+ lines and one line each from CH+ and OH+ have been detected, together with the two [CI] lines and the [NII]205um line. The observations in both single pointing mode with sparse image sampling and in mapping mode with full image sampling allow us to disentangle two molecular emission components, one due to the compact circum-nuclear disk (CND) and one from the extended region encompassing the star forming ring (SF-ring). Radiative transfer models show that the two CO components are characterized by density of n(H_2)=10^4.5 and 10^2.9 cm^-3 and temperature of T=100K and 127K, respectively. The comparison of the CO line intensities with photodissociation region (PDR) and X-ray dominated region (XDR) models, together with other observational constraints, such as the observed CO surface brightness and the radiation field, indicate that the best explanation for the CO excitation of the CND is an XDR with density of n(H_2) 10^4 cm^-3 and X-ray flux of 9 erg s^-1 cm^-2, consistent with illumination by the active galactic nucleus, while the CO lines in the SF-ring are better modeled by a PDR. The detected water transitions, together with those observed with the \her \sim PACS Spectrometer, can be modeled by an LVG model with low temperature (T_kin \sim 40K) and high density (n(H_2) in the range 10^6.7-10^7.9 cm^-3).Comment: Accepted for publication on the Astrophysical Journal, 30 August 201
