    BACKGROUND: Advanced glycation end-products play a role in diabetic vascular complications. Their optical properties allow to estimate their accumulation in tissues by measuring the skin autofluorescence (SAF). We searched for an association between SAF and major adverse cardiovascular events (MACE) incidence in subjects with Type 1 Diabetes (T1D) during a 7 year follow-up. METHODS: During year 2009, 232 subjects with T1D were included. SAF measurement, clinical [age, sex, body mass index (BMI), comorbidities] and biological data (HbA1C, blood lipids, renal parameters) were recorded. MACE (myocardial infarction, stroke, lower extremity amputation or a revascularization procedure) were registered at visits in the center or by phone call to general practitioners until 2016. RESULTS: The participants were mainly men (59.5%), 51.5 +/- 16.7 years old, with BMI 25.0 +/- 4.1 kg/m(2), diabetes duration 21.5 +/- 13.6 years, HbA1C 7.6 +/- 1.1%. LDL cholesterol was 1.04 +/- 0.29 g/L, estimated Glomerular Filtration Rates (CKD-EPI): 86.3 +/- 26.6 ml/min/1.73 m(2). Among these subjects, 25.1% were smokers, 45.3% had arterial hypertension, 15.9% had elevated AER (>/= 30 mg/24 h), and 9.9% subjects had a history of previous MACE. From 2009 to 2016, 22 patients had at least one new MACE: 6 myocardial infarctions, 1 lower limb amputation, 15 revascularization procedures. Their SAF was 2.63 +/- 0.73 arbitrary units (AU) vs 2.08 +/- 0.54 for other patients (p = 0.002). Using Cox-model, after adjustment for age (as the scale time), sex, diabetes duration, BMI, hypertension, smoking status, albumin excretion rates, statin treatment and a previous history of MACE, higher baseline levels of SAF were significantly associated with an increased risk of MACE during follow-up (HR = 4.13 [1.30-13.07]; p = 0.02 for 1 AU of SAF) and Kaplan-Meier curve follow-up showed significantly more frequent MACE in group with SAF upper the median (p = 0.001). CONCLUSION: A high SAF predicts MACE in patients with T1D

    Biomechanical analysis of sutures used for mesh fixation in the donor area after removal of the rectus abdominis muscle

    Background: The use of synthetic mesh for abdominal wall closure after removal of the rectus abdominis is established but not standardised. This study compares two forms of mesh fixation: a simple suture, which fixes the mesh to the edges of the defect on the anterior rectus abdominis fascia; and total fixation, which incorporates the fasciae of the internal oblique, external oblique and transverse muscles in the suture, anchoring the mesh in the position of the removed muscle. Method: A total of 16 fresh cadavers were dissected. Two sutures were compared: simple and total. Three different sites were analysed: 5 cm above, 5 cm below and at the level of the umbilicus. The two sutures compared were tested in each region using a standardised technique. All sutures were performed with nylon 0, perpendicular to the linea alba. Each suture was secured to a dynamometer, which was pulled perpendicularly towards the midline until the rupture of the aponeurosis. `Rupture resistance` was measured in kilogram force. The mean among the groups was compared using the paired Student`s t-test to a significance level of 1% (p < 0.01). Results: The mean rupture resistance of the total suture was 160% higher than that of the simple suture. Conclusion: The total suture includes the external oblique, internal oblique and transverse fasciae, which are multi-directional, and creates a much higher resistance when compared with the simple suture. Total suture may reduce the incidence of bulging and hernias of the abdominal wall after harvesting the rectus abdominis muscle, but comparative clinical studies are necessary.     Evaluating the terrestrial carbon dioxide removal potential of improved forest management and accelerated forest conversion in Norway

    As a carbon dioxide removal measure, the Norwegian government is currently considering a policy of large-scale planting of spruce (Picea abies (L) H. Karst) on lands in various states of natural transition to a forest dominated by deciduous broadleaved tree species. Given the aspiration to bring emissions on balance with removals in the latter half of the 21st century in effort to limit the global mean temperature rise to “well below” 2°C, the effectiveness of such a policy is unclear given relatively low spruce growth rates in the region. Further convoluting the picture is the magnitude and relevance of surface albedo changes linked to such projects, which typically counteract the benefits of an enhanced forest CO2 sink in high-latitude regions. Here, we carry out a rigorous empirically based assessment of the terrestrial carbon dioxide removal (tCDR) potential of large-scale spruce planting in Norway, taking into account transient developments in both terrestrial carbon sinks and surface albedo over the 21st century and beyond. We find that surface albedo changes would likely play a negligible role in counteracting tCDR, yet given low forest growth rates in the region, notable tCDR benefits from such projects would not be realized until the second half of the 21st century, with maximum benefits occurring even later around 2150. We estimate Norway's total accumulated tCDR potential at 2100 and 2150 (including surface albedo changes) to be 447 (±240) and 852 (±295) Mt CO2-eq. at mean net present values of US12(±3)andUS 12 (±3) and US 13 (±2) per ton CDR, respectively. For perspective, the accumulated tCDR potential at 2100 represents around 8 years of Norway's total current annual production-based (i.e., territorial) CO2-eq. emissions.publishedVersio
