Application de la navigation active à la caractérisation automatique de structures anatomiques
Les volumes tridimensionnels obtenus par scanner hélicoïdal ou IRM permettent de se déplacer virtuellement à l'intérieur de structures anatomiques et de les observer. Nous proposons une nouvelle méthode pour guider la planification de trajectoires grâce à l'interprétation locale des images pendant la navigation à l'intérieur des cavités d'une structure anatomique et ce sans prétraitement sur le volume initial. Le capteur virtuel construit un modèle de la scène inconnue grâce à l'analyse de l'image. Cette exploration autorise la caractérisation quantitative et qualitative de la structure anatomique ce qui est fondamental dans cette nouvelle application de l'endoscopie virtuelle qui concerne la chirurgie vasculaire. Nous reportons en particulier les résultats obtenus sur des images volumiques de fantômes et de patients acquises par scanner hélicoïdal en conditions cliniques
Segmentación automática de vejigas en IRM dinámicas mediante contornos activos
Las imágenes de resonancia magnética dinámicas son empleadas usualmente por los especialistas para el estudio de prolapso en los principales órganos de la pelvis. El análisis de la dinámica de dichos órganos junto con marcadores anatómicos pueden contribuir al desarrollo sistemas de asistencia al diagnóstico y tratamiento, para lo cual es de interés delimitar dichos órganos de manera eficiente y con la mínima interacción posible.
En este trabajo se propone un método para la segmentación automática de la vejiga, uno de los principales órganos pélvicos, a través de las secuencias temporales de imágenes. El método consta de tres pasos: pre-procesamiento, para mejorar el rango dinámico de la secuencia de imágenes; determinación de regiones candidatas dentro del primer cuadro por análisis morfológico y, finalmente, la segmentación de toda la secuencia mediante un algoritmo híbrido volumétrico, denominado RG-Snakes, basado en crecimiento de regiones y contornos activos. Los resultados obtenidos sobre imágenes de IRMd fueron comparados contra segmentaciones de referencia realizadas por especialistas, mostrando un alto factor de coincidencia en su gran mayoría.Sociedad Argentina de Informática e Investigación Operativa (SADIO

COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study
Background:
The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms.
Methods:
International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms.
Results:
‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country.
Interpretation:
This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men
Characterization of surface motion patterns in highly deformable soft tissue organs from dynamic MRI: An application to assess 4D bladder motion
Dynamic MRI may capture temporal anatomical changes in soft tissue organs
with high contrast but the obtained sequences usually suffer from limited
volume coverage which makes the high resolution reconstruction of organ shape
trajectories a major challenge in temporal studies. Because of the variability
of abdominal organ shapes across time and subjects, the objective of this study
is to go towards 3D dense velocity measurements to fully cover the entire
surface and to extract meaningful features characterizing the observed organ
deformations and enabling clinical action or decision. We present a pipeline
for characterization of bladder surface dynamics during deep respiratory
movements. For a compact shape representation, the reconstructed temporal
volumes were first used to establish subject-specific dynamical 4D mesh
sequences using the LDDMM framework. Then, we performed a statistical
characterization of organ dynamics from mechanical parameters such as mesh
elongations and distortions. Since we refer to organs as non flat surfaces, we
have also used the mean curvature changes as metric to quantify surface
evolution. However, the numerical computation of curvature is strongly
dependant on the surface parameterization. To cope with this dependency, we
employed a new method for surface deformation analysis. Independent of
parameterization and minimizing the length of the geodesic curves, it stretches
smoothly the surface curves towards a sphere by minimizing a Dirichlet energy.
An Eulerian PDE approach is used to derive a shape descriptor from the
curve-shortening flow. Intercorrelations between individual motion patterns are
computed using the Laplace Beltrami operator eigenfunctions for spherical
mapping. Application to extracting characterization correlation curves for
locally controlled simulated shape trajectories demonstrates the stability of

Outils et méthodologies pour la chirurgie orthopédique assistée par ordinateur mini-invasive
Cette thèse présente une méthodologie innovante pour la navigation chirurgicale minimalement invasive (MIS) dans un cadre orthopédique. Trois applications cliniques finales valident les concepts étudiés dans chaque chapitre : les arthroplasties d épaule, de hanche et de genou. Dans un premier temps, les techniques généralement mises en œuvre dans la chirurgie ouverte assistée par ordinateur sont présentées pour une indication originale, l arthroplastie d épaule. Cette chirurgie bien qu ouverte, présente tous les défis d une chirurgie MIS, offrant peu de zones anatomiques numérisables par le chirurgien. Pour résoudre ces problèmes d accès, nous proposons l utilisation d un palpeur échographique, permettant un accès à des zones anatomiques plus étendues. Nous démontrons la faisabilité technique de celui-ci en proposant une chaîne de numérisation échographique complète basée sur l utilisation d une sonde échographique 7,5 MHz, associée aux techniques décrites de navigation. Les problèmes liés à l utilisation peropératoire de la sonde, celui du calibrage spatial et des traitements d images nécessaires sont résolus et validés. L ensemble des solutions proposées permet d envisager une utilisation semi-automatique du palpeur en routine clinique. Ce dernier est ensuite utilisé pour initialiser un modèle déformable, permettant d obtenir une reconstruction patient-spécifique d un genou distal sans utilisation d imagerie préopératoire ionisante et compatible avec les exigences de la chirurgie MIS. ConText-GAN: using contextual texture information for realistic and controllable medical image synthesis*
This study proposes an enhancement to the ConText-GAN, an image synthesis model using a controllable texture input. The improvement consists in using a texture feature fusion module to reduce the complexity of the model, and enable the use of the OASIS architecture for image generation.Clinical relevance— The ConText-GAN can be used to generate images of fake patients, which are useful in the medical field due to the scarcity of data. An example is given of the generation of images showing pathological muscle tissue in the context of neuromuscular diseases
