Le travail comme déterminant social de la santé pour les personnes utilisatrices des services de santé mentale (notes de recherche)
Les politiques de désinstitutionnalisation des soins psychiatriques entraînent le retour massif d'ex-patients psychiatriques au sein de la vie en société. Conformément à l'approche des déterminants sociaux de la santé et du bien-être, nous soutenons que les actions menées hors du cadre hospitalier sont les plus favorables au bien-être des personnes utilisatrices des services de santé mentale. En tête de liste des déterminants sociaux de la santé, il faut placer le travail qui constitue l'un des principaux vecteurs d'intégration sociale. Et pourtant, seulement 15 % des ex-patients psychiatriques parviennent à intégrer le marché régulier du travail. Les recherches qui expliquent cette réalité sont peu nombreuses. Notre équipe du GRASP (Groupe de recherche sur les aspects sociaux de la santé et de la prévention)/Centre FCAR souhaite remédier à cette situation en analysant les rapports de la santé mentale et du travail à l'aide des notions relatives au capital social, à la territorialité, l'empowerment, l'intersectorialité et au rétablissement (recovery).Work as a social determinant of health for mental health services consumers Deinstitutionalization policies of psychiatric services have led to a massive return of former psychiatric patients in society. In accordance with social determinants of health and well-being, the authors sustain that actions led outside the hospital environment are the most favorable to mental health services consumers. On the list of the social determinants, work must be placed on top and constitutes a major factor of social integration. Yet, only 15 % of former psychiatric patients succeed in integrating the regular labour market. There is still very little research explaining this reality. Conforme a una aproximación de los determinantes sociales de la salud y del bienestar, sostenemos que las acciones llevadas afuera del cuadro del hospitalario son más favorables para el bienestar de las personas usiarias de servicios de salud mental. En el primer lugar de la lista de determinantes sociales de salud debemos poner el trabajo, que constituye uno de los vectores principales de integración social. Sin embargo, solamente 15% de ex-pacientes psiquiátricos logran una integración del mercado de trabajo regular. Las investigaciones que explican esta realidad son poco numerosas. Em primeiro lugar na lista dos determinantes sociais na área da saúde, é preciso colocar o trabalho, que constitui um dos principais vetores de integração social. Entretanto, apenas 15% dos ex-pacientes psiquiátricos conseguem integrar-se ao mercado normal de trabalho. As pesquisas que explicam esta realidade são pouco numerosas. Nossa equipe do GRASP (Grupo de Pesquisa sobre os Aspectos Sociais da Saúde e da Prevenção)/Centro FCAR procura corrigir esta situação ao analisar as relações da saúde mental e do trabalho com a ajuda das noções relativas ao capital social, à territorialidade, à autonomização (empowerment), à intersetorialidade e à recuperação (recovery)

The James Webb Space Telescope Mission
Twenty-six years ago a small committee report, building on earlier studies,
expounded a compelling and poetic vision for the future of astronomy, calling
for an infrared-optimized space telescope with an aperture of at least .
With the support of their governments in the US, Europe, and Canada, 20,000
people realized that vision as the James Webb Space Telescope. A
generation of astronomers will celebrate their accomplishments for the life of
the mission, potentially as long as 20 years, and beyond. This report and the
scientific discoveries that follow are extended thank-you notes to the 20,000
team members. The telescope is working perfectly, with much better image
quality than expected. In this and accompanying papers, we give a brief
history, describe the observatory, outline its objectives and current observing
program, and discuss the inventions and people who made it possible. We cite

Albiglutide and cardiovascular outcomes in patients with type 2 diabetes and cardiovascular disease (Harmony Outcomes): a double-blind, randomised placebo-controlled trial
Background:
Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke.
Methods:
We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. This study is registered with ClinicalTrials.gov, number NCT02465515.
Findings:
Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. On Nov 8, 2017, it was determined that 611 primary endpoints and a median follow-up of at least 1·5 years had accrued, and participants returned for a final visit and discontinuation from study treatment; the last patient visit was on March 12, 2018. These 9463 patients, the intention-to-treat population, were evaluated for a median duration of 1·6 years and were assessed for the primary outcome. The primary composite outcome occurred in 338 (7%) of 4731 patients at an incidence rate of 4·6 events per 100 person-years in the albiglutide group and in 428 (9%) of 4732 patients at an incidence rate of 5·9 events per 100 person-years in the placebo group (hazard ratio 0·78, 95% CI 0·68–0·90), which indicated that albiglutide was superior to placebo (p<0·0001 for non-inferiority; p=0·0006 for superiority). The incidence of acute pancreatitis (ten patients in the albiglutide group and seven patients in the placebo group), pancreatic cancer (six patients in the albiglutide group and five patients in the placebo group), medullary thyroid carcinoma (zero patients in both groups), and other serious adverse events did not differ between the two groups. There were three (<1%) deaths in the placebo group that were assessed by investigators, who were masked to study drug assignment, to be treatment-related and two (<1%) deaths in the albiglutide group.
Interpretation:
In patients with type 2 diabetes and cardiovascular disease, albiglutide was superior to placebo with respect to major adverse cardiovascular events. Evidence-based glucagon-like peptide 1 receptor agonists should therefore be considered as part of a comprehensive strategy to reduce the risk of cardiovascular events in patients with type 2 diabetes.

Le rêve de Nabuchodonosor et sa vision de la statue composite dans le second chapitre du livre de Daniel appartiennent au folklore de la tradition judéo-chrétienne depuis l’Antiquité. Le personnage de Nabuchodonosor étant basé en grande partie sur Nabonide, le dernier roi de Babylone (556-539 av. J.C.), le présent article offre quelques réflexions sur les origines de cette vision, arguant notamment que le motif de la succession des empires remonte au règne de Nabonide, et que les exigences de Nabuchodonosor envers les Chaldéens contiennent une réminiscence de l’obsession de Nabonide pour ses propres rêves et leur aspect prémonitoire.Nebuchadnezzar’s dream of the composite statue in Daniel 2 has belonged to the traditional folklore of the Judeo-Christian tradition since Antiquity.
