    Role of ultrasonography in the management of twin gestation

    Twins represent 1%–2% of all pregnancies, yet continue to account for a disproportionate share of neonatal adverse events including neonatal intensive care admission, morbidity, and mortality. Ultrasonography is central to the proper diagnosis of the type of twinning. Ideally, ultrasonography is performed before 14 weeks of gestation to determine chorionicity and amnionicity. Correct identification of the chorionicity in a twin pregnancy facilitates proper counseling and management of the gestation, including ultrasonography follow‐up.     Surveillance of fetal lung lesions using the congenital pulmonary airway malformation volume ratio: natural history and outcomes

    ObjectivesThe congenital pulmonary airway malformation volume ratio (CVR) is a widely used sonographic measure of relative mass size in fetuses with lung malformations. The purposes of this study were to examine serial CVR measurements to understand longitudinal growth patterns and to determine correlation with postnatal imaging.MethodsAn institutional review boardĂą approved retrospective review was performed on fetuses referred for an echogenic lung malformation between 2002 and 2014. For each fetus, the CVR was prospectively calculated using 2D ultrasound and followed with advancing gestation.ResultsBased on 40 fetuses, the mean initial CVR was 0.51Ăą 屝 0.07 at 20.5Ăą 屝 0.3Ăą weeks of gestation. The CVR increased after 24Ăą weeks of gestation (pĂą =Ăą 0.0014), peaking at a CVR of 0.96Ăą 屝 0.11 at 25.5Ăą 屝 0.05Ăą weeks, followed by a significant decrease in the CVR to 0.43Ăą 屝 0.07 prior to term (pĂą <Ăą 0.0001). However, approximately one third showed no appreciable increase in size. The mean CVR was significantly correlated with postnatal chest computed tomography (CT) size dimensions (pĂą =Ăą 0.0032) and likelihood for lung resection (pĂą =Ăą 0.0055).ConclusionsFetal lung malformations tend to follow one of two distinct growth patterns, characterized by either (1) a maximal CVR between 25 and 26Ăą weeks of gestation or (2) minimal change in relative growth.     Reflections on undertaking the Probation Qualifying Framework scheme during the transforming rehabilitation changes

    This article reflects upon the author’s experience of undertaking the PQF (Probation Qualifying Framework) training scheme during the chaotic period of Transforming Rehabilitation. The author asserts that the uncertainty and precarious nature of the changes were detrimental to an effective learning environment, which ultimately promoted a practice culture of punitiveness and control and did not allow learners the space to be skilful and confident practitioners, comfortable working autonomously. Furthermore, the author contends there is an emerging culture within the NPS (National Probation Service) increasingly fostered on ‘risk management’, which is reflected in the vocational nature of PQF training and is contributing towards a widening cultural gap that is emerging between the community rehabilitation companies and NPS

    The ideology and discourse of the English Defence League: ‘Not racist, not violent, just no longer silent’

    The English Defence League (EDL) emerged in 2009 and quickly became a major ‘anti-Islamist’ street protest movement, able to attract thousands to its national demonstrations. Despite the violence and anti-Muslim rhetoric associated with its protests, the group claims to be an anti-racist human rights organisation dedicated to protecting liberal freedoms. This article employs a critical methodology to address these claims, analysing EDL literature alongside strategies identified as typical of racist discourse construction. The representations, narratives and rhetorical strategies used by the group support the analysis of EDL Islamophobia as a form of cultural racism that constructs opposing ‘British’ and ‘Muslim’ subjects and functions to maintain traditional ethno-cultural dominance of the former over the latter

    A Study of Time-Dependent CP-Violating Asymmetries and Flavor Oscillations in Neutral B Decays at the Upsilon(4S)

    We present a measurement of time-dependent CP-violating asymmetries in neutral B meson decays collected with the BABAR detector at the PEP-II asymmetric-energy B Factory at the Stanford Linear Accelerator Center. The data sample consists of 29.7 fb−1{\rm fb}^{-1} recorded at the ΄(4S)\Upsilon(4S) resonance and 3.9 fb−1{\rm fb}^{-1} off-resonance. One of the neutral B mesons, which are produced in pairs at the ΄(4S)\Upsilon(4S), is fully reconstructed in the CP decay modes J/ψKS0J/\psi K^0_S, ψ(2S)KS0\psi(2S) K^0_S, χc1KS0\chi_{c1} K^0_S, J/ψK∗0J/\psi K^{*0} (K∗0→KS0π0K^{*0}\to K^0_S\pi^0) and J/ψKL0J/\psi K^0_L, or in flavor-eigenstate modes involving D(∗)π/ρ/a1D^{(*)}\pi/\rho/a_1 and J/ψK∗0J/\psi K^{*0} (K∗0→K+π−K^{*0}\to K^+\pi^-). The flavor of the other neutral B meson is tagged at the time of its decay, mainly with the charge of identified leptons and kaons. The proper time elapsed between the decays is determined by measuring the distance between the decay vertices. A maximum-likelihood fit to this flavor eigenstate sample finds Δmd=0.516±0.016(stat)±0.010(syst)ps−1\Delta m_d = 0.516\pm 0.016 {\rm (stat)} \pm 0.010 {\rm (syst)} {\rm ps}^{-1}. The value of the asymmetry amplitude sin⁥2ÎČ\sin2\beta is determined from a simultaneous maximum-likelihood fit to the time-difference distribution of the flavor-eigenstate sample and about 642 tagged B0B^0 decays in the CP-eigenstate modes. We find sin⁥2ÎČ=0.59±0.14(stat)±0.05(syst)\sin2\beta=0.59\pm 0.14 {\rm (stat)} \pm 0.05 {\rm (syst)}, demonstrating that CP violation exists in the neutral B meson system.     Measurement of the Branching Fraction for B- --> D0 K*-

    We present a measurement of the branching fraction for the decay B- --> D0 K*- using a sample of approximately 86 million BBbar pairs collected by the BaBar detector from e+e- collisions near the Y(4S) resonance. The D0 is detected through its decays to K- pi+, K- pi+ pi0 and K- pi+ pi- pi+, and the K*- through its decay to K0S pi-.
