An adequacy of laboratory requests of vitamin D levels
Introducción: ante las nuevas implicaciones atribuidas a la vitamina D y la asociación con enfermedades tales como el cáncer, diabetes, enfermedades cardiovasculares, autoinmunes y mortalidad, no es de extrañar que se haya defendido la medición de los niveles de vitamina D en la población general. Sin embargo, no existen datos experimentales que demuestren la viabilidad y rentabilidad de la estrategia de cribado en la población y tampoco se ha comprobado la existencia de benefi cios para la salud, por lo que en la actualidad solo es aconsejable la medida de 25 (OH) vitamina D en los grupos de personas de alto riesgo como indican las guías clínicas internacionales.
Objetivos: analizar las peticiones de vitamina D comprobando si se adecuan a las guías clínicas.
Métodos: realizamos un estudio descriptivo transversal en el Área de Salud de Badajoz (España) estudiando las peticiones de determinación de vitamina D durante 12 meses consecutivos (n = 3.907). En dicho estudio revisamos el diagnóstico de petición y la historia clínica del paciente para discriminar entre peticiones que se adecuaban a las guías clínicas y peticiones injustificadas. Por último, realizamos el estudio económico.
Resultados: en nuestros resultados encontramos que casi un tercio de peticiones no se adecuaban a las guías clínicas, en patologías tales como diabetes, dislipemias e hipertensión en las que no está recomendada la medición de los niveles de vitamina D, por lo que suponía un exceso de gasto para el sistema sanitario. Gasto que se incrementa cada año, tanto es así que se ha producido un aumento en las peticiones de más del 1.000% en los últimos 6 años.
Conclusiones: concluimos la necesidad de crear protocolos de petición de vitamina D que se ajusten a las guías clínicas hasta que existan más estudios experimentales sobre las nuevas implicaciones de la vitamina D y así conseguir una correcta utilización de los recursos económicos del hospital.Introduction: Regarding the new implications assigned to vitamin D and its association with diseases such as cancer, diabetes, heart and autoimmune diseases and mortality, it isn’t surprising that the measurements of the levels of vitamin D in general population had been defended. Objectives: Analyse vitamin D petitions confi rming if they adequate to clinical guides.
Methods: We carried out a descriptive study in the health area of Badajoz (Spain) studying the petitions for vitamin D analysis for 12 consecutive months (n = 3,907), in which we checked the diagnosis requested and the medical records of the patient to select between petitions that fi t in the clinical guides, and those unjustifi ed. Lastly, we also carried out an economical study.
Results: In our results we found that almost a third of the petitions didn’t fi t the clinical guides, in pathologies like diabetes, dyslipidemia and hypertension where the measurement of the vitamin D levels is not recommended, so this would mean an enormous expense for public health. This cost it is growing every year, so it has been an increase in requests over 1,000% in the last 6 years.
Conclusions: We conclude with the idea of creating vitamin D petition protocols which fi t the clinical guides until more experimental studies about the new implications on vitamin D are published, and fi nally achieve a correct use of the economical resources of the hospital.peerReviewe
El Conjunto Mínimo Básico de Datos como herramienta de vigilancia epidemiológica de la Incidencia Diabetes Mellitus 1: Experiencia en Extremadura.
The 'gold standard' of knowledge of the epidemiological evolution of the diseases that require hospitalization for diagnosis are the medical records, although the data collection is slow, difficult and time consuming, so if another faster, easier and cheaper source of data was validated, it would be very useful for epidemiological studies. Objectives. To verify The Minimum Basic Data (CMBD) validity as a source of epidemiological vigilance, comparing the CMBD data to the collected information in the medical record. Methodology. The CMBD data compared with the records of the medical records of incident cases of type 1 diabetes mellitus (DM 1) in the population under 15 years of Extremadura during 2003-2007 in a retrospective observational study and descriptive with capture-recapture method. Results. The cases were selected from 208. In 2.40% of cases, the results do not coincide with diabetes coding. 6.73% of cases, the results in the CMBD are coded with other types of diabetes or other pathology. 20.75% of the coded cases as DM 1 in with ketoacidosis doesn´t have ketoacidosis in its history, and 69.71% the medical history matches with the CMBD. Discussion. Despite of its limitations, the CMBD has proved to be a valuable source of information. Conclusion. The CMBD serves as a source of information, but not as DM 1 in epidemiological surveillanceEl 'gold standard' del conocimiento de la evolución epidemiológica de las patologías que para su diagnóstico necesitan hospitalización, son las historias clínicas, aunque la recogida de datos de ella es lenta, dificultosa y costosa en tiempo, por lo que si validáramos otra fuente de datos que fuese más rápida, fácil y barata sería de gran utilidad para los estudios epidemiológicos. Objetivos. Verificar la validez del el Conjunto Mínimo Básico de Datos (CMBD) como fuente de vigilancia epidemiológica, comparando los datos del CMBD y los recogidos en la historia clínica. Metodología. Comparamos los datos CMBD con los registros de las historias clínicas de los casos de incidencias de la DM 1 en la población menor de 15 años de Extremadura durante 2003-2007 en un estudio observacional retrospectivo y descriptivo, con método de captura-recaptura. Resultados. Los casos seleccionados fueron de 208. En el 2,40 % de los casos no coincide con la codificación de diabetes. En un 6,73 % de los casos en el CMBD están codificados con otros tipos de diabetes u otra patología. De los codificados como DM 1 con cetoacidosis, el 20,75 % no consta cetoacidosis en la historia, y en el 69,71 % coinciden historia y el CMBD. Discusión a pesar de sus limitaciones, el CMBD ha demostrado ser una fuente de información valiosa. Conclusión El CMBD sirve como fuente de información, pero no como vigilancia de la epidemiológica DM 1
Spanish home enteral nutrition registry of the year 2014 and 2015 from the NADYA-SENPE Group
Objetivo: exponer los resultados del registro de nutrición enteral domiciliaria (NED) del año 2014 y 2015 del Grupo NADYA-SENPE.
Métodos: se recopilaron los pacientes introducidos en el registro desde el 1 de enero al 31 de diciembre de 2014 y la mismas fechas de 2015, y se procedió al
análisis descriptivo y analítico de los datos.
Resultados: en el año 2014, se registraron 3.749 pacientes y en 2015, 4.202; la prevalencia fue de 80,58 pacientes/millón de habitantes en el año 2014 y de
90,51 en 2015. Por sexos, hubo un 49,9% de mujeres en 2014 y un 50,3% en 2015. La edad media fue de 73 años (IIQ 59-83) en ambos años. Finalizaron 684
episodios de NED en 2014 y 631 en 2015, la causa principal fue el fallecimiento en el 54,9% y 50,4% de los casos, respectivamente. Los portadores de sonda
nasogástrica presentan una edad media superior a los pacientes con cualquier otra vía (p < 0,001). Se registraron 67 pacientes pediátricos en 2014 (56,7% niñas) y
77 en 2015 (55,8% niñas). La vía principal de administración fue la gastrostomía en el 52,0% de los casos de 2014 y sonda nasogástrica en el 50,8% de los casos
de 2015. La causa principal de fi nalización de la nutrición fue el fallecimiento (57,1% en 2014 y 38,5% en 2015). Se observó que los niños más pequeños eran los
que se alimentaban preferentemente por SNG (p 0,004 vs. 0,002).Tanto en pacientes pediátricos como en adultos el diagnóstico principal que motivó la necesidad
de NED fue la enfermedad neurológica que cursa con afagia o disfagia severa.
Conclusiones: se ha incrementado el número de pacientes del registro, así como el número de centros participantes y el número medio de pacientes comunicados
por cada centro respecto a años anteriores, sin que se hayan modificado sustancialmente las características de los pacientes, salvo mayor duración de los episodiosObjective: To present the results of the Spanish home enteral nutrition (HEN) registry of the NADYA-SENPE group for the years 2014 and 2015.
Methods: From January 1st 2014 to December 31st 2015 the HEN registry was recorded and afterwards a further descriptive and analytical analysis was done.
Results: In 2014, 3749 patients were recorded, and 4202 in 2015; prevalence was 80.58 patients/one million inhabitants in Spain in 2014 and 90.51 in 2015. There

other route (p-value < 0.001). Sisty-seven paediatric patients were recorded in 2014 (56.7% females) and 77 in 2015 (55.8% females). Median age at the beginning
of HEN among children was 5 months in 2014 and 5 months in 2015. The main route of administration was gastrostomy, in 52.5% in 2014 and nasogastric tube in
50.8% in 2015. 7 episodes fi nished in 2014 and 13 in 2015, having death as the main cause (57.1% in 2014 and 38.5% in 2015). It was found that were younger
children the ones who were mainly fed by nasogastric tubes (p-value 0.004 vs. 0.002). Among paediatric patients as well as adults, the main diagnosis leading to
HEN was neurological disease which gives aphagia or severe dysphagia.
Conclusions: There has been an increase in the number of patients in the registry as well as the participating centers and the number of patients per center, without
any signifi cant change in the characteristics of the patients other than longer duration of the episode
Fish Oil Enriched Intravenous Lipid Emulsions Reduce Triglyceride Levels in Non-Critically Ill Patients with TPN and Type 2 Diabetes. A Post-Hoc Analysis of the INSUPAR Study
There are no studies that have specifically assessed the role of intravenous lipid emulsions (ILE) enriched with fish oil in people with diabetes receiving total parenteral nutrition (TPN). The objective of this study was to assess the metabolic control (glycemic and lipid) and in-hospital complications that occurred in non-critically ill inpatients with TPN and type 2 diabetes with regard to the use of fish oil emulsions compared with other ILEs. We performed a post-hoc analysis of the Insulin in Parenteral Nutrition (INSUPAR) trial that included patients who started with TPN for any cause and that would predictably continue with TPN for at least five days. The study included 161 patients who started with TPN for any cause. There were 80 patients (49.7%) on fish oil enriched ILEs and 81 patients (50.3%) on other ILEs. We found significant decreases in triglyceride levels in the fish oil group compared to the other patients. We did not find any differences in glucose metabolic control: mean capillary glucose, glycemic variability, and insulin dose, except in the number of mild hypoglycemic events that was significantly higher in the fish oil group. We did not observe any differences in other metabolic, liver or infectious complications, in-hospital length of stay or mortality

El mundo vive un cambio de época caracterizado por una crisis civilizatoria
que afecta a la mayoría de las instituciones. La universidad no es
ajena a este fenómeno. Cada vez más voces coinciden en que la educación
convencional ha perdido aliento y vivacidad, por lo que se
vuelve obligatorio replantear el quehacer educativo y reconfigurar sus
propósitos.
Esta obra promueve e impulsa maneras distintas de entender e
implementar la universidad del siglo XXI. Encabezados por el gran
pensador Edgar Morin, sus autores trazan el reto de generar una metamorfosis
de la educación superior desde esquemas de complejidad y,
para ello, ofrecen un conjunto de propuestas y experiencias sobre los
caminos posibles para transformar las tareas universitarias.
Dirigidas a académicos, sociólogos y todo aquel que tenga la preocupación
intelectual de regenerar la organización de la universidad e,

Nutrición enteral domiciliaria en España: registro Nadya del año 2011-2012
Objective: To describe the results of the home enteral
nutrition (HEN) registry of the NADYA-SENPE group in
2011 and 2012.
Material and methods: We retrieved the data of the patients
recorded from January 1st 2011 to December 31st 2012.
Results: There were 3021 patients in the registry
during the period from 29 hospitals, which gives 65.39 per
million inhabitants. 97.95% were adults, 51.4% male.
Mean age was 67.64 ± 19.1, median age was 72 years for
adults and 7 months for children. Median duration with
HEN was 351 days and for 97.5% was their first event
with HEN. Most patients had HEN because of neurological
disease (57.8%). Access route was nasogastric tube
for 43.5% and gastrostomy for 33.5%. Most patients had
limited activity level and, concerning autonomy, 54.8%
needed total help. Nutritional formula was supplied from
chemist’s office to 73.8% of patients and disposables,
when necessary, was supplied from hospitals to 53.8% of
patients. HEN was finished for 1,031 patients (34.1%)
during the period of study, 56.6% due to decease and
22.2% due to recovery of oral intake.
Conclusions: Data from NADYA-SENPE registry
must be explained cautiously because it is a non-compulsory
registry. In spite of the change in the methodology of
the registry in 2010, tendencies regarding HEN have been

Material y métodos: Se recopilaron los datos introducidos
en el registro desde el 1 de enero de 2011 al 31 de diciembre

Resultados: Hubo 3021 pacientes en el registro durante
el periodo, procedentes de 29 hospitales, lo que da una
prevalencia de 65,39 casos por millón de habitantes.
97.95% fueron adultos, 51,4% varones. La edad media
fue 67,64 ± 19,1 años y la mediana 72 años para los adultos
y 7 meses para los niños. La duración media de la NED
fue 351 días y para el 97,5% fue el primer episodio con
NED. La mayoría de pacientes tenían NED por una enfermedad
neurológica (57,8%). La vía de acceso fue sonda
nasogástrica para el 43,5% y gastrostomía para el 33,5%.
La mayoría de pacientes tuvieron un nivel de actividad física
limitado y, respecto a la autonomía, 54,8% necesitaba
ayuda total. La fórmula de nutrición se suministró desde
las oficinas de farmacia para el 73,8% y los fungibles,
cuando fueron necesarios, desde los hospitales para el
53,8%. La NED se suspendió en 1.031 pacientes (34,1%)
durante el periodo de estudio, 56,6% debido a fallecimiento
y 22,2% debido a recuperación de la vía oral.
Conclusiones: Los datos del registro NADYA-SENPE
deben ser interpretados con precaución ya que se trata de
un registro voluntario. A pesar del cambio de metodología
del registro en 2010, las tendencias en NED se han mantenido,
salvo la importancia cuantitativa de la vía ora
Prognostic value of bioelectrical impedance analysis in head and neck cancer patients undergoing radiotherapy: a VALOR® study
IntroductionBioelectrical impedance analysis (BIA) serves as a method to estimate body composition. Parameters such as phase angle (PA), standardized phase angle (SPA), body mass cell (BCM), BCM index (BCMI), and fat-free mass (FFM) might significantly impact the prognosis of head and neck cancer (HNC) patients. The present study aimed to investigate whether bioelectrical parameters can be used to predict survival in the HNC population and establish the optimal cutoff points for predictive accuracy.MethodsA multicenter observational study was performed across 12 tertiary hospitals in Andalusia (a region from the south of Spain). A total of 494 patients diagnosed with HNC between 2020 and 2022 at different stages were included in this study, with a minimum follow-up period of 12 months. The BIA assessment was carried out during the first 2 weeks of radical radiotherapy treatment with chemotherapy or other systemic treatments. A multivariate logistic regression analysis of overall survival, complications, hospital admission, and palliative care and its relationship with BIA nutritional assessment was performed.ResultsSignificant prognostic factors identified in the multivariable analysis encompassed phase angle (PA), standardized phase angle (SPA), body cell mass (BCM), and BCM index (BCMI). Lower PA and BCM values were significantly associated with adverse clinical outcomes. A BCM threshold above 17 kg/m2 was the most significant predictor for predicting survival within the overall HNC population. The PA values of <5.1° in male and <4.8° in female patients showed the best predictive potential for mortality. Increased PA (as a continuous variable) demonstrated a significantly reduced risk for mortality (OR, 0.64; 95% CI, 0.43–0.94; p < 0.05) and a decreased likelihood of hospital admission (OR, 0.75; 95% CI, 0.52–1.07; p < 0.05). Higher BCM correlated with a lower risk of mortality (OR, 0.88; 95% CI, 0.80–0.96; p < 0.01) and a diminished probability of hospital admission (OR, 0.91; 95% CI, 0.83–0.99; p < 0.05).ConclusionBIA is a crucial tool in the nutritional assessment of HNC patients. BCM and PA are the main bioelectrical parameters used to predict clinical outcomes in this population. Future studies are needed to validate BIA variables in a large cohort to ensure whether early intensification of nutritional treatment would improve survival
Nutrición parenteral domiciliaria en España, 2019: informe del Grupo de Nutrición Artificial Domiciliaria y Ambulatoria NADYA
RESUMEN Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadyasenpe.com) del año 2019. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2019. Resultados: se registraron 283 pacientes (51,9 %, mujeres), 31 niños y 252 adultos procedentes de 47 hospitales españoles, lo que representa una tasa de prevalencia de 6,01 pacientes/millón de habitantes/año 2019. El diagnóstico más frecuente en los adultos fue “oncológico paliativo” y “otros” (21,0 %). En los niños fue la enfermedad de Hirschsprung junto a la enterocolitis necrotizante, las alteraciones de la motilidad intestinal y la pseudoobstrucción intestinal crónica, con 4 casos cada uno (12,9 %). El primer motivo de indicación fue el síndrome del intestino corto tanto en los niños (51,6 %) como en los adultos (37,3 %). El tipo de catéter más utilizado fue el tunelizado tanto en los niños (75,9 %) como en los adultos (40,8 %). Finalizaron 68 episodios, todos en adultos: la causa más frecuente fue el fallecimiento (54,4 %). Pasaron a la vía oral el 38,2 %. Conclusiones: el número de centros y profesionales colaboradores con el registro NADYA va incrementándose. Se mantienen estables las principales indicaciones y los motivos de finalización de la NPD

The most frequent diagnosis in adults was “palliative cancer” (22.0%), followed by “others”. In children it was Hirschsprung’s disease together with necrotizing enterocolitis, with four cases (17.4%). The first indication was short bowel syndrome in both children (60.9%) and adults (35.7%). The most frequently used type of catheter was tunneled in both children (81.0%) and adults (41.1%). Ending 75 episodes, the most frequent cause was death (52.0%) and change to oral feeding (33.3%). Conclusions: The number of centers and collaborating professionals in the registry of patients receiving HPN remains stable, as well as the main indications and reasons for termination of HPN.Objetivo: comunicar los datos de nutrición parenteral domiciliaria
