    Evaluation of stresses in a combined plane strain-simple shear test

    A biaxial testing device for sheet metal has been developed that can impose a combination of\ud plane strain and simple shear deformation. The specimen has a large width to height ratio and a small height\ud to thickness ratio. The forces in tensile and shear direction are easily measured and the tensile stress and shear\ud stress can easily be derived. For a full description of stresses, however, the stress in lateral direction should also\ud be known. This stress is a result of the constraint, imposed by the large width to height ratio and cannot be\ud measured directly. The strain in the specimen is measured on the surface. By imposing the Drucker normality\ud principle, the direction of the tangent to the yield surface is known and the unknown stress increment in lateral\ud direction can be obtained. Computer simulations are performed to test whether the intended approach can\ud recalculate all stress components from measurement of 3 in-plane strains and just 2 stresses. Without hardening,\ud good results are obtained for a complete interval between the pure shear point up to a point between uniaxial\ud stress and the plane strain point on the yield locus. With hardening, the algorithm requires a lot of data points\ud to avoid drifting from the exact solution. It is noted that, although the normality rule is used, it is not necessary\ud to have an a-priori knowledge of the yield functio

    Lessons learned from computer models on blue light therapy for psoriasis vulgaris

    Blue light irradiation has been clinically proven to reduce the symptoms of psoriasis vulgaris, a common chronic inflammatory skin disease that affects 2% - 3% of the world’s population. This dermatological condition is characterized by hyperproliferation and disturbed differentiation of keratinocytes, which is evident in lesional areas as thick flaky skin. The lesional areas also exhibit sustained inflammation, induced by immune cells, such as T cells and dendritic cells, infiltrating the affected skin. Blue light reduces the proliferation of keratinocytes and increases their differentiation in a wavelength and fluence dependent manner. Also, it induces apoptosis in T cells and suppresses the activation of dendritic cells. These effects can explain the symptom reduction after treatment. But, the efficacy shown in the clinical studies could be further improved by having a deeper understanding on the underlying mechanism of this therapeutic approach and optimizing the treatment regimens currently used. Diverse findings have been published in different studies of blue light therapy for psoriasis describing large, little or no therapeutic effect. These results may be due to variances in the main treatment parameters of the implemented protocols, i.e. fluence, intensity, and length of treatment. Computational methods can provide a suitable platform to investigate the complex interactions leading to the management of psoriasis by blue light therapy and optimize the treatment protocols. Here, we explore in silico the underlying mechanism of blue light irradiation of psoriatic skin and predict the outcome of a wide range of therapeutic regimens with varying fluence, intensity, and length of treatment. The computational model is defined by a set of ordinary differential equations describing the time evolution of keratinocytes as they move vertically through the layers of the epidermis. The results of our simulations suggest that the temporary decrease in the severity of psoriasis can be explained by the transient decline in the proliferative capacity of keratinocytes. However, it is still unclear how the effects of blue light on the immune system contribute to the reduction of psoriasis symptoms. Simulations implemented for several combinations of treatment parameters predict that high efficacy is achieved by protocols with long duration and high fluence levels, regardless of the chosen intensity. These predictions provide general guidelines for treatment. Our in silico approach constitutes a framework for testing diverse hypotheses on the underlying mechanism of blue light therapy and designing effective strategies for the treatment of psoriasis vulgaris

    Validation of the 28-joint Disease Activity Score (DAS28) and European League Against Rheumatism response criteria based on C-reactive protein against disease progression in patients with rheumatoid arthritis, and comparison with the DAS28 based on erythrocyte sedimentation rate

    Contains fulltext : 80996.pdf (publisher's version ) (Closed access)OBJECTIVE: To validate and compare the definition of the Disease Activity Score 28 based on C-reactive protein (DAS28 (CRP)) to the definition based on erythrocyte sedimentation rate (ESR). METHODS: Data were analysed from two randomised, double-blind, placebo-controlled trials of abatacept of 6-month and 12-month duration in patients with rheumatoid arthritis. European League Against Rheumatism (EULAR) response criteria and the proportion of patients in remission (DAS28 <2.6) based on the two DAS28 definitions were examined. Trends in radiographic progression (erosion score, joint space narrowing score and total score) and physical function (Health Assessment Questionnaire Disability Index (HAQ-DI)) across the EULAR responder states (none, moderate and good) were analysed. RESULTS: There was general agreement in determining the EULAR responder state using both DAS28 definitions (kappa = 0.80, 95% CI 0.76 to 0.83). Overall, there was 82.4% agreement on the EULAR response criteria; when disagreements occurred, the DAS28 (CRP) yielded a better EULAR response more often then DAS28 (ESR) (12.6% vs 4.9%, respectively). There was also agreement in determining remission: kappa = 0.69 (95% CI 0.60 to 0.78). Radiographic progression decreased in patients treated with abatacept across EULAR states (from none to moderate to good) based on both definitions. For patients treated with placebo, the trend was not as pronounced, with radiographic scores higher for moderate vs non-responders. For physical function, similar trends were observed across the EULAR states for both DAS28 definitions. CONCLUSIONS: The DAS28 (CRP) has been validated against radiographic progression and physical function. While the DAS28 (CRP) yielded a better EULAR response more often than the DAS28 (ESR), the validation profile was similar to the DAS28 (ESR), indicating that both measures are useful for assessing disease activity in patients with rheumatoid arthritis

    Towards automatic pulmonary nodule management in lung cancer screening with deep learning

    The introduction of lung cancer screening programs will produce an unprecedented amount of chest CT scans in the near future, which radiologists will have to read in order to decide on a patient follow-up strategy. According to the current guidelines, the workup of screen-detected nodules strongly relies on nodule size and nodule type. In this paper, we present a deep learning system based on multi-stream multi-scale convolutional networks, which automatically classifies all nodule types relevant for nodule workup. The system processes raw CT data containing a nodule without the need for any additional information such as nodule segmentation or nodule size and learns a representation of 3D data by analyzing an arbitrary number of 2D views of a given nodule. The deep learning system was trained with data from the Italian MILD screening trial and validated on an independent set of data from the Danish DLCST screening trial. We analyze the advantage of processing nodules at multiple scales with a multi-stream convolutional network architecture, and we show that the proposed deep learning system achieves performance at classifying nodule type that surpasses the one of classical machine learning approaches and is within the inter-observer variability among four experienced human observers.Comment: Published on Scientific Report

    Maladaptive personality traits in adolescence: Psychometric properties of the Personality Diagnostic Questionnaire-4+

    AbstractThe Personality Diagnostic Questionnaire-4+ (PDQ-4+) is a self-report used for the assessment of personality disorder traits, however, its psychometric characteristics have yet to be tested in community samples of adolescents. The main goal was to analyze the psychometric properties of the PDQ-4+ scores in a large sample of non-clinical adolescents (N=1,443; M=15.9 years; SD=1.2). The PDQ-4+ scores showed adequate psychometric properties. Reliability of the subscales, incorporating a Likert-type 5-point response format, ranged from .62 to .85. The study of the internal structure at item level revealed that the PDQ-4+ subscales were essentially one-dimensional. Analysis of the internal structure at the subscale level by means of exploratory factor analysis and exploratory structural equation modeling yielded a possible three-dimensional solution. The PDQ-4+ subscales correlated moderately with emotional and behavioural variables measured by the Strengths and Difficulties Questionnaire. The results have clear implications for the understanding of maladaptive personality traits in adolescents
