    FOCUS-PDCA管理在降低静脉用药调配中心差错的应用

    目的探讨静脉用药调配中心(Pharmacy intravenous admixture service,PIVAS)的差错管理模式,充分发挥PIVAS员工的积极性与管理的意识,减少各类差错的发生。方法将FOCUS-PDCA管理引入我院PIVAS的差错管理,分析PIVAS药品调配过程中存在的差错,实施优化流程,制定相应的防控措施,并逐项改进。结果通过一年时间的FOCUS-PDCA管理,PIVAS外部差错率由0.050%降为0.00%,内部差错率由0.473%下降为0.356%,均有明显的降低。结论 FOCUS-PDCA管理方法引入PIVAS,能显著降低PIVAS的调配差错率,减少潜在的医疗风险,保障患者用药安全,有一定的推广应用价值
