Respostas hormonais em diferentes tipos de exercÃcios em portadores de insuficiência cardÃaca
Este estudo teve como objetivo identificar, descrever e analisar os tipos de exercÃcios fÃsicos recomendados e suas alterações hormonais em pacientes com insuficiência cardÃaca em reabilitação com exercÃcio fÃsico; bem como analisar os efeitos de uma reposição hormonal nestes indivÃduos. Para isso, foi realizada uma pesquisa de revisão de literatura, através de levantamentos bibliográficos em bases de dados, como Capes, Pubmed, Biremee Lilacs, na busca de artigos em periódicos especÃficos. Observou-se que, através de um programa de caminhadas, houve uma diminuição dos hormônios angiotensina II, aldosterona, vasopressina, e aumento do peptÃdeo natriurético atrial (PNA), houve também uma alteração positiva no T3. Este estudo pode ser útil para futuras intervenções de um profissional qualificado na escolha do exercÃcio para reabilitação, visando uma melhor qualidade de vida do portador de insuficiência cardÃaca
Notificação de incidentes: barreiras e estratégias para promover a cultura de segurança
Objective: The purpose was to identify the barriers of underreporting, the factors that promote motivation of health professionals to report, and strategies to enhance incidents reporting. Method: Group conversations were carried out within a hospital multidisciplinary team. A mediator stimulated reflection among the subjects about the theme. Sixty-five health professionals were enrolled. Results: Complacency and ambition were barriers exceeded. Lack of responsibility about culture of reporting was the new barrier observed. There is a belief only nurses should report incidents. The strategies related to motivation reported were: feedback; educational intervention with hospital staff; and simplified tools for reporting (electronic or manual), which allow filling critical information and traceability of management risk team to improve the quality of report. Conclusion: Ordinary and practical strategies should be developed to optimize incidents reporting, to make people aware about their responsibilities about the culture of reporting and to improve the risk communication and the quality of healthcare and patient safet
Assessment of the adherence to and costs of the prophylaxis protocol for venous thromboembolism
OBJECTIVES: Evaluate adherence to the therapeutic prophylaxis protocol for venous thromboembolism (VTE) as well as the costs of this practice. METHODS: A descriptive and cross-sectional study was conducted at a State General Hospital in Brazil through reports of drug dispensions, prescriptions and risk stratification of patients. Adherence to the VTE prophylaxis protocol was monitored. The tests for VTE diagnosis measured the adherence to therapeutic prophylaxis treatment, and the purchase prices of the drugs went into the calculation of drug therapy costs. The level of adherence to prescriptions for VTE prophylaxis in the hospital was classified as ‘‘adherence’’, ‘‘non-adherence’’ and ‘‘justified non-adherence’’ when compared with the protocol. RESULTS: Protocol adherence was observed for 50 (30.9%) patients, and non-adherence was observed for 63 (38.9%) patients, generating an additional cost of 514.71/month in savings due to a reduction in the number of daily administrations of unfractionated heparin while still providing an effective method for preventing VTE. Twenty-six patients stratified as having medium to high risk of VTE who did not receive prophylaxis were identified, generating $154.41 in savings. However, these data should be evaluated with caution since the risks and outcomes associated with not preventing VTE outweigh the economy achieved from not prescribing a drug when a patient needs it. The only case of VTE identified during the study period was related to justified non-adherence to the protocol. CONCLUSION: The protocol is based on scientific evidence that describes an effective therapy to prevent VTE. However, the protocol should be updated because the justifications for non-adherence are based on scientific evidence, and this justified non-adherence generates savings and yields effective disease prevention
Influência da força muscular respiratória pré-operatória na evolução clÃnica após cirurgia de revascularização do miocárdio
O objetivo foi avaliar se diferenças na força muscular respiratória pré-operatória: a) se relacionam com outros fatores de risco pré-operatórios; e b) influenciam o risco de surgimento de complicações pulmonares pós-operatórias (CPP), os tempos de permanência em ventilação mecânica (VM) e em unidade de terapia intensiva (UTI) e hospitalar após cirurgia de revascularização do miocárdio. O estudo foi observacional, prospectivo, tipo coorte. Vinte e quatro indivÃduos concluÃram o estudo e, após análise dos valores da manovacuometria pré-operatória, foram alocados em: G1, com pressão inspiratória máxima (PImáx) >;70% do previsto (n=13, 54%); e G2, PImáxThe aim of this study was to assess whether preoperative respiratory muscle strength: a) is related to other preoperative risk factors and b) represents a higher risk to postoperative pulmonary complications (PPC), higher time under mechanical ventilation (MV), intensive care unit (ICU) and hospitalization, after myocardial revascularization surgery. Twenty-four patients were studied and, after the analysis of preoperative measures taken, divided into two groups: G1, with maximum inspiratory pressure (MIP) over 70% predicted value (n=13, 54%); and G2, with MIP below 70% predicted value (n=11, 46%). At the statistical analysis, significance level was set at 5% (p;0.05). Almost half of G2 patients, who had lower MIP, also presented maximum expiratory pressure (MEP) below predicted value. As for time under MV, postoperative ICU and in-hospital times, besides number of PPC, no statistical differences were found between the groups. When compared to G1, G2 patients showed higher relative risk to developing postoperative pulmonary complications
Como tornar a reabilitação cardiovascular mais presente e efetiva?
Objetivo: embasar na literatura a prática dos profissionais que atuam com reabilitação cardÃaca. Método: trata-se de uma descrição da experiência dos profissionais de saúde que prestam assistência e acompanham os pacientes que realizam RCPM no Núcleo de Cardiologia e Medicina do Esporte da Universidade do Estado de Santa Catarina. Resultado: o programa existe cerca de 25 anos a atende de forma gratuita a população encaminhada de serviços públicos e privados da região da grande Florianópolis, com o objetivo de promover a reabilitação de seus pacientes através de programas baseados em exercÃcio fÃsico, com estratégias que englobam exercÃcios fÃsicos convencionais, dança de salão e atualmente a implementação da reabilitação do tipo Home Base. Conclusão: através do programa contatou-se que o preparo profissional e a intervenção por diferentes estratégias de exercÃcios podem favorecer a adesão, reduzindo os custos do tratamento e possuindo ação efetiva na reabilitação

The relationship between R&D, patents, human resource capability and productivity is widely discussed in the economic literature. The interaction between these areas is very important for economic analysis, as it is increasingly assumed to be the key issue in the role of technology in the production of the wealth of nations. This paper examines the correlation between these four sets of data in Brazil and represents only a preliminary stage in the investigation of the complex reldtionship between them. o' to The results show a strong correlation between R&D with patents and R&D with human resource capability.
