    Plane shear flows of frictionless spheres: kinetic theory and 3D soft-sphere discrete element method simulations

    We use existing 3D Discrete Element simulations of simple shear flows of spheres to evaluate the radial distribution function at contact that enables kinetic theory to correctly predict the pressure and the shear stress, for different values of the collisional coefficient of restitution. Then, we perform 3D Discrete Element simulations of plane flows of frictionless, inelastic spheres, sheared between walls made bumpy by gluing particles in a regular array, at fixed average volume fraction and distance between the walls. The results of the numerical simulations are used to derive boundary conditions appropriated in the cases of large and small bumpiness. Those boundary conditions are, then, employed to numerically integrate the differential equations of Extended Kinetic Theory, where the breaking of the molecular chaos assumption at volume fraction larger than 0.49 is taken into account in the expression of the dissipation rate. We show that the Extended Kinetic Theory is in very good agreement with the numerical simulations, even for coefficients of restitution as low as 0.50. When the bumpiness is increased, we observe that some of the flowing particles are stuck in the gaps between the wall spheres. As a consequence, the walls are more dissipative than expected, and the flows resemble simple shear flows, i.e., flows of rather constant volume fraction and granular temperature

    The Role of TH9 Lymphocytes in Rheumatoid Arthritis

    Background: Th9 cells are IL-9-secreting Th lymphocytes that are involved in the immunological responses underlying parasitic infections and allergic diseases. In the case of autoimmune diseases, Th9 cells seem to be involved in the pathogenesis of experimental autoimmune encephalomyelitis. No study has yet evaluated the effects of Th9 responses in rheumatic diseases such as rheumatoid arthritis (RA). Objectives: The aim of this study was determine the prevalence of Th9 lymphocytes in RA patients and identify their possible association with the discontinuation of biological treatment with infliximab (IFX). Methods: We enrolled 55 consecutive RA outpatients: 15 not receiving any immunosuppressive drug; 20 responders to IFX treatment; and 20 who had discontinued IFX because of adverse events or inefficacy and were being treated with other biological agents (ABA, TCZ, ETN or CTZ) and traditional immunosuppressive drugs. The matched control group consisted of 10 healthy subjects. After giving their informed consent, the subjects underwent blood sampling for the isolation of peripheral blood mononuclear cells (PBMCs). The PBMCs were cultured with/without IFX 50 mg/L for 18 hours, and the percentage of Th9 cells was assessed by means of flow cytometry. Th9 lymphocytes were identified as IFN\u3b3-, IL4-, IL17-, IL9-secreting CD4+ T cells. Results: Cytometric analysis revealed no significant decrease in the percentage of Th9 cells after IFX exposure in any of the groups, but there were significantly fewer cells in the healthy controls than the RA patients both before and after the IFX stimulation assay (Fig.1). The higher frequency of Th9 cells in the patients was not associated with higher levels of anti-nucleus autoantibodies or other auto-antibody subsets, or with a higher likelihood of experiencing an adverse event or lack of efficacy on IFX treatment. Conclusions IL-9 levels are increased in RA patients, in whom it plays a crucial role. Th9 cells are the major producers of IL-9, and their prevalence is higher in RA patients than in healthy subjects, although they do not seem to affect the outcome of biological therapies

    The aim of this study was to determine the prevalence of T helper 9 (Th9) lymphocytes in rheumatoid arthritis (RA) patients and to identify a possible association between the percentage of Th9 and the discontinuation of a biological treatment with an anti-tumor necrosis factor (TNF) (infliximab). We collected peripheral blood mononuclear cells (PBMCs) from 55 consecutive RA outpatients and 10 healthy controls. Th9 lymphocytes were identified as interferon gamma, interleukin (IL)4-, IL17-, IL9-secreting cluster of differentiation 4 (CD4)+ T cells. Cytometric analysis revealed no significant decrease in the percentage of Th9 cells after infliximab exposure in any of the groups, although it was lower in healthy controls than RA patients either before and after the infliximab stimulation assay. Th9 cells are IL-9-secreting T helper lymphocytes whose role in RA is still poorly known. IL-9 levels are increased in RA patients, in whom this cytokine plays a crucial role. Th9 cells are the major producers of IL-9, and their prevalence is higher in RA patients than in healthy subjects; however our experiment in vitro does not demonstrate an association between Th9 lymphocytes and the response to infliximab. Further studies are required to evaluate the real involvement of Th9 population in the immunogenicity of anti-TNF agents

    Deriving relativistic momentum and energy

    We present a new derivation of the expressions for momentum and energy of a relativistic particle. In contrast to the procedures commonly adopted in textbooks, the one suggested here requires only the knowledge of the composition law for velocities along one spatial dimension, and does not make use of the concept of relativistic mass, or of the formalism of four-vectors. The basic ideas are very general and can be applied also to kinematics different from the Newtonian and Einstein ones, in order to construct the corresponding dynamics.Comment: 15 pages, 2 figure

    Linear biocompatible glyco-polyamidoamines as dual action mode virus infection inhibitors with potential as broad-spectrum microbicides for sexually transmitted diseases

    The initial steps of viral infections are mediated by interactions between viral proteins and cellular receptors. Blocking the latter with high-affinity ligands may inhibit infection. DC-SIGN, a C-type lectin receptor expressed by immature dendritic cells and macrophages, mediates human immunodeficiency virus (HIV) infection by recognizing mannose clusters on the HIV-1 gp120 envelope glycoprotein. Mannosylated glycodendrimers act as HIV entry inhibitors thanks to their ability to block this receptor. Previously, an amphoteric, but prevailingly cationic polyamidoamine named AGMA1 proved effective as infection inhibitor for several heparan sulfate proteoglycan-dependent viruses, such as human papilloma virus HPV-16 and herpes simplex virus HSV-2. An amphoteric, but prevailingly anionic PAA named ISA23 proved inactive. It was speculated that the substitution of mannosylated units for a limited percentage of AGMA1 repeating units, while imparting anti-HIV activity, would preserve the fundamentals of its HPV-16 and HSV-2 infection inhibitory activity. In this work, four biocompatible linear PAAs carrying different amounts of mannosyl-triazolyl pendants, Man-ISA7, Man-ISA14, Man-AGMA6.5 and Man-AGMA14.5, were prepared by reaction of 2-(azidoethyl)-\u3b1-D-mannopyranoside and differently propargyl-substituted AGMA1 and ISA23. All mannosylated PAAs inhibited HIV infection. Both Man-AGMA6.5 and Man-AGMA14.5 maintained the HPV-16 and HSV-2 activity of the parent polymer, proving broad-spectrum, dual action mode virus infection inhibitors

    Extended kinetic theory applied to inclined granular flows: role of boundaries

    We compare the predictions of extended kinetic theory (EKT), where the roles of surface friction and correlation in fluctuation velocities are taken into account, with discrete element simulations of steady, fully-developed, inclined flows of identical spheres over bumpy bases, in the presence and absence of flat, frictional sidewalls. We show that the constitutive relation for the pressure of EKT must be modified in the proximity of the boundary, because of the influence of excluded volume and shielding associated with collisions of particles with the boundary itself. We also note that currently available boundary conditions for flows over bumpy planes in kinetic theory underestimate the energy dissipation. These two observations explain the lack of agreement of EKT with the simulations, in terms of the maximum angles of inclination for which steady, fully-developed flows are possible. That is, for some high angles of inclination, EKT does not have solutions, while steady flows are predicted in DEM. However, whenever a solution to the system of differential equations of EKT does exist, the predicted distributions of velocity, solid volume fraction and granular temperature satisfactorily match the numerical measurements. The incompressible, algebraic approximation of EKT, which ignores the conduction of energy in the energy balance, admits solutions for a wider range of angles of inclination, as in the simulations, but fails to reproduce the quantitative and qualitative behaviour of solid volume fraction and granular temperature in the two conductive layers at the top and bottom of the flow. When frictional sidewalls are added to the domain, we show that the spanwise ratio of shear stress to pressure is linearly distributed in the dense core region of the flow, confirming that the sidewalls exert, on average, a Coulomb-like resistance to the flow with an effective friction coefficient which is less than half the actual particle-wall friction

    The risk stratification of adverse neonatal outcomes in women with gestational diabetes (STRONG) study

    Aims: To assess the risk of adverse neonatal outcomes in women with gestational diabetes (GDM) by identifying subgroups of women at higher risk to recognize the characteristics most associated with an excess of risk. Methods: Observational, retrospective, multicenter study involving consecutive women with GDM. To identify distinct and homogeneous subgroups of women at a higher risk, the RECursive Partitioning and AMalgamation (RECPAM) method was used. Overall, 2736 pregnancies complicated by GDM were analyzed. The main outcome measure was the occurrence of adverse neonatal outcomes in pregnancies complicated by GDM. Results: Among study participants (median age 36.8 years, pre-gestational BMI 24.8 kg/m2), six miscarriages, one neonatal death, but no maternal death was recorded. The occurrence of the cumulative adverse outcome (OR 2.48, 95% CI 1.59–3.87), large for gestational age (OR 3.99, 95% CI 2.40–6.63), fetal malformation (OR 2.66, 95% CI 1.00–7.18), and respiratory distress (OR 4.33, 95% CI 1.33–14.12) was associated with previous macrosomia. Large for gestational age was also associated with obesity (OR 1.46, 95% CI 1.00–2.15). Small for gestational age was associated with first trimester glucose levels (OR 1.96, 95% CI 1.04–3.69). Neonatal hypoglycemia was associated with overweight (OR 1.52, 95% CI 1.02–2.27) and obesity (OR 1.62, 95% CI 1.04–2.51). The RECPAM analysis identified high-risk subgroups mainly characterized by high pre-pregnancy BMI (OR 1.68, 95% CI 1.21–2.33 for obese; OR 1.38 95% CI 1.03–1.87 for overweight). Conclusions: A deep investigation on the factors associated with adverse neonatal outcomes requires a risk stratification. In particular, great attention must be paid to the prevention and treatment of obesity

    The minimization of mechanical work in vibrated granular matter

    Experiments and computer simulations are carried out to investigate phase separation in a granular gas under vibration. The densities of the dilute and the dense phase are found to follow a lever rule and obey an equation of state. Here we show that the Maxwell equal-areas construction predicts the coexisting pressure and binodal densities remarkably well, even though the system is far from thermal equilibrium. This construction can be linked to the minimization of mechanical work associated with density fluctuations without invoking any concept related to equilibrium-like free energies

    Association of kidney disease measures with risk of renal function worsening in patients with type 1 diabetes

    Background: Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage 653 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage 653 CKD in a large cohort of patients affected by T1DM. Methods: A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage 653 CKD (eGFR < 60 mL/min/1.73 m2) or eGFR reduction > 30% from baseline was evaluated. Results: The mean estimated GFR was 98 \ub1 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR < 90 ml/min/m2 were independent risk factors for stage 653 CKD and renal function worsening. When compared to patients with eGFR > 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions: Albuminuria and eGFR reduction represent independent risk factors for incident stage 653 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening
