    Transparency in government institutions: a literature review

    This paper presents a literature review on the issue of transparency in government institutions. The review is carried out through a bibliometric survey of the Scopus database, brazilian government websites, and brazilian legislation. The review explores and evaluates government transparency as it pertains to Brazilian as well as international spheres. An analysis of the articles (from 2011 to 2012) found in the Scopus database using the keyword “transparency,” uncovered the following recurring themes: open government, e-government, government accountability, and communication. The theoretical scope was constructed from these issues, plus the more relevant Brazilian legislation as well as issues encountered on websites focusing on international governmental institutions. Thus, it was possible to construct a theoretical framework that should guide the development of future researches

    Functional anatomy of the sternoglossus muscle of the tamandua tetradactyla (linnaeus, 1758)

    The lesser anteater (Tamandua tetradactyla) (Linnaeus, 1758) presents a feed mechanism triggered thanks to the synergistic activity of the extrinsic muscles related to it, with the example sternoglossus muscle. This paper proposes characterizing this muscle, considering the origins and insertions, and discussing the results compared to similar species studied. Five anteaters donated by ICMBio were used. The samples were subjected to dissection procedures for access to the thoracic cavity and visualization of the muscle. The sternoglossus muscle of the lesser anteater originates on the side of the xiphoid process and continues with two bundles of muscles in the cranial direction that join near the base of the tongue, as part of its constitution. During the course of the muscle there is no connection to the hyoid bone and adjacent structures, connecting the cranium to the tongue, which justifies the nomenclature sternoglossus rather than sternohyoglossus. Thus, the combined muscle characteristics demonstrate a specific aspect within the analyzed species and highlight the main function performed by the muscles to move the tongue out of the oral cavity. Therefore, the absence of this muscle makes impossible the animal feed, since its directly related to the seizure of food through tongue

    Health-related quality of life in patients with type 1 diabetes mellitus in the different geographical regions of Brazil : data from the Brazilian Type 1 Diabetes Study Group

    Background: In type 1 diabetes mellitus (T1DM) management, enhancing health-related quality of life (HRQoL) is as important as good metabolic control and prevention of secondary complications. This study aims to evaluate possible regional differences in HRQoL, demographic features and clinical characteristics of patients with T1DM in Brazil, a country of continental proportions, as well as investigate which variables could influence the HRQoL of these individuals and contribute to these regional disparities. Methods: This was a retrospective, cross-sectional, multicenter study performed by the Brazilian Type 1 Diabetes Study Group (BrazDiab1SG), by analyzing EuroQol scores from 3005 participants with T1DM, in 28 public clinics, among all geographical regions of Brazil. Data on demography, economic status, chronic complications, glycemic control and lipid profile were also collected. Results: We have found that the North-Northeast region presents a higher index in the assessment of the overall health status (EQ-VAS) compared to the Southeast (74.6 ± 30 and 70.4 ± 19, respectively; p < 0.05). In addition, North- Northeast presented a lower frequency of self-reported anxiety-depression compared to all regions of the country (North-Northeast: 1.53 ± 0.6; Southeast: 1.65 ± 0.7; South: 1.72 ± 0.7; Midwest: 1.67 ± 0.7; p < 0.05). These findings could not be entirely explained by the HbA1c levels or the other variables examined. Conclusions: Our study points to the existence of additional factors not yet evaluated that could be determinant in the HRQoL of people with T1DM and contribute to these regional disparities

    Determinants of intensive insulin therapeutic regimens in patients with type 1 diabetes: data from a nationwide multicenter survey in Brazil

    Background: To evaluate the determinants of intensive insulin regimens (ITs) in patients with type 1 diabetes (T1D).Methods: This multicenter study was conducted between December 2008 and December 2010 in 28 public clinics in 20 Brazilian cities. Data were obtained from 3,591 patients (56.0% female, 57.1% Caucasian). Insulin regimens were classified as follows: group 1, conventional therapy (CT) (intermediate human insulin, one to two injections daily); group 2 (three or more insulin injections of intermediate plus regular human insulin); group 3 (three or more insulin injections of intermediate human insulin plus short-acting insulin analogues); group 4, basal-bolus (one or two insulin injections of long-acting plus short-acting insulin analogues or regular insulin); and group 5, basal-bolus with continuous subcutaneous insulin infusion (CSII). Groups 2 to 5 were considered IT groups.Results: We obtained complete data from 2,961 patients. Combined intermediate plus regular human insulin was the most used therapeutic regimen. CSII was used by 37 (1.2%) patients and IT by 2,669 (90.2%) patients. More patients on IT performed self-monitoring of blood glucose and were treated at the tertiary care level compared to CT patients (p < 0.001). the majority of patients from all groups had HbA1c levels above the target. Overweight or obesity was not associated with insulin regimen.     Benthic estuarine communities in Brazil: moving forward to long term studies to assess climate change impacts

    Abstract Estuaries are unique coastal ecosystems that sustain and provide essential ecological services for mankind. Estuarine ecosystems include a variety of habitats with their own sediment-fauna dynamics, all of them globally undergoing alteration or threatened by human activities. Mangrove forests, saltmarshes, tidal flats and other confined estuarine systems are under increasing stress due to human activities leading to habitat and species loss. Combined changes in estuarine hydromorphology and in climate pose severe threats to estuarine ecosystems on a global scale. The ReBentos network is the first integrated attempt in Brazil to monitor estuarine changes in the long term to detect and assess the effects of global warming. This paper is an initial effort of ReBentos to review current knowledge on benthic estuarine ecology in Brazil. We herein present and synthesize all published work on Brazilian estuaries that has focused on the description of benthic communities and related ecological processes. We then use current data on Brazilian estuaries and present recommendations for future studies to address climate change effects, suggesting trends for possible future research and stressing the need for long-term datasets and international partnerships

    Omecamtiv mecarbil in chronic heart failure with reduced ejection fraction, GALACTIC‐HF: baseline characteristics and comparison with contemporary clinical trials

    Aims: The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials. Methods and Results: Adults with established HFrEF, New York Heart Association functional class (NYHA) ≄ II, EF ≀35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure &lt; 100 mmHg (n = 1127), estimated glomerular filtration rate &lt; 30 mL/min/1.73 m2 (n = 528), and treated with sacubitril‐valsartan at baseline (n = 1594). Conclusions: GALACTIC‐HF enrolled a well‐treated, high‐risk population from both inpatient and outpatient settings, which will provide a definitive evaluation of the efficacy and safety of this novel therapy, as well as informing its potential future implementation

Para tanto, procuraremos efetuar um estudo sistemĂĄtico de trĂȘs dimensĂ”es do processo decisĂłrio e das relaçÔes entre o Executivo e o Legislativo no perĂ­odo: (1) um estudo de caso da elaboração dos dispositivos do tĂ­tulo "Da Ordem EconĂŽmica e Social" da Constituição de 1946; (2) a anĂĄlise agregada da produção legal do perĂ­odo 1946-1964, onde examinaremos o universo dos projetos de lei apresentados, bem como das leis aprovadas nesta etapa democrĂĄtica, procurando caracterizar o padrĂŁo de sugestĂŁo e aprovação de leis por ator relevante ao longo dos sucessivos governos. For that, we make a systematic study of three dimensions of decision making process and the relationship between the Executive and the Legislative during the period: 1) a case study of the elaboration of the title ÂżThe economic and social orderÂż of the Brazilian Constitution of 1946; 2) the aggregate analysis of the legal production during 1946-1964, where the universe of bills submitted was analyzed, as well as laws approved during this democratic age; trying to characterize the standard of suggestions and approval of the laws by relevant political actor over successive governments. From the evidences gathered through this aggregate analysis of this legal production, we seek to examine critically some dominant theories on the role of political parties and the parliament in the development of economic policy during the first experience of multi-party presidential system democracy in Brazil to demonstrate the hypothesis that the development of party system at this time changed significantly the pattern of legal production and relationship between the powers in the period studied; 3) Finally, we analyze critically the classical theories of Celso Furtado about the relationship between the Legislative and the process of economic development during this time of Brazilian democracyDoutoradoHistoria EconomicaDoutor em Desenvolvimento Economic
