This report summarizes the results of a partial evaluation of the 1988-89 Drug Abuse Resistance Education (DARE) effort implemented in all sixth grades in the Nashville, Tennessee, Metropolitan Public School system by trained instructors from the Nashville Metropolitan Police Department. As documented in the following pages, the evaluation effort had two major components. 1. An evaluation of the program by teachers and principals in all the participating schools. 2. Inner-City Study, an investigation designed to assess the impact of the program in elementary and middle schools that service those students thought to be most at risk to the threat of drugs. Tujuan penelitian ini untuk mengetahui pengaruh secara bersama kualitas pelayanan, promosi, dan after sales service terhadap kepuasan konsumen sepeda motor Honda PT. Nusantara Surya Sakti di Amurang.Metode penelitian yang digunakan asosiatif dengan teknik analisis regresi linear berganda.Sampel yang digunakan 100 responden dari populasi 998 pada tahun 2013.Hasil penelitian menunjukkankualitas pelayanan, promosi, dan after sales service berpengaruh signifikan secara bersama terhadap kepuasan pelanggan/konsumen namun secara parsial after sales service tidak berpengaruh dan tidak signifikan.Saran bagi manajemen PT. Nusantara Surya Sakti di Amurang, sebaiknya manajemen Perusahaan mempertahankan dan meningkatkan kualitas pelayanan sesudah pembelian (after sales service) agar pelanggan merasa puas. Importance of decision support implementation in emergency department vancomycin dosing

INTRODUCTION: The emergency department (ED) plays a critical role in the management of life-threatening infection. Prior data suggest that ED vancomycin dosing is frequently inappropriate. The objective is to assess the impact of an electronic medical record (EMR) intervention designed to improve vancomycin dosing accuracy, on vancomycin dosing and clinical outcomes in critically ill ED patients. METHODS: Retrospective before-after cohort study of all patients (n=278) treated with vancomycin in a 60,000-visit Midwestern academic ED (March 2008 and April 2011) and admitted to an intensive care unit. The primary outcome was the proportion of vancomycin doses defined as “appropriate” based on recorded actual body weight. We also evaluated secondary outcomes of mortality and length of stay. RESULTS: The EMR dose calculation tool was associated with an increase in mean vancomycin dose ([14.1±5.0] vs. [16.5±5.7] mg/kg, p<0.001) and a 10.3% absolute improvement in first-dose appropriateness (34.3% vs. 24.0%, p=0.07). After controlling for age, gender, methicillin-resistant staphylococcus aureus infection, and Acute Physiology and Chronic Health Evaluation II score, 28-day in-hospital mortality (odds ratio OR 1.72; 95% CI [0.76–3.88], p=0.12) was not affected. CONCLUSION: A computerized decision-support tool is associated with an increase in mean vancomycin dose in critically ill ED patients, but not with a statistically significant increase in therapeutic vancomycin doses. The impact of decision-support tools should be further explored to optimize compliance with accepted antibiotic guidelines and to potentially affect clinical outcome

Evaluation and comparison of native and recombinant LipL21 protein-based ELISAs for diagnosis of bovine leptospirosis

A 21-kDa leptospiral lipoprotein (LipL21) was evaluated for its diagnostic potential to detect bovine leptospirosis by ELISA. Both native LipL21 (nLipL21) and recombinant LipL21 (rLipL21) proteins were tested and compared regarding diagnostic efficiency, and no statistically significant difference was observed. The sensitivity of rLipL21 ELISA for 62 microscopic agglutination test (MAT) positive sera was 100% and the specificity with 378 MAT negative sera was 97.09%. Thus, rLipL21 protein-based ELISA could be used as an alternative to MAT for the diagnosis of bovine leptospirosis

Leptospira seroprevalence among ugandan slaughter cattle: comparison of sero-status with renal Leptospira infection

Despite evidence of both human and animal; Leptospira; exposures in Uganda, the epidemiology of the disease is still not well-investigated. Contact with animals and their environments have been pointed out as potential source of infection with; Leptospira; species in humans; and cattle may be an important reservoir in Uganda. In this cross-sectional study, we estimated the prevalence of anti-; Leptospira; antibodies by the standard microscopic agglutination test (MAT); and associated risk factors among slaughtered cattle. We also compared the performance of the MAT used in this study against a; lipL32; based real time PCR (qPCR) assay previously conducted on the kidneys and urine of the same slaughter cattle as tested in this reported study. Of 500 cattle sampled, 27.8% (95% CI 23.9-32.0) tested positive (titer ≥ 100) to at least one; Leptospira; serovar, with the majority of seropositive cattle reacting to serovars Tarassovi (sg Tarassovi) (11.6%), Sejroe (Sg Sejroe) (7.8%), and Australis (Sg Australis) (5.2%). Older animals had 2.8 times (95% CI 1.0-8.2,; p; -value 0.055) greater odds of being seropositive than younger ones (<1.5 years). The sensitivity and specificity of the MAT over the qPCR were 65.9% (95% CI 50.1-79.5) and 75.9% (95% CI 71.7-79.7), respectively; with a negative predictive value of 95.8% and positive predictive value of 20.9%. In conclusion, slaughter cattle in this study were significantly exposed to pathogenic; Leptospira; species of mainly the Tarassovi, Sejroe, and Australis serogroups, with seroprevalence being higher among older cattle. The high specificity and negative predictive value of MAT as used in this study when compared to the qPCR assay may imply a rather strong association between seronegativity and absence of renal; Leptospira; infection. However, MAT predictability for renal; Leptospira; infection may be interpreted cautiously since predictive values of diagnostic tests are dependent on prevalence
