    Fragmentation of care during pregnancy in the Volta Region of Ghana: A social network analysis approach

    To determine the extent of care fragmentation during pregnancy and delivery in the Volta Region of Ghana. The National Health Insurance Claims Data for the Volta Region for the period January –December 2013 was used. Pregnant women who delivered at a health facility and made at least 3 visits were included in the study. Social network analysis (SNA) approach was used to determine care fragmentation. Fragmentation of care during delivery was defined to be any delivery at a facility different from the woman’s most frequently visited antenatal care (ANC) facility. Network metrics (weighted in-degree and weighted out-degree) were used to determine extent of care fragmentation and also the key facilities contributing to the fragmentation. Overall, 14,474 pregnant women were included in the study with 15% (2,185) having cesarean section (CS). A total of 6,025 (42%) of all the pregnant women visited more than one facility during ANC and delivery, out of which 960 (16%) had CS. About 26% (3,769) of all deliveries and 32% (696) of all CS deliveries were fragmented. Fragmentation among those that had CS was significantly higher compared to those that had vaginal delivery (VD) (32% versus 25%, 2=45.88, p<0.001). Among those who visited multiple facilities, 63% (73% CS and 61% VD, 2=49.22, p<0.001) were fragmented. In addition, 15% of all deliveries (36% among those who visited multiple facilities) and 20% of all CS deliveries (45% among those who visited multiple facilities) were performed at facilities that the pregnant women never received ANC services from. There is high level of care fragmentation during the critical period of delivery among pregnant women who visited more than one facility. This fragmentation is particularly higher among those that had CS compared to vaginal delivery. This calls for policy to ensure coordination and continuity of care during pregnancy

    Artikel ini mengusulkan model pengukuran kinerja (pemetaan) kepemimpinan yang didasarkan kepada keempat sifat Rasulullah SAW, yaitu shiddiq, tabligh, amanah, dan fathanah. Studi kasus yang digunakan adalah implementasi kepemimpinan terhadap tiga kepala daerah tingkat Kotamadya/Kabupaten Provinsi Jawa Barat. Proses kajian diawali dengan mengidentifikasi variabel atau kriteria beserta satuan ukurannya dari masing-masing sifat Rasulullah. Selanjutnya, dilakukan survei di daerah yang menjadi target kajian, yaitu Kab-kota A, B, dan C. Untuk pemetaan, digunakan konsep perhitungan yang dikembangkan oleh tim Technology Atlas Project-Unitet Nation dan akhirnya dapat dihitung Indeks Kepemimpinan (IK) untuk setiap pimpinan di daerahnya masing-masing. IK tersebut menggambarkan tingkat kepemimpinan relatif menurut pandangan masyarakat, apakah sudah sesuai dengan sifat kepemimpinan yang telah dicontohkan oleh Rasulullah

    Spin transport theory in ferromagnet/semiconductor systems with non-collinear magnetization configurations

    We present a comprehensive theory of spin transport in a non-degenerate semiconductor that is in contact with multiple ferromagnetic terminals. The spin dynamics in the semiconductor is studied during a perturbation of a general, non-collinear magnetization configuration and a method is shown to identify the various configurations from current signals. The conventional Landauer-B\"{u}ttiker description for spin transport across Schottky contacts is generalized by the use of a non-linearized I-V relation, and it is extended by taking into account non-coherent transport mechanisms. The theory is used to analyze a three terminal lateral structure where a significant difference in the spin accumulation profile is found when comparing the results of this model with the conventional model.Comment: 17 pages, 10 figure

    UBE2A and UBE2B are recruited by an atypical E3 ligase module in UBR4

    UBR4 is a 574 kDa E3 ligase (E3) of the N-degron pathway with roles in neurodevelopment, age-associated muscular atrophy and cancer. The catalytic module that carries out ubiquitin (Ub) transfer remains unknown. Here we identify and characterize a distinct E3 module within human UBR4 consisting of a ‘hemiRING’ zinc finger, a helical-rich UBR zinc-finger interacting (UZI) subdomain, and an N-terminal region that can serve as an affinity factor for the E2 conjugating enzyme (E2). The structure of an E2–E3 complex provides atomic-level insight into the specificity determinants of the hemiRING toward the cognate E2s UBE2A/UBE2B. Via an allosteric mechanism, the UZI subdomain modestly activates the Ub-loaded E2 (E2∼Ub). We propose attenuated activation is complemented by the intrinsically high lysine reactivity of UBE2A, and their cooperation imparts a reactivity profile important for substrate specificity and optimal degradation kinetics. These findings reveal the mechanistic underpinnings of a neuronal N-degron E3, its specific recruitment of UBE2A, and highlight the underappreciated architectural diversity of cross-brace domains with Ub E3 activity.</p

    Robust isothermal electric switching of interface magnetization: A route to voltage-controlled spintronics

    Roughness-insensitive and electrically controllable magnetization at the (0001) surface of antiferromagnetic chromia is observed using magnetometry and spin-resolved photoemission measurements and explained by the interplay of surface termination and magnetic ordering. Further, this surface in placed in proximity with a ferromagnetic Co/Pd multilayer film. Exchange coupling across the interface between chromia and Co/Pd induces an electrically controllable exchange bias in the Co/Pd film, which enables a reversible isothermal (at room temperature) shift of the global magnetic hysteresis loop of the Co/Pd film along the magnetic field axis between negative and positive values. These results reveal the potential of magnetoelectric chromia for spintronic applications requiring non-volatile electric control of magnetization.Comment: Single PDF file: 27 pages, 6 figures; version of 12/30/09; submitted to Nature Material

    Political hashtag publics and counter-visuality: a case study of #fertilityday in Italy

    In 2016 the Italian health ministry launched the ‘Fertility Day’ campaign, aimed at tackling Italy’s low birth rate. Under the accusation of delivering sexist and racist messages, the campaign became a trending topic on Twitter, and a protest was launched to be held during Fertility Day. By applying a combination of digital methods and visual content analysis to the #fertilityday Twitter stream, this paper contributes to existing research on the deliberative strength of political hashtag publics, with a particular focus on their power structures, communication patterns and visual content use. Findings on gatekeeping dynamics downsize optimistic views on the democratising potential of Twitter’s socio-technical infrastructure as they point to the emergence of online satirical media and ‘tweetstars’ – along with mainstream news media– as main producers of spreadable content, with ordinary users only surfacing when traditional media elites and new satirical actors lack or lose interest in the debate. Results confirm that political hashtag publics follow acute event communication patterns, with users highly engaged in retweeting and referencing external material and visual content playing a key role in these gatewatching practices. The transient counter-visuality – or critical stance – of tweets with user-manipulated images, however, also suggests that the deliberative potential of these publics is not easily sustainable over time

    Higgs lepton flavour violation: UV completions and connection to neutrino masses

    We study lepton violating Higgs (HLFV) decays, first from the effective field theory (EFT) point of view, and then analysing the different high-energy realizations of the operators of the EFT, highlighting the most promising models. We argue why two Higgs doublet models can have a BR(h → τ μ) ∼ 0.01, and why this rate is suppressed in all other realizations including vector-like leptons. We further discuss HLFV in the context of neutrino mass models: in most cases it is generated at one loop giving always BR(h → τ μ) < 10−4 and typically much less, which is beyond experimental reach. However, both the Zee model and extended left-right symmetric models contain extra SU(2) doublets coupled to leptons and could in principle account for the observed excess, with interesting connections between HLFV and neutrino parameters
