    Total thyroidectomy associated to chemotherapy in primary squamous cell carcinoma of the thyroid

    Primary squamous cell carcinoma of the thyroid (PSCCT) is a rare malignant disease with rapid fatal prognosis. The onset is generally characterized by sudden bilateral latero-cervical lymphadenopathy. The Authors report patient of 58-year-old who referred for evaluation of rapidly aggravating bilateral latero-cervical lymphadenopathy. The US highlighted the presence of a hypoechoic nodular lesion characterized by peri and intra-nodular vascularization. Multilayer CT showed diffused involvement of mediastinal and bilateral latero-cervical lymph nodes, with no evidence of primary pulmonary neoplasia or elsewhere. The patient underwent total thyroidectomy. The peri-isthmic tissue was removed due to the presence of a small roundish formation, that was due to lymph node metastasis at histological examination. Histological diagnosis: PSCCT. The immunohistochemical panel of the thyroid lesion was indispensable for the differential diagnosis between PSCCT, medullary carcinoma, anaplastic carcinoma, and thyroid metastasis of neoplasia with unknown primitiveness. The patient underwent chemotherapeutic treatment with Carboplatin and Paclitaxel with modest improvement of dysphagia symptoms and reduction of 10-15% of the target lesions. The clinical course was characterized by loco-regional progression of the disease with exitus in 10 months after diagnosis. Survival and quality of life after surgical therapy and chemotherapy were like that of patients undergoing only chemotherapy. Due to the extreme rarity of the neoplasia, 60 cases described in Literature, no exclusive guidelines are reported for PSCCT. More extensive case studies are needed to evaluate the effects of total thyroidectomy with intent R0/R1 on improving survival and quality of life of patients with PSCCT

    Transitions of cardio-metabolic risk factors in the Americas between 1980 and 2014

    Background: Describing the levels and trends of cardio-metabolic risk factors associated with non-communicable diseases (NCDs) is vital for monitoring progress, planning prevention and provide evidence to support policy efforts. We aimed to analyse the transition in body-mass index (BMI), obesity, blood pressure, raised blood pressure (RBP) and diabetes in the Americas, 1980-2014. Methods: Pooled analysis of population-based studies with data on anthropometric measurements, biomarkers for diabetes, and blood pressure from adults aged 18+ years. A Bayesian model was used to estimate trends in BMI, RBP (systolic blood pressure ≥140 mmHg or diastolic blood pressure ≥90 mmHg) and diabetes (fasting plasma glucose ≥7.0 mmol/l, history of diabetes, or diabetes treatment) from 1980 to 2014 in 37 countries and 6 sub-regions of the Americas. Findings: 389 population-based surveys from the Americas were available. Comparing the 2014 with the 1980 prevalence estimates, the obesity ratio was the largest in the non-English-speaking Caribbean sub-region (4.71 in men and 2.50 in women) showing that the prevalence in 2014 for men is almost five times larger than it was in 1980. The English-speaking Caribbean sub-region had the largest ratio regarding diabetes (2.14 in men and 2.13 in women). Conversely, the ratio for RBP signals that the frequency of this condition has diminished across the region; the largest decrease was found in North America (0.56 in men and 0.54 in women). Interpretation: Despite the generally high prevalence of cardio-metabolic risk factors across the Americas region, estimates also show a high level of heterogeneity in the transition between countries

    Cartilage Oligomeric Matrix Protein (COMP): A Biomarker of Arthritis

    Arthritis is a chronic disease with a significant impact on the population. It damages the cartilage, synovium, and bone of the joints causing pain, impairment, and disability in patients. Current methods for diagnosis of and monitoring the disease are only able to detect clinical manifestations of arthritis late in the process. However, with the recent onset of successful treatments for rheumatoid arthritis and osteoarthritis, it becomes important to identify prognostic factors that can predict the evolution of arthritis. This is especially critical in the early phases of disease so that these treatments can be started as soon as possible to slow down progression of the disease. A valuable approach to monitor arthritis would be by measuring biological markers of cartilage degradation and repair to reflect variations in joint remodeling. One such potential biological marker of arthritis is cartilage oligomeric matrix protein (COMP). In various studies, COMP has shown promise as a diagnostic and prognostic indicator and as a marker of the disease severity and the effect of treatment.     Dynamic Tests on a Steel Frame Equipped with Hysteretic and Visco-Re-Centring Energy Dissipating Bracing Systems

    Abstract An extensive program of dynamic experimental tests, named JetPacs (Joint Experimental Testing on Passive and semiActive Control Systems), has been carried out at the Structural Laboratory of the University of Basilicata considering a 1:1.5 scaled three-dimensional steel frame, derived from a 2-storey, 1-bay prototype building. The JetPacs Project was developed within the RELUIS 2005-08 project, founded by Italian Dept. of Civil Protection. This work refers to the experimental tests on the model equipped with two different EDB systems based on Hysteretic Dampers (HD) and visco-re-centering devices (SMA+VD). The devices were designed with the purpose of limiting the inter-storey drifts so that the frame yielding is surely prevented. The visco-re-centering device was obtained by coupling Viscous Dampers (VD), with re-centering device based on shape memory alloy (SMA) wires, while the Hysteretic Damper was constituted by low-carbon U-shaped steel plates. The main objective of this paper is to compare the seismic response of the structure with SMA+VD and HD EDB's in order to calibrate a new performance based design procedure for the evaluation of the mechanical characteristics of SMA+VD devices. In this paper, the experimental results of the shaking table tests, in terms of comparison of both design solutions which lead to a comparable level of protection for the framed structure, are shown. The results obtained by numerical nonlinear time history analysis (NTHA) of both design solutions are also showed and compared with experimental ones

    Risks of Leveraged Products

    Leveraged investments have become a fundamental feature of modern economies. The new financial products allow people to take greater-than-usual exposures to risk factors. This thesis analyzes several different aspects of the risks involved by some frequently used leveraged products: CDOs, CDSs, and hedge funds. It is shown that these risks have indeed several facets and that misjudging them can have severe effects for both individual investors and the global financial stability. However, although leveraged products can be more complex than other financial instruments, their characteristics in terms of risks and returns can usually be understood rather well by careful scholars. The aim of this thesis is to contribute to a better understanding of some of the features of leveraged products and provide useful insights on how to best use these new instruments

    Prevalence of zoonotic helminths in italian house dogs

    Introduction: Dogs may act as potential sources of zoonotic parasites, e.g. intestinal helminths like Toxocara spp., Ancylostoma spp., Echinococcus spp. In particular circumstances, the environment contaminated by parasitic elements represents a source of infection for people and animals. The present study has evaluated the presence of zoonotic helminths in house dogs from central and north-eastern Italy. Methodology: Stool samples from 493 dogs were examined by a qualitative copromicroscopic technique and differences in prevalence of zoonotic parasites were statistically examined in relation to canine individual data. Results: 48/493 (9.7%) were positive for at least one parasite. Helminths recovered were Trichuris vulpis (5.5%), Toxocara canis (4.3%), Ancylostoma spp. (0.6%) and Eucoleus aerophilus (0.4%), while no cestodes were detected. Age and living with other dogs resulted risk factors for T. canis infection. Conclusions: The health risk associated with the occurrence of parasitic nematodes in privately owned dogs, along with the current anthelmintic treatment plans, are discussed

    Affective Contagion: How Attitudes Expressed by Others Influence Our Perception of Actions

    Vitality forms represent a fundamental aspect of social interactions by characterizing how actions are performed and how words are pronounced on the basis of the attitude of the agent. Same action, such as a handshake, may have a different impact on the receiver when it is performed kindly or vigorously, and similarly, a gentle or rude tone of voice may have a different impact on the listener. In the present study, we carried out two experiments that aimed to investigate whether and how vocal requests conveying different vitality forms can influence the perception of goal-directed actions and to measure the duration of this effect over time. More specifically, participants were asked to listen to the voice of an actor pronouncing “give me” in a rude or gentle way. Then, they were asked to observe the initial part of a rude or a gentle passing action, continue it mentally, and estimate the time of its completion. Results showed that the perception of different vitality forms expressed by vocal requests influenced the estimation of action duration. Moreover, we found that this effect was limited to a certain time interval (800 ms), after which it started to decay
