    D2 receptor occupancy of olanzapine pamoate depot using positron emission tomography : an open-label study in patients with schizophrenia

    A long-acting depot formulation of olanzapine that sustains plasma olanzapine concentrations for over a month after a single injection is currently under development. This multicenter, open-label study explored D2 receptor occupancy of a fixed dose of olanzapine pamoate (OP) depot given every 4 weeks. Patients (nine male, five female) with schizophrenia or schizoaffective disorder previously stabilized on oral olanzapine were switched to OP depot 300 mg by intramuscular injection every 4 weeks for 6 months. No visitwise within-group significant changes were found in Brief Psychiatric Rating Scale Total or Clinical Global Impressions-Severity of Illness scores, although seven patients received oral olanzapine supplementation during the first four injection cycles. To minimize impact on D2 occupancy, positron emission tomography (PET) scans were not completed during injection cycles that required supplemental oral olanzapine. Two patients reported transient injection site adverse events, which did not result in discontinuation. The most frequently reported treatment-emergent adverse events were insomnia, aggravated psychosis, and anxiety. Mean striatal D2 receptor occupancy, as measured by [11C]-raclopride PET, was 69% on oral olanzapine (5–20 mg/day) and 50% (trough) on OP depot at steady state. Following an initial decline, occupancy returned to 84% of baseline oral olanzapine occupancy after six injections. Over the study period, D2 receptor occupancy and plasma olanzapine concentrations were significantly correlated (r=0.76, Pless than or equal to0.001). OP depot resulted in mean D2 receptor occupancy of approximately 60% or higher at the end of the 6-month study period, a level consistent with antipsychotic efficacy and found during treatment with oral olanzapine. However, supplemental oral olanzapine or another dosing strategy may be necessary to maintain adequate therapeutic response during the first few injection cycles.peer-reviewe

    Safety and efficacy of diaphragm pacing in patients with respiratory insufficiency due to amyotrophic lateral sclerosis (DiPALS)

    Background: Non-invasive ventilation is part of the standard of care for treatment of respiratory failure in patients with amyotrophic lateral sclerosis (ALS). The NeuRx RA/4 Diaphragm Pacing System has received Humanitarian Device Exemption approval from the US Food and Drug Administration for treatment of respiratory failure in patients with ALS. We aimed to establish the safety and efficacy of diaphragm pacing with this system in patients with respiratory muscle weakness due to ALS. Methods: We undertook a multicentre, open-label, randomised controlled trial at seven specialist ALS and respiratory centres in the UK. Eligible participants were aged 18 years or older with laboratory supported probable, clinically probable, or clinically definite ALS; stable riluzole treatment for at least 30 days; and respiratory insufficiency. We randomly assigned participants (1:1), via a centralised web-based randomisation system with minimisation that balanced patients for age, sex, forced vital capacity, and bulbar function, to receive either non-invasive ventilation plus pacing with the NeuRx RA/4 Diaphragm Pacing System or non-invasive ventilation alone. Patients, carers, and outcome assessors were not masked to treatment allocation. The primary outcome was overall survival, defined as the time from randomisation to death from any cause. Analysis was by intention to treat. This trial is registered, ISRCTN number 53817913. Findings: Between Dec 5, 2011, and Dec 18, 2013, we randomly assigned 74 participants to receive either non-invasive ventilation alone (n=37) or non-invasive ventilation plus diaphragm pacing (n=37). On Dec 18, 2013, the Data Monitoring and Ethics Committee (DMEC) recommended suspension of recruitment on the basis of overall survival figures. Randomly assigned participants continued as per the study protocol until June 23, 2014, when the DMEC advised discontinuation of pacing in all patients. Follow-up assessments continued until the planned end of the study in December, 2014. Survival was shorter in the non-invasive ventilation plus pacing group than in the non-invasive ventilation alone group (median 11·0 months [95% CI 8·3-13·6] vs 22·5 months [13·6-not reached]; adjusted hazard ratio 2·27, 95% CI 1·22-4·25; p=0·009). 28 (76%) patients died in the pacing group and 19 (51%) patients died in the non-invasive ventilation alone group. We recorded 162 adverse events (5·9 events per person-year) in the pacing group, of which 46 events were serious, compared with 81 events (2·5 events per person-year) in the non-invasive ventilation alone group, of which 31 events were serious. Interpretation: Addition of diaphragm pacing to standard care with non-invasive ventilation was associated with decreased survival in patients with ALS. Our results suggest that diaphragmatic pacing should not be used as a routine treatment for patients with ALS in respiratory failure.     A Performer\u27s Guide to the Songs of Arthur Honegger (1892-1955).

    This monograph describes the interpretive and performance problems posed by Honegger\u27s 48 melodies. Written over a thirty-year period, this repertoire constitutes a major contribution to the genre, and as such stands alongside better known works of Milhaud and Poulenc. Although a wealth of performance material is currently available, Honegger\u27s melodies raise a variety of issues, ranging from details of pronunciation and translation to wider problems of interpretation: why did he choose the texts he did; how did he set these texts; what is the relationship between the form of the music and the form of text; and how does the music enhance poetic images. The study has four chapters. The first chapter surveys the four main phases in Honegger\u27s career, the place of melodies within these phases, and Honegger\u27s general approach to song composition. Once the background has been sketched, Chapter 2 deals with melodies written during the first phase of Honegger\u27s career (1914-1920). Next, Chapter 3 discusses melodies from the 1920s, when Honegger\u27s career took off. Finally, Chapter 4 examines Honegger\u27s late melodies; and it shows how he consolidated his compositional style in the 1940s. The paper concludes with three appendixes: a song catalog, which gives place and date of publication, information about dedications and first performances, as well as ranges, tessituras, and moods; translations of the song texts, which provide exact, word-by-word equivalents; a phonetic transcription of the song texts, which details the appropriate pronunciation

    Using Monuments to Teach About Racism, Colonialism, and Sexism

    This chapter examines how an interdisciplinary high-impact practice approach to teaching and learning using selected contested monuments can reveal intersections of racism, colonialism, and sexism, and lay the foundation for students’ civic engagement. In place-based and virtual experiences, students observe and investigate local and national monuments, integrating knowledge from multiple disciplines, including history, psychology, art, culture, and tourism. Students make critical analyses about how monuments reveal power relationships in our society. Students from various disciplines explore the origin of contested monuments, the evolving national and local debates around them, and their effect on students’ learning to evaluate historical, contemporary, and cultural perspectives while developing their own supported opinions to nurture civic engagement and lifelong learning
